Effect of Visual acuity for Change of Astigmatism axis in Myopic astigmatism (근시성 난시안에서 난시축 변화가 시력에 미치는 영향)

  • The purpose of this study is to compare the status of eyesight according to the change of astigmatism axis in myopic astigmatism and to minimize errors in making astigmatic glasses based on accurate optometry and prescription. The subjects were 93 males and females(186 eyes) who have myopic astigmatism without any ocular disease or systemic disease. We performed comparative analysis on the status of visual acuity according to the change of astigmatism axis to 5, 10 and 15 degree in corrected eyesight 1.0. The direct astigmatism was the most common astigmatism type among the 186 eyes. After all subjects were perfectly corrected into 1.0, the change of astigmatism axis affected eyesight; The results suggested that the more change was made in astigmatism axis, the worse their eyesight would become. The main astigmatism type was changed from direct astigmatism to inverse astigmatism as age increased. The change of the astigmatism axis resulted in failing of corrected eyesight. Therefore, the convergence of examination and correction for astigmatism strength and axis is necessary when conducting refraction inspection for astigmatism.

A Study for the Change of Astigmatism Axis When the Fixation Point Moved Far Distance to Near Distance (원거리에서 근거리 주시 시 난시축 변화에 대한 연구)

  • When the fixation point moved far distance to near distance, the change of astigmatism type in total astigmatism showed no-changed eyes: 1,020 eyes (63.8%), changed eyes: 578 eyes(36.1 %). The change of with-the-rule into oblique-astigmatism, oblique-astigmatism into with-the-rule, against-the-rule into oblique-astigmatism of change was plenty the most respectively. In corneal astigmatism was no-changed eyes: 1,164 eyes (72.8%), changed eyes: 434 eyes(27.1%). The change of with-the-rule into oblique-astigmatism, oblique-astigmatism into with-the-rule, against-the-rule into with-the-rule astigmatism was plenty the most respectively. In residual astigmatism is no-changed eyes: 935 eyes(58.5%), changed eyes: 663 eyes(41.4%). The change of with-the-rule into oblique-astigmatism, oblique-astigmatism into against-the-rule, against-the-rule into oblique-astigmatism was plenty the most respectively. When the fixation point moved far distance to near distance, the change of astigmatism axis in total astigmatism was no-changed eyes: 761 eyes(48.5%), cyclotorsioned eyes the above 10 degrees: 837 eyes(52.3%). In corneal astigmatism was no-changed eyes: 846 eyes(52.9%), cyclotorsioned eyes the above 10 degrees : 752 eyes(47%). In residual astigmatism was no-changed eyes: 614 eyes(38.4%), cyclotorsioned eyes the above 10 degrees : 984 eyes(62.5%). The magnitude of cyclotorsion of astigmatism axis in total astigmatism was Counter clockwise rotation: 31 degrees, clockwise rotation: 20 degrees. In coneal astigmatism was Counter clockwise rotation: 25 degrees, clockwise rotation: 27 degrees. In residual astigmatism was Counter clockwise rotation: 33 degrees, clockwise rotation: 35 degrees.

A Study on Variation of Astigmatism Axis according to the Age (연령에 따른 난시축 변화에 관한 연구)

  • In this study, the variation of astigmatism axis according to the age was studied 4227 eyes of healthy eyes. The refractive errors were measured by objective and subjective refraction. The astigmatism among the total eyes was 76.5%. With-the-rule, against-the-rule and oblique astigmatism were 51.4%, 31.7% and 16.9%, respectively. The frequency of with-the-rule was the highest and against-the-rule was the lowest of all the astigmatism in the refractive error. In relation to the age and astigmatism, the with-the-rule was 68.3% in the 10's age group which was the highest among the astigmatism and the against-the-rule was 10.3% in the 50's age group and upward which was the lowest among the astigmatism. The difference in the with-the-rule and against-the-rule ratio was a little in the 30's age group. As they aged, the tendency of the with-the-rule was increased between 10's and 20's age group continually. And the tendency of the against-the rule was increased between 30's and 50's age group and upward. There was a decrease in with-the-rule and an increase in against-the-rule, with the age increase. The oblique astigmatism relationship to age could not be determined.

