Relationship of sodium consumption with obesity in Korean adults based on Korea National Health and Nutrition Examination Survey 2010~2014 (2010년~2014년 국민건강영양조사 자료를 이용한 성인의 나트륨 섭취와 비만과의 관련성)

  • Purpose: Excess sodium intake may contribute to the etiology of hypertension and cardiovascular disease risk. World Health Organization (WHO) recommends a daily sodium intake of less than 2 g. The aim of this study was to estimate the association of sodium intake with obesity in Korean adults. Methods: This study used Dietary intake and Health data on 22,321 subjects aged 30 years and over from the Korea National Health and Nutrition Examination Survey (KNHANES) 2010~2014. Information on dietary intake was obtained by the one day 24-hour recall method in KNHANES, and sodium intake was classified into five groups (< 2,000 mg, 2,000~4,000 mg, 4,000~6,000 mg, 6,000~8,000 mg, ${\geq}8,000mg$). Obesity was defined as having a body mass index (BMI) higher than $25kg/m^2$. Intake of sodium and obesity status were analyzed by logistic regression with SPSS Statistics 23. Results: Men tended to have a higher sodium intake than women (p < 0.001). After adjusting for age, sex, year, daily energy intake, education level, smoking status, drinking status, physical activity, and chronic diseases and comparing the highest sodium intake group (${\geq}8,000mg$) with the lowest intake group (< 2,000 mg), the OR of obesity was 1.351 (95% CI: 1.032~1.767) in men. The OR of obesity in the sodium intake group (4,000~6,000 mg) was 1.232 (95% CI: 1.063~1.427) in women. Conclusion: Our findings suggest an independent relationship between sodium intake and as increased risk of obesity in Korean adults, implying the necessity for future research on low-sodium diet intervention in relation to obesity.

Comparison of the health and nutritional status of Korean elderly considering the household income level, using the 2018 Korea National Health and Nutrition Examination Survey (가구소득수준에 따른 남녀 노인의 건강 및 영양섭취 실태 비교: 2018년 국민건강영양조사 자료를 이용하여)

  • Purpose: This study examined the dietary behavior, health status and nutrient intake by considering the level of household income of elderly people using data obtained from the Korea National Health and Nutrition Survey (KNHANES VII) 2018. Methods: The study subjects were 1,355 elderly people over 65 years old (558 men, 797 women). Based on their household income, participants were classified into three groups: low-income, middle-income, and high-income. The variables consisted of general characteristics, dietary behavior, health status, health related behavior, and dietary intakes. Dietary data were estimated by the 24-hour dietary recall. Results: In men, the low-income group encompassed older, less educated, less employed, and living with family of first generation. However, in women, there were no differences in employment by the level of income, and women living alone had lower income than subjects living with family. Elderly men in the high-income group had a significantly higher level of nutrient intake (energy, protein, fat, phosphorous, riboflavin, niacin and vitamin C). Men in the low-income group consumed a significantly lower intake of fruits, seaweeds and eggs, including total food. Women in the low-income group had significantly less intake of protein, fat, calcium, phosphorous, iron, vitamin A, riboflavin, and niacin whereas women in the high-income group had significantly higher intake of sugar & sweet, eggs, and beverages, including total food. Conclusion: These results suggest that the level of household income is an important factor that influences food and nutrient intake in the Korean elderly. The socioeconomic status needs to be considered differently among elderly men and women when implementing food assistant programs and designing nutrition education programs.

