본 논문에서는 블록체인 기반의 투명성/신뢰성을 제공하는 조작 불가능한 확률 제어 시스템을 제안한다. 해당 시스템은 클라이언트에 의해 질의 된 확률값을 블록체인상에서 산출해냄으로써, Legacy 시스템 아키텍처의 한계인 조작 가능성을 원천적으로 배제할 수 있다. 이는 블록체인 참여 노드 간의 데이터를 동일하게 공유하여 투명성을 확보하고, 이를 기반으로 데이터에 대한 신뢰성을 확보한 확률 제공 기능이다. 특히 해당 시스템은 Private/Permissioned 구조 기반의 블록체인 네트워크를 기반으로 운영 노드에 의해서 유지/관리되어 별도의 트랜잭션 수수료가 블록체인상에서 발생하지 않는다. 또한 Public Blockchain 메인 네트워크상의 미래 블록에 대한 정보를 확률값 산출 Seed에 활용함으로써, Non-deterministic 한 환경을 제공한다. 이는 클라이언트가 확률 질의에 대한 검증 과정을 직접 수행하거나 Third-party 검증을 통해 확률값에 대한 조작 여부를 확인할 수 있다.

The digital workflow of optical impressions by the intraoral scanner and CADCAM manufacture of dental prostheses is actively developing. The complex process of traditional impression taking, definite cast fabrication, wax pattern making, and casting has been shortened, and the number of patient's visits can also be reduced. Advances in intraoral scanner technology have increased the precision and accuracy of optical impression, and its indication is progressively widened toward the long span fixed dental prosthesis. This case report describes the long span implant case, and the operator fully utilized digital workflow such as computer-guided implant surgical template and CAD-CAM produced restoration after the digital impression. The provisional restoration and customized abutments were prepared with the optical impression taken on the same day of implant surgery. Moreover, the final prosthesis was fabricated with the digital scan while utilizing the same customized abutment from the provisional restoration. During the data acquisition step, stl data of customized abutments, previously scanned at the time of provisional restoration delivery, were imported and automatically aligned with digital impression data using an 'A.I. abutment matching algorithm' the intraoral scanner software. By using this algorithm, it was possible to obtain the subgingival margin without the gingival retraction or abutment removal. Using the digital intraoral scanner's advanced functions, the operator could shorten the total treatment time. So that both the patient and the clinician could experience convenient and effective treatment, and it was possible to manufacture a prosthesis with predictability.

연구개는 그 주위조직과 더불어 생리적 기능에 있어 음성, 연하, 및 호흡 등에 중요한 역할을 한다. 종양등의 이유로 인한 연구개의 외과적 절제는 그 해부학적 크기와 모양의 영구적인 변화로 인해 여러기능의 상실을 초래한다. 이에 따른 결함은 과도한 비음, 연하중 음식물이나 유동물의 비관으로의 누출 등을 들 수 있다. 연구개와 그 주위조직의 생리적인 기능의 숙지 및 이를 응용한 보철물(Palatopharyngeal Obturator)의 적절한 제작은 이러한 기능적 결함을 수복하는데 이용되어진다. 본 증례에서는 종양으로 인해 연구개를 절제한 환자의 보철적 수복에 있어, 구강내 온도에 따라 그 형상이 변화하는 인상재를 이용하여, 측-후 인두벽의 운동을 기능적으로 인기하는 방법을 이용하였다. 이러한 방법으로 제작된 보철물은 종양으로 인한 연구개의 절제 후 생리적인 기능의 장애를, 올바른 Palatopharyngeal 폐쇠를 유도함으로써 음질의 향상, 연하등의 기능을 회복시키는데 큰 도움이 될 수 있음을 보여준다.

