부식성 식도염에 관한 임상적 고찰 (Clinical Study of Corrosive Esophagitis)

근래 국민 생활이 향상되고 독, 극약물에 대한 규제가 강화되었을 뿐 아니라 사회 인식도 개선됨으로서 부식성 약물을 오연하는 경우는 점차 감소하고 있다. 그러나 물질 문명의 발달과 이를 뒤따르지 못하는 정서 생활의 부조화로 인하여 자살을 기도하고저 연하하는 경우는 아직도 적지않게 발생하고 있으며 또한 연하하는 부식제의 종류도 급격한 산업의 발달과 공업화로 인하여 다양해지고 있다. 식도 부식증에 관하여는 각종 산성(Pitkin, 1935 Carmody, 1930) 및 알카리성(Tree, 1942, Tucker, 1951) 부식제에 대한 연구 보고가 있었으며 이에 따른 협착증에 대해서는 Salzer(1920), Bokey(1924) 등이 조기 확장법을 주장한 이후 Spain(1950)에 의해 Cortison의 투여 효과가 소개되었고 또한 항생제의 동시투여로 그 치료 면에서 괄목할만한 발전을 하여 왔다. 그러나 이러한 치료 방법들에 못지않게 조기 식도경 검사를 시행하여 그 결과에 따라서 치료 방향을 결정하는 것은 매우 중요한 점으로 이 러한 시도가 최근의 추세라고 하겠다. 이에 저자들은 1971년부터 1981년 3월까지만 10년 3개월간 연세대학교 부속병원 이비인후과에서 입원 가료한 부식성 식도염 환자 96명에 대한 역학적 조사를 시행하여 아래와 같은 결과를 얻었기에 보고하는 바이다. 1) 성별 빈도는 남자와 여자의 비가 1대 1.7이며 연령 분포는 21세에서 30세가 38명 (39.6%)으로 가장 많았다. 2) 약물을 먹은 동기는 자살 목적이 80명 (83.3%), 사고의 경우는 10세미만이 9명 (56.3%)으로 가장 많았었다. 3) 약물은 빙초산 41명 (41.8%), 가성소다 20명 (20.4%), 염산 17명 (17.3%)의 순이었으며 연도별 빈도를 볼때 가성소다의 경우는 감소하였으나 산성부식제 특히 빙초산의 급격한 증가를 나타내었다. 4) 위세척을 한 경우는 57명 (59.4%)으로 대부분이조기 도착한 경우였다. 5) 대부분에서 Nasogastic tuhe를 삽입하였으며 (83.3%), 삽입하지 않은 16명 (16.7%)은 늦게 내원이 10명, 삽입 실패 4명, 기타 2명이었다. 6) 기관절개술이 요구된 경우는 호흡기장애가 있었던 17명 (17.7%)이었으며 분비물의 축적이 6명 (35.3%), 상기도폐쇄가 11명 (64.7%)이었다. 7) 조기 식도경 검사를 시행한 11명 (11.5%)중 48시간이내가 6명 (54.5%)으로 가장 많았다. 조기 식도경검사 결과 ; 점막에 중증도의 궤양이 8명 (72.7%)으로 대부분을 차지하였고 점막의 종창과 부식 정도가 비교적 경하였던 6명 (54.5%)은 입원 가료가 필요하지 않아서 곧 경구 음식 투여를 시작하였으나 입원 가료가 필요했던 중(重)증의 경우 그 입원 일수는 평균 4주간 이었다. 8) 본 병원에 처음 내원하여 치료를 받은 22명중 Nasogastric tube를 넣은 기간은 평균 11.6일 이었으나 최근에는 tube를 삽입하여둔 기간은 대체로 입원기간과 일치하며 이는 조기 식도경 검사를 시행하여 병변의 정도에 따라 결정하였다. 9) 대부분의 경우에서 Steroid를 투여하였으며 투여하지 않거나 투여가 지연된 경우는 47명 (48.9%)으로서 내용별 분류를 보면 (1) 약물의 종류로 인한 경우(산성, 미상) 12명, (2) 경(輕)하다고 인정된 경우 9명, (3) 늦게 내원한 경우 11명, (4) 비적응증(심한 감염, 합병증, 임신등)이 7명, (5) 기타(사망, 기록미비)가 8명이었다. 10) 협착으로 인한 확장 치료(Bougienage)를 요한 경우가 7명, 급식위루술(feeding gastrostomy)을 시행한 경우가 6명, 그 외에 수술적 치료를 요한 경우가 4명이었다. 11) 입원가료중 합병증이 생긴 경우는 27명 (28.1%)이었으며 심폐합병증, 심한출혈, 식도및 위장 천공, 신부전증의 순으로 빈도를 나타내었으며 사망 및 빈사상태에 회원이 8명이었다. 12) 식도 협착증이 있었던 경우는 13명 (56.5%)이었으며 협착 부위 및 빈도는 (1) 식도의 중간 부위 협착이 5명, (2) 식도의 상부 협착이 3명, (3) 하부 협착이 3명, (4) 전반적인 협착이 1명이였으며, (5) 하인두 및 유문협착이 1명이었다.