Study on nutrition, dietary and health status of middle-aged Korean men according to sedentary hours: based on the 2019 Korea National Health and Nutrition Examination Survey (중장년 한국 남성의 좌식 시간에 따른 영양, 식이 및 건강행태 연구: 국민건강영양조사 제8기 1차년도(2019년) 자료를 이용하여)

  • Purpose: Adult Korean men belonging to the main economically active population are known to have long sedentary hours. This study was undertaken to determine the difference and relevance of sedentary hours on the nutrition, diet, and health status of adult men, and to suggest how to prevent health risk factors. Methods: Subjects (n = 1,068) were classified into 4 groups based on their sedentary hours, ranging from the first quartile (Q1) having the least hours spent sitting, to the fourth quartile (Q4) spending the longest hours. Results: Subjects belonging to Q4 had the lowest average age, the largest waist circumference, and the highest level of education. Among those engaged in economic activities, the ratio of white-collar workers was significantly higher in Q4. Accordingly, the rate of not doing high-intensity or moderate-intensity physical activity while working was also the highest in Q4. A significant difference was obtained in the drinking frequency between groups, but this was found to be associated with the average working hours rather than sedentary hours. The proportion of not doing aerobic exercise was higher with longer sitting hours. The highest diagnosis of diabetes (8.8%) was obtained in the Q4 group. Among the factors related to cardiovascular disease, only low density lipoprotein-cholesterol showed a significant difference, with Q4 being significantly higher than Q1. Considering energy and nutrient intake, vitamin B1 and calcium intake were the lowest in the group with the longest sitting hours, as well as the least consumption of vitamin C than the recommended estimated average requirement. Conclusion: The results of this study suggest that the health and nutritional status of Korean adult men are affected by sedentary hours. This should be recognized as a health risk factor and guidelines need to be developed for sedentary lifestyle management.

