Outcomes of Chronic Peritoneal Dialysis by Various Modalities in Korean Children - A Single Center Study (소아 환자에서 다양한 복막투석 방법간의 결과 비교-단일기관 연구)

  • Purpose : A single center cross sectional retrospective study was performed to compare the outcomes of different peritoneal dialysis(PD) modalities in Korean children. Methods : Among children dialyzed with PD between the year 2004 and 2007, 35 children had reliable data on PD adequacy after 3 to 15 months of dialysis. Subjects were grouped by their modalities; 17, 13 and 5 children were on continuous ambulatory PD(CAPD), continuous cyclic PD(CCPD) and nightly intermittent PD(NIPD), respectively. Body weight and height, number of patients taking anti-hypertensives and laboratory data including biochemical and hemoglobin levels were compared. Dialysis adequacy including weekly Kt/Vurea, creatinine clearance (Ccr) and daily water removal were also compared. Patients were sub-grouped by their peritoneal permeability characteristics. Results : The percentage of patients taking anti-hypertensives, monthly change in Z-scores of body weight and height and laboratory data did not differ among the groups. Patients on CAPD and CCPD showed similar dialysis adequacies. Weekly dialytic Ccr was significantly lower in the NIPD group compared to the others. But total Ccr was not different when residual renal function was added. Weekly dialytic Ccr by CAPD was significantly higher than that of CCPD in low and low-average transporters. Conclusion : We propose that modality can be selected flexibly according to the patients' preferences. And peritoneal permeability characteristics provide valuable information for adjusting PD prescriptions in ultrafiltration failure or in inadequate dialysis. Further study of other clinical performance measures should be performed to clarify the comparable outcomes in different PD modalities.

Effects of Soil Bulk Density on Saturated Hydraulic Conductivity and Solute Elution Patterns (토양의 용적밀도에 따른 포화수리전도도 및 음이온의 용출양상)

  • The effects of bulk densities(${\rho}_b$) on saturated hydraulic conductivity (Ksat) and solute elution patterns were investigated from five different bulk densities ranging from $1.1Mg/m^3$ to $1.5Mg/m^3$ with each increment of $0.1Mg/m^3$. The hydraulic conductivities observed were divided into two stages: (1) a linearly decrease with increase in bulk density up to $1.4Mg/m^3$, (2) a steady state where the bulk density is greater than $1.4Mg/m^3$. Using the saturated hydraulic conductivity at the steady state, we figured out the equation describing the correlation between bulk densities(${\rho}_b$) and saturated hydraulic conductivity(Ksat) as follows: $Ksat=-19.2({\rho}_b{^2})+6{\rho}_b+15.5$, (r=0.985). Electrical conductivity(EC) measured from the leachate of the soil column showed that EC at the same pore volume were decreased with an increase in the bulk density from $1.2g/cm^3$, $1.5g/cm^3$, as shown in the time taken to collect the same pore volume at each respective bulk density. The maximum relative concentrations (C/Co=1) from the breakthrough curves for the anions of $Cl^-$, $NO_3{^-}$ and $SO_4{^{2-}}$, which are weakly adsorbed on the soil particles, moved to the right of the graph, while a distinctive retardation occurs at the bulk density between $1.3Mg/m^3$ and $1.4Mg/m^3$. The time taken to recover about 90% of indigenous sulphate was approximately twice as those of chloride and nitrate, resulting in slightly stronger adsorption characteristics for sorption sites on the soil surface. Thus, we can conclude that the salt accumulation in green house soil might be significantly influenced by it's bulk density at the soil depth, as well as the adsorption capacity of ions for the sorption sites in soils.

GafChromic Film Dosimetry for Stereotactic Radiosurgery with a Linear Accelerator (선형가속기를 이용한 정위방사선 치료 시 GafChromic Film을 이용한 선량측정)

  • Purpose: The purpose of this study was to evaluate whether a GafChromic film applied to stereotactic radiosurgery with a linear accelerator could provide information on the value for acceptance testing and quality control on the absolute dose and relative dose measurements and/or calculation of treatment planning system. Materials and methods: A spherical acrylic phantom, simulating a patient's head, was constructed from three points. The absolute and relative dose distributions could be measured by inserting a GafChromic film into the phantom. We tested the use of a calibrated GafChromic film (MD-55-2, Nuclear Associate, USA) for measuring the optical density. These measurements were achieved by irradiating the films with a dose of 0-112 Gy employing 6 MV photon. To verify the accuracy of the prescribed dose delivery to a target isocenter using a five arc beams (irradiated in 3 Gy per one beam) setup, calculated by the Linapel planning system the absolute dose and relative dose distribution using a GafChromic film were measured. All the irradiated films were digitized with a Lumiscan 75 laser digitizer and processed with the RIT113 film dosimetry system. Results: We verified the linearity of the Optical Density of a MD-55-2 GafChromic film, and measured the depth dose profile of the beam. The absolute dose delivered to the target was close to the prescribed dose of Linapel within an accuracy for the GafChromic film dosimetry (of $\pm$3$\%$), with a measurement uncertainty of $\pm$1 mm for the 50$\~$90$\%$ isodose lines. Conclusion: Our results have shown that the absolute dose and relative dose distribution curves obtained from a GafChromic film can provide information on the value for acceptance. To conclude the GafChromic flim is a convenient and useful dosimetry tool for linac based radiosurgery.

