AN EPIDEMIOLOGIC STUDY ON THE PEDIATRIC PATIENTS IN DEPARTMENT-OF PEDIATRIC DENTISTRY, CHOSUN UNIVERSITY DENTAL HOSPITAL FOR LAST 10 YEARS(1990-1999) (최근 10년간 조선대학교 치과병원 소아치과에 내원한 신환에 관한 역학적 연구(1990-1999))

  • The purpose of this study was to make public oral health plan more effective and the improvement of the hospital management system for better clinical dental practice. Distribution and trends were examined in all patients who had been examined and diagnosed at Department of Pedodontics, Dental Hospital, Chosun University over 10 year-period from 1990 to 1999 Results were as follows, 1. The number of patients per year was increasing trend after 1996 and higher visiting rate in male(55.9%) than in female(44.1%). 2. Age distribution had shown $3\sim4$ year-old cup being the largest (23.4%) and each percentage of $5\sim6,\;7\sim8,\;0\sim2,\;9\sim10,\;11\sim12,\;13\sim14,\;above\sim15$ year-old group was 19.9%, 17.7%, 14.6%, 12.6%, 8.0%, 3.3%. 0.5%. 3. Geographic distribution showed a majority of patients in Kwang-Ju (83.0%). Group in the suburbs of Kwang-Ju(Jang-Sung group) was 5.4%. 4. Dental caries showed the highest percentage(40.5%) in chief complaints and percentage of oral exam, orthodontic problem, oral pain, trauma, supernumerary teeth, swelling was 13.9% 12.6%, 8.8%, 7.4%, 5.5%, 4.9%. 5. In all patients with traumatic problem, crown fracture showed the highest percentage(41.4%) and percentage of subluxation was 19.4%. 6. Majority(78.7%) of patients were not refered, and percentage of patients refered from local clinic was 20.5%. 7. Patients who had periodic check-up comprised 19.6%, and percentage of after 3, 6, 9, 12, 18, 24month was 36.7%, 22.2%, 13.5%, 11.3%, 5.4%, 11.0%.

THE DISTRIBUTIONS AND TRENDS IN MALOCCLUSION PATIENTS - A 10 year study of 2155 patients from YDSH (연세대학교 영동세브란스병원 교정과에 내원한 부정교합 환자의 분포 및 경향에 관한 연구)

  • Distributions and trends were examined in 2155 malocclusion patients who had been examined and diagnosed at Deparment of Orthodontics in Youngdong Severance Hospital over a 10 year-period from 1984 to 1993. The results were as follows ; 1. The number of patients per you had nearly quadrupled during the 10 year-period, with females($58.3\%$) outnumbering males ($41.7\%$). 2. Age distribution had shown 7-12 year-old group being the largest($53.3\%$), but percentages of 7-12 year-old and 13-18 year-old groups had been decreasing while that of above-19 patients had gradually incresed. 3. Distributions in the types of malocclusion according to the Angle's Classification had shown $27.8\%$ for Class I, $22.6\%$ for Class II division 1, $7.9\%$ for Class II division 2, and $41.6\%$ for Class III. Percentages of Class I patients had decreased while percentages of Class II and III patients had increased. 4. Non-extraction cases ($75.4\%$) had outnumbered extraction cases ($24.6\%$), with a general trend toward fewer extraction cases over the 10 year period. 5. Patients who had orthognathic surgery had been increasing, with Class I, II, and III cases comprising $8.8\%,\;16.9\%\;and\;74.3\%$, respectively. There had been three times as newly 2-jaw operations as 1-jaw operations. 6. Nearly $4\%$ of the subjects had facial asymmetries, with Class III cases being the most frequent. 7. Patients with TMD symptoms comprised $4.6\%$, with an increasing trend. The symptoms had occurred more frequently in older patients and in females. 8. Geograpic distributions showed majority (3/4) of patients from the nearby areas, namely Gangnam-gu, Seocho-gu and Songpa-gu.

