A Clinical Study of Ocular Dimention and Visual Acuity Before and After Cataract Surgery Over Ninety Years Old (90세 이상 초고령 환자들의 백내장 수술전.후 안수치와 시력에 관한 임상연구)

  • Propose: Due to the rapid growth of medical technologies and the increasing population of older people, we investigated clinical status of ocular dimensions and visual acuity for pre and post cataract surgeries of people over age 90. Methods: From March 2007 to February 2009, we investigated eighty-two eyes of forty-eight patients who had undergone cataract surgeries at an ophthalmic clinic (Ansung, Kyungi-do), investigated maximum corrected vision, axial length, anterior chamber depth and accompanied ocular diseases before and after the surgeries based on the collected data. Results: As patients aged, axial length unchanged but anterior chamber depth decreased over all due to the increase of intraocular lens thickness, and men tended to have a higher degree than women. Seventy-one (86.6%) of eighty-two eyes showed improved corrected vision than before surgeries and forty-three (52.4%) eyes could see more than visual acuity of 0.5. Conclusions: Patients with the systemic disease and accompanied ocular disease showed low vision less than 0.5 after cataract surgery compared to same healthy age peoples. But the others improved correction visual acuity more than 0.5, so the cataract surgery was surely necessary for people over 90 years old and also the presence of ocular disease could have a great influence on correction visual acuity.

The Epidemiological Study on the Cataract of the Rural Aged Population Using a Simplified Screening System (간편한 선별검사법에 의한 농촌 노인인구의 백내장에 대한 역학조사)

  • The purpose of this study is to determine the prevalence of cataracts in a rural area of Kyung-Buk province. Cataract is the main cause of blindness and visual impairment in the world. Recently, the number of age-related cataract surgeries has increased remarkably. In spite of such an increase in the number of patients, there are still many cataract patients with highly deteriorated visual function who have had no occasion to receive an ocular examination. In order to screen such patients the author assessed 636 individual(1272 eyes) aged 50 years or more who had wanted to be examined generally in two area. Chilgok and Munkyung, Kyung-Buk province using a simplified cataract screening system recently proposed by Sasaki et al. Kanazawa Medical University in Japan. The results obtained are as follows. The number of visual impairment patients in this study group was 493(77.5%). They were screened by cataract screening system. Primary screening examination detected 448 subjects to be suspected of cataract while the subsequent secondary examination narrowed this number of subjects to 308(48.4%). Final number of subjects to be diagnosed of cataract was 421(66.2%). The rate of incidence, according to the age, was 27.5% in the 50 year-olds age group, 62.5% in the 60's age group, 86.1% in the 70's age group and 94.3% in the 80's age group and upwards. This results concluded that cataract is the main cause of blindness and visual impairment and is an important public eye health care problem of aged population in rural Korea. Methods of tackling the cataract problem(both backlog and incident), and other eye health needs are recommended. The need to extend eye health service to the rural areas is emphasized.

The Spatial Characteristics of Universal Design (UD) Tourist Attractions in Seoul (서울시 유니버설 디자인(UD) 관광지의 공간적 특성에 관한 연구)

  • In 1991, the United Nations World Tourism Organization (UNWTO) declared "tourism accessible for all" recommending the practice of the right to enjoy tourism. According to the Ministry of Health and Welfare of Korea (2019), the disabled, who are the most vulnerable in tourism, accounted for 5.1% of the total population, and the number of the elderly over 65 is expected to increase to 20.3% by 2025. In particular, the need for customized policies has been raised as the proportion of disabled people among the elderly aged 65 and over continues to increase. Thus, this study identified the spatial characteristics of Universal Design (UD) tourist destinations considering the tourism vulnerable groups. Administrative units (425 dongs) in Seoul were used as spatial units for analysis. As a research method, first, a spatial model was specified through LM verification, and then spatial regression analysis was performed. As a result of the analysis, the spatial characteristics of UD tourist destinations were found to have positive (+) effects on the number of universally certified businesses, the number of restaurants, and the number of bus stops that were available to the vulnerable. It was confirmed that there are a large number of universal certified businesses, restaurants, and bus stops in dongs with UD tourist destinations. The findings will provide policy implications when promoting the right to enjoy tourism in the future and improving Korean universal design quality.

A Study on Dental Hygiene Students' Knowledge, Attitude and Behavior towards the Elderly in Busan (부산지역 일부 치위생과 학생의 노인에 대한 지식과 태도 및 행동에 관한 연구)

