An Analysis of the Quality of Life and the Affecting Factors of the Elderly (노인의 삶의 질과 영향요인에 관한 연구)

  • The purpose of this study is to examine the demographic, economic, and psychological factors that influence the quality of life (QoL) of the elderly. The data in the 'Survey on the Elderly in 2011', were collected from The Ministry of Health & Welfare and the Korea Institute for Health and Social Affairs. The samples included 3,312 out of the total of 15,146 elderly people aged from 65 to 70 years. Multiple regression analysis and path analysis were used to analyze the research model. The findings are as follows. First, the most critical factor that affected the QoL of the elderly was "subjective standard of living" with other factors including "subjective physical condition level", "depression", "total annual income", "the number of chronic disease", "self-esteem", "economic supports from their adult children", "education level", "living with their adult children or not", "sex", and "home ownership", in that order. Second, based on the path analysis results, the QoL of the elderly was influenced by "subjective physical condition level", "subjective standard of living", "depression", "self-esteem", "total annual income", "education level", and "the number of chronic disease", in that order. In particular, "depression" had a negative direct effect and an indirect effect on the QoL of the elderly, whereas "self-esteem" had a positive direct effect and indirect effect on their QoL. Third, "labor force participation status" did not affect the QoL of the elderly. However, it implied the mediating effect of "self-esteem" and "depression" on their QoL. These results show that demographic, economic, and psychological factors heavily influence the QoL of the elderly. Specifically, "depression" and "self-esteem" turn out to be mediating variables that strongly influence the QoL of the elderly. Also, "labor force participation status" does not have a positive direct effect on the QoL of the elderly but has a positive mediating effect of "depression" and "self-esteem" on their QoL.

A Comparative Study on Family Support, Self-esteem, and Health Status between the Institutionalized Elderly People and the Home-staying Ones (시설노인과 재가노인의 가족지지, 자존감 및 건강상태 비교연구)

  • This study aims to provide the fundamental data for substantial nursing intervention in the elderly through a comparative appreciation on family support, self-esteem, and health status between the institutionalized elderly people and the home-staying ones. The subjects of this study are the institutionalized 108 elderly people of E and C Public Homes and the home-staying 109 elderly ones of O-Nho In Jeong(a kind of public recreational facilities for the aged) over the age of 65. The instruments for this research are based upon the tool(11 items, 5 points for each) for measuring family support developed by Choi, Young Hee(1984), a self-esteem scale done by Rosenberg (1965), the tools(20 items) for checking the health status of the elderly done by Lee, Young-Ja(1989). The sampling for this study has been carried on from July, 2000 until November, 2000. Questionnaire data were drawn up by personal interviews. The analyses of collected data are based on general characteristics calculated at the rate of 100 percentage to the average, t-test, ANOVA(some difference on a level with p<.05 being subsequently confirmed by DMR) for family support, self-esteem and health status, and Pearson Correlation to verify the hypothetical correlation among the subjects' family support, self-esteem and health status. The results of this study are as follows: 1. The difference between two groups in the light of family support, self-esteem and health status. (1) Family support - The rate of the family support that the institutionalized elderly people perceive turned out to be 22.13, that of the home-staying ones 30.99. (2) Self-esteem - The rate of the self-esteem that the former perceives proved to be 25.59, that of the latter 32.28. (3) Health Status - The rate of the health status that the former perceives turned out to be 39.67, that of the latter 51.60. 2. Family support, self-esteem, health status in terms of demographic characteristic (1) Family support - The group of institutionalized elderly people shows a tendency to be chiefly influenced by the death or life of the spouse and the number of the children; the group of the home staying ones to be chiefly influenced by the educational level (2) Self-esteem - The group of institutionalized elderly people shows a tendency to be chiefly influenced by educational level; the group of the home staying ones to be chiefly influenced by the amount of pocket money, the pocket money provider and the family main supporter. (3) Health Status - The group of institutionalized elderly people shows a tendency to be chiefly influenced by educational level; the group of the home staying ones to be chiefly influenced by age, the death or life of spouse, religion, and the educational level. 3. Correlation among family support, self-esteem, and health status The rate of correlation between family support and health status proved to be the highest (r=.549). After came the rate of correlation between health status and self-esteem, which turned out to be(r=.506). The last came the rate of correlation between family support and self-esteem, which proved to be(r=.406). According to this study, there is a conspicuously close correlation among family support, self-esteem, and health status for the elderly. Thus, it would be indispensable to seek out a variety of nursing intervention ways how the elderly could promote family support, self-esteem, and health status.

