The Chemopreventive Effect of Heat-treated Radish Complex Extracts on Rat Colonic Aberrant Crypt Foci Induced by 1,2-Dimethylhydrazine (1,2-Dimethylhydrazine에 의해 유발된 Colonic Aberrant Foci에 대한 초임계 열처리된 무복합추출물의 대장암 효과)

  • The present study was designed to investigate the chemopreventive effects of supercritical heat radish extracts with anti-constipation acitivity in loperamide-treated rats, on 1,2-dimethylhydrazine(DMH)-induced aberrant crypt foci(ACF) in rat colonic mucosa. Supercritical heat radish extracts was administered in the drinking water(3.2 mg/ml) during the initiation (for 12 weeks) or postinitiation phase(for 9 weeks) of DMH-induced carcicogenesis. Supercritical heat radish extracts coupled with DMH for 12 weeks decreased significantly total number of aberrant crypts(pre-neoplastic lesions) and the number of ACF to 35% and 30%, respectively. The number of ACF with more than four aberrant crypts (crypt multiplicity) was significantly reduced in the colon of rats receiving Supercritical heat radish extracts for 12 weeks. In the group receiving Supercritical heat radish extracts after post-initiation for 9 weeks, only the number of ACF with ${\geq}4$ abberant crypts significantly decreased. Supercritical heat radish extracts, given alone for 12 weeks, did not induced the development of ACF. These results indicate that Supercritical heat radish extracts possess chemopreventive effects on chemically induced rat colon carcinogenesis.

Transverse Colon Cancer with Duodenal Fistula (십이지장루를 동반한 횡행결장암 1예)

  • A 57-year-old male visited our hospital due to a growing abdominal mass for 1 month. The patient was diagnosed as transverse colon cancer with duodenal fistula, and then was treated with neoadjuvant concurrent chemoradiation therapy (2 cycles of FOLFOX-4, 3-dimensional conformal radiation therapy: 3,000 cGy in 10 fractions). Despite the improvement of colon cancer and associated inflammation, the symptom of colonic obstruction was aggravated. Thus transverse colon segmentectomy was done. After surgery, he have received adjuvant 12 cycles of FOLFOX-4 chemotherapy. Now, he is currently being followed up in cure state.

  • A clinical and statistical study was done for 123 patients with histologically proven colorectal malignant tumor from 1983 to 1986 at the department of anatomical pathology. Yeung Nam University Hospital. The results were as follows: 1. Ratio between male and female was 1.6:1 and incidence was most prevalent in 7th decades comprising 29.3%. 2. Location of tumor was the most frequent in rectum(65%). 3. Frequent symptoms and signs in case of right colon were pain, abdominal mass and bowel habit change. In left colon, they were pain, bowel habit change and bloody stool or melena. In rectum, they were bloody stool or melena, bowel habit change and pain. 4. Duration of symptom was 1 to 3 months(33.3%). 5. The most frequent histological type was adenocarcinoma(82.9%). 6. According to Dukes's classification, 32.9% of the tumor were stage $C_2$. 7. Operative procedures were Mile's operation(47.0%), right hemicolectomy(19.8%) and lower anterior resection(11.7%). 8. Polyps were the most frequent associated disease. 9. The most common complication was wound infection(11.1%).

Anticancer Effect of Ascorbic Acid and Saengshik on CT-26 Colon Cancer (CT-26 결장암에 대한 비타민 C와 생식의 항암효과)

  • Uncooked powered diet (Saengshik) composed of grains, vegetables, mushrooms and fruits have various physiological functions including strong antioxidant and potent anticancer effects by many kinds of bioactive phytochemicals. The objective of present study was to identify the anticancer effects of vitamin C and saengshik on colon cancer induced by CT-26 cell line in BALB/c mice. As the result, the tumor volumes of vitamin C-mixed diet group (VC) showed no significant differences compared with control group (C) after subcutaneous injection of CT-26 cell lines. However saengshik group (S) showed a significant effect, inhibiting the growth of cancer by 56.2% ($4.8{\pm}9.0\;mm^3$), 48.1% ($80.8{\pm}60.0\;mm^3$), 43.2% ($135.2{\pm}117.2\;mm^3$), 55.5% ($233.6{\pm}248.2\;mm^3$), 69.2% ($304.6{\pm}442.5\;mm^3$) and 70.7% ($464.9{\pm}705.9\;mm^3$) respectively as compared with C group at an interval of 5 days after injection of the CT-26 cells into mice. Also the final tumor volume of S group exerted a significant differences as compared with one of C group (p<0.05). Especially in the case of S group (n=10), the tumors in 2 of 10 mice entirely disappeared at 25th day. Our results suggest that saengshik possess a strong inhibitory action against tumor growth induced by CT-26 colon cancer cell line in the mice. Further studies of saengshik are required to confirm the cancer prevention effect and possibility of adjuvant cancer therapy.