The International Atomic Energy Agency's Statute in Article III.A.5 allows it“to establish and administer safeguards designed to ensure that special fissionable and other materials, services, equipment, facilities and information made available by the Agency or at its request or under its supervision or control are not used in such a way as to further any military purpose; and to apply safeguards, at the request of the parties, to any bilateral or multilateral arrangement, or at the request of a State, to any of that State's activities in the field of atomic energy”. Safeguards are essentially a technical means of verifying the fulfilment of political obligations undertaken by States and given a legal force in international agreements relating to the peaceful uses of nuclear energy. The main political objectives are: to assure the international community that States are complying with their non-proliferation and other peaceful undertakings; and to deter (a) the diversion of afeguarded nuclear materials to the production of nuclear explosives or for military purposes and (b) the misuse of safeguarded facilities with the aim of producing unsafeguarded nuclear material. It is clear that no international safeguards system can physically prevent diversion. The IAEA safeguards system is basically a verification measure designed to provide assurance in those cases in which diversion has not occurred. Verification is accomplished by two basic means: material accountancy and containment and surveillance measures. Nuclear material accountancy is the fundamental IAEA safeguards mechanism, while containment and surveillance serve as important complementary measures. Material accountancy refers to a collection of measurements and other determinations which enable the State and the Agency to maintain a current picture of the location and movement of nuclear material into and out of material balance areas, i. e. areas where all material entering or leaving is measurab e. A containment measure is one that is designed by taking advantage of structural characteristics, such as containers, tanks or pipes, etc. To establish the physical integrity of an area or item by preventing the undetected movement of nuclear material or equipment. Such measures involve the application of tamper-indicating or surveillance devices. Surveillance refers to both human and instrumental observation aimed at indicating the movement of nuclear material. The verification process consists of three over-lapping elements: (a) Provision by the State of information such as - design information describing nuclear installations; - accounting reports listing nuclear material inventories, receipts and shipments; - documents amplifying and clarifying reports, as applicable; - notification of international transfers of nuclear material. (b) Collection by the IAEA of information through inspection activities such as - verification of design information - examination of records and repo ts - measurement of nuclear material - examination of containment and surveillance measures - follow-up activities in case of unusual findings. (c) Evaluation of the information provided by the State and of that collected by inspectors to determine the completeness, accuracy and validity of the information provided by the State and to resolve any anomalies and discrepancies. To design an effective verification system, one must identify possible ways and means by which nuclear material could be diverted from peaceful uses, including means to conceal such diversions. These theoretical ways and means, which have become known as diversion strategies, are used as one of the basic inputs for the development of safeguards procedures, equipment and instrumentation. For analysis of implementation strategy purposes, it is assumed that non-compliance cannot be excluded a priori and that consequently there is a low but non-zero probability that a diversion could be attempted in all safeguards ituations. An important element of diversion strategies is the identification of various possible diversion paths; the amount, type and location of nuclear material involved, the physical route and conversion of the material that may take place, rate of removal and concealment methods, as appropriate. With regard to the physical route and conversion of nuclear material the following main categories may be considered: - unreported removal of nuclear material from an installation or during transit - unreported introduction of nuclear material into an installation - unreported transfer of nuclear material from one material balance area to another - unreported production of nuclear material, e. g. enrichment of uranium or production of plutonium - undeclared uses of the material within the installation. With respect to the amount of nuclear material that might be diverted in a given time (the diversion rate), the continuum between the following two limiting cases is cons dered: - one significant quantity or more in a short time, often known as abrupt diversion; and - one significant quantity or more per year, for example, by accumulation of smaller amounts each time to add up to a significant quantity over a period of one year, often called protracted diversion. Concealment methods may include: - restriction of access of inspectors - falsification of records, reports and other material balance areas - replacement of nuclear material, e. g. use of dummy objects - falsification of measurements or of their evaluation - interference with IAEA installed equipment.As a result of diversion and its concealment or other actions, anomalies will occur. All reasonable diversion routes, scenarios/strategies and concealment methods have to be taken into account in designing safeguards implementation strategies so as to provide sufficient opportunities for the IAEA to observe such anomalies. The safeguards approach for each facility will make a different use of these procedures, equipment and instrumentation according to the various diversion strategies which could be applicable to that facility and according to the detection and inspection goals which are applied. Postulated pathways sets of scenarios comprise those elements of diversion strategies which might be carried out at a facility or across a State's fuel cycle with declared or undeclared activities. All such factors, however, contain a degree of fuzziness that need a human judgment to make the ultimate conclusion that all material is being used for peaceful purposes. Safeguards has been traditionally based on verification of declared material and facilities using material accountancy as a fundamental measure. The strength of material accountancy is based on the fact that it allows to detect any diversion independent of the diversion route taken. Material accountancy detects a diversion after it actually happened and thus is powerless to physically prevent it and can only deter by the risk of early detection any contemplation by State authorities to carry out a diversion. Recently the IAEA has been faced with new challenges. To deal with these, various measures are being reconsidered to strengthen the safeguards system such as enhanced assessment of the completeness of the State's initial declaration of nuclear material and installations under its jurisdiction enhanced monitoring and analysis of open information and analysis of open information that may indicate inconsistencies with the State's safeguards obligations. Precise information vital for such enhanced assessments and analyses is normally not available or, if available, difficult and expensive collection of information would be necessary. Above all, realistic appraisal of truth needs sound human judgment.