서울지역 중년층의 영양섭취실태와 생활습관

  • 노년기의 독립적인 생활유지와 삶의 질을 향상시킬 수 있는 바람직한 영양섭취 방법을 모색하기 위한 종적연구의 대상자로 자원한 서울지역의 건강한 40-64세의 중년 293명(남 121명, 여172명)을 대상으로 생활습관과 영양섭취실태를 조사한 결과는 다음과 같다. 대상자의 평균 연령은 남자 52.0$\pm$7.3세, 여자 519$\pm$6.9세였고, 대부분 부부 중심으로 조사하였다. 교육수준은 남자의 71.9%, 여자의 39.5%가 대학졸업 이상이었으며, 남자의 43.0%가 전문직, 24.8%가 행정ㆍ사무직이었고, 가계의 한 달 평균 수입이 200만원 이상자가 72.5%로써 본 조사의 자원자는 대체적으로 고학력, 전문직종의 중류층이었다. 건강에 대한 염려를 가끔 또는 자주 하는 대상자가 66.9%로 건강에 대한 관심은 높았으나 건강을 지키기 위한 실천은 낮은 것으로 나타났다. 그 예로 지난 1년간 체중이 증가하고 있는 대상자가 37.5%였고, 이들 중 75%가 2kg 이상의 체중이 늘고 있지만 체중을 줄이려고 노력하는 대상자는 37.5%뿐이었다. 음주는 남성의 56.5%가 평균 주 2회 정도 하는 편이었고, 1회 음주량은 소주로 1병 이하가 대부분(88%)이었다. 현재 흡연자는 남자의 40.5%였고, 금연자는 30.6%로 비교적 흡연율이 낮았다. 식사시간은 대부분(83.6%) 규칙적이었다. 식사를 주 2-3회 이상 거르는 대상자가 남자는 23%인 반면 여자는 49%였고, 이들 대부분(60.3%)이 아침을 먹지 않았다. 1주일에 1번 이상 외식하는 대상자는 남자의 84%, 여자의 66%였다. 식사시 식품의 배합에 대하여 별로 관심이 없다는 사람이 50%였지만 실제로 식품군별 섭취 시에는 상당한 관심이 있는 것으로 나타났다. 즉, 육류의 기름기를 대충 또는 거의 다 제거하고 먹는 대상자가 남자의 68%, 여자의 88%였고, 육류를 자주 먹지 않거나 소량씩 먹는 대상자가 94%, 생선.두부 및 콩제품을 하루 1끼 이상 섭취하는 대상자가 87%, 채소를 하루 1번 이상 섭취하는 대상자가 96%인 반면, 인스턴트식품이나 패스트푸드를 주 2-3회 이상 먹는 대상자는 23% 정도였다. 영양지식점수(15.4$\pm$3.1/25)와 영양태도점수(50.5$\pm$4.9/75)는 비교적 양호하였다. 전체적으로 오답률이 높은 문항은 체중조절에 있어서 식사와 운동의 관계, 당뇨병 환자의 흰쌀밥 섭취 문제, 비타민제의 섭취와 감기, 고단백 식사의 문제점 등에 관한 것이었다. 반면 철분 섭취와 빈혈의 관계, 비만의 위험성, 동물성 기름의 섭취와 혈중 콜레스테롤치, 우유가 Ca의 급원이면서 성인에게도 필요하다는 것 등은 잘 인식되고 있었다. 조사 대상자의 평균 에너지 섭취량은 RDA의 77.3 $\pm$ 13.6%였으나, 이들 대부분(80%)의 활동 정도가 가벼운 활동에 속하므로 에너지가 부족하다고 할 수는 없다. 반면 평균 단백질 섭취량은 RDA의 109.9 $\pm$ 23.5%로 충분하였고, 3대 열량영양소 즉, 단백질:지방:탄수화물의 구성비는 16:22:62로 지방의 섭취가 약간 높으나 바람직한 구성비를 보였다. 권장량의 75% 이하를 섭취하고 있는 영양소는 칼슘(70.7$\pm$23.9%)뿐이었다. 과체중자의 비율을 체질량지수(BMI) 25이상으로 구분하면 남자의 45%, 여자의 23%가 과체중자로 나타나나, 23이상으로 구분하면 남자의 71%, 여자의 52%가 과체중자로 분류되었다. 이를 WHR의 기준치로 분류하면 남자의 25%, 여자의 26%가 과체중자로 나타났다. 대부분 대상자의 hemoglobin치(99%), hematocrit치(90%), 혈장 albumin치(99.7%)는 양호하였다. 혈당이 140mg/㎗ 이상인 대상자는 2.7%에 불과하였다. 혈중 총콜레스테롤치가 240mg/㎗ 이상인 대상자는 19%, 중성지방치가 250mg/㎗ 이상인 대상자는 9%, HDL-콜레스테롤치가 35mg/㎗ 이하인 대상자는 2.4%로 혈중 지질 profile은 양호하였다. 이상의 결과로 보아 본 연구 대상자는 영양섭취상태와 생화학적 건강상태가 양호하다고 판단되므로 종적연구의 대상자로 적합하다고 사료된다.