Hospice and Palliative Care for the Terminal Patients with Colorectal Cancer (말기 대장직장암 환자의 호스피스 완화의료)

  • Purpose: Colorectal ranter is the 4th leading cause of cancer death in Korea and the prevalence is increasing continuously. This study was aimed to figure out the problems through the clinical consideration about terminal colorectal ranter patients who had died in hospice unit. Methods: We retrospectively reviewed the medical records in 78 patients with colorectal ranter who had admitted, received palliative care, and died in a hospice unit between April 2003 and November 2006. Results: The median age of patients was 59.6 years with 45 men (58%) and 24 women (42%). The median survival in hospice and palliative care was 36 days. The median hospitalization was 22 days. The most prevalent reason for admission was pain (38 patients, 49%), and the most common symptom was also pain (70 patients, 90%). Forty eight patients (62%) took analgesics before hospice referral. Twenty seven patients (65%) of 45 patients with intestinal obstruction have been performed palliative procedures. Median survival of patients with palliative procedure was higher than that of no palliative procedure group (47 days vs 19 days, P-value=0.005). Conclusion: The duration of hospice and palliative care was not enough to care the terminal colorectal cancer. Therefore, we suggest that proper education and information should be provided to physician, patients and their family members for effective hospice and palliative care.

  • In der vorliegenden Arbeit geht es um die vergleichende Betrachtung von deutschen Urteilen OLG Munchen, Urt. v. 21. 4. 2011 - Az. 1 U 2363/10; BGH, Urt. v. 22. 5. 2012 - VI ZR 157/11) und einer Reihe von koreanischen Urteilen im Bereich des Arzthaftungsrechts. Sie behandelt die Kausalitat von Tatbestand und Rechtswidrigkeitszusammenhang in der normativen Haftungszurechung. In Korea gilt die sog. Adaquanztheorie noch entscheidend als bewertendes Zurechnungskriterium - sogar manchmal als umgangssprachliches Homonym im Sinne der Verh$\ddot{a}$ltnism$\ddot{a}{\ss}$igkeit angewendet -, die dogmengeschichtlich von Deutschland $\ddot{o}$bernommen wurde. Doch wie aus den deutschen Urteilen ersichtlich, ist sie dort schon $\ddot{u}$berwunden. Die Ergebnisse der betrffenden koreanischen Urteile sind zwar nicht unbillig, deren Urteilsbegrundungen aber theoretisch bzw. praktisch nicht $\ddot{u}$berzeugend. Nach allgemeiner Ansicht kommt es vielmehr auf den Schutzzweck an, der auch bei der Anwendung des ${\S}$ 393 KBGB gelten kann. Schlie${\ss}$lich wurde die $\ddot{U}$bertragung des praxisgerechten L$\ddot{o}$sungsansatzes in deutschen Urteilen auf rechtsvergleichende Weise dazu beitragen, die Zivilrechtspraxis in der koreanischen Justiz nachvollziehbarer machen.

  • A components of garlic (Allium sativum) have anti-proliferative effects against various types of cancer. We aimed to investigate the capacity of garlic compounds to anti-tumor on a various cancer cell lines. Fractionation of garlic extract, guided by antiproliferative activity against human gastric cancer (AGS) cells, has resulted in the isolation of N-benzyl-N-methyldecan-1-amine (NBNMA). We investigated the effect of newly isolated NBNMA from garlic cloves on the inhibition of the growth of CT-26, AGS, HepG2, HCT-116, MCF7, B16F10, and Sarcoma-180 cells for in vitro and CT-26 colon carcinoma cells in vivo. NBNMA exhibited an antiproliferative effect in CT-26 cells by apoptotic cell death. NBNMA exhibited down-regulation of anti-apoptotic Bcl-2 proteins and up-regulation of apoptotic Bad protein expression in western blot analyses. In addition, NBNMA meagre activated caspase 3 and caspase 9, initiator caspases of the extrinsic and intrinsic pathways of apoptosis. NBNMA treatment at a dose of 10 mg/kg for 21 days in experimental mice implanted with tumors resulted in significant reduction of the tumor weight (43%). NBNMA exhibited both in vitro and in vivo anticancer activity. These results indicate that NBNMA has promising potential to become a novel anticancer agent from garlic cloves for the treatment of colon carcinoma cancer.