Fuzzy logic based Control Theory has gained much interest in the industrial world, thanks to its ability to formalize and solve in a very natural way many problems that are very difficult to quantify at an analytical level. This paper shows a solution for treating membership function inside hardware circuits. The proposed hardware structure optimizes the memoried size by using particular form of the vectorial representation. The process of memorizing fuzzy sets, i.e. their membership function, has always been one of the more problematic issues for the hardware implementation, due to the quite large memory space that is needed. To simplify such an implementation, it is commonly [1,2,8,9,10,11] used to limit the membership functions either to those having triangular or trapezoidal shape, or pre-definite shape. These kinds of functions are able to cover a large spectrum of applications with a limited usage of memory, since they can be memorized by specifying very few parameters ( ight, base, critical points, etc.). This however results in a loss of computational power due to computation on the medium points. A solution to this problem is obtained by discretizing the universe of discourse U, i.e. by fixing a finite number of points and memorizing the value of the membership functions on such points [3,10,14,15]. Such a solution provides a satisfying computational speed, a very high precision of definitions and gives the users the opportunity to choose membership functions of any shape. However, a significant memory waste can as well be registered. It is indeed possible that for each of the given fuzzy sets many elements of the universe of discourse have a membership value equal to zero. It has also been noticed that almost in all cases common points among fuzzy sets, i.e. points with non null membership values are very few. More specifically, in many applications, for each element u of U, there exists at most three fuzzy sets for which the membership value is ot null [3,5,6,7,12,13]. Our proposal is based on such hypotheses. Moreover, we use a technique that even though it does not restrict the shapes of membership functions, it reduces strongly the computational time for the membership values and optimizes the function memorization. In figure 1 it is represented a term set whose characteristics are common for fuzzy controllers and to which we will refer in the following. The above term set has a universe of discourse with 128 elements (so to have a good resolution), 8 fuzzy sets that describe the term set, 32 levels of discretization for the membership values. Clearly, the number of bits necessary for the given specifications are 5 for 32 truth levels, 3 for 8 membership functions and 7 for 128 levels of resolution. The memory depth is given by the dimension of the universe of the discourse (128 in our case) and it will be represented by the memory rows. The length of a world of memory is defined by: Length = nem (dm(m)+dm(fm) Where: fm is the maximum number of non null values in every element of the universe of the discourse, dm(m) is the dimension of the values of the membership function m, dm(fm) is the dimension of the word to represent the index of the highest membership function. In our case then Length=24. The memory dimension is therefore 128*24 bits. If we had chosen to memorize all values of the membership functions we would have needed to memorize on each memory row the membership value of each element. Fuzzy sets word dimension is 8*5 bits. Therefore, the dimension of the memory would have been 128*40 bits. Coherently with our hypothesis, in fig. 1 each element of universe of the discourse has a non null membership value on at most three fuzzy sets. Focusing on the elements 32,64,96 of the universe of discourse, they will be memorized as follows: The computation of the rule weights is done by comparing those bits that represent the index of the membership function, with the word of the program memor . The output bus of the Program Memory (μCOD), is given as input a comparator (Combinatory Net). If the index is equal to the bus value then one of the non null weight derives from the rule and it is produced as output, otherwise the output is zero (fig. 2). It is clear, that the memory dimension of the antecedent is in this way reduced since only non null values are memorized. Moreover, the time performance of the system is equivalent to the performance of a system using vectorial memorization of all weights. The dimensioning of the word is influenced by some parameters of the input variable. The most important parameter is the maximum number membership functions (nfm) having a non null value in each element of the universe of discourse. From our study in the field of fuzzy system, we see that typically nfm 3 and there are at most 16 membership function. At any rate, such a value can be increased up to the physical dimensional limit of the antecedent memory. A less important role n the optimization process of the word dimension is played by the number of membership functions defined for each linguistic term. The table below shows the request word dimension as a function of such parameters and compares our proposed method with the method of vectorial memorization[10]. Summing up, the characteristics of our method are: Users are not restricted to membership functions with specific shapes. The number of the fuzzy sets and the resolution of the vertical axis have a very small influence in increasing memory space. Weight computations are done by combinatorial network and therefore the time performance of the system is equivalent to the one of the vectorial method. The number of non null membership values on any element of the universe of discourse is limited. Such a constraint is usually non very restrictive since many controllers obtain a good precision with only three non null weights. The method here briefly described has been adopted by our group in the design of an optimized version of the coprocessor described in [10].