  • As the number of old people grows in today's modern society, dental hygienist's role is more emphasized than ever before for oral hygiene management of the aged. It is also important to find out college students' knowledge, attitude and behavior towards the elderly. Therefore, this study is conducted through interview with college students who are majoring in dental hygiene of health care in Busan and will be in charge of oral hygiene. According to the interview, this study obtains the following results: 1. The level of undergraduate students' knowledge of old people shows a total score of $14.53{\pm}2.35$ (correct response rate is 69.9%). 2. The average score of undergraduate students' attitude towards old people is in a neutral range(50~70), recording 63.12(${\pm}7.22$) on a scale of 100. 3. The average score of undergraduate students' behavior towards old people is lower than a neutral range(43~60), recording 39.09(${\pm}13.43$) on a scale of 85 and showing negative behavior. 4. Experience of living together with the elderly and participating in volunteer jobs lead to significant differences in undergraduate students' attitude towards old people. 5. Experience of living together with the elderly causes significant differences in undergraduate students' behavior towards old people. That is, undergraduate students living together with the elderly show a positive behavior towards old people. 6. Undergraduate students' knowledge shows a strong positive relationship with their attitude, but it has a bit positive correlation with behavior even though there are no statistically significant differences between knowledge and behavior. No relationship is found between behavior and attitude. That is, undergraduate students have more positive attitude as their knowledge of the elderly is higher, but their behavior is not positive.

A exploratory study for Influential factors of smartphone over-dependence in elder and older Group (장·노년층의 스마트폰 과의존 영향요인에 대한 탐색적 연구)

  • Smartphone use has become a part of everyday life, not only for adolescents but also for elderly and older people. Thus, this study explored the effect of demographic variables, the amount of smartphone usage, type of smartphone use and psychosocial variables on smartphone dependence of the elder group. We analyzed the data of 5627 (50s and 60s) of the Internet overdependence survey data of the Korea Information Society Agency in 2016. As a result of hierarchical multiple regression analysis, More frequency of smartphone use of weekday and weekend, and usage time of smartphone of weekday were related to greater smartphone dependence. More smartphone use for information seeking and games were associated with greater smartphone dependence. And, as loneliness and anxiety increased, smartphone dependence increased, while smartphone dependence decreased, as social capital increased. Through this study, it is necessary to understand characteristics related to smartphone dependence of elder and older group, and to use this knowledge to solve their smartphone dependence. Future studies also need to identify the causal relationship between predictors of smartphone dependence in elderly and older people.

The Effects of Pet Dog-Assisted Activities on Self-Esteem, Depression and Cognitive Function among Elderly People (애완견 매개활동이 노인의 자기존중감, 우울 및 인지기능에 미치는 효과)

  • Objectives: The purpose of this study was to investigate the effects of pet dog-assisted activities on self-esteem, depression and cognitive function of elderly people Method: The experimental group(pet dog-assisted group) consisted of 22 people over 65 years old out of about 90 elderly people registered with a home for the aged and the control group(non-pet dog assisted group) consisted of 22 people over 65 years old out of about 90 elderly people registered with another home for the aged in Daegu. Both groups had similar general characteristics, state of health, conduct on health and self-esteem, depression and cognitive abilities. Nineteen people who participated in the program at least 10 times out of 12, were chosen for the final analysis from the experimental group. For the control group, 20 people who responded to all three tests, which had been conducted before, 1 week after, and four week after the experiment, were analyzed. Result: Mean(${\pm}SD$) differences between before and 1 week after the experiment for self-esteem, depression, and cognitive function were 5.84(${\pm}3.06$), -3.26(${\pm}1.76$), 1.47(${\pm}1.26$), respectively. Those between before the experiment and 4 weeks after the experiment was 5.68(${\pm}3.64$), -3.94(${\pm}2.32$), 1.63(${\pm}0.96$), respectively. Those for control group between before and after the experiment were -0.40(${\pm}1.27$), 0.00(${\pm}0.92$), 0.15(${\pm}0.93$), respectively. Those for control group between before and 4 weeks after the experiment were -0.45(${\pm}2.24$), 0.25(${\pm}1.68$), 0.15(${\pm}0.93$), respectively. All the differences between experiment and control group in mean differences between before and 1 week after experiment, between before and 4 weeks after experiment were statistically significant(p<0.05). Conclusion: This study suggests that the pet-assisted activities can be useful solution for elderly people who have psychological and emotional problems caused by retrogression of physical, mental and social ability.

Clinical Characteristics and Prognosis of Elderly Patients Receiving Prolonged Mechanical Ventilation in the Medical Intensive Care Unit at a University Hospital (한 대학병원 내과계 중환자실에서 장기간 기계환기를 받은 노인 환자들의 특징 및 예후)

  • Background: As the number of older-aged people increases, the number of elderly patients who receive critical care services is expected to increase substantially. The objective of this study was to examine the clinical characteristics and outcomes of elderly patients who receive mechanical ventilation for more than 30 days in the medical intensive care unit (MICU) at a university hospital. Methods: We retrospectively examined forty-one elderly patients (${\geq}65$ years old) who were receiving mechanical ventilation, from April 2004 to March 2007, for periods exceeding 30 days at the MICU at Eulji University Hospital. Results: The MICU and hospitalmortality rate were 60.9% and 65.9%, respectively. The mean length of the ICU stay was 57.5 days and the mean duration of mechanical ventilation was 49.3 days. The most common reason for MICU admission was acute respiratory failure (73.2%), followed by sepsis (12.2%), neurological problems (9.8%), and gastrointestinal bleeding (4.9%). The Acute Physiology and Chronic Health Evaluation (APACHE) II scores were higher for the nonsurvivors than for the survivors (28.0 vs. 25.0, respectively, p=0.03). The nonsurvivors received more red blood cell (RBC) transfusions during their ICU stay than did the survivors (84.0% vs. 43.8%, respectively p=0.007). The factors associated with hospital death were the APACHE II score and if the patient had received a RBC transfusion. Conclusion: The APACHE II score and a RBC transfusion were predictors of increased hospital mortality for the elderly patients who were on prolonged mechanical ventilation. These predictors may assist physicians to make clinical decisions for this patient population.