  • 본 연구는 환자의 의료기관 선택기준에 관한 조사를 실시하는데 있다. 외래, 중 병원에서 검사한 사항을 CD로 담아 타 병원 진료 후 현 병원에 내원한 대상자와 본 병원에서 진료 후 CD를 가지고 타 병원으로 전원 하는 환자 및 보호자를 대상으로 2009년 10월 5일부터 10월 26일까지 본 연구에 동의하는 대상자 302명을 대상으로 하였다. 내원 전원 환자의 물리적 특성에서 주차시설, 주위시설, 의료진 서비스 품질에서는 간호사 친절도, 전문성, 설명, 의료 서비스 품질 특성에서는 의료기기 최신설비, 병원 평판에서는 의료인력 의술, 주위 환경, 병원 외적 요인에서는 진료의 권유 항목이 유의하였다(p<0.05). 내원 시 병원 선택 변수 평균치 검정 결과 중 성별에서는 만족도, 연령별에서는 의료진 서비스품질, 거주지에서는 병원외적요인, 병원지역에서는 병원평판, 만족도, 진료기관에서는 병원평판, 만족도, 진료구분에서는 만족도에서 유의하였다(p<0.05). 전원 시 병원 선택 변수 평균치 검정결과 중 연령별에서는 의료진 서비스 품질, 거주지에서는 병원외적요인, 병원지역에서는 병원평판, 만족도, 진료기관에서는 병원평판, 만족도, 진료 구분에서는 만족도가 유의하였다(p<0.05). 본 연구에서는 내원 전원 환자는 물리적 환경, 의료서비스 품질, 병원 평판 등 을 병원 선택에 중요한 요인들로 지각 하고 있으며 간호사와 주위 사람들의 권유에 의하여 병원을 선택한다고 나타났다. 내원 환자는 의료진 서비스 품질, 병원 평판, 만족도에서 평균치가 높게 나타났다. 전원 환자는 물리적 환경, 의료진 서비스품질, 의료 서비스 품질, 병원 평판, 만족도에서 평균치가 높게 나타났다. 자료 분석은 SPSS 12.0 Window Program을 이용하였다.

A Study of Types and Distribution of Orthodontic Patients in the Department of Dentistry (치과에 내원한 교정환자 실태에 관한 연구)

  • 174 patients who visited in the Department of Dentistry, College of Medicine, Yeungnam University from 1983. 7. 15 to 1986. 11. 30 were surveyed on the tendency of patient distribution and the state of Angle's classification. The results were as follows : 1. There was increased visiting rate of patient per year. Female outnumbered male by a ratio of 1.3 : 1. 2. 8-15 age group was 74% in total visiting in the most frequency. The average visiting numbers of each month were higher among the vacation and was about 74% in that period. 3. As showed the living distribution, Nam Gu and Su Sung Gu's patients were 53% of the total. 4. As the motive of visiting in Dental Department, patients of 30% in total patients visited for themself to receive orthodontic treatment and others was introduced patients. 5. By classification, Class I malocclusion was 63% in total visiting patient, Class II malocclusion 17%, ClassIII malocclusion 20%. Crowding among Class I malocclusion was 60%.

  • The number of patients requesting services of pediatric dental clinics has been steadily increasing in South Korea. The pediatric dental clinics are comprised of hospital-based clinics and local clinics. The purpose of this study was to analyze new-patient distribution and to survey the motives for visiting the department of pediatric dentistry at Seoul National University Dental Hospital(SNUDH), utilizing questionnaires for parents from May to July 2010. The results are as follows: 1. Age distribution showed that 0-3 year-olds formed the largest group of 29.3%. The average age of patients was 6.2 years old. 2. In the surveys for the parents, the major reason for visiting SNUDH was the referral from other clinics(30.6%). 3. The 75% of patients previously visited other clinics before visiting SNUDH. 1) The greater number of patients(52%) visited general practitioner's clinic than pediatric specialists(48%). 2) More than 90% of the patients visited SNUDH after visiting other clinics for reassuring diagnosis and referrals. 3) The crucial deterring factor of being treated at SNUDH was difficult accessibility.

