Generally, Bibliotherapy is aimed to cure the emotional or psychological wounds by using books for common users as self-treatment. But, Bibliotherapy for disabled people is carried out by alternative formats instead of a book. In library for the disabled, it is developing various bibliotherapy programs, but not yet activated. In order to carry out bibliotherapy, it is necessary to improve the bibliotherapy catalog preferentially. However, most Bibliotherapy catalogs are created for the common users, not for disabled people specially. How to make the special catalog for disabled people and catalog of alternative formats according to pathological symptom or situation is still a big problem. Basing on this premise, the author examined the necessity of bibliotherapy catalog for disabled people, and the present situation of alternative materials for the bibliotherapy. And then, the author proposes plans of bibliotherapy for how to build the bibliotherapy for disabled people.

The purpose of this study was to identify the perception of alternative medicine and factors affecting the intention to use for nurses and caregiver. In order to achieve the purpose of the study, this study was conducted to examine the awareness of alternative medicine, use experience, and intention to use for 400 workers in hospitals and nursing homes. The results of the analysis are summarized as follows. Recognition of alternative medicine effected on reliability of alternative medicine, awareness and use experience. In addition, experience with alternative medicine has a negative impact on the intention to use alternative medicine. In conclusion, this study suggested that the development of alternative medicine requires efforts to improve the therapeutic effect of alternative medicine.

제1형 고셔병(Gaucher disease type 1)은 리소좀 효소인 산성 ${\beta}$-글루코시다아제(acid ${\beta}-glucosidase$)의 결핍으로 인한 리소솜 축적 질환이다. 효소 활성도가 감소되어 기질이 축적되어, 간비종대, 빈혈, 혈소판감소증 및 골질환을 포함한 전신 증상이 발생한다. 재조합 효소 단백을 정맥 주입하는 효소 대체요법(Enzyme replacement therapy)는 지난 20년 넘게 고셔병의 표준 치료법이었다. 그러나 성공적인 효소 대체요법에도 불구하고, 심각한 폐증상과 골격 증상 등 고셔병 치료에 여전히 해결되지 않는 문제들이 남아 있다. 기질 감소 치료(Substrate reduction therapy)는 기질의 생합성을 억제하여 축적을 감소시킨다. 최근 새로운 경구용 기질감소 치료제인 엘리글루스타트(eliglustat)가 적합한 CYP2D6 대사 표현형을 가진 고셔병 성인 환자를 위한 1차 치료제로 미국과 유럽에서 승인되었다. 엘리글루스타트가 아직 한국에서는 쓰이지 않고 있지만, 본 종설에서는 문헌 검토를 통해 고셔병의 새로운 치료로서의 효소 대체요법을 소개하고자 한다. 아직 확고한 결론을 도출하기에는 연구 결과가 제한적이기는 하지만, 현재까지의 데이터에 따르면 엘리글루스타트는 임상 효능에 있어서 효소 보충 요법에 비열등성을 보인다. 장기 결과에 대한 추가 연구가 필요하지만, 엘리글루스타트의 승인은 해당 1형 고셔병 성인 환자들에게 경구 치료제라는 새로운 선택을 가능하게 하였다. 향후 국내에서 엘리글루스타트가 처방 가능해 지면, 각 환자 마다 철저한 평가를 통해 치료법을 선택할 수 있도록 해야 할 것이다. 나아가, 국내 1형 고셔병 환자들을 위해 엘리글루스타트의 사용에 관한 임상적 지침 또한 조만간 개발될 필요가 있다.

여기서는 보완대체의학의 임상효능을 평가하기 위한 일반적인 연구원칙들을 제시하였다. 어떻게 우리가 보완대체의학을 평가할 것인가? 그것이 정말 통상의학과 다른가? 우리는 보완대체의학을 통상의학과 비교하여 우리의 연구 질문들을 다르게 제시해야만 하는가? 보완대체의학은 안전하고 경제적인가? 그리고 우리가 이러한 전문적인 이슈들을 어떻게 제시할 것인가? 보완대체의학 연구는 관심을 가진 임상가들에게 지적인 도전을 제시 한다. 과학적으로 평가하는 것은 가능하긴 하지만 어렵다. 특정 약물치료를 통한 무작위대조 임상시험들은 침술, 동종요법, 수기요법과 같은 유사한 분야의 학문들보다 계획하고 투여하는 것이 훨씬 쉽다. 보완대체의학 분야에서 임상연구의 전술과 전략들은 개념상의 연속되는 도전들을 제시할 것이다.

