A STUDY ON THE LATERAL CEPHALOMETRIC TOMOGRAPHY OF TMJ ARTHROSIS (악관절증의 측방두부계측 단층방사선학적 연구)

  • The author obtained individualized lateral cephalometric tomograms from 23 young adults (46 of left and right normal TMJ) with normal occlusion and 20 patients (14 of patient asymptomatic TMJ and 26 of patient symptomatic TMJ) with clicking and painful TMJ after the analysis of submental vertex view. Individualized lateral cephlometric tomogram analysis and TMJ space analysis were performed after tracing each film. All data from these analysis was recorded and statistically processed with CYBER computer system. 1. The results were obtained as follows. In submental vertex view, the mean condylar angulation of Rt. side in normal group was 20.348°±6.358°, Lt. side was 18.870°±7.777° and Rt. side in patient group was 19.350°±7.576° Lt. side was 17.750°±6.146° respectively. The mean condylar angulation of Rt. side was larger than Lt. side in normal and patient group. 2. When the mandible was moved from centric occlusion to centric relation, condylar position relating to the glenoid fossa was placed posteriorly and superiorly in normal TMJ group and patient symptomatic TMJ group. 3. In centric relation position, the proportion of anterior space to posterior space was 1.593 for normal TMJ group, 1.604 for patient asymptomatic TMJ group and 1.671 for patient symptomatic TMJ group. In centric occlusion position, 1.390 for normal TMJ group, 1.539 for patient asymptomatic TMJ group and 1.196 for patient symptomatic group. Normal TMJ group, patient asymptomatic TMJ group and patient symptomatic TMJ group and patient symptomatic TMJ group revealed significant difference in ∠C₂ measurement. (ANOVA-test, p<0.05) 5. Normal group and patient group revealed significant difference in Fh, ∠C₁and ∠C₂ measurement. (T-test, p<0.05) 6. There were strong positive correlation (0.8771) between Fp and Fm, and strong negative correlation (-0.9039) between ∠C₂ and ∠C₁ from the lateral cephalometric tomogram analysis.

Accuracy and reproducibility of landmark of cone beam computed tomography (CT) synthesized cephalograms (Cone beam computed tomography로 합성된 두부규격 방사선사진에서의 각 계측점의 정확도와 재현성에 관한 연구)

  • Introduction: Cone beam computed tomography (CBCT) has various advantages and is used favorably in many fields in dentistry. Especially, CBCT is being used as basic diagnostic tool for 3-dimensional analysis in orthognathic patient. Two-dimensional cephalograms can be synthesized from CBCT digital imaging and communications in medicine (DICOM) data. In this study, conventional cephalograms and CBCT were taken simultaneously, and representative landmarks were located and analyzed in its accuracy and reproducibility. Materials and Methods: Ten patients who had orthognathic surgery in Wonkwang University Daejeon Dental Hospital participated in this study. For each patient, CBCT and conventional cephalogram was taken. By using Ondemand (Cybermad, Korea), 2-dimensional cephalograms was established on CBCT. In addition, 19 landmarks were designated and measured by 3 orthodontists twice a week. After these landmarks were transferred to a coordinate, distance of landmark and axis, standard error, distribution degree were measured, compared and analyzed. Results: Comparing the CT ceph group and conventional cephalogram group, CT ceph group had shown shorter distance of landmark and axis in S, Hinge axis, Bpt, Ba, Or, Corpus left. Standard error of the mean shows that CT ceph group has better reproducibility in Or, Corpus left, Hinge axis at X axis and Na, U1R, U1T, Bpt, PNS, Ba Corpus left, Hinge axis at Y axis. In both groups, mean error was less than 1.00 mm, no significant difference were found between CT ceph group and conventional cephalogram group in all measurements. Furthermore, comparing two groups, each 17 landmarks out of 19 had its characteristic in distribution degree. Conclusion: No significant difference were found between CBCT composed cephalographic radiograph and conventional cephalograghic radiograph, clinical application may be possible if improved.