New Perspectives on Sunday School of Korean Church for Next Generation (다음 세대와 한국교회 주일학교의 새 전망)

  • In the early 21st century, the global COVID-19 pandemic, which has arisen during the development of the technological science of the Fourth Industrial Revolution, has been a great challenge in all fields including politics, economy, industry, education and religion in Korean society. To prevent the spread of the COVID-19 epidemic, the Korean government announced 'social distancing guidelines,' focused on the 'prohibition of three conditions'(crowd, closeness, airtight) for safety reasons. These quarantine guidelines made it more difficult for Korean churches and Sunday schools to operate. In general, looking at the statistical data of the major denominations of the Korean Church in the second half of the 20th century, shows that the Church has entered a period of stagnant or declining growth. Data also show that the number of students attending Sunday School is decreasing. The researcher identified four causes of the crisis faced by the Korean church and Korean Sunday school entering the 21st century. These trends are influenced by the tendencies of postmodernism, the deconstruction of modern universalism, the certainty and objectivity of knowledge, and the grand narrative and worldview of diffusion. Moreover, it is a phenomenon in which the young population decreases in contrast to the increasing elderly population in the age of population cliff in Korean society. Sunday Schools are also facing a crisis, as the youth population, who will become the future heroes of the Korean church, is declining. Finally, constraints of Church and Sunday school education activities are due to COVID-19 Pandemic. As analysis shows the loss of the Church's educational vision and a decrease in the passion for education. Accordingly, the researcher suggests four new strategies for the next generation of Korean Sunday schools, whose ranges from 200 members or less; this range covers the majority of Sunday School program run by churches in Korea. First, in the age of postmodernism, a time of uncertainty and relativism, Christian Societies requires teachers who are certain of absolute Christian truth and faith. Second, in an era of declining population cliffs for younger generations, a shift to a home-friendly Sunday school paradigm is needed. Third, during the COVID-19 pandemic, educational activities must appropriately utilize face-to-face and non-face-to-face communication. Finally, even in difficult times, Korean Sunday school should nevertheless remember the Lord's great commandment(Matthew 28:18-20) and restore the vision and passion of education to announce and teach the gospel. The researcher hopes that this study will provide small, positive steps in rebuilding Korean Sunday school educational activities for future generations in difficult times.

Physical Functioning and Related Factors in the Elderly People Admitted Long-term Home Care Insurance (재가급여 장기요양인정 노인들의 신체기능 상태 및 관련요인)

  • This study was performed to determine the levels of physical functioning and to reveal its association with socio-demographic characteristics, health status and health related behaviors in the elderly people admitted long-term home care insurance. The interviews were performed, during the period from December 1, 2011 to March 31, 2012, to 618 home care elderlies admitted long-term care insurance dwelling in Daejeon city. As a results, the distribution of impaired ADL and IADL group of all subjects were 45.6% in ADL, and 48.4% in IADL. in consequently, the rate of impaired IADL group was higher than that of impaired ADL group. In logistic regression analysis, the adjusted odds ratio of the impaired ADL and IADL group were significantly increased in the group of male, living with spouse or family, bear for living expenses with son and daughter, health insurance, irregular exercise, seldom with frequency of going out, seldom with activity of hobbies, Subjective health status is good, and disability of body, visual acuity, hearing ability and mastication ability is good, and without urinary incontinence and amnesia. In conclusion, the level of physical functioning in the elderly people admitted long-term home care insurance was decreased in IADL than ADL. And the level of ADL and IADL were related with the various variables of socio-demographic characteristics, health status and health related behaviors.

A Critical Approach on Family Support, Social Security, and its Direction: Focusing on Old Parents and Children from Divorced Families (가족부양 쟁점에 관한 일고찰: 노인과 이혼가족 아동을 중심으로)

  • The study is to analyze support-related issues, particularly associated with elderly and children care, which have been a fundamental agenda of family welfare policy. We are expected for societal-level controversy on the relationship of family support and social security in the near future because of rapid increase in ageing and divorce rate. Accordingly, we need to establish social consensus on the extent to which both family support system and social support system are responsible for the life maintenance of the elderly and the children from divorced family. We also clarify the various characteristics(including scope, degree, period of time, and need/manageability) of support provided by family members. Considering the changes of family structure and population composition, the policy of supporting the old and children should go for social responsibility. Findings show that civil law and social welfare law have unclear application on the relationship between family support and social security, and contents in family support. In particular, public assistance law strongly emphasizes the principle of family support while social insurance laws provide only limited and insufficient family maintenance. The suggestions of further study on support-related issues are given in the rapidly changing society and the increasing economic instability.
