In this study, an augmented reality (AR) app that detects the screw-assembly state of a car valve-body and assists the assembly work is developed and the effectiveness of the app is shown through testing. The app creates the contents indicating the screw-assembly position and order, and the screw-assembly state. Then, the contents are registrated onto the valve-body image on a smart-phone screen to be shown to the worker during assembly. To this end, the features are extracted from the 2D image of the valve-body and the location of the valve-body is tracked. By extracting the areas where the screws are to be assembled, and periodically determining the luminance of these areas, it is checked whether the screws are assembled in order at the predetermined position of the valve-body. When an error is detected during assembly, a warning sound is notified to the worker, and the worker can check the assembly state on the smart-phone screen and handle the error, immediately. Study results found that it takes about 65 ms to detect the assembly state of the five screws, and the assembly state is detected without error for 1 hour.
Pelvic fractures are high-energy injuries, often accompanied by damage to the adjacent tissues and organs. For patients with pelvic trauma, active treatment is required early in the injury, because mortality can increase if appropriate treatment is not provided. In most cases, however, minimally invasive surgery is considered because extensive surgery cannot be performed due to the patient's condition. Percutaneous fixation of the pubis has been introduced because it can be applied easily to achieve the stability of the anterior part of the pelvis. Although many studies introduced percutaneous fixation of pubic bone fractures, most describe screw fixation for nondisplaced fractures. When treating displaced fractures with percutaneous screw fixation, it is difficult for the guide pin or drill bit to avoid the joint surface. Using a bent guide pin could allow easy insertion of the cannulated screw while avoiding the articular surface.
Purpose: To determine and to compare the effects of cyclic loading on the fixation strength of different femoral fixation methods in ACL reconstruction. Materials and Methods: Biomechanical test using an Instron(R) machine (Model No.5569. Mass, U.S.A) were carried out to compare the pull out strength of six different femoral fixation techniques after a cyclic loading in 72 Yorkshire pig knees. The graft-bone complex was cyclically loaded between 30N and 150N at 50 mm/min rate for 1000 cycles and maximal tensile testing was performed. A preload of 30N was applied to the graft along the axis of the tunnel 15 minutes. ANOVA and the Duncan multiple comparison test was used for the statistical analysis. Results: The mean maximum tensile strength of femoral fixation before and after the cyclic loading test were 1003.4$\pm$145N and 601.1$\pm$154N in hamstring-LA screw(R) group, 595.5$\pm$104N and 360.7$\pm$56N in hamstring-Bioscrew(R) group, 1431.7$\pm$135N and 710.7$\pm$114N in hamstring-Semifix(R) group, 603.6$\pm$54N and 459.1$\pm$46N in hamstring-Endobutton(R) fixation group, 1067.4$\pm$145 and 601.8$\pm$134N in the BPTB-Titanium interference screw group, and 987.1$\pm$168N and 588.7$\pm$124N in the BPTB-Bioscrew(R) group. And these data illustrated that cyclic loading reduces the maximum tensile strength by 40 $\%$, 39 $\%$, 50 $\%$, 24 $\%$, 44 $\%$, 40 $\%$ respectively. Conclusions: With the results of these experiments it should be emphasized that rehabilitation exercises after anterior cruciate ligament reconstruction should be executed with precaution as the repetitive flexion and extension of the knee would compromise the maximum tensile strength of the graft tendon.
Purpose: The purpose of this study is to compare the removal torque between prefabricated and customized implant abutment screw. Materials and methods: Three types of implant system (Osstem, Astra, Zimmer) were used. For each system, prefabricated abutment screw (control group) and customized abutment screw (test group) were used to connect the fixture and the abutment (n = 6). Digital torque gauze was used to control the tightening torque and the screws were tightened under each manufacturer's recommendation. 10 minutes after the connection the same tightening torque was applied, and 5 minutes after the second connection, the removal torque was measured. This procedure was repeated 10 times. In the cyclic loading test, 10 minutes after the first connection to the 6 groups (n = 3), the same tightening torque was applied, and a total of 1,000,000 time loading was applied at 30 degree angle to long axis with 50 N load. Repeated measures of ANOVA test (${\alpha}$=.05) was used as statistics to evaluate the effect of repeated loading number on the removal torque. Independent t-test was used to evaluate the difference in removal torque after cyclic loading. Results: The removal torque significantly decreased as the number of loading repetition increased (P<.05). In the 10 time repetition test, there was no significant difference between the prefabricated and customized implant abutment screw of the 3 implant system (P<.05). Also in the cyclic loading test, there was no significant difference between the prefabricated and customized implant abutment screw of the 3 implant system (P<.05). Conclusion: Within the limitation of this study, there was no significant difference in the removal torque between the prefabricated abutment screw and customized abutment screws.