중이 병변에 관한 수술적 처치는 그 병소 부위가 고막에 국한되어 있거나 중이 강 전벽부 및 내면부의 경우 수술시야가 좋아서 그 처치가 용이하며 술후 청력개선 및 치유경과도 만족할만하며 다양한 수술적 처치방법이 연구되어 널리 시행되고 있다. 그러나 아직도 중이강 후벽이나 상고실에 있는 병소의 처리방법에 대하여서 많은 문제점을 노출시키고 있으며 그 이유로서는 첫째 수술 시야의 장애, 둘째 안면신경 손상의 위험, 셋째 등골 주변부 병소 조작에 의한 외임프의 유출 및 미로 손상의 위험, 넷째 상고실 부위가 너무 협소하여 수술시 병변 확인이 힘든 경우등을 들수 있다. 특히 Staged operation (단계수술)과 Intact Canal Wall Typanoplasty(외청도 보존형 유양동 삭개술)이 소개된 이후로는 더욱더 중이강후벽에 대한 해부학적 지식이 더욱 절실하게 되었으며, 두개골의 인종적 차이를 감안 할때 한국인에 대한 중이강 후벽의 구조적 연구를 필요로 하게 되었다. 중이강의 후벽(유돌벽)은 Reichert's 연골에서 기원되며 제 2새궁(Second branchial arch)에서 발생되며 이 후벽의 구조물중 Sinus tympani는 이관의 내피와(endothelial pouch) 중 후낭 (Saccus posticus) 에서 함기화 (pneumatization)된 중이봉소로서 이부위 병변의 제거가 중이수술시 중요한 의의를 갖는 이유는 만성중이염에 의해 염증성반응이 오래 지속되면 비 후된 점막에 의해 Sinus tympani가 밀폐되고, 지속되는 자극에 의해 주로 안면신경관의 골성부와 등골 및 후반규관(Posterior Semicircular Canal)의 골성부에 육아조직을 형성하기 쉬울뿐더러 진주종에 의해 이 부위의 침식성 파괴를 이르켜 각각의 조직을 노출시킴으로 인해 중요한 합병증을 초래하기 쉬우며 수술로 인한 중대한 후유증이 병발하기 쉬운 곳이므로 임상면에서 중요하다. 그러므로 오래 전부터 이곳 병변의 안전한 제거를 위해 여러사람의 끊임없는 연구가 있었다. Amjad (1968), Donaldson (1970)등은 각각 24개, 20개의 측두골 계측을 실시하여 Sinus tympani의 형태 해부하적 구조에 대하여 연구한 결과 Sinus tympani의 크기와 모양은 여러형태로 변형이 많으며, 특히 주위 중요한 조직과 연관성이 많은 Sinus tympani의 가장 은밀한 심부 (Deepest portion)는 거의 대부분 후부로 신전되는 경우의 변형이 많아 주위조직의 손상없이 Sinus tympani의 병변을 제거하기 힘든 형태라는 결론을 내리고 이곳의 수술적 처리에서 발생 할 수 있는 여러 문제점을 지적하였다. Jako (1965) 등은 Sinus tympani병소의 안전한 제거를 위해 Sinus tympani를 확인할 수 있는 광학거울 (Fine-grade optical mirror)을 이 부위에 넣고 반사시야에서 수술적 처치를 시행할 것을 주장했으나, 수술시의 출혈등에 의한 명료시야의 확보에 문제점이 있어 널리 시행되지 않았다. Saito(1971)등은 Sinus tympani의 접근방법으로서 종래의 중이강내에서 접근방식 (Tympani approach) 보다 유양동을 통한 접근(mastoid approach)이 보다 편리하고 확실한 방법일 것이라는 생각으로 42개의 측두골에 대한 미세해부학적 계측을 실시하였다. 그러나 연구결과에서 그는 Sinus tympani의 상부는 안면신경과 지나치게 가까운 거리에 있어 접근이 용이하지 않았고 후부는 이낭(Otic capsule)에서 발생한 중치밀골(Hard compact bone)로 되어 있으며 안면신경 과 후반규관 사이가 협소하여 수술시 micro-drill burr로서 제거하기가 용이치 않다는 사실을 밝혀내고 mastoid approach로는 Sinus tympani 접근이 힘들다는 결론을 지었다. Goodhill (1973)등은 술자의 위치에서 수술의 전 과정을 시행하는 보편적인 방법에서는 Sinus tympani의 병변제거가 힘들므로 술자의 위치의 반대방향에서 현미경적 시야를 확보함으로써 은밀한 부위의 병변제거가 용이하다는 Circumferential approach 방법을 주장하였으나 수술부위의 감염성이 높다는 점이 문제점으로 지적되었다. 한편 Sinus tympani와 함께 수술시 문제점이 될 수 있는 후벽구조의 일부인 facial recess는 내피와(Endothelial pouch)중 상낭(Saccus superior)과 내낭(Saccus medius)에서 발생한다. 최근 Intact Canal Wall Tympanoplasty에 대한 활발한 연구가 진행되고 이에 따라 단계수술을 시행하는 빈도가 증가함에 따라 이 수술의 성공여부에 관건이 되는 중이강과 유양동의 통기(Ventilation)를 유지시키기 위해서 facial recess에 대한 많은 연구가 있었다. Sheehy(1967)등은 facial recess를 개방하는 이유로서는 첫째 facial recess의 병변이 제거되고 둘째 골성고실륜(bony annulus)을 손상시키지 않고 난원창과 중이강 후부를 노출시킬 수 있으며 셋째 atticotomy를 시행치 않고도 안면신경의 일부와 상고실 (epitympsnum) 을 노출시키는 것이 용이하며 넷째 수술후 유양동의 통기를 유지시키는 Posterior tympanotony를 주장하였다. 중이수술을 성공적으로 시행하려면 중이강후벽의 정확한 형태해부학적 구조를 이해하고 이 부위 병변을 제거하는 용이한 접근방법에 대한 술자의 고도의 기술이 요구되므로 이에 저자는 다양한 해부학적 변형에 대하여 연구할 필요성을 느껴 Sinus tympani와 facial recess를 중심으로 한국인 정상성인의 중이강후벽의 크기와 모양을 계측하였다. 연구대상은 측두골 35개 대상으로 능형융기 (pyramidal eminence) 안면신경, 등골 및 반규관등을 중심으로 주변부 구조의 계측을 시도하였다. 계측의 정확성과 정밀성을 높이기 위해 각각의 은밀한 부위에 Elastic impression material 인 PERMLASTIC
