Tc-99m myocardial perfusion agents such as Tc-99m sestamibi or Tc-99m tetrofosmin has advantages over T1-201 for myocardial perfusion scan fuck as low attenuation and easy availability. However, Tc-99m agents do not redistribute so they need to be given T times, namely after stress and at rest. To evaluate whether rest image is needed in patients showing normal stress image, 43 patients who underwent both myocardial perfusion scan and coronary angiography and showed normal stress images were evaluated. Findings of rest images of them were evaluated whether they change the diagnosis or treatment plans. Among 43 patients who showed normal stress myocardial perfusion imaging, 31 (72.1%) showed no additional informations. However, among 5 patients with vasospastic angina 4 (80%) showed abnormal rest images in spite of normal stress images. So, when vasospastic angina is suspected clinically, rest image could be helpful in identifying patients with coronary vasospasm. In conclusion, rest myocardial perfusion images were not helpful in 72.1 % of patients with angina when stress images were normal. In only exception was those with vasospastic angina.
Apparent washout of T1-201 may occur between redistribution and reinjection images. To examine the frequency of it, we prospectively compared 4-hour redistribution and reinjection images in 63 consequent patients. All patients underwent pharmacological stress test using 0.56 mg/kg dipyridamole. Immediately after the 4-hour redistribution images, 1 mCi T1-201 was injected at rest, and images were reacquired 10 minutes after reinjection. The stress, redistribution, and reinjection images were then analyzed semiquantitatively (0=no uptake, 1=faint uptake, 2=mildly diminished uptake, 3=normal uptake). Of the 100 abnormal myocardial regions on the stress images, 54 showed either complete or partial reversibility on 4-hour redistribution images. After reinjection 11(21.2%) of these regions demonstrated apparent T1-201 washout due to low differential uptake of the tracer. Such lesions would appear irreversible if redistribution imaging is not performed before reinjection. Thus 4-hour redistribution imaging should be performed for assessment of myocardial ischemia and viability.
Purpose: Using rest T1-201/dipyridamole stress gated Tc-99m-MIBI/ 24 hour delay T1-201 SPECT, we investigated the predictive values of the markers of the stress-rest reversibility (Rev), T1-201 rest perfusion (Rest), T1-201 24 hour redistribution (Del) and Tc-99m-MIBI gated systolic thickening (Thk) for wall motion improvement after coronary artery bypass surgery. Materials and Methods: In 39 patients (M;F= 34:5, age $58{\pm}8$), preoperative and postoperative (3 months) SPECT were compared. 24 hour delayed SPECT was done in 16 patients having perfusion defects at rest. Perfusion or wall motion was scored from 0 to 3 (0: normal to 3: defect or dyskinesia). Wall motion was abnormal in 142 segments among 585 segments of 99 artery territories which were surgically revascularized. Results: After bypass surgery, ejection fraction increased from $37.8{\pm}9.0%$ to $45.5{\pm}12.3%$ in 22 patients who had decreased ejection fraction preoperatively. Wall motion improved in 103 (72.5%) segments among 142 dysfunctional segments. Positive predictive values (PPV) of Rev, Rest, Del, and Thk were 83%, 76%, 43%, and 69% respectively. Negative predictive values (NPV) of Rev, Rest, Del, and Thk were 48%, 44%, 58%, and 21%, respectively. Rest/gated stress/delay SPECT had PPV of 74% and NPV of 46%. Though univariate logistic regression analysis revealed Rev (p=0.0008) and Rest (p=0.024) as significant predictors, stepwise multivariate test found Rev as the only good predictor (p=0.0008). Conclusion: Among independent predictors obtained by rest T1-201/ stress gated Tc-99m-MIBI/ delayed T1-201 myocardial SPECT for wall motion improvement after bypass surgery, stress-rest reversibility was the single most useful predictor.
Coronary angiography should be performed to verify the patency of bypass graft if coronary artery disease is said to be surgically corrected. Rest/stress myocardial SPECT could be used as a noninvasive alternative to prove patency of bypass graft and functional significance of graft patency by showing improvement of rest perfusion and perfusion reserve after operation. We evaluated the outcome of coronary artery bypass graft surgery by comparing rest/dipyridamole-stress myocardial SPECT before and after operation. Myocardial SPECT were performed $19{\pm}23$ days before and $108{\pm}19$ days after operation in 44 patients(M:F=25:19). Segmental perfusion were scored with 0(normal) to 3(defect) independently for each image set of rest and stress. Perfusion scores were compared between pre and postoperative images. Rest/stress images of pre and post-operative SPECT were compared in paired sets. Postoperative change of perfusion was determined as no change, improvement or aggravation for each artery territory. Postoperative outcome of patients were analyzed. Overall, 74%(158 segments) of 215 segments which had perfusion decrease before operation showed improvement. Among 60 artery territories, 37 territories(62%) improved. Among 42 patients with perfusion decrease in preoperative SPECT, 21 patients(47%) improved after operation. Severe persistent defects improved in 43%(6/14). We concluded that bypass surgery improved myocardial perfusion in dipyridamole stress induced or persistent decrease and that rest/stress myocardial SPECT could be used as a guide for which artery should be operated.
Purpose: Dipyridamole stress myocardial perfusion SPECT could predict prognosis, however, long-term follow-up showed change of hazard ratio in patients with suspected coronary artery disease. We investigated how long normal SPECT could predict the benign prognosis on the long-term follow-up. Materials and Methods: We followed up 1169 patients and divided these patients into groups in whom coronary angiography were performed and were not. Total cardiac event rate and hard event rate were predicted using clinical, angiographic and SPECT findings. Predictive values of normal and abnormal SPECT were examined using survival analysis with Mantel-Haenszel method, multivariate Cox proportional hazard model analysis and newly developed statistical method to test time-invariance of hazard rate and changing point of this rate. Results: Reversible perfusion decrease on myocardial perfusion SPECT predicted higher total cardiac event rate independently and further to angiographic findings. However, myocardial SPECT showed independent but not incremental prognostic values for hard event rate. Hazard ratio of normal perfusion SPECT was changed significantly (p<0.001) and the changing point of hazard rate was 4.4 years of follows up. However, the ratio of abnormal SPECT was not. Conclusion: Dipyridamole stress myocardial perfusion SPECT provided independent prognostic information in patients with known and suspected coronary artery disease. Normal perfusion SPECT predicted least event rate for 4.4 years.