A RADIOGRAPHIC STUDY OF CHANGES OF UPPER RESPIRATORY AIRWAY SPACE AFTER ORTHOGNATHIC SURGERY OF BOTH JAWS IN PATIENTS WITH SKELETAL CLASS III MALOCCLUSION (골격성 제3급 부정교합자의 양악 수술 후 상기도 공간의 변화에 관한 두부 계측 방사선학적 연구)

  • Purpose: The aim of this study is the changes of upper respiratory airway space in patients with mandibular prognathism after 2-jaw orthognathic surgery in patients with skeletal classs III malocclusion. Method: We measured the lines between selected upper airway landmarks on lateral cephalometric x-ray films of skeletal class III 64 persons who had not been operated yet, were 6 months after operation. The test subjects were divided into 3 groups according to maxillary movement, as follows; maxillary advancement (MA) group, maxillary posterior impaction (MPI) group, maxillary posterior impaction and superior repositioning (MPI+MSR) group. Result: In this study, nasopharyngeal airway space in MPI+MSR group was significantly increased after operation (p<0.05). Oropharygeal and hypopharyngeal airway space in MA group and MPI group were significantly decreased after operation (p<0.05). From hyoid bone to anterior mandible point distance in MA group and MPI group were significantly decreased after operation (p<0.05). Conclusion: Oropharygeal and hypopharyngeal airway space were influenced more by mandibular set-back than maxillary movement. Maxillary movement surgery as well as mandibular setback surgery should be taken into consideration in order to minimize symptoms related to obstructive sleep apnea syndrome after operation.

A Case Report of Hemifacial Microsomia (반측안면 왜소증의 치험 1례)

  • This is a case report and review of literature that deals with hemifacial microsomia corrected by costochondral graft, Lefort I osteotomy and bilateral intraoral sagittal split ramus osteotomy. Patient, 23 years old female, had visited to treat the esthetic problem due to a deviation of jaw. On the basis of clinical and radiographic examinations, she was diagnosed as hemifacial microsomia. First, costochondral graft was performed to bridge the defect between glenoid fossa and body of mandible. After 11 months, Patient was performed a Lefort I osteotomy and bilateral intraoral sagittal split ramus osteotomy to create a symmetric jaw. Patient was satisfied with final esthetics and there have been no evidence of infection ill now.

MEAN VALUES OF LATERAL CEPHALOMETRIC ANALYSIS FROM KOREAN ADULTS WITH NORMAL OCCLUSION IN RELATION TO THE DIAGNOSIS OF OBSTRUCTIVE SLEEP APNEA SYNDROME (폐쇄성 수면무호흡증 진단을 위한 두부규격 방사선사진 계측 분석에 의한 한국 성인 정상교합자의 정상치에 관한 연구)

  • Purpose : The lateral cephalometric Korean norms of the skeletal and pharyngeal dimension were analyzed for the diagnosis of obstructive sleep apnea syndrome(OSAS). Materials and Methods : The lateral cephalometric radiographies were taken in male(n=53) and female(n=50), who had a normal profile, class I occlusion, normal ANB $(0{\sim}4^{\circ})$ and normal Wits $Appraisal(-4{\sim}0mm)$. The X-rays were traced by two oral and maxillofacial surgeons twice. The significant differences between male-female and examiners were tested. Results : The angle of mandibular line to Nasion-Sella line was $31.12^{\circ}/33.79^{\circ}$ (in male/in female, p=0.0018). The lower Gonion angle was $73.74^{\circ}/73.74^{\circ}(p=0.9978)$. The length of the soft palate was 36.93mm/34.35mm(p=0.0002). The pharyngeal airway space was 13.42mm/11.55mm at mandibular plane level(PAS(ML))(p=0.0025). The hyoid was placed inferiorly to mandibular plane 10.18mm/7.72mm(p=0.0051). The results from this study are to be used for the diagnosis of OSAS.

  • In this paper, we developed a Mac-Yule measurement system which consider psychological stable state of patience. The developed system consist with a hardware device that can derive a EEG, respiration and pulse wave, and a software which acquire a biological signal and signal processing The EEGs are derived with bipolar method from frontal head. The respiration signals obtain from nasal front with a transducer which consist with thermistor bridge. The pulse waves are detected from earlobe with photoplethysmograph method. A power spectrum of EEG are used as the decision parameters of psychological stable state of patience. The decision of Mac-Yule are defined as origin text method that of numbers of pulse to 1 respiration period. As the results of experiment with developed system, we could have a spectrum band discretion of EEG signal, stable respiration signal detection and automatic gain controlled pulse signal with realtime. And then, we could detect Mac-Yules from processed signals.