The Effectiveness of CT and MRI Contrast Agent for SUV in 18F-FDG PET/CT Scanning (18F-FDG PET/CT 검사에서 정량분석에 관한 CT와 MRI 조영제의 효과)

  • In this study, among various factors having influence on SUV, we intended to compare and analyze the change of SUV using CT(4 type) and MRI(3 type) contrast agents which are commercialized now. We used Discovery 690 PET/CT(GE) and NEMA NU2 - 1994 PET phantom as experimental equipment. We have conducted a study as follows; first, we filled distilled water to phantom about two-thirds and injected radioisotope(18F-FDG 37 MBq), contrast agent. Second, we mixed CT contrast agent with distilled water and MRI contrast agent with that water separately. And then, we stirred the fluid and filled distilled water fully not to make air bubble. In emission scan, we had 15minutes scanning time after 40 minutes mixing contrast agent with distilled water. In transmission scan, we used CT scanning and its measurement conditions were tube voltage 120 kVp, tube current 40 mA, rotation time 0.5 sec, slice thickness 3.27 mm, DFOV 30 cm. Analyzing results, we set up some ROIs in 10th, 15th, 20th, 25th, 30th slice and measured SUVmean, SUVmax. Consequently, all images mixed 3 types of MRI contrast agent with distilled water have high SUVmean as compared with pure FDG image but there was no statistical significance. In SUVmax, they have high score and there was statistical significance. And other 4 images mixed 4 types of CT contrast agent with distilled water have significance in both SUVmean and SUVmax. Attenuation correction in PET/CT has been executed through various methods to make high quality image. But we figured out that using CT and MRI contrast agents before PET/CT scanning could make distortion of image and decrease diagnostic value. In that reason, we have to sort out the priority of examination in hospital not to disturb other examination's results. Through this process, we will be able to give superior medical service to our customers.

Crustal Characteristics and Structure of the Ulleung Basin, the East Sea (Japan Sea), Inferred from Seismic, Gravity and Magnetic Data (탄성파 및 중자력자료에 의한 울릉분지의 지각특성 및 구조 연구)

  • Depths to four seismic sequence boundaries and the thickness of each sequence were estimated and mapped based on multi-channel seismic data in the Ulleung Basin. These depth-structure and isopach maps were incorporated into the interpretation of gravity and magnetic anomaly maps. The sediment thickness ranges from 3,000 m to 4,000 m in the central basin, while it reaches 6,000 m locally along the southwestern, western, and southeastern margins. The acoustic basement forms a northeast-southwest elongated depression deeper than 5000 m, and locally deepens up to 7,500 m in the southwestern and western margins. Low gravity anomalies along the western and southern margins are associated with basement depressions with thick sediment as well as the transitional crust between the continental and oceanic crusts. Higher gravity anomalies, dominant in the central Ulleung basin, broaden from southwest toward northeast, are likely due to the shallow mantle and a dense crust. A pair of magnetic elongations in the southeastern and northwestern margins appear to separate the central Ulleung basin from its margin. These magnetic elongations are largely dominated by intrusive or extrusive volcanics which occurred along the rifted margin of the Ulleung basin formed during the basin opening. The crust in the central Ulleung Basin, surrounded by the magnetic elongations, is possibly oceanic as inferred from the seismic velocity. The oceanic crust can be mapped in the central zone where it widens to 120 km from the southwest toward northeast. Bending of the crustal boundary in the southern part of the Ulleung Basin suggests that the Ulleung Basin has been deformed by a collision of the Phillipine plate into the Japan arc.