무령왕릉의 보존대책을 수립하기 위하여 누수 현상, 고분구조 벽체거동 상황 및 구조안전점검, 고분 내에 서식하는 조류의 제거, 고분내 습기 및 결로현상 제거를 위한 공기조화시설 등을 포함하는 종합 정밀저사를 1996년 5월 1일 부터 1997년 4월 30일 까지 1년간 수행하였다. 이러한 연구를 수행하기 위하여 기초 지반에 대한 정밀측량, 고분 내외부의 년중 온습도 모니터링, 지반의 내부물성 파악을 위한 지구물리 탐사, 시추 자료에 대한 각종 물성실험, 지반의 투수계수 측정 및 지구화학적 분석을 실시하였다. 고분 내외부에 대한 정밀측량 결과 봉분 중심의 현실 중심보다 북동쪽으로 5m 이격되어 있다. 무령왕릉의 경우는 발굴 당시에도 유사한 분포를 하였으나, 이로 인하여 불균형적인 토압을 발생시키는 원인이 되고 있다. 벽돌깨짐 상황 조사결과 무령왕릉의 조사 대상 벽돌 6025장중 1972년에는 435장 파손되었던 것이 1996년 조사결과 1072장이 파손된 것으로 밝혀져 파손율이 2.5배로 증가하였다. 6호분의 경우는 이보다 더 심하여 벽돌 파손율이 2.9배로 증가하고 있다. 1972년 상황은 약 1450년간 진행된 것이고 1996년의 상황은 불과 24년간 진행된 것임을 감안할 고분이 발굴된 이후 벽돌의 균열은 상당한 가속도로 진행되어 온것이 사실이며 이대로 진행된다면 더욱 더 가속화될 것으로 판단된다. 고분벽체의 거동상태를 계측한 결과 무령왕릉 동측벽의 경우 우기와 건기에 각각 2.95 mm/myr및 1.52 mm/myr의 거동을 보여 우기에 지하수 유입에 의한 지반의 약화로 인하여 건기보다 2배정도 거동하는 양상을 보인다. 한편, 연도 입구의 호벽은 건기에 전실쪽으로 0.43 mm/myr, 연도쪽으로 2.05 mm/myr의 거동을 보여 무령왕릉에서는 가장 심한 거동을 보이고 있다. 6호분의 거동현황은 건기에만 측정된 바, 현실의 동측벽과 서측벽이 각각 벽체 뒤쪽으로 7.44 mm/myr, 현실안쪽으로 3.61 mm/myr의 거동을 보여 조사대상 5호분, 6호분, 7호분, 중 가장 심한 거동을 보이고 있다. 이는 고분 벽돌의 깨짐이 6호분이 가장 심하다는 사실과 무관하지 않은 것으로 판단된다. 봉분내부의 토양층구조에 대한 지오레이다 영상단면을 분석한 결과 무령왕릉 연도상부의 누수지방지층이 심하게 균열되어 있음을 발견하였다. 이 곳은 고분내부로 직접누수가 발생하는 곳이다. 직접누수와 지하수 형태로 유입된 침투수는 고분군 주위의 지반의 함수비를 증가시켜 지반의 지지력을 약화시키고 또한 고분내로 서서히 유입되어 고분내부의 습도를 100%로 유지시키는 주된 원인이다. 이러한 높은 습도는 고분내의 남조류의 번식을 가져왔으며 남조류의 번식은 현재 6호분이 가장 심각하고 7호분이 우려되는 수준이며 5호분은 문제가 없는 것으로 판단된다. 이와 같이 고분군의 발굴후 인위적인 환경변화와 지속적인 강우침투 및 배수 불량의 영향은 고분군의 안정성에 상당한 위험을 초래하였으며, 현 상태는 각 고분에 대한 보강이 불가피한 것으로 판단된다. 고분 벽돌의 깨짐, 고분 벽체의 거동, 조류의 서식등을 포함하여 송산리 고분군에서 발생되고 있는 보존상의 제반 문제점들을 일차적으로 누수 및 침투수에 의한 결과이다. 그러므로 무엇보다도 고분군 내부 및 고분 주변으로의 강우 및 지하수 침투를 막는 차수 대책이 시급한 것으로 판단된다. 또한 이미 발생한 변위가 더 이상 진행되지 않도록 하중을 경감하고 토압의 균형을 이루는 보강대책이 시급한 실정이다. 고분군의 보존대책으로는 고분의 구조안전에 관하여 근본적인 구조변경이 불가피한 직접 보강대책보다 간접적인 보강대책이 바람직한 것으로 판단된다. 간접적 보강대책으로서는 현 봉분규모의 축소, 불균등토압의 조정, 강우침투 방지를 위한 최종복토시스템, 유도배수구의 설치 및 능선 상부로부터의 지하수 차단시설을 포함한다. 이러한 차수대책은 고분군의 제문제를 해결하는 가장 근본적인 대책이라 할 수 있다. 고분내 결로현상 및 습기 제거를 위하여 항온항습장치를 전실에 설치하고 덕트를 통하여 고분내 현실로 순환시키는 방법을 제안하며 외부 온도의 영향을 덜 받게 하기 위하여 전실상부 토양을 현재보다 두껍게 하고 전실의 천정마감재를 열전도도가 낮은 목재로 교체하며 출입문의 이동등을 제안하였다. 금번 조사 연구를 통하여 얻어진 각종 보수 및 보존대책은 고분구조 자체에 가능한 한 최소의 변화를 주면서 가장 시급한 문제점부터 해결하도록 제시되었다. 설계 및 시공단계에서는 보수의 경중을 가려 순서에 입각하여 이루어져야만 하며 설계 및 시공단계에서 고분의 구조안전에 영향을 주지 않도록 각별한 주의가 요망된다. 끝으로, 상기의 보수대책이 실시된 후 이에 대한 평가 및 향후의 영구 보존을 위한 벽체경사 계측, 함수비 및 지하수위 계측, 온습도 측정 등을 포함하는 사후계측시스템은 필수적이라 할 수 있다. 각종 계측자료를 이용한 장기적이고도 세심한 분석을 통하여 완벽한 보존 관리가 이루어져야할 것으로 사료된다.로 사료된다.