Determinants of Health-Promoting Behavior in the Elderly (노인의 건강증진행위와 관련된 변인에 관한 연구)

  • This study was undertaken to grasp health-promoting behavior of the elderly and to identify variables related to them in order to facilitate nursing intervention for health promotion of this population. The subjects for this study were 291 old persons obtained by cluster sampling from twenty general social welfare centers located in Tague. Data were collected by self-reported questionnaires from August 13 to September 13, 1996. Questionnaires were developed based on Sherer and others' Self-Efficacy scale, Rosenberg's Self-Esteem scale, Wallston and other's Multidimensional Health Locus of Control scale, Northern illinois University's Helath Self Rating Scale, Walker and others' Health Promotion Lifestyles Profile. Analysis of the data was done by use of descriptive statistics, stepwise multiple regression, Pearson Correlation Coefficient, MANOVA, t-test, and ANOVA. The results were summarized as follows : 1. For the practice of health-promoting behavior, the mean score was 2.89 and range was 3.59 to 2.09. The factor of the highest mean score was regular diet(M=3.42) and factor of the lowest mean score as stress management(M=2.27). 2. The combination of self-efficacy, internal health locus of control, family number, and average monthly pocket money explained 30.0% of the variance of health-promoting behavior. 3. With regard to the relationship between health-promoting behavior and cognitive perceptual factor, self-efficacy correlated positively with health-promoting behavior(r=.4951, P=.0001), self-esteem correlated positively(r=.3263, P=.0001), internal health locus of control correlated positively(r=.3244, P=.0001), perceived health status correlated positively(r=.1355, P=.0274). 4. According to age(F=2.50, P=.0431), sex(t=2.14, P=.0332), marital status(F=7.85, P=.0005), education(F=5.44, P=.0003), family number(F=11.18, P=.0001), people living together(F=7.21, P=.0009), previous occupation(F=5.83, P=.0001), average monthly pocket money(F=7.27, P=.0001), there were differences of health-promoting behavior. The above findings show that health-promoting behavior are related to demographic characteristics, four cognitive perceptual factors(self-efficacy, self-esteem, internal health locus of control, perceived health status). On the basis of the above findings the following recommendations are made ; 1. Nursing interventions enhancing exercise or activity, accountability for health, stress management of the elderly must be provided. 2. Nursing strategies enhancing self-efficacy which is most significant effect on health-promoting behavior must be developed.

The Intervention Effect of Cognitive Improvement Program for Elderly with Mild Cognitive Impairment (경도인지장애 노인의 인지향상 프로그램 중재효과)

  • Purpose: This study was conducted to identify the effects of a group cognitive improvement program on cognitive function, depression and self-esteem in elderly individuals with mild cognitive impairment. Methods: This was an experimental study that employed a pre-post design of a non-equivalence control group. The subjects were 52 elderly people with mild cognitive impairment, 25 of whom were assigned to the experimental group and 27 to the control group. The program was conducted for a total of 12 sessions for 60 minutes each. Data were analyzed using the ${\chi}2-test$, Fisher's exact test, and Independent t-test with the SPSS 20.0 program. Results: After the intervention, the group who participated showed improvement in all areas of cognitive function based on MMSE-KC (F=26.37, p.<0.001), the Rey Complex Figure Test: copy (F=20.66, p.<0.001), Immediate memory of Seoul Verbal Learning Test-Elderly's version (F=29.68, p.<0.001), delayed memory (F=45.79 p.<0.001), memory recall (F=28.97, p.<0.001), Forward of Digit Span Test (F=9.25, p=.004), backward (F=8.33, p.=0.006), language comprehension (F=13.42, p.<0.001), and digit symbol coding (F=17.74, p.<0.001) relative to the control group. Moreover depression (F=24.09, p.<0.001) was decreased in program participants, whereas self-esteem (F=40.24, p.<0.001) was increased. Conclusion: The program could be a useful intervention because the results show that the group cognitive improvement program has a significant effect on cognitive function, depression and self-esteem in elderly with mild cognitive impairment.

An exploration of the relationship between crime/victim characteristics and the victim's criminal damages: Variable selection based on random forest algorithm (범죄 및 피해자 특성과 범죄피해 내용의 관계 탐색: 랜덤포레스트 알고리즘에 기초한 변인선택)

  • The current study applied the random forest algorithm to Korean crime victim survey data collected biennially between 2010 and 2018 to explore the relationship between crime/victim characteristics and the victim's criminal damages. A total of 3,080 cases including gender, age (life cycle stage), type of crime, perpetrator acquisition, repeated victimization, psychological damage (depression, isolation, extreme fear, somatic symptoms, interpersonal problems, moving out to avoid people, suicidal impulses, suicide attempts), and emotional changes after victimization (changes in self-protection confidence, self-esteem, confidence in others, confidence in legal institutions, and respect for Korean legal system/law) were analyzed. Considering the features of data that are difficult to apply traditional statistical techniques, this study implemented random forest algorithms to predict crime and victim characteristics using the victim's criminal damages (psychological damage and emotional change) and selected good predictors using VSURF function in VSURF package for R. As a result of the analysis, it was confirmed that the relationship between the type of crime and depression, extreme fear, somatic symptoms, and interpersonal problems, between perpetrator acquisition and somatic symptoms and interpersonal problems, and between repeated victimization and changes in respect for Korean legal system/law. Gender and life cycle stage (youth/adult/elderly) were found to be related to extreme fear and changes in self-protection confidence, respectively. However, more empirical evidence should be aggregated to explain the results as meaningful. The results of this study suggest that it is necessary to enhance the experts' knowledge and educate them on cases about the relationship between crime/victim characteristics and criminal damage. Strengthening their interview strategy and knowledge about law/rules were also needed to increase the effectiveness of the Korean victim assessment system.