Purpose: The aim of this study was to evaluate the clinical performance of screw- and cement-retained implant prosthesis (SCP) design in terms of retained preload of abutment screws and cement washout. Materials and methods: Patients with the partial posterior edentulous areas comprised the study group. Implants were placed, and SCPs were delivered after 3 to 6 months healing. Follow-up examinations were performed. The implant survival rate and the prosthetic success rate were evaluated. The retained preload ratio of abutment screws and the prosthetic decementation ratio were measured. Results: Twenty one SCPs (forty three implants)in twenty patients were followed up to 64 months. All of the implants survived during the follow-up period (mean follow-up: 34 months). The prosthetic success rate was 100 % considering no abutment, screw, porcelain or metal frame fractures, as well as no screw loosening. The retained preload ratio of SCPs at the end of follow-up period was 97.61% (${\pm}16.29$) and the decementation ratio was 9.5 %. Conclusion: Within the limitations of this clinical study, SCP design showed favorable short-term clinical performances in respect of screw loosening and cement washout.

Implant prostheses were classified into screw-retained prosthesis and cement-retained prosthesis by their method of retaining, and there is screw and cement retained implant prosthesis (SCRP) which has been made reflecting the strengths of these two. The advantages of the SCRP technique are easy retrievability and passive fit of implant prostheses. However, the occlusal screw holes of implant prostheses can be thought as a disadvantage with respect to esthetics and occlusion. Inappropriately positioned implants also limited the use of the SCRP technique. The present study is reporting about the case where nine implants (US II, OSSTEM, Seoul, Korea) were placed in maxilla and eight in mandible respectively in fully edentulous patients. Then, the cement-retained prosthesis was applied for the part in which the screw hole positioned improperly, and screw-retained prosthesis for properly positioned implants so that the combined screw-cement prosthesis has been produced where the satisfying result has shown in both function and esthetics. Three-year follow-up has been done for the patient.

The purpose of this study was to observe and analyze the initial marginal bone resorption changes according to the patterns of cover screw exposures during healing period followed by implants installation. Total 64 fixtures(TiUniteTM, NobelBiocare, Sweden) were installed in partially edentulous jaws of 28 patients, who were selected retrospectively and were shown at least one cover screw exposure. Cover screw exposures were defined at 1 month recall. According to the patterns of exposures, groups were categorized into group 1 (No exposure), group 2 (pin-point exposure), group 3 (less than 1/2 of cover screw), group 4 (more than 1/2 of cover screw), group 5 (total exposure). Periapical radiographs were taken in purpose of changes of marginal bone level between installation and 2 month recall. Healing abutments were secured on the exposure groups at 2 month recall. Results were as follows: 1. Marginal bone resorptions were identified whenever cover screws were exposed. 2. Group 2 and 3 were shown significantly increased bone loss more than other group (P <.05). 3. Group 4 and 5 were shown significantly increased bone loss more than group 1, however, less than other groups ( P <.05). Conclusionally, cover screw exposure may cause marginal bone resorptions, therefore, early connection of healing abutment is clinically helpful.
