The Patient Recognition, Acceptability and Evaluation of Feasibility for Day Surgery (종합병원 일부 입원환자의 당일수술에 대한 태도와 당일수술 적용 가능성 평가)

  • Objectives . This study was conducted in order to investigate the degree of recognition, acceptability, and altitude towards day surgery of patients who were hospitalized with diseases that were candidates for day surgery; in order to analyze the average length of stay for treatment of the ailments; and to analyze the percentage of patients who could be discharged on the same day after the surgery ,using the post-anesthesia discharge scoring system. Methods : Data was collected between February 1 and March 31, 1999 from 353 patients who received surgery for cataract, adenoid hypertrophy, inguinal hernia, strabismus, ptosis, cholelithiasis, hemorrhoid, or anal fistula, at a general hospital in Daegu city. The patients were interviewed and surveyed by a post-anesthesia discharge scoring system(PADS) in order to collect data on patient condition such as vital signs, activity and mental status, pain, nausea and vomiting, surgical bleeding, intake and output after the surgery. Results : Among the 353 patients, 52.7% were after of the day surgery and 52.7% were interested in day surgery. Of the respondents, 43.1% said 'my ailment was not serious and the surgery was simple' and 30.4% said 'according to my condition rest at home was desirable' as the reasons for wanting day surgery Alternatively, 56.5% of those declining day surgery said the 'uncertainty of staying home' was the reason. The greatest concern in discharging within 24 hours after surgery was a post-op emergency situation. On the other hand, the shortened hospitalization was the largest advantage of day surgery with 39.1% responding this way, followed by the savings in hospitalization costs (25.8%) and emotional stability (13.7%). The majority of those surveyed (47.6%) believed that discharge should be determined within 1-2 days after the surgery. The average hospital stay was 3.1 days for dischargeable ailments. Pain (45.6%), nausea and vomiting (10.5%), and headache (7.9%) were the common symptoms following surgery. The percentage of patients who were able to be discharged within 24 hours after surgery revealed 95.2% were dischargeable after approximately 3 hours, 99.2% dischargeable after 12 hours, and 100% dischargeable after 24 hours. Conclusions : According to the PADS score, the cataract extract and strabismus correction patients were eligible for day surgery and the further evaluation concerning the reason for delayed recovery of the other diseases is needed.

Medical Service Satisfaction on the Patients Utilizing a Day-Case Surgical Center in Korea (국내 당일수술센터 이용환자의 의료서비스 만족도분석)

  • 본 연구는 국내 당일수술센터 이용환자의 의료서비스 만족도와 관련된 요인들을 규명하기 위해 시도하였다. 설문조사는 2009년 1월 13일부터 2월 13일까지 한 종합병원에서 독립적으로 운영하는 당일 수술센터를 이용하는 환자들로부터 도출했다. 연구결과 당일수술환자는 수술과 입원경험은 적었고 본인의 건강 관심도는 높은 것으로 나타났다. 수술대기시간과 수술시간은 의료서비스 질에 유의한 차이가 있었다(P<0.05). 다중 로지스틱 회귀분석에서 당일수술환자의 인식도에 영향을 주는 것은 연령과 교육수준으로 나타났다(P<0.05). 따라서 당일수술률을 증가하기 위해서 당일수술의 이점에 대한 정보를 제공하고 수술전 교육이 필요하다.

Factors Influencing on Medical Service Satisfaction of Patients Utilizing a Day-Case Surgery Center in Korea (국내 당일수술센터 이용환자의 의료서비스 만족도에 영향을 미치는 요인)

  • This study was attempted to identify the factors relating to the medical service satisfaction on the patients utilizing a day-case surgery center in Korea. A questionnaire survey was conducted to patients were utilizing a day-surgery center, independently operated by one general hospital from January 13 to February 13, 2009. The results of this study can be summarized as follows. First, in the characteristic related with health and medical situations, day-surgery patients had little experience with surgery or hospitalizing and much interest in their health. Secondly, there were significant differences in regard to the quality of medical service for surgery waiting time and surgery time(p<0.05). Thirdly, multiple regression analysis revealed that age and level of education affected patients perception of day surgery(p<0.05). In conclusion, in order to increase the rate of day-surgery, it is necessary to give information about the advantages of day-surgery center and preoperative education.

Camparison between the 1 Day and the 2 Day Protocols of Lymphoscintigraphy and Sentinel Node Biopsy using Subareolar Injection in Breast Cancer Patients: A Retrospective Study (유륜하 주사에 의한 유방암 환자의 전초림프절 스캔과 전초림프절 생검에 있어서 당일검사와 전날검사의 비교: 후향적 연구)