A Study on the Accommodative Astigmatism of Near Vision (근거리 주시 시 조절성 난시에 대한 연구)

  • Purpose: The purpose of this study was an accurate grasp of the astigmatic change due to eyes accommodation on near vision and suggested problems occurring accommodative astigmatism using near glasses. Methods: 154 subjects(308eyes) from 20 to 40 years selected for this study who had many opportunity of near vision. First, far vision corrective refraction measured using the phoropter, and then both eyes opened simultaneously for maintaining the function of binocular put away dot chart 40 cm. One eye was fogging and the other eye measured near vision astigmatism axis and degrees astigmatism using cross cylinder, and then compared with far astigmatism. Results: Increased degree of astigmatism persons on near vision more than decreased or did not changed degree of astigmatism persons, which could affect visual acuity more than 0.75 diopters in 30 eyes with an increase of 9.7% of total subjects. Direct astigmatism and oblique astigmatism were increased on near vision. Astigmatic axes were rotated base in direction on both eyes and 66.9% of subjects were more than ${\pm}$5$^{\circ}$ rotation. Due to the rotation axis of astigmatism, residual astigmatism is expected to occur and expect to adversely affect the eyes. Conclusions: Long-term using the glasses without correction of increased astigmatic and rotated axis on near vision should occur new residual astigmatism and increase the asthenopia also. Considered to be taken astigmatic change on near vision during near refraction examination.

Ten-year Refractive Error and Astigmatism Changes in Korean Subjects (한국인의 10년간 굴절이상과 난시 축 변화)

  • Purpose: This study was to investigate the changes of refractive error and astigmatism associated with age in Korean subjects between the ages of 6 and 80 years during 10-year period. Methods: 220 normal subjects (345 eyes) who visited ophthalmic clinic was recruited and followed for 10 years between 1999 and 2009, cycloplegic manifest refraction being performed annually. Visual acuity was tested on a Han's chart. Results: The mean 10-year change in the spherical equivalent refraction (SER) of age 6 to 10 years old and 10 to 20 years was -3.649D and -2.165D respectively. There was no change of refractive error in age 21 to 40 years. The myopic shift decreased with age from 41 up to 69 years but increased slightly in patients 70 years and older; the hyperopic shift showed the opposite trend. The distribution of refractive error over the 10 years in aged 6 to 10 and 11 to 20 years was shifted myopic. The incidence of medium (> -3.01D) to high myopia at age 6 to 10 years was 4.8% and after 10 years was 62.5%. The 10-year change of astigmatism axis was in "with the rule" direction for younger age group and in a "against the rule" direction for older subjects. Conclusions: This study has documented refractive error changes in Korean subjects and confirmed reported trends of myopic shift from age 6-20 years and hyperopic shift before age 70 years and a myopic shift thereafter. The axis of astigmatism turns to "against the rule" after 40's.

Axial Rotation of Toric Soft Lens by Corneal Astigmatism and Change of Posture (각막난시와 자세 변화에 의한 토릭소프트렌즈의 축 회전)

  • Purpose: The present study was conducted to investigate the axial rotations of toric soft lens during the change of lens wearer's posture, and the relationship between its rotation and corneal astigmatism. Methods: The amount, direction, and speed of toric soft contact lens rotation were measured for 42 eyes (aged 20s) with the rule astigmatism in the straight and lying postures, and it compared between their changes according to corneal astigmatism. Results: There was no significant difference in the axial rotation of lens for the astigmatism prescription between the straight and lying postures. However, the rotation angle was significantly different according to the posture of lens wearer. Rotating directions in straight posture were nasal direction for 20 eyes and temporal direction for 22 eyes. In lying posture, lenses of most wearers were rotated to a direction of lying posture, and the initial rotating speed was very fast in initial wearing for -0.75 D toric lenses, but consistency for -1.25 D toric lenses. The rotation angle in lying posture showed significantly different according to the amount of corneal astigmatism, the lens speed was also significantly different according to the wearing time but not the amount of corneal astigmatism. Conclusions: The axial rotation of toric soft lens was different by the lens wearer's posture and its amount was the greater with the higher degree of corneal astigmatism. Thus, these factors should be considered for the development of toric lens design.

A Study on the Relationship Between the Off-Axis Cylinder and Corrected Vision of Astigmatism (난시안의 교정축 이탈과 교정시력과의 관계 연구)

  • This study was classified and compared astigmatism's refractional abnormal degrees with visual acuity state of full correction which turned on axises of only 5 degree, 10 degree, and 15 degree. Subjects of this study were 57 college students (114 eyes) who had neither eye diseases nor binocular abnormality, were from their twenties to fifties, with myopia. It appeared that 30.8% of subjects who had astigmatism wore glasses with wrong axis of astigmatism. After accurate correction of the visual acuity and degrees of astigmatism, when we moved to corrected axises at 5 degree, 10 degree, 15 degree, failure of visual acuity with one line or more were 56.1%, 84.2%, 93.8%, respectively. When we comapre the completely-corrected visual acuity with the visual acuity with dricted axes, the bigger the width of visual acuity's weakness was the bigger the drifted angle. The change of normal visual acuity according to drifting angle of corrected axises of astigmatism, when we compared with full correction, appeared 0.94 in 5 degree, 0.87 in 10 degree, and 0.79 in 15 degree. Drift of 5 degree from fully corrected axis, corresponded to difference of visual acuity about one line, drift of 10 degree to 1.8 line difference of visual acuity, and drift of 15 degree about to 2.6 line difference. Through this study, we were sure that, in the case of drifting away from the right axis of astigmatism, it lead to visual weakness and asthenopia. Therefore we darely advise that optometrists should make mistake of axis least by confirming accucacy of corrected axis after despensing of spectacles of astigmatism.