Effects of Copper and Zinc Sources on Growth Performance, Nutrient Digestibility, Carcass Traits and Meat Characteristics in Finishing Pigs (형태별 구리 및 아연 급여가 비육돈의 생산성, 영양소 소화율, 도체 및 육질 특성에 미치는 영향)

  • This study investigated the effects of copper and zinc sources on growth performance, nutrient digestibility, carcass traits and meat characteristics in finishing pigs. Dietary treatments included 1) inorganic copper ($CuSO_4{\cdot}5H_2O$ 30ppm), 2) organic copper (Cu-methionine, 30 ppm), 3) inorganic zinc (ZnO, 80 ppm) and 4) organic zinc (Zn-methionine, 80 ppm). ZnO treatment improved the ADFI (average daily feed intake) compared to Cu-met and Zn-met treatments (p<0.05) during 5 weeks of treatment. However, during the entire experimental period, the ADG (average daily gain), ADFI and F:G ratio (feed conversion ratio) were not significantly different among the treatments (p>0.05). DM and N digestibility were not significantly different among the treatments after 5 weeks (p>0.05). At the end of the experiment, DM and N digestibility were decreased with Zn-met treatment relative to the other three treatments. The pH value of meat from $CuSO_4$ treated pigs was greater than Zn-met and ZnO treated pigs (p<0.05). Sensory evaluation was carried out for randomly selected (n = 16) paired loin samples. Meat color levels were increased (p<0.05) with Cu treatments compared to Zn treatments. $CuSO_4$ decreased the marbling of meat relative to the other treatments (p<0.05). The firmness was greater with ZnO treatment compared to Cu-met and ZnO treatments (p<0.05). These results indicate that inorganic Zn can improve growth performance, however, inorganic Cu and Zn are as effective as organic Cu and Zn at improving nutrient digestibility, carcass traits and meat quality. This form of organic additive can be friendlier to the environment than inorganic supplements.

A Study on Light Condition between Pinus densiflora and Quercus variabilis Natural Mixed Forest Stands by Using the Hemispherical Photo Method (수관사진법을 이용한 소나무-굴참나무 천연림에 있어서의 광 조건 연구)

  • This study was performed to obtain the basic data to present rational silvicultural tending plan. It makes these widely distributed pine-oak mixed stand and each of pure stand in middle province on object of this study and do comparative analysis of light condition about stand parameter and natural regeneration according to each slope(north, west and south) conditions of location in central part of South Korea. Sample plots for pine-oak mixed stand and pine and oak pure stand were established on each of southern, northern and western slopes based upon site and growth conditions of the slope. Sample plot was a circle of 0.05ha with a diameter of 25.24 m. A sample plot has between 30 and 40 tree in it. Total 23 sample plots were established; 9 pure pine stands, 8 pine-oak mixed stands. and 6 pure oak stands across lower, middle, upper parts of slopes. Relative light intensity within a stand was' measured by crown- photo(fish-eye lens; 180$^{\circ}$) system through fish-eye lens and by comparing each plot with the denuded through PAR-sensor. The crown closure ratio of pure pine stand (75%) shows much lower than that of mixed stand (90.9%) and pure oak stand (93%). The relative light intensity within a stand showed an opposite result. The crown closure of mixed stands tended to become gradually low as the slope moves from the north to the south, but the relative light Intensity within the stand tended to rise. By analyzing the relationship between the relative light intensity within a stand and stand parameter, light intensity within a stand tended to decrease as the diameter and N/ha increase. Number of oak seedlings and light intensity within a stand is in a straight-line regression relation. In particular, the number of oak seedlings was the highest in mixed stands on the southern slope. But no single pine seedling was found. The unfavorable conditions of l0cm thick litter layer and low relative light intensity in a stand (ranging between 4% and 8%) is considered to prevent pine seeds from germinating.

A study on the shear bond strengths of veneering ceramics to the colored zirconia core (착색지르코니아 코어와 전장 도재 사이의 전단결합강도에 관한 연구)