  • Purpose: Lymphoscintigraphy and sentinel node biopsy are used in detection of axillary lymph node metastasis in breast cancer patients, but standardized technique is not established. We compared the results of the injection the morning of surgery (1 day protocol) with the subareolar injection the day before surgery (2 day protocol) with the subareolar injection in patients with breast cancer having lymphoscintigraphy and sentinel node biopsy. Materials and Methods: This study included 349 patients who underwent the breast cancer operation during 2001-2004. One hundred seventy one patients (1 day protocol, 1 hour) was injected 0.8ml of Tc-99m Tin-Colloid (37 MBq) by subareolar injection on the morning of surgery. One hundred seventy eight patients (2 day protocol, 16 hour) was injected 0.8 ml of T c-99m Tin-Colloid (185 MBq) on the afternoon before surgery. Lymphoscintigraphy was performed in sitting position and sentinel node localization was performed by hand-held gamma probe during operation. Result: In the 1 day protocol, 153 cases (89.5%) of the sentinel node were localized by lymphoscintigraphy and 150 cases (87.7%) were localized by gamma probe. In the 2 day protocol, 159 cases (89.3%) were localized by lymphoscintigraphy and 154 cases (86.5%) were localized by gamma probe. There was no significant difference in localization of sentinel node between the 1 day and the 2 day protocol by lymphoscintigraphy and gamma probe (p>0.05, p>0.05). Conclusion: There was no difference the result of localization of sentinel node with subareolar injection between the 1 day and the 2 day protocol in breast cancer patients. Because the 2 day protocol allows the enough time of performing lymphoscintigraphy, it is more useful in localization of sentinel node in breast cancer patients.

  • 전통적으로 수술 전 평가는 수술 전날 밤에 시행되는 것이 일반적이었다. 그러나 외래수술을 받는 환자의 증가로 인하여 많은 환자들이 수술 당일 아침에 마취과 의사를 처음 만나게 되는 경우가 많고 심지어는 심각한 내과적 문제를 가진 환자들도 충분한 수술 전 평가가 이루어지지 않은 사태에서 수술실로 들어오게 되는 경우가 있다. 그러나 이러한 상황에서도 마취과의사는 주술기(perioperative)의 환자 상태를 관리하는 주된 역할자로서 수술 전 환자에 대한 평가를 충실히 해야 한다. 수술 전 평가는 환자의 현재 상태를 평가하는 것이고, 수술 중 발생할 수도 있는 상황에 대비하며, 수술실을 넘어서 중환자실, 통증치료실에서의 마취의사의 역할로 이어지도록 하는 것이다. 최근 발전하고 있는 수술 전 평가 클리닉은 환자의 수술준비의 기초적인 정보를 제공한다. 이는 마취의사에게는 상당한 장점과 편리성을 제고하는 것이고, 환자 관리의 질적 향상과 급변하는 병원관리정책의 요구에 부합하는 것이다.

The Effects of Early First Oral Water Intake on Thirsty Feeling, Nausea, and Vomiting in Child under Ambulatory Surgery (당일수술 후 최초 경구수분섭취 시간단축이 소아에서의 갈증감소 및 오심, 구토 발생에 미치는 영향)

  • Purpose : The purpose of this study was to find effects of early oral water intake on thirsty feeling, nausea, and vomiting after ambulatory surgery. Method : Sixty patients who received PET(Pressure Equalizing Tube) insertion surgery under general anesthesia were conveniently sampled and divided into two groups. Experimental group was instructed to drink water after two hours and control group was directed to intake water after three hours postoperatively. Data were collected from May 1, 2002 to August 31, 2002 at ambulatory surgery center of Asan Medical Center. The thirsty feeling, nausea and vomiting score between experimental and control group were compared at the time of recovery, recovering consciousness, 2hr, 3hr, right before discharge and/or 24hr(telephone interview) postoperatively. The collected data were analyzed by SPSS 8.0 statistic program. Result : Result of this study are as follows. 1) There was a statistically significant difference(t=-10.31, p=0.00) in the occurrence of thirsty feeling at the 3hr postoperatively between experimental and control groups. 2) There was a statistically significant difference($x^2=6.66$, p=0.02) in the incidence of nausea during the 2hr postoperatively between experimental and control groups. 3) There was no difference($x^2=0.35$, p=0.55) between groups in vomiting scores. Conclusion : It was generally known that the delay of oral water intake decreases post operative complications. But, this study shows that early oral water intake decreases discomfort of thirsty and there was no adverse effects on the occurrence of postoperative nausea and vomiting.

Effects of Surgery Information Service on One-Day Surgery Patients' Anxiety and Satisfaction with Nursing Care (간호정보 제공이 당일수술 환자의 불안과 간호만족도에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of surgery information service on one-day surgery patients' anxiety and satisfaction with nursing care. Methods: The study used the nonequivalent control group time difference design. Sampling and measurement of the control group participants (n=30) was completed first. Later, participants in the experimental group (n=30) were sampled, intervened, and measured. The experimental group participants received the surgery information services twice: before and after the surgery, 20~30 minutes for each of the sessions. The anxiety was measured with the State-Trait Anxiety Inventory developed by Spielberger (1975), and modified by Kim & Shin (1978). The patient satisfaction with nursing care was measured with the Patient Satisfaction with Nursing Care Scale developed by La Monica and colleagues (1986), and modified by Shin (1999). The data was collected between February 1 and May 30, 2006. Results: The results were as follows: 1) The level of anxiety in the experimental group was not significantly different from that in the control group. 2) The level of patient satisfaction with nursing care in the experimental group was significantly higher than in the control group (t=-4.53, p=.00). Conclusion: These findings suggested that the one-day vocal cord surgery information service could be a useful nursing intervention to improve patient satisfaction with nursing care, but not for controlling the anxiety of one-day surgery patients.