A Numerical Analysis of Load Transfer Behavior of Axially Loaded Piles (축하중 재하말뚝의 하중전이 거동에 대한 수치해석)

  • The behavior of axially loaded pile was analyzed by two methodologies: one is the finite difference method using load transfer curves recommended by API(1993) , and the other is the numerical analysis using the FLAC program. From both analyses, load-displacement curves and load distributions along the depth were evaluated appropriately for the measured. The analysis using the FLAC could capture the nonlinearity of load-displacement curve even for unloading and reloading cases, since the unloaded stress paths of fill layer elements occurred on the failure envelop. Futhermore, the measured load transfer curves were compared with the API recommendations and with the calculations obtained front the results of the FLAC analysis for the interpretation of the transfer behavior between the soil and the pile under axial loadings. It was concluded that the atrial behavior of open ended piles at Pusan could be evaluated by both the finite difference analysis using API load transfer curves and the numerical analysis using FLAC.

구치(臼齒)의 근원심(近遠心) 경사도(傾斜度)에 관(關)한 두부방사선계측학적(頭部放射線計測學的) 연구(硏究)

  • This study was designed to get the informations of the mesiodistal axial inclinations of the posterior teeth and of the relationships between these and other angular measurements of facial bony structures in normal occlusion and malocclusion groups using lateral roentgenocephalograms. The subjects consisted of 73 normal occlusions (31 males 42 females), 38 Class II Division 1 malocclusions (17 males 21 females) and 47 Class III malocclusions (19 males 28 females). The findings of this study are as follows : 1. In mandible, the posterior teeth axes of Class II Division 1 malocclusion group were inclined more mesially and those of Class III malocclusion group were inclined more distally than normal occlusion group. In maxilla, Class II Division 1 malocclusion group showed more distal inclination and Class III malocclusion group showed more mesial inclinaton of 1st, 2nd premolars and more distal inclination of 1st, 2nd molars than those of the normal occlusion group. 2. There was a tendency for teeth axes to maintain nearly the same inclination in relation to occlusal plane irrespective of various OMA and OPA in each group. 3. F M A, P M A and O P A were the largest in Class II Division 1 malocclusion group and O M A, GoA were the largest in Class 111 malocclusion group. 4. There were high correlationships between mandibular teeth inclinations related to mandibular plane and 4 angular measurements except OPA, and between maxillary teeth inclinations related to palatal plane and OPA.

  • Bimaxillary Protrusion can be treated effectively in growing patients and in adults with conventional orthodontic therapy. However, In the adult patient, combined surgical and orthodontic treatment modalities may offer distinct advantages over such conventional therapy. In those cases complicate by vertical jaw dysplasia, sagittal dysplasia, or transverse skeletal discrepancy in addition to bimaxillary protrusion, the possibilities of obtaining successful results through orthodontic treatment alone greatly diminish. Surgical retraction of both maxillary and mandibular anterior segments with subapical osteotomies and ostectomies in the extraction site may be a good treatment alternative. Treatment time and possible adverse effects of lengthy orthodontic therapy may be reduced and optimum esthetic improvement may be facilitated. On the following cases, patient who had bimaxillary protrusion with Angle class III malocclusion was treated with combined orthodontic - surgical therapy by anterior subapical segmental osteotomies.

A CEPHALOMETRIC STUDY ON MESIODISTAL AXIAL INCLINATION OF POSTERIOR TEETH IN OPEN BITE AND DEEP BITE (개방교합과 과개교합에서 구치의 근원심 치축경사도에 관한 두부방사선계측학적 연구)

  • For the purpose of investigating mesiodistal axial inclination of posterior teeth in normal occlusion group, open bite and deep bite group and investigating the correlationship between the axial inclination of posterior teeth and overbite of anterior teeth, a cephalometric study was performed on the subjects consisted of normal occlusion group(40), open bite group(71 : Angle's Class I, Class II, division 1 25, Class III 25) and deep bite group(64 : Angle's Class I 23, Class II, division 1 21, Class III 20). Mesiodistal axial inclination of posterior teeth to occlusal, mandibular and palatal plane were measured. The findings of this study were as follows : 1. Upper and lower posterior teeth were more mesially inclined to occlusal plane in open bite group than in deep bite group. 2. Lower posterior teeth were more mesially inclined in deep bite group than in open bite group in Angle's Class II, division 1 malocclusion but there were no significant differences in Angle's Class I and Class III malocclusion. 3. There was no significant correlationship between the axial inclination of posterior teeth to each plane and overbite of anterior teeth in open bite group. 4. There was a significant correlationship between the axial inclination of upeer and lower second premolar to occlusal plane and overbite of anterior teeth in Angle's Class I, Class II, division 1 and Class III malocclusion.