  • Statement of problem: Delamination of veneering porcelain from underlying ceramic substructures has been reported for zirconia-ceramic restorations. Colored zirconia cores for esthetics have been reported that their bond strength with veneered porcelain is weaker compared to white zirconia cores. Purpose: This study aimed to investigate the shear bond strength by manufacturing the veneering porcelain on the colored zirconia core, using the layering technique and heat-pressing technique, and to evaluate the clinical stability by comparing the result of this with that of conventional metal ceramic system. Material and methods: A Metal ceramic (MC) system was tested as a control group. The tested systems were Katana zirconia with CZR (ZB) and Katana Zirconia with NobelRondo Press (ZP). Thirty specimens, 10 for each system and control, were fabricated. Specimen disks, 3 mm high and 12 mm diameter, were fabricated with the lost-wax technique (MC) and the CAD-CAM (ZB and ZP). MC and ZB specimens were prepared using opaque and dentin veneering ceramics, veneered, 3 mm high and 2.8 mm in diameter, over the cores. ZP specimens were prepared using heat pressing ingots, 3 mm high and 2.8mm in diameter. The shear bond strength test was performed in a Shear bond test machine. Load was applied at a cross-head speed of 0.50 mm/min until failure. Mean shear bond strengths (MPa) were analyzed with the One-way ANOVA. After the shear bond test, fracture surfaces were examined by SEM. Results: The mean shear bond strengths (SD) in MPa were MC control 29.14 (2.26); ZB 29.48 (2.30); and ZP 29.51 (2.32). The shear bond strengths of the tested systems were not significantly different (P > .05). All groups presented cohesive and adhesive failures, and showed predominance of cohesive failures in ceramic veneers. Conclusion: 1. The shear bond strengths of the tested groups were not significantly different from the control group (P >.05). 2. There was no significant different between the layering technique and the heat pressing technique in the veneering methods on the colored zirconia core. 3. All groups presented cohesive and adhesive failures, and showed predominance of cohesive failures in ceramic veneers.

Tuberculin Survey to Estimate the Prevalence of Tuberculosis Infection of the Elementary Schoolchildren under High BCG Vaccination Coverage (고 비시지 접종률 상태에서 초등학생들의 투베르쿨린 조사를 통한 감염률 추정 조사)

  • Background: Although the prevalence of tuberculosis infections (PTBI) is one of the basic epidemiologic indices, no survey has been carried out since 1995 because the nation-wide tuberculosis prevalence survey was changed to a surveillance system. Subjects without a BCG scar are examined in a tuberculin survey. However, it is very difficult to select these subjects under high vaccination coverage. It is important to evaluate the impact of BCG vaccinations on the tuberculin response and estimate the PTBI regardless of the BCG vaccination status. Methods: A nation-wide, school-based cross-sectional tuberculin survey was carried out among first graders in elementary school in 2006. A total of 5,148 children in 40 schools were selected by quota sampling. Tuberculin testing with 0.1 ml of two tuberculin units of PPD RT23 was carried out on 4,018 children. The maximum transverse diameter of induration was measured 48 to 72 hours later. The presence of a BCG scar was checked separately. Results: There were no BCG scars in 6.3% of the subjects. The mean induration size of tuberculin testing was $3.7{\pm}4.4mm$, which included 1,882 (46.8%) subjects with an induration size of 0 mm. The PTBI was 10.9% (439 subjects) using a cut-off point of ${\geq}10mm$ (conventional method). The annual risk of tuberculosis infections (ARTI) was 1.9% when the mean age of the subjects was assumed to be 6 years. There was no difference in the PTBI according to the presence or absence of a BCG scar [11.2% vs 7.6% (OR: 1.54, 95% CI: 0.98~2.43)]. Using a mirror image technique with 16 mm as the cut-off point, the PTBI and ARTI had decreased to 2.4% and 0.4% respectively. Conclusion: PTBI and ARTI, as estimated by conventional methods, appear to be high among BCG vaccinated children. A mirror image technique is more suitable for estimating the indices in a country with an intermediate burden of tuberculosis than the conventional method.

Urban archaeological investigations using surface 3D Ground Penetrating Radar and Electrical Resistivity Tomography methods (3차원 지표레이다와 전기비저항 탐사를 이용한 도심지 유적 조사)

  • Ongoing and extensive urbanisation, which is frequently accompanied with careless construction works, may threaten important archaeological structures that are still buried in the urban areas. Ground Penetrating Radar (GPR) and Electrical Resistivity Tomography (ERT) methods are most promising alternatives for resolving buried archaeological structures in urban territories. In this work, three case studies are presented, each of which involves an integrated geophysical survey employing the surface three-dimensional (3D) ERT and GPR techniques, in order to archaeologically characterise the investigated areas. The test field sites are located at the historical centres of two of the most populated cities of the island of Crete, in Greece. The ERT and GPR data were collected along a dense network of parallel profiles. The subsurface resistivity structure was reconstructed by processing the apparent resistivity data with a 3D inversion algorithm. The GPR sections were processed with a systematic way, applying specific filters to the data in order to enhance their information content. Finally, horizontal depth slices representing the 3D variation of the physical properties were created. The GPR and ERT images significantly contributed in reconstructing the complex subsurface properties in these urban areas. Strong GPR reflections and highresistivity anomalies were correlated with possible archaeological structures. Subsequent excavations in specific places at both sites verified the geophysical results. The specific case studies demonstrated the applicability of ERT and GPR techniques during the design and construction stages of urban infrastructure works, indicating areas of archaeological significance and guiding archaeological excavations before construction work.
