Collision Tumor of Meningioma and Anaplastic Astrocytoma - A Case Report - (뇌수막종과 역형성 성상세포종의 혼합종양 - 증 례 보 고 -)

  • Multiple primary brain tumors of different cell types are rare, accounting for 0.4% of all the primary brain tumors. Phakomatosis, irradiation, trauma and other factors have been associated with multiplicity of brain tumors. When these tumors are close or intermixed, the term "collision" has been used, and in these cases an explanation might be that one tumor stimulating the other. We report a patient with collision tumor of meningioma and anaplastic astrocytoma, who did not have a history of trauma, irradiation, or phakomatosis.

  • Cerebral paragonimiasis is an important parasitic infestation found in Eastern Asia, West Africa, and Central/South America. It is frequently associated with abscess, granuloma or cystic lesions in chronic stage. Chronic symptoms include seizure, visual disturbance, movement and perception disorders, usually resulting from the spaceoccupying lesions. The authors report a case of chronic cerebral paragonimiasis combined with meningioma. A 69-year-old man presented with generalized seizure and impaired consciousness. CT scan and MR image of the brain revealed two intracranial lesions ; one was cerebral paragonimiasis in the left temporo-occipital lobe, and the other a huge falx meningioma. Two lesions were successfully removed in staged operations and the generalized seizure was controlled after removal of the cerebral paragonimiasis.

Secretory Meningioma with Severe Peritumoral Edema - Case Report - (심한 부종을 동반한 분비성 뇌수막종 - 증례보고 -)

  • The authors report a 42-year-old woman presented with a falx meningioma with disproportionately severe peritumoral edema. Histological examination including immunohistochemical staining resulted in the diagnosis of secretory meningioma. In addition to tumor size, edema could not be explained by location, growth rate, vascular involvement, or other factors. We conclude that secretory meningiomas may possess an innate ability to cause brain edema.

Stereotactic Radiosurgery for Intracranial Tumors; Early Experience with Linear Accelerator (두개강내 종양에 대한 방사선 뇌수술의 역할)

  • Between August 1988 and December 1991, 24 patients with intracranial tumors were treated with stereotactic radiosurgery(RS) using a 10 MV linear accelerator at Severance Hospital, Yonsei University College of Medicine. There were 5 meningiomas, 3 craniopharyngiomas, 9 glial tumors, 2 solitary metastases, 2 acoustic neurinomas, 2 pineal tumors, and 1 non-Hodgkin's lymphoma. Ten patients were treated as primary treatment after diagnosis with stereotactic biopsy or neuroimaging study. Nine patients underwent RS for post-op. residual tumors and three patients as a salvage treatment for recurrence after external irradiation. Two patients received RS as a boost followed by fractionated conventional radiotherapy. Among sixteen patients who were followed more than 6 months with neuroimage, seven patients (2 meningiomas, 4 benign glial tumors, one non-Hodgkin's lymphoma) showed complete response on neuroimage after RS and nine patients showed decreased tumor size. There was no acute treatment related side reaction. Late complications include three patients with symptomatic peritumoral brain edema and one craniopharyngioma with optic chiasmal injury. Through this early experience, we conclude that stereotactically directed single high doses of irradiation to the small intracranial tumors is effective for tumor control. However, in order to define the role of radiosurgery in the management of intracraniai tumors, we should get the long-term results available to demonstrate the benefits versus potential complications of this therapeutic modality.

In Vivo H-1 MR Spectroscopy of Intracranial solid Tumors (두개강내 고형성 종양의 H-1 자기공명분광법)

  • Purpose: To assess the ability of in vivo H-1 MRS to determine the degree of malignancy and to characterize the histopathologic type of intracranial solid tumors. Materials and Methods: In vivo H-1 MR spectra of the pathologically-proven 81 intracranial soild tumors (low-grade glioma 17 cases, high-grade glioma 31 cases, lymphoma 9 cases, meningioma 8 cases, central neurocytoma 4 cases, medulloblastoma 3 cases, PNET 3 cases, metastasis 2 cases, others 4 cases) were analyzed. H-1 MR spectroscopy was performed on a 1.5T MR unit using PRESS sequence with a TR of 2000ms, a TE of 270 or 135ms and a voxel size of $2{\times}2{\times}2cm^3$ for all spectra. N-acetyl aspartate (NAA)/Creatine complex(Cr), Choline complex (Cho)/Cr, and lactate (Lac)/Cr ratios were measured on the peak heights of each resonance and compared among the different tumors. Results: All intracranial solid tumors demonstrated decreased NAA, elevated Cho and lactate, and variable Cr levels. All tumors showed increased Cho/Cr and Lac/Cr, whereas NAA/Cr level was decreased. Mean Cho/Cr and Lac/Cr ratios were significantly higher in high-grade gliomas than in low-grade gliomas. However, NAA/Cr ratio showed no significant difference between low-grade and high-grade gliomas. Very high Cho peaks were seen in lymphomas, meningiomas, medulloblastomas, and neurocytomas in addition to high-grade gliomas. Conclusion: H-1 MRS may be useful in differentiating between low-grade and high-grade gliomas, however cannot characterize the histologic types or subtypes of tumors.