Social-environment factors by region for cause of death of elderly people in Korea (노인의 사망원인에 대한 거주지역간 사회 환경요인분석)

  • Objectives. The purpose of this study is to analyze the social-environment factors by region for cause of death elderly people in Korea and to study the factors of longevity. Methods. The study included 16 regions with a total of 177,585 elderly peoples. The data in this study was collected from The National Statistical Office, Republic of Korea. Results. Those regions the highest cerebrovascular disease were Incheon County in that order. The correlation of social-environment by cause of death factors were divorce (+0.832), air pollution of Pb ; lead (+0.879), smoking (+0.895), fatness (+0.666), local tax revenue (+0.756), air pollution of SO2 (+0.602) and dirt road (+0.863). Conclusions. We should learn to live long and healthily from residence harmonized with natural environment. Longevity of elderly peoples is to be fostered for the promotion of health by control the social-environment factors.

Comparison of the health behavior and nutrition status of young-old women according to the vitality in their quality of life: based on the 2019, 2021 Korea National Health and Nutrition Examination Survey (전기노인 여성의 삶의 질 중 기운에 따른 건강행태와 영양상태 비교: 2019년, 2021년 국민건강영양조사 자료를 이용하여)

  • Purpose: This study aimed to identify the general characteristics, chronic diseases, health behavior, mental health, and nutritional status of young-old women based on their vitality. Methods: This study used data from the 2019 and 2021 Korea National Health and Nutrition Examination Survey (KNHANES). The subjects were 1,113 young-old women aged 65 to 74 years old. The health-related quality of life concept with an 8-item questionnaire was used to measure the quality of life. Subjects were categorized into 4 groups (always, often, sometimes, never) based on their vitality. General characteristics, chronic diseases, health behavior, dietary behavior, food intake, and nutrient intake were compared among the groups. Results: Age, education level, household income, employment, fruit intake, dietary supplements, abundance of food, and nutrition labeling recognition were associated with the vitality of the subjects. Young-old women with arthritis, diabetes, and osteoporosis displayed lower vitality. Moreover, subjective health status, exercise, activity restrictions, and average daily sitting hours were related to vitality, while no significant difference was found in vitality between smoking and drinking. In terms of mental health factors, higher vitality was associated with 6-8 hours of sleep, lower stress levels, and reduced depression. The high-vitality group exhibited a higher intake of potatoes, starch, mushrooms, fruits, meat, milk, animal oils, and beverages than the low-vitality group. Additionally, the group with higher levels of vitality showed a higher intake of protein, fat, saturated fatty acids, monounsaturated fatty acids, polyunsaturated fatty acids, n-6 fatty acids, dietary fiber, sugars, phosphorous, potassium, magnesium, iron, zinc, and riboflavin. Conclusion: This study suggests that the vitality of young-old women is related to socioeconomic factors, health behavior, mental health, and food intake. To maintain a vibrant lifestyle in elderly women, it is necessary to have social and economic stability, prevent arthritis, diabetes, and osteoporosis, exercise regularly, get sufficient sleep, maintain mental health, and have a balanced diet.

Factors associated with the bone mineral density in Korean adults: Data from the 2010-2011 Korean National Health and Nutrition Examination Survey (KNHANES) V (우리나라 성인남녀의 골밀도 관련요인: 국민건강영양조사 제 5기 1, 2차 년도(2010년, 2011년))

  • Objective: The purpose of this study was to identify the correlates of bone mineral density of Korean adults. Methods: This study was based on the data from the 2010-2011 Korean National Health and Nutrition Examination Survey (KNHANES, 2010~2011) V. Among 8,473 subjects of the survey, who were adults 19 years old or over with bone mineral density data, the current study excluded those whose data include a missing study variable or the response of 'I don't know', and finally analyzed the data of 5,986 subjects (2,692 males and 3,294 females). This study employed the following statistical methods of analysis: t-test, analysis of variance (ANOVA), and hierarchical multivariate linear regression analysis. Results: In multivariate analysis, in the case of men, the significant associated factors in bone mineral density were age, education level, body mass index (BMI), waist circumference, total muscle mass, exercise, and weight control; in the case of women, the significant associated factors in bone mineral density were age, education level, economic activity, BMI, total muscle mass, exercise, weight control. fat intake, uptake female hormone, menopause, and age of menarche. Conclusion: For both sexes, the strongest modifiable factor in influencing bone mineral density was total muscle mass. Therefore, to prevent osteoporosis and promote health, active health education and interventions such as regular exercise for total muscle mass increase should be implemented from an early growth period.

A Study on the Serum Concentrations of Glucose, Insulin and Lipid in Elementary School Children by Body Weight (체중에 따른 일부 초등학교 아동의 혈청 중 포도당, 인슐린 및 지질농도에 관한 연구)

  • The purpose of this study was to investigate anthropometric values, serum biochemical levels and their correlation in elementary school children. The subjects were 98 boys and girls in the $4{\~}6$ grade and divided into three groups by body size. The mean height of under-weight group (boys: $14.9\pm7.8\;cm$, girls: $144.9\pm9.6\;cm$) was the highest of the groups. The systolic and diastolic blood pressure of all the groups ranged from $111.56\pm13.51\;mmHg/72.86\pm7.26\;mmHg\;to\;117.50\pm10.00\;mmHg/75.31\pm9.51\;mmHg$, all within the normal levels. As subject was fatter, serum concentrations of triglyceride, total cholesterol, LDL-cholesterol insulin and atherogenic index (AI) in the boys, and serum concentrations of triglyceride, insulin and AI in the girls were significantly higher The concentrations of serum glucose, insulin, triglyceride, total cholesterol and LDL- cholesterol were significantly positively correlated with BMI and WHR. No significant correlation was found with blood pressure and serum lipid concentrations. Serum insulin levels was significantly positively correlated with serum glucose and triglyceride concentrations. Thus, this study implicates that obesity in childhood may be relevant to hyperlipidemia, atherosclerosis and diabetes mellitus.

