호흡곤란에 대한 적절한 대처는 의사로서 숙지하여야 할 가장 중요하고도 기본적인 개념 중의 하나이다. 상부 기도에 발생하는 여러 가지 임상적 상황은 흔히 흡기성 천명을 동반하는 호흡 곤란으로 나타나고, 이에 대해 흔히 기도 삽관이나, 기관절개술 등이 행해 지지만, 그 발생 원인에 대해서 감별점들을 숙지하지 않으면, 원발 질환에 대한 합리적인 치료 뿐만 아니라 응급상황에서 적절하게 기도를 확보하는 데에도 문제가 생길 수 있다. 후두 상부에 일어나는 기도폐쇄의 상황은 비강에서부터 인두와 성문 상부, 식도 입구부에 이르는 해부학적 위치에 발생하는 다양한 질환들에 의한다. 비강과 비인두에는 pyriform aperture stenosis, choanal atresia, lacrymal duct cyst, 또는 teratoma나 encephalocele과 같은 질환이 발생할 수 있다. 구강, 인두부에서는 다양한 종류의 안면골격의 이상이나 설거대증, 또는 설갑상선, 갑상설관낭종, 또는 유피종등이 설기저부에 발생하기도 하고, 흔한 이유로 심한 편도-아데노이드 비대가 심각한 호흡곤란을 일으키기도 한다. 특히 소아에서는 이물의 가능성도 항상 염두에 두어야한다 이와 같이 기도 협착의 위치에 따라서 임상적 표현 양상이 구별될 수 있고, 또 부위에 따라 다양한 질환이 감별되어야 하므로 발생 위치에 따른 각 질환의 이해가 적절한 기도의 관리를 위해서 필수적이다.

Grisel's syndrome is a non-traumatic subluxation of the atlantoaxial joint with an inflammatory condition in the adjacent soft tissues. Due to the instability of the cervical spine, careful airway management is crucial to prevent potential cervical spinal cord injury following airway manipulation. We successfully secured the patient airway using a supraglottic airway device (I-gel) in a patient who had previously diagnosed with Grisel's syndrome. The operation was successfully completed, and the patient recovered without any neurological complications. I-gel can be a good option for airway management during general anesthesia in a patient diagnosed with Grisel's syndrome.

천식은 기도가 수축하고 염증이 생기는 흔한 만성 호흡기 질환이다. 천식은 평소에도 기관지에 염증이 존재하므로 가벼운 자극에도 쉽게 기도가 좁아지게 되어 천명음(숨쉴 때 쌕쌕거리는 소리), 기침, 가슴이 답답함, 호흡곤란 등의 증상이 나타나게 된다. 만일 치료를 하지 않고 그대로 방치한다면 오랫동안 폐 기능이 감소된다. 아직까지 천식을 완치할 수 있는 치료법은 발견되지 않았지만, 올바른 방법으로만 하면 성공적으로 관리할 수 있는 질병이다.

주로 여름철에 발생하는 익사사고는 강에서 멱을 감는 소년층에서 가장 많다. 대부분은 안전수칙을 잘 지키지 않거나 수영미숙 등으로 사고가 많이 일어나며 또한 수심이 갑자기 깊어지는 경우에 일어나기도 한다. 그 외에도 수영장이나 바다에서도 익사가 발생하고 있다. 익사사고는 기도에 물이 들어가는 질식사가 주원인이나, 실제로 기도나 폐에 물의 양이 많지 않은 경우가 대부분이며 때로는 기도에 물이 전혀 없는 후두경련으로 인한 기도폐쇄 즉 건성익사도 있다. 사고 시 소생술 시작이 늦으면 늦을수록, 소생술자의 경험이 미숙할수록 치사율은 높으며 나이가 어릴수록 소생 후에도 더 위험하다.

The purpose of this study was to revitalize management of Hapki-do gymnasium based on customer. The Subjects of this study were 264 persons who have trained Hapki-do. Also, subjects were chosen by convenience sampling method. Data were analized with SPSS 12.0 version for frequency and crosstab analysis. Following is the analysis result. First of all, it is necessary repairing facilities and operating systematic education program. Secondly, it is necessary a discriminatory education program. Further study on demand features of Hapkido gymnasiums will contribute to expand and to strengthen the base of Hapkido development. This will aid Hapkido to gain dominant position in the competition of martial arts and sports industry.