A Clinical Study for Total Astigmatism in Korean (한국인 전체난시에 관한 임상적 연구)

  • In this study, total astigmatism and corneal astigmatism were studied 328 eyes of healthy Korean for astigmatism. Total astigmatism was measured by fogging-and-dial technique, and corneal astigmatism was measured with Topcon OM-4 keratometer. Residual astigmatism was determined by a discrepancy between total and corneal astigmatism. The results were as follows: As they aged, there were a statistically significant changes in total and corneal astigmatism but not in residual astigmatism. And the amount of with-the-rule total and corneal astigmatism had a tendency to decrease, but residual astigmatism had no changes. From 30 years, total astigmatism showed a tendency to become against the rule. The average diopteric value was +0.342D in total astigmatism. +0.920D in corneal astigmatism and -0.579D in residual astigmatism.

Changes of Corrective Astigmatism Values Depending on Position of Circle of Least Confusion in Astigmatic Refining Test Using Cross Cylinder (크로스실린더를 이용한 난시정밀검사에서 검사 전 최소착란원 위치에 따른 난시교정값의 변화)

  • Purpose: To investigate the changes of corrective values of astigmatism caused by the position of circle of least confusion on retina in refining astigmatic test using cross cylinder. Methods: 62 subjects (115 eyes) aged $22.24{\pm}2.48$ years participated for this study. After astigmatic test using a radial chart, refining test was performed using a cross cylinder in a condition of maximum plus to maximum visual acuity (MPMVA). Astigmatic refining test was repeatedly performed in each condition of which S+0.75 D, S+0.50 D, S+0.25 D, S-0.25 D, S-0.50 D, and S-0.75 D are added to spherical lenses of MPMVA. The measured values were compared with the values in MPMVA condition. Results: As compared with values in condition of MPMVA, change of astigmatic axis was increased with add the power of (+) spherical lenses and (-) spherical lenses. In same spherical condition, change of astigmatic axis was decreased with increment of astigmatic power (p<0.05). The corrective power of astigmatism was reduced with increment of (+) spherical lenses (p<0.05), and was raised with increment of (-) spherical lenses compared with the power in MPMVA condition. In case of adding (+) spherical lenses, difference of astigmatic power increased with increment of corrective astigmatism power in same test condition. Conclusions: In order to obtain a proper values for corrective astigmatism, position of circle of least confusion should be accurately adjusted before the performing an astigmatism's refining test.

A Study of Accommodative Response on Emmetropia (정시안의 조절반응량에 관한 연구)

  • The purpose of this research was to measurement real accmmnodative response amount of emmetropia using both eyes open-view auto-refractometer. We classified 377(male 146, female 231)eyes showing good naked vision at least 1.0, with a spherical equivalent refraction of under ${\pm}0.50D$ and cylindercal refraction of under -0.75D, from 10 to 59(mean $25.8{\pm}12.4$) years into 5 groups with 10 year interval and analyzed change of the near astigmatism and accommodative response amount to distinction of age. Age comparison, total mean 1.47D, 10 years age group 1.63D, 20 years age group 1.57D, 30 years age group 1.71D, 40 years age group 0.97D, and 50 years age group 0.05D. It was no difference between 10 to 30 years age group, But it showed the decre8.se which is remarkable from above 40 years age group and it does not almost response from above 50 years age group. Sexual difference, the man appeared some highly in male(mean $26.4{\pm}12.3$ years) 1.56D m female(mean $26.4{\pm}12.3$) 1.43D. Near-astigmatism power change, mean +0.12D, 10 years age group +0.06D, 20 years age group +0.16D, 30 years age group +0.09D, 40 years age group +0.05D, and 50 years age group +0,06D. No significant difference of astigmatism powers among the an groups, but from all years age group the astigmatism power appeared more highly far-astigmatism than near-astigmatism. Near-astigmatism axis change in the far-astigmatism changes at under ${\pm}10^{\circ}$ 103 eyes, above ${\pm}10^{\circ}$ rotation clockwise direction 108 eyes, above ${\pm}10^{\circ}$ rotation counter-clockwise direction 101 eyes. It was analyzed near-astigmatism axis change.