Association between dietary protein intake and overweight and obesity among Korean children and adolescents: data from the 2014-2019 Korea National Health and Nutrition Examination Survey (한국 아동 및 청소년의 단백질 섭취와 과체중 및 비만과의 연관성: 2014-2019년 국민건강영양조사 자료를 활용하여)

  • Purpose: Proteins are major components of the body and essential nutrients for proper growth and development. However, studies on protein intake in children and adolescents are insufficient. A few previous studies have reported the relationship with growth indicators, but results vary depending on the source of protein. Therefore, the current study investigates the relationship between protein intake and overweight and obesity among children and adolescents in Korea. Methods: Based on the 2014-2019 Korea National Health and Nutrition Examination Survey, 5,567 children and adolescents aged 6-18 years, who participated in a 24-hour dietary recall with information on height and weight, were included in this study. Protein intake was estimated as percentage of total energy (% of energy) and was classified into animal and plant protein according to the food source. Overweight and obesity were defined using the 2017 pediatric and adolescent growth chart. Results: Total protein intake of the subjects was estimated as 14.5% of total energy (animal protein 8.3% and plant protein 6.3%). The group with the highest total protein intake had a higher odds ratio (OR) of overweight/obesity than those with the least protein intake (OR, 1.36, 95% confidence interval (CI), 1.10-1.67, p for trend = 0.003). When classified by food source, the group with the highest animal protein intake had a significantly higher OR of overweight/obesity than subjects with the lowest intake (OR, 1.30, 95% CI, 1.05-1.61, p for trend = 0.016). However, plant protein was not significantly associated with overweight/obesity. Conclusions: These findings suggest that a high intake of animal protein in children and adolescents increases the risk of being overweight and obese. In order to develop normal growth and prevent obesity in the future, it is necessary to determine an appropriate protein intake level through nutrition education programs and prospective studies on balanced protein intake.

Associations of Metabolic Syndrome with Glaucoma in Korean - Based on the Korean National Health and Nutrition Examination Survey 2005, 2007-9, 2010 (한국인의 대사증후군과 녹내장 간의 상관관계 -2005, 2007-9, 2010국민건강영양조사 이용)

  • Purpose: This study was conducted to assess the association of metabolic syndrome and its components with glaucoma. Methods: We investigated the associations of metabolic syndrome and its components (abdominal obesity, impaired fasting glucose, high blood pressure, and dyslipidemia) with glaucoma using data from 19,162 adults aged 40 or above among the Korean National Health and Nutrition Examination Survey III (2005), IV (2007-9), V (2010). The logistic regression analysis was used for assessing those associations after adjusting demographic, lifestyle, and social economic status and for assessing the association of metabolic medication intakes with glaucoma risks. Results: After adjusting for age and sex, the risk of glaucoma was significantly higher in the subjects with impaired fasting glucose(Odds Ratio (95% confidence interval): 1.78 (1.25, 2.53)) and metabolic syndrome (1.45 (1.01, 2.08)) than subjects without those. These associations increased when additional adjusting for smoking, alcohol use, regular physical activity, income, education status(impaired fasting glucose: 1.89 (1.29, 2.77), metabolic syndrome: 1.52 (1.03, 2.25)). Glaucoma prevalence was borderline significantly increased according to the number of metabolic abnormalities(age and sex adjusted p for trend = 0.055). Use of antihypertensive medication was significantly associated with the risk of glaucoma. Conclusions: Metabolic syndrome and impaired fasting glucose were significantly associated with the increased glaucoma risk. Use of antihypertensive medication was also significantly associated with the increased glaucoma risk.
