The Necessity for End-of-Life Care Education: A Preliminary Analysis with Interns at Two University Hospitals (임종돌봄에 대한 교육의 필요성: 2개 대학병원의 인턴을 대상으로 예비 분석)

  • Purpose: This study was performed to explore the current state of end-of-life (EoL) care education provided to new interns at two university hospitals. Methods: A questionnaire was given to incoming interns (N=64). The levels of acquired knowledge and experience of clinical observation were measured. Seven areas for self-assessment questions were identified and used to analyze the interns' attitudes towards EoL-related education and practice. Results: On average, participants learned five elements (nine in total) from EoL-related classes and two (seven in total) from clinical observation. The most frequently educated element was how to deliver bad news (96.9%) in the classroom setting and how to control physical symptoms (56.5%) in clinical observation. Less than 20% received training on EoL care communication, including discussion of advanced directives. Compared with participants who had no EoL training, those who had EoL training showed positive attitudes in all seven categories regarding overall satisfaction, interest and preparedness in relation to EoL-care classes and practice. Conclusion: Although interns are responsible for caring of dying patients, their EoL training in classroom and clinical settings was very insufficient. Further research should be conducted to establish an education system that provides sufficient knowledge and training on EoL care.

A Study on Empathy Experiences of Welfare Practitioners of Single-Household Elderly Support Center (독거노인종합지원센터 돌봄서비스 실천인력의 공감경험에 관한 연구)

  • The purpose of this study was to understand empathy experiences of welfare practitioners who work with elderly people living alone. We conducted focus group interviews with 14 service-managers and life-managers who work at Single-Household Elderly Support Center, and analyzed the data using inductive subject analysis, one of the qualitative research methodologies. It was found that 2 subjects and 5 essential themes about empathy experiences of care service employees. first, care service employees were aware of the importance of empathic technology, while also having difficulty empathizing with clients. And sometimes they were feeling compassion Fatigue. second, they thought that there was a lack of empathy education for care service employees and therefore recognized the need for empathy programs that can act as a platform. Based on the results of this research, we suggested practical measures to improve empathy ability of practitioners who working with the elderly living alone.

  • This article examines the reason why Indonesian Javanese are using their traditional medicine jamu. Tracing the cultural logic of Javanese, this study observes the space and the process in which the demand on jamu is culturally constructed. In order to address this, the article focuses on Javanese family and their royal court, the spaces where jamu originated from and has been used. Then, the discussion proceeds to identify jamu as a medium to construct rukun(harmony) and tentram(peace), as well as to express and ensure Javanese cosmology. Along with the research, the article argues that: 1) jamu is an embodiment of Javanese local knowledge system into which Javanese consciousness, notions, knowledge, and techniques integrate; 2) the health achieved by using jamu is what Javanese call tentram, which encompasses physical, mental, spiritual and social dimensions of health; 3) the health of tentram has the value of a resource that becomes the foundation to build a good, prosperous society.

Current status of meal and snack service in elementary care classes in Gyeonggi area (경기도 지역 초등돌봄교실의 급·간식 운영 현황)

  • Purpose: This study aimed to investigate the current status of food service management in elementary care classes. Methods: A focus group interview with seven care class managers and a survey with 101 care class managers using a self-administered questionnaire in Gyeonggi province were conducted. Results: In the focus group interview, purchased meals and snacks were evaluated as low quality by the care class managers. Frequent use of frozen or processed food and products with low prices were also reported as problems. Care class managers were in charge of meal and snack planning without any guidelines or expert advices. The results of the survey show that most schools serve purchased snacks and meals. The average unit costs of one meal and snack were 4,062 and 1,463 Won, respectively. The average unit costs of snacks during semester (p = 0.015) and vacation (p = 0.039) were significantly lower in rural than urban areas. The percentages of schools that prepared nutrition standards for meal and snack planning in elementary care classes were only 7.4% and 10.9%, respectively. The meal menus were mostly provided by catering service companies, and the snack menus were planned mostly by the care class managers. Menu planning by the care class managers was more usual in rural than urban areas (p = 0.054 for meal planning and p = 0.008 for snack planning). Just 33.7% of schools introduced safety standards for food service in elementary care classes, and more than half of the respondents (56.4%) did not do a regular medical check-up. Only 33.7% of the respondents received education for food safety. Conclusion: These results show the necessity for establishment of detailed guidelines for food service in elementary care classes and for the introduction of a food safety and nutrition education program customized for care class managers.

In-home care Service's Enhance Scheme to the Elderly Patriots & Veterans through the Unity Management with the Regional Social Welfare System (지역사회복지체계와 통합 운영을 통한 고령유공자 재가돌봄서비스 확충 방안)

  • It is growing a necessity of consideration and management to the relevant policies for the elderly Patriots and Veterans systematically in order to increase the life-supported services since the rapid growth of the aging society. It is difficult to ensure the outcome of cost-effectiveness about the policies because of the residual welfare view and the limitation of the government budget in terms of the traditional Bohun(Korean independent movement activists, veterans and sacrificed advocators for democratization) service, though. In addition, it is not realized the individual activity and daily life-supported services yet to the elderly Patriots & Veterans since the concentration of security income and medical care. Accordingly, it is proper to quest a new Bohun policy from the traditional residual welfare view to universal or ubiquitous welfare service, which will enable them to get the various care service benefit in terms of cost-effectiveness. In this vein, the study is trying to figure out the alternative supply mechanism in-home care service to the elderly Patriots and Veterans; especially, the study emphasizes on the linkage between the regional social welfare service system and Bohun welfare service system.