Personal Growth through Spousal Bereavement in Later Life (노년기 배우자 사별 후 적응과정에서의 개인적 성장)

  • This study purposes to explore the growing process through spousal bereavement in later life, and to develop the theory. A qualitative research was conducted, and the participants were 17 seniors. The analysis according to Strauss and Corbin's grounded theory(1998), resulted in 143 concepts, 43 subcategories, and 19 categories. Range analysis according to paradigm showed that the causal conditions were 'marital relationships', 'independent/dependent tendencies', and 'emotional readiness for the death of a spouse', and the phenomena were 'depression', 'hopelessness', 'daily stress', 'psychological intimidation', 'regret', and 'sense of being freed'. The contextual conditions that affect these phenomena were 'desire for intimate personal relationships' and 'desire to maintain independence'; the action/interaction strategies to manage the phenomena were 'facing reality' and 'efforts for construction of the new life'; and the mediating conditions that promote or suppress these action/interaction strategies were 'social support' and 'spirituality'. The results were 'reconstruction of the meaning in life', 'increase in self-esteem', 'reinforcement of social network' and 'embrace and acceptance'. Furthermore, when personal growth after bereavement of a spouse was analyzed focusing on changes over time, the growth process consisted of three steps: 'sadness and despair', 'embracing and moving forward', and 'personal growth'. The pattern analyses were performed to typify recurring relations by category, and 5 types were derived. The results of our study show that personal growth after spousal loss is an integrative process in life after crisis, and can be conceptualized as the process of overcoming the despair that immediately follows the death of a spouse, seeking a new life by actively taking control, and discovering a strengthened self.

A Study on the Origin of The Triple Value(三達尊) in Ancient China-Mainly with the Aged Consciousness in the Book of Odes (중국 고대 삼달존(三達尊) 사상의 연원 고찰 - 『시경(詩經)』에 보이는 기로의식(耆老意識)을 중심으로 -)

  • This article is designed to study the conceptualization process of one of the ancient Chinese classical literature heritages, the so-called, the Triple Virtue(三達尊). By showing the principle meanings and the newly embodied symbolism of this Triple Virtue, this article is prepared to encourage revitalization of the moral virtues and self-identical pride among the elderly and to promote the young people's social consciousness of respecting the elderly. The author identifies the philosophical origins of the Triple Virtue, implying that the virtuous trinity is composed of morality, position and age, by analyzing poems in "Daya(大雅)", "Xiaoya(小雅)" of the Book of Odes and archives in "Zhoushu(周書)" of the Book of Documents(尙書). The author especially emphasizes that the concept of Triple Virtue was created by governing classes for meeting the political needs in the Zhou Dynasty. Moreover, by regarding King Wen of the Zhou Dynasty as the symbolic representation in the beginning era of the Western Zhou Dynasty and Shao BoHu as the embodiness representation in the end of the Western Zhou Dynasty, the author performs an in-depth study related to the above two great men. Finally, the author sheds lights on how symbolic and embodiness representations had played significant roles in formulating a typical model of the Triple Virtue in the following generations.

Health and quality of life for Korean people in ageing society (고령화 사회에서 한국인의 건강과 삶의 질)

  • Korean society is rapidly becoming an ageing society. The Korean may have to live longer than did their parents. Increasing life expectancy and changing social structure, Korean people are getting interested in quality of life, and well-being is becoming a matter of concern. And, the Korean is fully aware of the importance of health for well-being or good life. This concern about health may bring about specific behaviors related to health. Although health care expenditures of Korea are currently smaller than those of other developed countries, it is continuously increasing. Large portion of increased amount of health care expenditures is to spend for disease prevention and expansion of long-term care facilities. Constructs of well-being of the Korean, not living in western culture, may be different from those of people living in western society. Health is not top-ranked to importance for quality of life in previous studies. It does not mean health isn't determinant factor for good life or well-being. Health is an essential element for well-being. It has been proved in several researches which examined poor quality of life caused by certain diseases and management of health-related quality of life. Some theories relate to health-seeking behaviors suggested the health belief or the attitude toward health, intention to do health behavior, perceived behavioral control, and self efficacy as important factors which could predict health-related behaviors. With getting older, people decline in physical and physiological functions and become vulnerable to chronic diseases. Quality of life depends on how to adjust to these changes in senescence. Social supports, especially supports from offspring, are very important to quality of life in senescence, because supports from offspring have influence on pride of the older, they may be afraid of disclose the conflict with their offspring. Avoiding self-disclosure exclude other source of social supports and harm individual's health, therefore psychological intervention is needed to. Increasing life expectancy of the Korean, Korean government has to provide numerous long-term care facilities as well as psychosocial supports. The Korean, so far, does not recognize that psychologist could render great service to promoting individual or community health and improving individual's quality of life. It is highly expected that psychologists take actively interested and involve in health related to quality of life.