Factors Associated with the Prehospital Delay in Acute Myocardial Infarction (급성 심근경색증 환자의 병원내원시간 지연에 관련된 요인)

  • In patients with acute myocardial infarction (AMI), the delay from symptom onset to hospital arrival has a critical effect on morbidity and mortality. This study examined to find out the determinants of the prehospital delay in patients with AMI. The study sample consisted of 597 patients hospitalized with AMI between Jan and Dec 2009. Demographic, medical history, and clinical data were abstracted from the hospital medical records of patients with confirmed AMI, the prehospital delay was categorized as less than or greater than 6 hours. Older age, low socioeconomic status(medical aid), and low use of Emergency medical system were associated with delays in seeking emergency care for Acute myocardial infarction. Education programs to improve patient knowledge of acute coronary syndrome symptoms and promote patient responsiveness with regard to seeking medical care should be used to reduce the prehospital delay time, especially in the low socioeconomic group.

Characteristics of patients with decreased mental status while staying in an emergency department: Comparative study with alert patients (응급실 체류 중에 의식이 저하된 환자의 특성: 의식명료 환자와의 비교연구)

  • This was a retrospective study that compared the characteristics of patients with decreased mental status (DMS) during their stays in an emergency department (ED) with that of patients with alert mental status (AMS). The participants in this study consisted of 90 patients with DMS who were staying in an ED along with 100 randomly selected patients with AMS. The DMS group contained more instances of cerebrovascular disease, liver disease, diabetes, and hypertension compared to the AMS group (p=0.001, p=0.003, p<0.001, and p=0.002, respectively). With regard to triage, the DMS group had higher severity than the AMS group (p<.001). The final diagnosis for the DMS group was as follows: 64.9% neurological and 63.3% related to the digestive system, which differed from that of the AMS group (p<.001). Fifty-six (62.2%) patients with DMS had their statuses discovered by nurses, while nine patients in the DMS group had received cardiopulmonary resuscitation. These results suggest that ED nurses frequently assess the consciousness of patients who they suspect may have neurologic or digestive diseases and are classified as high severity, even when the patients are alert.

THE STUDY ON THE ORTHODONTIC PATIENTS WHO VISITED DEPARTMENT OF ORTHODONTICS, SEOUL NATIONAL UNIVERSITY HOSPITAL DURING LAST 10 YEARS(1985-1994) (최근 10년간 서울대학교병원 교정과에 내원한 부정교합 환자에 관한 고찰 (1985년 - 1994년))

  • With socioeconomic development and change of esthetic recognition, the demand for orthodontic treatment and number of orthodontic patients has been increasing so rapidly. And frequency of malocclusion was changed. So this study was done in an attempt to provide an epidemiologic study so that we can accomodate their orthodontic needs adequately and to obtain the reliable quantitative information regarding the characteristics of orthodontic patients who visited Department of Orthodontics, Seoul National University Hospital from 1985 to 1994. Following results were obtained. 1. The total number of orthodontic patients of SNUDH during 1990-1994 increased in comparing with that of 1985-1989. And it showed that the number of annual patients was increasing trend. 2. The total number of female patients was 1.59 times as high as that of male. It showed that the annual percentage of female patients has been increasing and that of male patients has been decreasing. So demands for orthodontic treatment of malocclusion of female patients were higher than that of male patients. 3. Each total percentage of class I, class II div.1, class II div. 2 and class III was 35.98%, 14.00%, 1.74% and 48.28%. The annual percentage of class I group had been decreasing but it has increased at 1994. However that of Class III group had been increasing until 1991 but it has been decreasing. 4. Each percentage of less than 6 year-old group, D to 8 year-old group, 8 to 12 year-old group, 12 to 18 year-old group and more than 18 year-old group was 2.65%, 8.63%, 32.50%, 27.74% and 28.48%. Annual percentage of 12 to 18 year-old group ( middle & high school students group ) had been decreasing but it has been increasing. However 18 year-old group had been increasing but it has been decreasing. So entrance examination for college and university is an important factor to distribution of age group.