식생활은 인간 생활의 주체이고 먹는다는 것은 그 수단이다. 그중 중요한 하나의 명제는 인간이 놓여진 여러 환경에서 어떻게 건강을 유지하고 그 개체가 소유하고 있는 능력을 최대치까지 생리적으로 성장 발전시킴과 동시에 최대한 수명을 연장시키기 위한 식물 섭취방법을 마이크로 레벨까지 해명하는데 있다. 인간은 일생동안 엄청난 양의 음식물을 먹는다(70세 수명일 경우 200만 파운드 즉 체중의 1,400배). 그러나 먹기는 먹되 자신의 건강과 장수를 위하여 어떤 음식을 어떻게 선택하여 어떻게 먹어야 하는 문제가 매우 중요하다. 최근 우리나라도 국민 소득이 늘면서 식생활은 서구화 경향으로 기우는 듯하다. 공해를 비롯한 수입식품 등 여러 가지 문제점이 제기됨에 따라 자연식과 건강식을 주장하는 소리가 높이 일고 있다. 그중에는 축산 식품이 콜레스테롤 함량이 다른 식품에 비하여 높게 함유하고 있다는 것으로 심혈관질환의 주범인양 무차별 강조하는 나머지 육식공포 내지는 계란 등의 혐오감 마저 불러 일으키는 경향까지 있는 듯하다. 따라서 본논고에서는 축산식품중의 콜레스테롤 함량수준이 과연 성인병의 주범인지 아니면 다른 지방산과 관련해서 올바르게 평가하고 그 문제점과 대책을 개관해 보고 요약하면 다음과 같다. 1. 사람은 유사이래 본능적으로 주변의 식물이나 동물의 고기를 먹고 성장하여 자손을 증식시키고 어느 사이에 늙으면 죽음을 맞이 하는 싸이클을 반복하면서 기나긴 세월동안 진화를 하여 오늘날의 인간으로서의 자태를 이루었다. 유인원과 같은 인류의 선조들은 수렵을 통해 육식을 많이 하였을 것이므로 인간은 원래 육식동물이 아닐까? 구석기시대의 유물을 보면 많은 뼈가 출토되고 “얄타미라”나 “라스코” 동굴벽화가 선명하게 묘사되고 있다. 2. 우리나라 선조 승구족의 일파가 백두산을 비롯한 만주 송화강 유역에 유입되면서 수렵과 목축을 주요 식품획득의 수단으로 식품문화권을 형성하면서 남하하여 한반도 민족의 조상인 맥족(貊族)으로 맥적(貊炙)이라고 하는 요리(오늘날의 불고기)를 먹었다는 기록이 있다. 3. 인간의 수명을 1900년대로 거슬러 올라가서 뉴질랜드가 세계최장수국(호주는 2위)로서 평균수명은 남자 58세, 여자 69세인 반면 일본과 한국은 당시 남자 36세, 여자 37세이던 것이 일본은 1989년에 이르러 세계 최장수국으로 등장했으나 1990년 당시 뉴질랜드
최근 10년간 우리나라 경제의 급격한 발전으로 이에 부수적으로 일어나는 대기오염은 생활환경을 파괴하는 오염형태의 하나로서 직접 또는 간접으로 인체에 미치는 피해는 격심하여가고 있으며 이것이 심각한 사회문제로 대두되었다. 대기오염이 인체에 미치는 영향에 관하여 병리나 임상적으로는 많은 연구보고가 있으나 호흡기로의 기시부인 비강이나 인의 영향에 관한 연구보고는 많지 않은 실정인 차제에 대기오염과 비, 인후질환자의 관계를 추구하여 그 대책을 논하는 것은 의의 있는 일이라 하겠다. 이러한 연구의 일환으로서 연세의대 공해연구소의 대기오염도조사에서 가장 심한 곳으로 확인된 부산시 우암동 지역에 위치한 S 공업고등학교 학생 469명은 조사군으로 하고 대기오염도가 낮은 K고등학교 학생 345명을 대조군으로 하여 내과, 안과, 비, 인후과적 검사를 실시하여 얻은 성적과 그 외의 문제점들을 검토하고자 한다. I. 위해오염물질 대기오염물질은 자극성가스와 질식성가스로 구분되며 이비인후과 질환과 밀접한 관계가 있는 것은 자극성 가스이며 여기에는 질소산화물, 유황산화물, 탄화수소와 그리고 광합성작용에 의하여 2차적으로 발생하는 강력한 자극성물질인 PAN(Peroxy acyl nitrate) 등을 열거할 수 있으며 이들 가스는 산화성이 강하기 때문에 점막이 부착되어 괴양 및 2차감염을 일으켜 인체에 피해를 준다. 이리한 오염물질은 고체연료(특히 석탄)나 액체연료의 불완전 연소때와 각종 차량의 배기가스로서 배출된다. 대표적인 오염물질인 일산화탄소, 질소산화물 및 유황산화물의 조사지역과 대조지역의 오염도는 다음과 같다. II. 인체에 미치는 피해 1. 일반적인 피해 대기오염이 인체에 미치는 영향은 오염물질의 물리적 화학적 성상 및 오염물질의 농도, 양, 그리고 폭로기간 등에 따라 다르다. 각 오염물질별 건강피해를 보면 (1)
현재 이과영역에서 청각기능을 진단하기 위해 순음 및 언어청각검사와 Impedance청각검사등을 이용한 여러가지 특수청각검사법이 사용되고 있으나 우리나라에서는 이에 대한 정상한국인 기준치의 연구가 몇몇 학자에 의해서만 산발적으로 보고되어 왔다. 금번 본 교실에서는 정상 청각기능을 가진 의과대 남학생 100명을 대상으로 일련의 특수청각검사를 실시하여 다음과 같은 기준치를 얻었기에 보고하는 바이다. 1) 언어판별치(PB score)는 전부 90%이상이었고 95%의 예가 우측귀에서는 94%이상이었고 좌측귀에서는 92%이상이었다. 2) M.C.L.(most comfortable level)은 우측귀가 45
The objective of this research is to test the feasibility of developing a statewide truck traffic forecasting methodology for Wisconsin by using Origin-Destination surveys, traffic counts, classification counts, and other data that are routinely collected by the Wisconsin Department of Transportation (WisDOT). Development of a feasible model will permit estimation of future truck traffic for every major link in the network. This will provide the basis for improved estimation of future pavement deterioration. Pavement damage rises exponentially as axle weight increases, and trucks are responsible for most of the traffic-induced damage to pavement. Consequently, forecasts of truck traffic are critical to pavement management systems. The pavement Management Decision Supporting System (PMDSS) prepared by WisDOT in May 1990 combines pavement inventory and performance data with a knowledge base consisting of rules for evaluation, problem identification and rehabilitation recommendation. Without a r.easonable truck traffic forecasting methodology, PMDSS is not able to project pavement performance trends in order to make assessment and recommendations in the future years. However, none of WisDOT's existing forecasting methodologies has been designed specifically for predicting truck movements on a statewide highway network. For this research, the Origin-Destination survey data avaiiable from WisDOT, including two stateline areas, one county, and five cities, are analyzed and the zone-to'||'&'||'not;zone truck trip tables are developed. The resulting Origin-Destination Trip Length Frequency (00 TLF) distributions by trip type are applied to the Gravity Model (GM) for comparison with comparable TLFs from the GM. The gravity model is calibrated to obtain friction factor curves for the three trip types, Internal-Internal (I-I), Internal-External (I-E), and External-External (E-E). ~oth "macro-scale" calibration and "micro-scale" calibration are performed. The comparison of the statewide GM TLF with the 00 TLF for the macro-scale calibration does not provide suitable results because the available 00 survey data do not represent an unbiased sample of statewide truck trips. For the "micro-scale" calibration, "partial" GM trip tables that correspond to the 00 survey trip tables are extracted from the full statewide GM trip table. These "partial" GM trip tables are then merged