Perfusion RRI of the Brain Using Oxygen Inhalation (산소 호흡을 이용한 뇌의 관류 자기공명영상)

  • Purpose : To know the possibility of clinical application of MRI using oxygen inhalation as a perfusion MRI Materials and methods : Two healthy volunteers and three patients of one moyamoya disease, one acute infarction and one meningioma were studied using a 1.5 Tesla MRI unit. Oxygen (15 liters/min) mixed with room air was given using face mask from 8 second to 35 second during the study. Images were acquired 25 times (scan time per study were 1.6 seconds) using susceptibility contrast EPI (echo planar image) sequence. Difference maps were acquired by early (study 12-18), and late (study 19-25) O2 inhalation image groups minus pre-O2 inhalation image group (study 3-9) with a Z-score of 0.7-1.0 using VB31C program of Magneton Vision. The resulting perfusion images were created by superimposition of difference maps on corresponding T1 weighted anatomic images. On moyamoya patient, similar perfusion images were acquired after Gd-DTPA injection, and compared with O2 inhalation perfusion images. Results ; The author can get the perfusion images of the brain by oxygen inhalation with susceptibility contrast EPI sequence at the volunteers, and the patient of moyomoya disease, acute infarction and meningioma. On moyamoya patient, perfusion images with O2 inhalation are similar with perfusion images by Gd-DTPA injection. Conclusion 1 This study has demonstrated that the susceptibility contrast EPI by oxygen inhalation can be used as the clinically useful perfusion MRI technique

  • We report a case of 54 years old male with malignant meningioma originating in the posterior fossa with multiple recurrences, intracranial and extracranial metastases. In spite of gross total removal of tumor and conventional external radiation therapy(CERT), 2 more recurrences, 5 more intracranial metastases and 1 extracranial metastasis to the rib were developed. We tried fractionated stereotactic radiation therapy(FSRT) and CERT to the intracranial metastasis with satisfactory result. Extracranial metastasis to the rib was resected and histological finding was similar to that of original tumor.

Manic Patient with Meningioma Treated with Low dose Risperidone and Valproic Acid (저용량 리스페리돈과 발프로에이트로 치료된 뇌수막종에 의한 조증)

  • We describe the case of a 73 year-old female patient, YSG, who initially presented with a manic episode without any previous psychiatric history and was later diagnosed as having a meningioma in the left frontal lobe. YSG's symptoms were characterized by hyperactivity, insomnia, aggressive behavior with an auditory hallucination. She showed no abnormal signs on a complete neurologic examination. A gadolinium-enhanced MRI study showed a huge, extra-axial mass with homogenous enhancement in the left high convexity of the frontal lobe. Her manic symptoms subsided after administration of risperidone 1mg and valproic acid 500mg daily, for three weeks without surgical resection of the tumor. These findings suggest that YSG's mania might have resulted from the left-sided frontal tumor, and that her symptoms were treated rapidly by small doses of risperidone combined with valproic acid. Medical staff who care for manic patients should be aware of this possibility of a organic lesion without evidence of neurologic disease.

False-positive I-131 Uptake in Meningioma (갑상선암 환자에서 관찰된 뇌수막종의 위양성 옥소 섭취)

  • We experienced a case with meningioma showing false positive I-131 uptake. A 55-years old female patient underwent high dose (150 mCi) radioactive iodine therapy to ablate remnant tissue after total thyroidectomy for papillary thyroid cancer. in addition to intense tracer uptake in thyroid bed, there was mild but focal abnormal uptake in left frontal lobe of the brain on post-therapy I-131 whole body scan. Subsequent brain MR imaging showed single mass lesion in left frontal lobe and the mass was resected under the impression of brain metastasis of thyroid carcinoma. Pathologic report confirmed meningioma from the surgical specimen.

  • Acupuncture is a complementary medical treatment whose use in cancer patients has been recommended by the American Cancer Society (ACS) for the treatment of cancer and treatment-related symptoms. Pain, nausea, breathlessness, vasomotor symptoms and limb edema have all been found to respond to this treatment modality. This has become quite familiar to many Koreans not only for pain, but also for many other health problems, both in acute and chronic conditions. Actually, acupuncture is a therapeutic technique that is part of a larger system of traditional oriental medicine. There are several styles of acupuncture. We treated one 72-year-old female patient with Dongshiqixue Acupuncture Technique who had refused surgical operation for brain meningioma showing right hemiparesis, dysarthria, headache and dizziness. During the acupuncture treatment, there were no other adverse effects. After six day's treatment, she could ambulate and make usual life all by herself. On her brain magnetic resonence image follow-up, there was no interval change in meningioma. After discharged from our hospital, she has kept up her independent daily life as before. So it is suggested that some acupuncture treatment should be effect on brain meningioma clinically.
