The purpose of this study was to investigate the frequency of falls, fear of falling, perceived health status, and number of disease according to cognitive function in community-dwelling the elderly women. This study utilized a descriptive survey design. Data were collected for six months with 311 elderly women who visited in public health center at S city. After obtaining Institutional Review Board(IRB) approval, a face-to-face, and private interview was conducted with each participant who was eligible and agreed to participate in this study by trained graduate-level nursing students. The questionnaires consisted of MoCA-K, K-MMSE, Fall Efficacy Scale, PHS scale. The collected data were analyzed with SPSS/WIN 17.0 program, which was used descriptive statistics, Chi-Square test and t-test, Correlation. The major findings of this study were as follows; 1) The average age of the subjects was 71.68±5.13 and cognitive function score was 22.14±4.32. approximately 35% of participants had fallen within one year. 2) there were significant differences in perceived health status according to cognitive function. 3) fear of fall and cognitive function, perceived health status and cognitive function were significant correlation. In conclusion, this study will contribute to establish strategies for preventing fall. Interventions for fall prevention and fear of fall enhancement should be developed with the consideration of the level of cognitive function in the elderly. Reducing the fear of falling through the comprehensive nursing intervention will ultimately improve the quality of life in the elderly.

This study was conducted to identify the predictors influencing fear of falling in community-dewelling elderly. A secondary data analysis was performed using data of 65 years or older elderly who participating in the 2014 Korean Elderly Survey(Jun. 11th ~ Sep. 4th 2014). Multivariated logistic regression analysis was used to identify the risk factors of fear of falling. 81.5% of the elderly had fear of falling. There were significant factors, sex(OR=2.87, p<.001), education(OR=1.73, p<.001), presence of cohabiting spouse(OR=1.19, p<.001), experience of falling(OR=6.35, p<.001), arthritis(OR=1.83, p<.001), depression(OR=2.25, p<.001), cognitive deficit (OR=1.24, p<.001), instrumental activities of daily living (OR=1.98, p<.001), exercise(OR=1.29, p<.001) and the number of chronic diseases(over 3: OR=2.53, p<.001, 1~2: OR=1.73, p<.001), predicting fear of falling in community-dwelling elderly. Fear of falling was associated with multifactorial condition, and arthritis was an important factor in chronic disease. This study will provide fundamental information on programming and a policy proposal related to fear of falling.
The purpose of this study was to investigate the changes in temporospatial variables in healthy elderly and healthy adults during usual walking, narrow base walking and centerline-guided walking. Twenty healthy elderly and nineteen healthy adults were participated in this study. In each conditions, the subjects were walked on a 6m walkway at comfortable self-selected speeds under three conditions : (1) usual walking, (2) walking within a 50% of the distance between the subject's ASIS (3) walking along a centerline. GAITRite system was used for kinematic analysis to assess the temporospatial variables. There were no significant changes in healthy adults(p>.05), but walking speed, cadence, H-H base support, functional ambulation performance were significantly decreased progressively as pathway narrowed in elderly adults(p<.05). The results show that elderly people had more difficulty with walking on narrow pathway for fear of falling. This study provides data for use in basic research into safe walking and preventing falling for elderly.
Purpose: The purpose of this study was to investigate the experience of falls and identify risk factors associated with falls. Methods: The participants were 233 community-dwelling elderly people in two cities. Data were collected by interviewing the elderly with structured questionnaires from May to June, 2012. For data analysis, descriptive statistics and multiple logistic regression were performed using SPSS version 17.0. Results: Age, educational level, living arrangement, health status, the use of assistive devices, the number of medication, hypertension, arthralgia, dizziness, fall efficacy, fear of falling, activity of daily living, and quality of life were significant associated with fall experience. Through multiple logistic regression analysis, arthralgia, dizziness, fall efficacy, and fear of falling were identified as significant predictors of falls. Conclusion: It is important to identify the risk factors for falls among the elderly living in community. Fall prevention interventions should be multifactorial, especially for the elderly who were identified as the high risk group.

고령화 사회로 진입하면서 노인들은 노화과정에 의한 보행능력의 감소 및 근력 약화와 같은 신체적 변화로 인해 잦은 낙상을 경험한다. 이에 따라 낙상 사고를 감지하는 연구가 활발히 진행되고 있다. 낙상은 사전 예방도 중요하지만 사고 발생 후의 신속한 대처도 중요하다. 낙상을 감지하고 의료진에게 즉시 낙상정보를 제공하여 후속적 조치를 취하는 것은 사고 후 대처의 핵심이다. 본 논문에서는 스마트폰 환경에서 사용자의 낙상 후 방향을 판별하기 위해 두 가지 센서 데이터의 특정 값들을 추출하였으며, 이에 5 가지 기계학습 알고리즘을 적용하였다. 사용자는 스마트폰을 착용한 상태로 전후좌우 4 방향 낙상 실험을 진행하며 스마트폰 내에 내장된 3 축 가속도 센서와 3 축 자이로 센서값을 측정한다. 피험자 11 명을 대상으로 낙상 실험 결과, 5 가지의 분류기 중 k-NN에서 98.6%의 인식률을 나타내었다. 뽑아낸 특징 값과 분류 알고리즘은 낙상의 방향 검출에 유용한 것으로 판단된다.