The goal of suction denture is to enhance retention of dentures by the formation of negative pressure between the denture base and the underlying tissue and sealing around the denture by the mucosa. This patient has been converted to a completely edentulous state after the extraction of residual teeth. Fabrication of suction denture was planned because it was estimated that the conventional compete would be hard to achieve retention due to the absorption of residual ridge and lack of sublingual fold. Through appropriate clinical and laboratory technique such as preliminary impression on the mandibular rest position, provisional vertical dimension determination with Centric Tray^® (Ivoclar Vivadent AG, Schaan, Liechtenstein), closed mouth definitive impression and jaw relation record using individual tray with Gnathometer M^® (Ivoclar Vivadent AG, Schaan, Liechtenstein), artificial teeth arrangement considering stability of the denture, and proper polymerization technique that minimizes polymerization shrinkage, restoring the patient with suction denture resulted in satisfaction throughout the function and asesthetics.

The patient was a 69-year-old male who had decreased VDO and overbite of the anterior teeth due to severe teeth wear. Complete mouth rehabilitation was planned to restore esthetics and function. Due to the presence of a subgingival finish line, poor fit of the margin of the temporary crown, and insufficient oral hygiene management, gingival overgrowth and bleeding occurred in many teeth, and inaccurate impressions were taken. After dividing each arch and taking partial impressions, a transfer coping was made using pattern resin on each working model die, and after being placed on the abutment teeth, a master cast was made through pick-up impression taking to produce prosthesis. In this case, the final prosthesis produced on the second master cast were fitted in terms of proximal contact, marginal fit, vertical occlusal dimension, facial appearance, esthetics, and occlusion. After complete mouth rehabilitation, the patient was satisfied with function and esthetic appearance.

치관부 파절이나 치아결손 혹은 치아형태의 이상이나 변색된 치아등으로 보철치료가 요구되는 경우 저작, 연하, 발음등의 회복과 심미적인 회복이 중요한 목적이 되며, 전치부에서는 심미성이, 구치부에서는 기능적인 회복이 더욱 강조된다. 보철치료후 치료목적을 달성하고 건강한 구강내의 상태를 유지하기 위해서는 보철물이 지대치 및 주위조직에 생물학적으로 조화되어 병적상태를 유발시키지 않아야 하며, 이를 위해서는 시술전에 올바른 진단과 적절한 치료계획을 세우고, 지대치 형성, 인상채득 및 임시관 장착, 금속관 및 가공의치 제작, 구강내의 시적(try-in), cementation등 모든 과정을 주위조직과 관련하여 주위 깊게 행하여야 한다.

전 세계인의 삶을 송두리째 바꾸어 놓은 코로나19로 인해 산업 전반에도 큰 변화가 예상된다. 특히 가처분 소득 감소와 소비 심리 수축으로 꼭 필요한 것에만 지갑을 여는 구매자들이 늘어날 것으로 전망된다. 과거보다 더욱 깐깐해진 구매자의 시선을 잡기 위해 가장 중요한 것 중 하나가 패키징 디자인이다. 사람에 대한 첫 인상이 3초 안에 결정되는 것처럼 좋은 패키징 디자인으로 구매자의 시선을 한번에 사로잡을 수 있기 때문이다. 포스트코로나시대를 맞아 국내 최대 디자이너 커뮤니티이자 산업 디자인 콘테스트 플랫폼인 '라우드소싱'과 함께 패키징 제조업체들이 주목해야 할 패키지 디자인에 대한 시리즈를 총 4회에 걸쳐 진행한다.

Fixed dental prostheses such as inlay, onlay, crown, and bridge fabricated by CAD/CAM technique combined with digital impressions is getting popular due to the recent rapid progress of digital impression taking system. For the scope of implant prosthesis, however, digital intra-oral scan hasn't been actively utilized for the fabrication of superstructures. In this case report, 6 cases of titanium-milled custom abutment based on the iTero intra-oral scan data were introduced, five of them were restored with screw-type prosthesis after cementation (SCRP) and the clinical results were satisfactory on restoring the function and esthetics.