The mediating effect between the degree to provide emotional labor and personal relationship in the intent to stay for Care worker (돌봄 여성노동자의 감정노동수행정도와 직무지속의사와의 관계에 있어 대인관계의 매개효과)

  • The purpose of this study is to find out the relationship among the degree to provide emotional labor, personal relationship and the intent to stay of female care service workers. Specifically, this study focuses on the mediating effect of the personal relationship (client and agency relation). The path analysis and structural equation modeling analysis were performed on the collected data using SPSS18.0. and AMOS 8.0. And Sobel test conducted for examine the mediating effect. The results are the followings. First, the result of the analysis showed that agency relationship was an indirect factor as the partial mediating effect on relationship between the degree to provide emotional labor and the intent to stay for the care women workers. The result suggests that education utilizing various techniques and strategies to overcome the difficulty accompanied by emotional labor such as communication education and the government try to effort managing and controlling the agency which impact on the labor condition of care workers. Second, the government also tries to provide the trust and collaboration network system which construct a good relationship between the care worker and the agency.

Factors Affecting the Satisfaction of Elderly Care Service in the Elderly Living Alone (독거노인의 노인돌봄서비스 만족도에 영향을 미치는 요인)

  • The purpose of this study is to examine the factors affecting the satisfaction of elderly care service in the elderly living alone and to provide basic data for the improvement of the satisfaction of elderly care service. The subjects of this study were 301 elderly people aged 65 or older who were receiving care services for the elderly in J city, Chungbuk province. Multiple regression analysis was conducted to examine the influencing factors. The results of this study showed that the factors affecting the satisfaction of elderly care service of the elderly living alone were the manager factor, service factor, institutional factor, and income level. This shows that the higher the life manager, service factor, and institution factor, and the elderly living alone with low income level, the higher the satisfaction of the elderly care service. Especially, the factors of life manager had the greatest effect on the satisfaction of elderly care service, followed by institutional factors and service factors. Based on these results, policy proposals are as follows: First, improvement of professionalism and treatment of life managers, second, professional and systematic service support and strengthening of social safety net, and third, improvement of appropriateness, accessibility and persistence for quality improvement of elderly care service.

The meaning and challenges of the introduction of the Customized Care Services for Older Adults (노인맞춤돌봄서비스의 도입 의미와 과제)

  • The South Korean government has introduced a new Customized Care Service for Older Adults by eliminating the six existing care services for them since 2020. It is a significant change of care service system for older adults and the service would play a key role in providing preventive services for them. The aim of the study is to examine the meaning and tasks of the introduction of the Customized Care Services for Older Adults in South Korea by using the framework of Gilbert and Terrell. The study found that the new service was designed under the government's policy contexts of the establishments of public-based social service providers and the emphasis of community care, and it has brought about a number of significant institutional changes of care system in terms of allocation, benefits, delivery, and finance. The institutional coverage of the service was increased to lower 70% of older adults and a large number of kinds of health and social care services would be provided for them. The sphere of living was set in each local area and the new service providers are entirely in charge of providing the services in each sphere. The financing system was changed to support service providers by providing government subsidies. Although a number of positive results would be expected to occur for the elderly, it is likely that some challenging tasks would happen in the fields as follows: the inadequate coverage, the vagueness of the application of self-care principle and the decrease in the amounts of direct care services, the inappropriate area of service provision, and the supplier-oriented provision of services rather than user-oriented.

The Effect of Nurse-led Religious and Spiritual Intervention on Psychological Distress among Hospice Care Patients: A Systematic Review and Meta-analysis

  • The aim was to evaluate the impact of nurse-led religious and spiritual interventions (RSI) on the psychological distress of hospice care patients. Pubmed, CINAHL, Cochrane Library, Embase, RISS, KISS, and ScienceOn were searched until December 2023. A systematic review and meta-analysis were performed on 10 studies involving a total of 597 hospice care patients. Overall, nurse-led RSI appeared to have beneficial effects in reducing anxiety (SMD = -1.09, 95% CI = -1.67 to -0.50) and depression (SMD = -1.12, 95% CI = -1.83 to -0.40). These findings suggest that, if applied appropriately, nurse-led RSI can be used as a method to alleviate psychological distress among hospice care patients.

A Study on the Experience of Social Support in the Education and Care of Children of Married Migrant Women (결혼이주여성의 자녀 교육과 돌봄에서 사회적 지지 경험연구)

  • This study explored the experience of social support in the education and rearing of children of immigrant women through international marriage and found its essential meaning. First of all, the husband's social support was very important, but the relationship with the husband had a different effect on childrearing and education. Parents-in-law had a positive and negative impact on child rearing and education of them due to cultural conflicts between the two countries. Their own mother was a strong support that gave them great strength just by being there, and as their children grew up, they regarded their mother as the source of bilingual education for their children. Other supporters around them were Korean friends who connected Korean society by sharing information on child care and education. Friends who spoke and communicated in their native language were emotional and psychological supporters that bonded the same experience of parenting and education for their children. In conclusion, the research participants expected a better life for themselves and their children by using a multi-layered social support system as well as a transnational family network in the process of child education and care. Accordingly, it was proposed to systematically improve the laws, systems, and policy support so that the social support system can be further strengthened at the family, community, and transnational levels for the education and care of children of immigrant women through international marriage.