목표 : 본 연구는 인천 지역에 사는 24~60개월 영유아를 가진 부모들의 보완대체요법에 대한 인식을 조사한 연구로, 보완대체요법 활용을 통한 한방과의 연계에 대한 가능성을 타진하기 위해 연구되었다. 방법 : 박지현(2007)이 사용한 질문지를 본 연구에 맞게 수정 보완하여 사용하였다. 수집한 자료는 SPSS 12를 사용하여 분석하였다. 결과 : 1) 영유아를 둔 한국의 모든 어머니는 자녀의 건강에 관심이 많았으며, 올바른 생활 습관을 갖게 하는 것이 가장 중요하다고 생각하였다. 2) 실제 80% 정도의 부모가 자녀에게 보완대체요법을 제공하였으며, 주로 예방차원에서 제공하기 때문에 질병이 발생하기 전에 사용을 하였다. 3) 보완대체요법은 질병이 발생했을 때 사용을 하지만, 보완대체요법의 사용은 주로 병원치료와 병행하는 경우가 많았다. 4) 직접 보완대체요법을 사용한 부모들의 80% 정도는 긍정적인 반응을 보였고, 이들 중 81% 정도는 다시 사용을 하겠다는 반응을 보였다. 결론 : 영유아 부모들은 건강 증진을 위한 보완대체의학에 대해 긍정적으로 인식하고 있었다. 이러한 결과는 한의학 영역에서 영유아 부모들에게 자녀의 건강을 효과적으로 증진시킬 수 있는 전문적인 의료지식뿐만 아니라 이들이 한의학에 접근할 수 있는 다양한 방법을 고안할 필요성을 제시하고 있다.

고령화 시대에 있어서 노인복지시설을 이용하는 많은 노인들이 본인의 의지와 관계없이 일반적으로 경험하게 되는 건강관리 관련 프로그램은 대체의학과 밀접한 관계를 가지고 있다. 따라서 대체의학을 노인관련 의료복지에 적용함으로써 노인성질환에 대한 예방과 치료에 활용하고 치료를 통하여 노인의료복지의 수준을 제고하는데 노력을 할 필요성이 있다. 본 논문은 노인들의 일상생활에서의 대체의학의 요법을 통해서 포괄적인 건강증진을 위한 실천적 방안을 제시하고자 하였다.

This study is a descriptive research study to analyze knowledge and attitude about CAM and aromatherapy for health department students in H8 university health department students(528 students) in Chungcheongdo-do. The results of this study showed that the treatment methods and therapeutic advantages of CAM showed a positive attitude and the recognition of the therapeutic effect on aromatherapy was somewhat higher. However, the knowledge level of aromatherapy was rather low. The students of the department of health science showed a relatively high awareness of the need for education in CAM, but aromatherapy showed a relatively low perception. Therefore, in order to reduce patient welfare and symptom in health department, it is necessary to develop knowledge training for knowledge application and effective treatment and development of clinical practice program for various treatment methods.

목 적 : 흡연에 따른 건강적 피해가 많이 알려져 있음에도 불구하고 특히 여성 및 청소년 계층의 흡연이 증가하고 있으며 폐암 사망자수는 급격히 늘어나고 있는 상황이다. 완전한 금연이 실패하는 주요 이유는 담배의 중독성에서 비롯되며 금단현상을 극복하지 못해 일어나는 것으로 알려져 있다. 흡연의 중독성은 담배 주요성분 중의 하나인 니코틴(nicotine)에 의해 유발되며 따라서 모든 금연 요법 및 금연 치료보조제 들은 이 니코틴 작용을 어떻게 효과적으로 억제 또는 대체하느냐에 초점을 맞추고 있다. 최근 금연치료 요법으로 한방 침이 주목 받고 있으며 본 논문을 통해 한방 침의 금연효과에 대한 신경학적 기전을 고찰하고자 한다. 방 법 : 금연을 돕는 전형적인 보조 치료제 및 치료법이 몇 종류 개발되어 사용 중에 있으며 대표적으로 니코틴 대체 요법(nicotine replacement therapy, NRT)이나 항우울제로 사용되는 bupropion 등을 들 수 있다. 이 치료 방법들은 뇌의 도파민계 신경전달 체계에 영향을 미쳐 금연 효과를 발휘하게 되는데 실질적인 금연성공확률은 그리 높지 않은 것으로 알려져 있다 따라서 침의 자극에 대한 도파민계 신경전달 조절효과를 중심으로 고찰함으로써 침의 금연효과에 대한 의과학적 기전을 설명하고자 하였다. 결 과 : 침자극은 우수한 금연효과를 가져올 수 있는 치료법으로 기존의 금연 치료요법 및 치료제 들을 보완할 수 있는 보다 확실한 치료요법 중의 하나이며 특히 금연 후에 오는 금단현상을 효과적으로 완화시키는 작용을 한다. 그리고 이 같은 효과는 부분적으로 도파민계를 비롯한 신경전달계를 조절함으로써 가능한 것으로 판단된다. 결 론 : 본 논문을 통해 니코틴에 의한 금단현상의 신경학적 기전과 금연과 관련된 신경전달체계에 대한 침자극의 효능에 대해 고찰하였으며 기존의 금연보조치료법을 대체할 수 있는 우수한 의학적 치료법으로써의 침치료법을 제시하였다.

The purpose of this study was to examine the correlation between the effectiveness of SUKI alternative therapy and scalp massage, and to contribute to the application and activation of hair SUKI alternative therapy. The research method was first to examine the understanding of SUKI alternative therapy through the concept of SUKI alternative therapy, the characteristics of SUKI alternative therapy, and points to be aware of during SUKI alternative therapy. The correlation between SUKI alternative therapy and scalp massage was investigated by reviewing research and others. As a result, SUKI replacement therapy had significant effects such as stress, depression, high blood pressure, muscle pain, skin wrinkles and elasticity. has been shown to affect treatment. Therefore, it is expected that SUKI replacement therapy can be applied to hair SUKI replacement therapy to improve the health and quality of life of modern people.