Purpose: The purpose of this study was to provide basic data to develop a Fall Prevention Education Program by comparing and analyzing fall experiences and the factors that influence elderly people with dementia suffering falls. Methods: The participants were 302 patients with dementia aged 60 years or older with nursing records of hospitalization in three nursing homes located in a metropolitan area. The SPSS/Win 21.0 package was used to analyze the collected data. A logistic regression analysis was performed to identify the influence factors related to fall experiences. Results: For men, fall experiences were more frequent given the following factors: overweight; high fall risk tendency; aged 90 and above; married; dementia duration period of 5 to 9 years; and taking medication for dementia. For women, the factors included age range of 80 to 89; overweight; obese; both low and high fall risk tendency; separation by death; and having spent less than 1 year in nursing homes. Conclusion: We anticipate positive results in fall prevention education programs for the elderly with dementia if the results of this study are used as basic data, and interventions are customized to consider the sex and the relevant influence factors as to fall experiences.
Purpose: The purpose of this study was to investigate the prevalence, characteristics, and factors related to falls occurring among the community-dwelling elderly. Methods: The subjects were 299 community-dwelling-elderly in Jeju Province. Data were collected via in-person interviews by five visiting nurses in October, 2012. Fall risk assessment tools developed by the CDC, K-ADL, HDS-K, and GDS were used for data collection. Results: A significant number of subjects (34.1%) had experienced a fall at least once within the last six months. Living arrangements, strokes, urinary incontinence, main daily activities, and depression were significant factors related to these falls. Other risk factors included heavy use of medications, difficulties in walking and standing, needing arms to stand up from a chair, and hearing loss. Conclusion: The results reveal critical factors related to falls among the community-dwelling elderly. These information should be used to develop and implement fall prevention programs in communities.
Purpose: This study aims to determine the association between fear of falling, falls efficacy, and fall prevention behaviors in older adults in outpatient and identify the factors affecting fall prevention behaviors. Methods: This was a descriptive study. A total of 137 older adults aged 60 or older with more than one disease in an outpatients setting were recruited from a university hospital in Seoul. Data were collected from March 11 through April 8, 2016. Results: The experience of a fall was significantly associated with age (t=-2.71, p=.008), gender (𝛘2=14.32, p<.001), education (𝛘2=6.05, p=.049), income (𝛘2=9.93, p=.019), physical activity status (𝛘2=16.84, p<.001), and the number of diseases (t=-2.89, p=.004). Fall experience was not significantly associated with marital status (𝛘2=3.79, p=.285). Multiple regression analysis revealed that the most influential factor of fall prevention behaviors was the fear of falls (β=-.37, p<.001), followed by the number of diseases (β=-.20, p=.013). Conclusion: It is necessary to provide preventive fall education with older adults in outpatients settings considering the fear level of falling and the numbers of accompanying diseases.

The purpose of this study was to investigate the effects of health exercise program to old Women's walking ability, depression and WHOQOL-BREF. Data were collected from April to June, 2007 from the falls experienced 70 women elderly. All subjects participated in 12 week health exercise program which was designed in order to develop walking ability. The data were analyzed using frequency, %, paired t-test. The results of this study were as followings; First, there was significant differences in the average time of chair stand (t=2.291, p=.025), one leg standing(Rt. leg)(t=2.236, p=.029), step length between before and after(t=4.015, p=.000) training of 12 week health exercise program. Second, there was non significant differences in depression(t=1.044, p=.300) but, significant differences in WHOQOL-BREF(t=3.528, p=.001). The WHOQOL-BREF in general quality of life(t=2.923, p=.005), physical(t=3.039, p=.003), psychological(t=2.481, p=.016), social(t=2.531, p=.014) and environment domain(t=4.259, p=.000) were significant differences. The results suggest that the 12 week health exercise program can improve the muscle endurance and balance, QOL.
Objective: The purpose of this study was to investigate the local stability of the lower extremity joints and muscle activation patterns of the lower extremity during walking between falling and non-falling group in the elderly women. Method: Forty women, heel strikers, were recruited for this study. Twenty subjects (age:72.55±5.42yrs; height:154.40±4.26cm; mass:57.40±6.21kg; preference walking speed:0.52±0.17m/s; fall frequency=1.70±1.26 times) had a history falls(fall group) within two years and Twenty subjects (71.90±2..90yrs; height:155.28±4.73cm; mass:56.70±5.241kg; preference walking speed: 0.56±0.13m/s) had no history falls(non-fall group). While they were walking on a instrumented treadmill at their preference speed for a long while, kinematic and EMG signals were obtained using 3-D motion capture and wireless EMG electrodes, respectively. Local stability of the ankle and knee joint were calculated using Lyapunov Exponent (LyE) and muscles activation and their co-contraction index were also quantified. Hypotheses were tested using one-way ANOVA and Mann-Whitey. Spearman rank was also used to determine the correlation coefficients between variables. Level of significance was set at p<.05. Results: Local stability in the knee joint adduction-abduction was significantly greater in fall group than non-fall group(p<.05). Activation of anterior tibials that acts on the foot segment dorsal flexion was greater in non-fall group than fall group(p<.05). CI between gastrocnemius and anterior tibials was found to be significantly different between two groups(p<.05). In addition, there was significant correlation between CI of the leg and LyE of the ankle joint flexion-extention in the fall group(p<.05). Conclusion: In conclusion, muscles that act on the knee joint abduction-adduction as well as gastrocnemius and anterior tibials that act on the ankle joint flexion-extention need to be strengthened to prevent from potential fall during walking.