and a partial GM TLF is created. The GM friction factor curves are adjusted until the partial GM TLF matches the 00 TLF. Three friction factor curves, one for each trip type, resulting from the micro-scale calibration produce a reasonable GM truck trip model. A key methodological issue for GM. calibration involves the use of multiple friction factor curves versus a single friction factor curve for each trip type in order to estimate truck trips with reasonable accuracy. A single friction factor curve for each of the three trip types was found to reproduce the 00 TLFs from the calibration data base. Given the very limited trip generation data available for this research, additional refinement of the gravity model using multiple mction factor curves for each trip type was not warranted. In the traditional urban transportation planning studies, the zonal trip productions and attractions and region-wide OD TLFs are available. However, for this research, the information available for the development .of the GM model is limited to Ground Counts (GC) and a limited set ofOD TLFs. The GM is calibrated using the limited OD data, but the OD data are not adequate to obtain good estimates of truck trip productions and attractions .. Consequently, zonal productions and attractions are estimated using zonal population as a first approximation. Then, Selected Link based (SELINK) analyses are used to adjust the productions and attractions and possibly recalibrate the GM. The SELINK adjustment process involves identifying the origins and destinations of all truck trips that are assigned to a specified "selected link" as the result of a standard traffic assignment. A link adjustment factor is computed as the ratio of the actual volume for the link (ground count) to the total assigned volume. This link adjustment factor is then applied to all of the origin and destination zones of the trips using that "selected link". Selected link based analyses are conducted by using both 16 selected links and 32 selected links. The result of SELINK analysis by u~ing 32 selected links provides the least %RMSE in the screenline volume analysis. In addition, the stability of the GM truck estimating model is preserved by using 32 selected links with three SELINK adjustments, that is, the GM remains calibrated despite substantial changes in the input productions and attractions. The coverage of zones provided by 32 selected links is satisfactory. Increasing the number of repetitions beyond four is not reasonable because the stability of GM model in reproducing the OD TLF reaches its limits. The total volume of truck traffic captured by 32 selected links is 107% of total trip productions. But more importantly, ~ELINK adjustment factors for all of the zones can be computed. Evaluation of the travel demand model resulting from the SELINK adjustments is conducted by using screenline volume analysis, functional class and route specific volume analysis, area specific volume analysis, production and attraction analysis, and Vehicle Miles of Travel (VMT) analysis. Screenline volume analysis by using four screenlines with 28 check points are used for evaluation of the adequacy of the overall model. The total trucks crossing the screenlines are compared to the ground count totals. L V/GC ratios of 0.958 by using 32 selected links and 1.001 by using 16 selected links are obtained. The %RM:SE for the four screenlines is inversely proportional to the average ground count totals by screenline .. The magnitude of %RM:SE for the four screenlines resulting from the fourth and last GM run by using 32 and 16 selected links is 22% and 31 % respectively. These results are similar to the overall %RMSE achieved for the 32 and 16 selected links themselves of 19% and 33% respectively. This implies that the SELINICanalysis results are reasonable for all sections of the state.Functional class and route specific volume analysis is possible by using the available 154 classification count check points. The truck traffic crossing the Interstate highways (ISH) with 37 check points, the US highways (USH) with 50 check points, and the State highways (STH) with 67 check points is compared to the actual ground count totals. The magnitude of the overall link volume to ground count ratio by route does not provide any specific pattern of over or underestimate. However, the %R11SE for the ISH shows the least value while that for the STH shows the largest value. This pattern is consistent with the screenline analysis and the overall relationship between %RMSE and ground count volume groups. Area specific volume analysis provides another broad statewide measure of the performance of the overall model. The truck traffic in the North area with 26 check points, the West area with 36 check points, the East area with 29 check points, and the South area with 64 check points are compared to the actual ground count totals. The four areas show similar results. No specific patterns in the L V/GC ratio by area are found. In addition, the %RMSE is computed for each of the four areas. The %RMSEs for the North, West, East, and South areas are 92%, 49%, 27%, and 35% respectively, whereas, the average ground counts are 481, 1383, 1532, and 3154 respectively. As for the screenline and volume range analyses, the %RMSE is inversely related to average link volume. 