현재 골 유착성 임플랜트는 안전하고 유용한 보철 수복 분야의 하나로 자리잡아 가고있다. 과거 완전 무치악 환자에게 있어서 저작 기능의 회복이란 면에 중점을 두었던 경향에서 현재 골유착 임플랜트의 높은 성공률에 의해 부문 무치악, 단일치 수복에 까지 다양하게 사용되고 있다. 특히 심미성과 인접치의 손상을 가하지 않는다는 면에서 단일치 수복에서 골유착 임플랜트 치료에 관심이 모아지고 있다. 단일치 수복에 있어서 심미성이 중요한 문제로 대두되면서 이런 수복에 사용될 수 있는 다양한 계통의 임플랜트들이 개발되어 사용되고 있다. 이중 가장 기본적인 Branemark 계통의 CeraOne 지대주, 최근에 개발된 전부도재 지대주인 CerAdapt 전부주조 지대주인 UCLA 계통등은 높은 성공률을 보이며 많이 사용되고 있다. 임상가들에 의해 나사유지형 보철물이 착탈성이란 관점에서 선호되고 있다. 시멘트유지형 보철물에서는 나사공이 없는 온전한 교합면을 유지한다는 면과 교합력을 받는 면적에 있어서 더 넓은 면을 가진다는 점등 더 많은 장점을 가진다고 주장하는 사람도 있다. 임상가들의 기호에 의해 선택되어지는 나사 유지형과 시멘트 유지형에 있어서 교합력이 야기하는 하중하에서 각각의 응력 분포를 분석할 필요성이 요구되었다. 이 연구는 단일치 수복에 사용되는 CeraOne, CerAdapt UCLA 계통에서 각각의 나사 유지형과 시멘트유지형에서의 응력분포를 삼차원 유한요소법으로 분석하고 상부 구조물에 가해지는 수직하중, 수평 하중, 경사 하중에 의해 야기되는 응력을 비교 분석한 것이다. 본 연구의 연구 결과는 다음과 같다. 1. CeraOne, CerAdapt, UCLA 지대주 모두 시멘트유지형보다 나사유지형에서 응력집중이 컸다. 2. CeraOne 시멘트유지형인 1번 모델에서 응력분산이 유리하였고, UCLA 나사 유지형인 6번 모델에서 가장 불리하였다. 3. 모든 모델에서 고정체 경부에서 가장 큰 응력 집중이 있었고 이것은 UCLA 지대주에서 가장 컸다. 4. 상부 구조물에서 주된 응력의 집중은 교합면에서 일어났다. 5. 골은 상부 피질골, 즉 고정체경부와 만나는 부위에서 가장 큰 응력의 집중이 일어났으며 수평, 경사 하중시 응력집중 양상은 힘을 가한 쪽의 반대쪽 고정체 경부에 응력이 집중되는 양상을 보였다. 6. 전체 모델과 골, 고정체 모두에서 수평 하중과 경사 하중시 보다는 수직 하중시 더 적은 응력값을 보였다.

Purpose: The purpose of this finite element analysis study is to introduce the novel Lock screw system and analyze its mechanical property to see if it can prevent abutment screw loosening. Materials and Methods: The Lock screw is a component tightened on the inside of the implant abutment which applies compressive force to the abutment screw head. To investigate the effect, modeling was done using CAD program and it was analyzed by finite element analysis under various load conditions. First, the preload was measured according to the tightening torque of the abutment screw then it was compared with the theoretical value to verify the analytical model. The validated analytical model was then divided into those with no external load and those with 178 N, and the tightening torque of the lock screw was changed to 10, 20, 30 Ncm respectively to examine the property of stress distribution on the implant components. Results: Using Lock screw under various loading conditions did not produce equivalent stresses beyond the yield strength of the implant components. In addition, the axial load was increased at the abutment-abutment screw interface. Conclusion: The use of Lock screw does not exert excessive stress on the implant components and may increase the frictional force between the abutment-abutment screw interface, thus it is considered to prevent loosening of the abutment screw.
Statement of problem: Within the elastic limit of the screw, the greater the preload, the tighter and more secure the screw joint. However, additional tensile forces can incur plastic deformation of the abutment screw when functional loads are superimposed on preload stresses, and they can elicit the loosening or fracture of the abutment screw. Therefore, it is necessary to find the optimum preload that will maximize fatigue life and simultaneously offer a reasonable degree of protection against loosening. Another critical factor in addition to the applied torque which can affect the amount of preload is the joint connection type between implant and abutment. Purpose: The purpose of this study was to evaluate the influence of tightening torque on the implant-abutment screw joint stability. Material and methods: Respectively, three different amount of tightening torque (20, 30, and 40 Ncm) were applied to implant systems with three different joint connections, one external butt joint and two internal cones. The initial removal torque value and the postload (cyclic loading up to 100,000 cycles) removal torque value of the abutment screw were measured with digital torque gauge. Then rate of the initial and the postload removal torque loss were calculated for the comparison of the effect of tightening torques and joint connection types between implant and abutment on the joint stability. Results and conclusion: 1. Increase in tightening torque value resulted in significant increase in initial and postload removal torque value in all implant systems (P < .05). 2. Initial removal torque loss rates in SS II system were not significantly different when three different tightening torque values were applied (P > .05), however GS II and US II systems exhibited significantly lower loss rates with 40 Ncm torque value than with 20 Ncm (P < .05). 3. In all implant systems, postload removal torque loss rates were lowest when the torque value of 30 Ncm was applied (P < .05). 4. Postload removal torque loss rates tended to increase in order of SS II, GS II and US II system. 5. There was no correlation between initial removal torque value and postload removal torque loss rate (P > .05).