Bioactive Molecules Produced by Probiotics to Control Enteric Pathogens (프로바이오틱스가 생산하는 생리활성 물질의 장내 유해균 억제 효과)

  • There is a burgeoning number of products on the market that contain probiotics, but do they do you any good? What exactly are probiotics? They have been defined as living organisms that, when ingested in sufficient quantities, provide health benefits beyond basic nutrition. They are often referred to as "friendly bacteria" or "good bacteria." Probiotics have been claimed, amongst other things, to (i) reduce the incidence of colon cancer and other diseases of the colon, such as IBS, (ii) stimulate the immune system, (iii) have anti-hypertensive and anti-cholesterolemic properties, (iv) mitigate against the effect of antibiotics on the intestinal microbiota, and (v) protect against gastrointestinal infections. However, the scientific basis for many of these claims is not well-established. Indeed, the European Food Safety Authority has denied the use of several health claims associated with probiotics, particularly those related to mitigation of diarrhea following consumption of antibiotics. Thus, there is a need for research on the mechanisms of action of probiotics. We have been mainly interested in the use of probiotics to control enteric infections. There are several possible modes of action to explain how probiotics may protect the host from enteric pathogens, including competitive exclusion and immunomodulation. We have shown that probiotics produce bioactive molecules that interfere with bacterial cell-cell communication (also called quorum sensing), and this results in a down-regulation of virulence genes that are responsible for attachment of the pathogen to the gastrointestinal epithelium. These bioactive molecules act on a variety of bacteria, including enterohemorrhagic and enterotoxigenic Escherichia coli, Salmonella, Clostridium difficile and Clostridium perfringens, and there is evidence that they can inhibit the formation of biofilms by Listeria monocytogenes. These bioactive molecules, which are peptidic in nature, can exert their effects not only in vitro but also in vivo, and we have shown that they mitigate against E. coli O157:H7 and Salmonella in mice and Salmonella and E. coli K88 infections in pigs. They can be delivered in foods such as yoghurt and maintain their activity.

Use of Cellulose and Recent Research into Butyrate (섬유소의 이용과 butyrate의 최근 연구)

  • On earth, there are about 5,400 kinds of mammals, of which about 1,000 kinds are herbivores. Among herbivores, about 250 kinds are known to be ruminants. As for cattle and sheep, which are ruminants, fermentation takes places mainly in their rumen; in contrast, for pigs and men, which are non-ruminants, fermentation takes place mainly in their caecum, colon, and rectum. As for the kind and dominance of rumen microorganisms, Bacteroidetes account for 51% and Firmicutes for 43%. As for the dominance of the large intestine microorganisms in men, Firmicutes account for 65% and Bacteroidetes for 25%. Cell wall components are decomposed by microorganisms, and short chain fatty acids (SCFAs) are generated through fermentation; the ratio of acetate, propionate, and butyrate generate is 60:25:15. Butyrate absorbed through the primary butyrate transporter MCT1 (mono carboxylate transports-1) in the intestines activates such SCFA receptors as GPR43 and GPR41. Butyrate has a strong anti-tumorigenic function. Butyrate is characterized by the fact that it has an effect on many cancer cells, contributes to the coordination of functions in the cells, and induces cancer apoptosis. Butyrate activates caspase but inhibits the activity of HDAC (histone deacetylase), so as to induce apoptosis. In addition, it increases p53 expression, so as to induce cell cycle arrest and apoptosis. Anti-inflammation actions of SCFA include the reduction of IL-8 expression in intestinal epithelial cells, the inhibition of NO synthesis, and the restraint of the activity of NF-${\kappa}B$ (nuclear factor ${\kappa}B$), so as to suppress the occurrence of cancers caused by inflammation. Butyrate plays an important role in maintaining physiological functions of intestinal mucous membranes and is used as a cure for inflammatory bowel disease (IBD).

Comparison between FDG Uptake and Pathologic or Immunohistochemical Parametersin Pre-operative PET/CT Scan of Patient with Primary Colorectal Cancer (원발성 대장-결장암 환자의 치료 전 PET/CT 스캔에서 FDG 섭취 정도와 병리학적 및 면역조직화학적 지표들과의 비교)

  • Purpose: To evaluate the relationship between F-18 FDG uptake of tumor in PET/CT scan and pathological or immunohistochemial parameters of colorectal cancer. Materials and Methods: 147 colorectal cancer patients who underwent both pre-operative F-18 FDG PET/CT scan and surgery were included. In cases with perceptible FDG uptake in primary tumor, the maximum standardized uptake value (SUVmax) was calculated. The pathologic results such as site, size, depth of invasion (T stage), growth pattern, differentiation of primary tumor, lymph node metastasis and Dukes-Astler & Coller stage and immunohistochemical markers such as expression of EGFR, MLH1, MSH2 and Ki-67 index were reviewed. Results: 146 out of 147 PET/CT scans with colorectal cancer showed perceptible focal FDG uptake. SUVmax showed mild positive linear correlation with size of primary tumor (r=0.277, p=0.001) and Ki-67 index (r=0.226, p=0.019). No significant difference in F-18 FDG uptake was found according to site, depth of invasion (T stage), growth pattern, differentiation of primary tumor, presence of lymph node metastasis, Dukes-Astler & Coller stage and expression of EGFR. Conclusion: The degree of F-18 FDG uptake in colorectal cancer was associated with the size and the degree of Ki-67 index of primary tumor. It could be thought that FDG uptake of primary tumor has a correlation with macroscopic and microscopic tumor growth.