'The SELINK adjustments of productions and attractions resulted in a very substantial reduction in the total in-state zonal productions and attractions. The initial in-state zonal trip generation model can now be revised with a new trip production's trip rate (total adjusted productions/total population) and a new trip attraction's trip rate. Revised zonal production and attraction adjustment factors can then be developed that only reflect the impact of the SELINK adjustments that cause mcreases or , decreases from the revised zonal estimate of productions and attractions. Analysis of the revised production adjustment factors is conducted by plotting the factors on the state map. The east area of the state including the counties of Brown, Outagamie, Shawano, Wmnebago, Fond du Lac, Marathon shows comparatively large values of the revised adjustment factors. Overall, both small and large values of the revised adjustment factors are scattered around Wisconsin. This suggests that more independent variables beyond just 226; population are needed for the development of the heavy truck trip generation model. More independent variables including zonal employment data (office employees and manufacturing employees) by industry type, zonal private trucks 226; owned and zonal income data which are not available currently should be considered. A plot of frequency distribution of the in-state zones as a function of the revised production and attraction adjustment factors shows the overall " adjustment resulting from the SELINK analysis process. Overall, the revised SELINK adjustments show that the productions for many zones are reduced by, a factor of 0.5 to 0.8 while the productions for ~ relatively few zones are increased by factors from 1.1 to 4 with most of the factors in the 3.0 range. No obvious explanation for the frequency distribution could be found. The revised SELINK adjustments overall appear to be reasonable. The heavy truck VMT analysis is conducted by comparing the 1990 heavy truck VMT that is forecasted by the GM truck forecasting model, 2.975 billions, with the WisDOT computed data. This gives an estimate that is 18.3% less than the WisDOT computation of 3.642 billions of VMT. The WisDOT estimates are based on the sampling the link volumes for USH, 8TH, and CTH. This implies potential error in sampling the average link volume. The WisDOT estimate of heavy truck VMT cannot be tabulated by the three trip types, I-I, I-E ('||'&'||'pound;-I), and E-E. In contrast, the GM forecasting model shows that the proportion ofE-E VMT out of total VMT is 21.24%. In addition, tabulation of heavy truck VMT by route functional class shows that the proportion of truck traffic traversing the freeways and expressways is 76.5%. Only 14.1% of total freeway truck traffic is I-I trips, while 80% of total collector truck traffic is I-I trips. This implies that freeways are traversed mainly by I-E and E-E truck traffic while collectors are used mainly by I-I truck traffic. Other tabulations such as average heavy truck speed by trip type, average travel distance by trip type and the VMT distribution by trip type, route functional class and travel speed are useful information for highway planners to understand the characteristics of statewide heavy truck trip patternS. Heavy truck volumes for the target year 2010 are forecasted by using the GM truck forecasting model. Four scenarios are used. Fo~ better forecasting, ground count- based segment adjustment factors are developed and applied. ISH 90 '||'&'||' 94 and USH 41 are used as example routes. The forecasting results by using the ground count-based segment adjustment factors are satisfactory for long range planning purposes, but additional ground counts would be useful for USH 41. Sensitivity analysis provides estimates of the impacts of the alternative growth rates including information about changes in the trip types using key routes. The network'||'&'||'not;based GMcan easily model scenarios with different rates of growth in rural versus . . urban areas, small versus large cities, and in-state zones versus external stations. cities, and in-state zones versus external stations.
The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70
