Effects of Transformational Leadership on the Organizational Effectiveness of Long-Term Care Facilities : Focusing on the Care Giver's Mediating effect of Empowerment (노인장기요양기관의 변혁적 리더십이 조직효과성에 미치는 영향 : 요양보호사의 임파워먼트 매개효과를 중심으로)

  • The purpose of this study is to examine effects of empowerment on relationship between supervisors transformational leadership recognized by care givers and organizational effectiveness represented as the job satisfaction and the organizational commitment. For the purpose, survey were conducted with care givers working in long-term care facilities in Seoul, Gyeonggi-do and Incheon, and final data were resulted from analysis of 283 examines of those care givers. The findings of this study were as follows: first, transformational leadership of long-term care facilities had a significant influence on the job satisfaction and organizational commitment. Second, the care giver's empowerment had a influence on the job satisfaction and organizational commitment. Third, empowerment worked as mediators between the transformational leadership of long-term care facilities and care giver's job satisfaction and organizational commitment. Based upon these findings, we can propose practices towards improving a long-term care facilities administrator's transformational leadership and implication of program development and provide an institutional strategy aiming to care giver's empowerment improvement.

A Study on the Hospital Worker's Security Stress (국내 병원 종사자들의 보안스트레스에 관한 연구)

  • The need for protecting computerized medical information has been growing as information and communication technology has rapidly been developed. Government is also promoting institutional improvement measures for the information protection of medical institutions. With more emphasis upon the efforts to enhance the information protection capability of the medical institution, however, it could increase stress to the hospital workers and cause a reduction in the quality of essential medical services of medical institutions. Therefore, it is necessary to research the stress from hospital workers to strengthen the protection of information that medical institutions to manage and its antecedents. Based on the results of relevant researches, this study selected several factors that affect the security stress, including work overload, invasion of privacy and work uncertainty. The sample for this study was 123 hospital workers who were surveyed by questionnaire. The factor analysis and multiple regression method were employed for the analysis of this study. The results of this study showed that work overload, invasion of privacy and some of work uncertainty were the main factors positively influencing the security stress.

  • The study examined the impact of medical servicescape, human services, and effects on perceived value and customer satisfaction. From July 1, 2016 to August 31, 2016, a total of 300 outpatients visiting the university hospital and general hospital in Ulsan area participated the study. As a result, the general features of the Servicescape were more important to the aged and the higher the academic background, the higher the scores of Human Service and perceived value were revealed. There was a positive correlation between the service schedule of the medical institution, human service, perceived value, and customer satisfaction. And the factors affecting customer satisfaction were in order of human service (${\beta}=0.382$), perceived value (${\beta}=0.376$). The service scapegoats and personal services offered by customers visiting healthcare providers play an important role in understanding the internal health and needs of customers, and this research results will serve as a basic material for strengthening the competitiveness of medical institutions.

The impact of healthcare accreditation on patient safety and quality, job satisfaction and organizational commitment: about a general hospital (의료기관인증이 병원 근로자가 지각하는 환자안전과 질, 직무만족 및 조직몰입에 미치는 효과 -일개 종합병원을 대상으로-)

  • This study was conducted to examine the effects of healthcare accreditation on patient safety and quality, job satisfaction and organizational commitment. One group pretest posttest design was used. The study included 405 participants, who have worked for more than one year in a general hospital. The treatment was healthcare accreditation which was prepared from Apr. 2012 and tested by Korea Institution for Healthcare Accreditation on Nov. 2012. Data was analyzed using paired t-test and One-Way ANOVA. This study found patient safety and quality(t=7.28, p=.000), job satisfaction(t=7.13, p=.000) and organizational commitment(t=3.44, p=.001) increased significantly after healthcare accreditation. The results of this study suggest that healthcare accreditation has the potential to improve patient safety and quality, job satisfaction and organizational commitment.

Complex impact of Patient Safety and Medical Quality on Hospital Management Activities due to Healthcare Accreditation Adoption (의료기관인증제 도입에 따른 환자안전과 의료의 질이 병원경영활동에 미치는 복합영향)

  • The purpose of this study was to confirm the complex relationship between patient safety-medical quality and hospital management activities to observe the mediating effect of organizational culture in the relationship due to healthcare accreditation adoption. For this, we conducted a 4 weeks survey, from September 22, 2014 to October 17, 2014 to the employees of 6 hospitals that could accommodate less than 300 patients or more than 300 patients located in Daegu and Gyeongbuk. The study includes 377 participants who have worked for more than one year in the hospital. All of hospital with less than 300-beds or more than 300-beds, such as patient safety-medical quality, organizational culture, hospital management activities found a positive correlation among variables. The mediating effect of organizational culture in the relationship between patient safety-medical quality and hospital management activities was shown by the partial mediating effect and high causal effect. Therefore, these study suggest that patient safety-medical quality due to healthcare accreditation adoption has the potential to improve hospital management activities.

  • This study is to help fine some factors of how the Initial food consumption ability affects articulation production and intelligibility in children at aged 4-15 with cerebral palsy. According to the factor analysis of articulation production, here are some positive articulation productions; the ability of mumbling and swallowing food, biting and swallowing and liquid diet, the experience of food consumption therapy, the use of specific methods or tools for food consumption, the behavior-problem during a meal and the beginning of the neck control. Therefore, the study finds that the Initial food consumption ability in children with spastic cerebral palsy may affect articulation production and intelligibility.

  • Tracheostomy may be used to assure airway protection in various clinical situation. It, as a known operation, has a history spanning 2000 years. The first clear account of a successful tracheostomy was recorded in 1546 by Brasavola. Until 1718 the term "bronchotomy" was used to describe the procedure. Heister then introduce the term "tracheotomy and this was later adopted and popularized by Trousseau about 1830. The term "tracheostomy" appeared in medical literature after 1820 and the two terms "tracheostomy" and "tracheotomy" are used interchangeably today. Indications include relief of upper airway obstruction, facilitation of pulmonary toilet, diminution of dead space and need for prolonged mechanical ventilation. The extent of indication of tracheostomy has a tendency to increase, thus oral and maxillofacial surgeons have some opportunities to face a situation that require tracheostomy. So, we reported retrospective study of 31 cases of tracheostomy patient in oral and maxillofacial surgery with reference review to reveal the significance of surgical skill and management capability of emergercy state maxillofacial surgery patients.

  • The subjects of following research were 663 medical practitioners working at hospitals or bigger sized medical facilities in Pusan. Among the entire research subjects, it was shown that medical practitioners are notably and positively affected when their supervisors' type of leadership was either servant leadership or transformational leadership. The types of leaderships of supervisors that are recognized by medical practitioners were transformational leadership, transactional leadership, servant leadership. The effect of supervisors' leadership according to different type of role are as following. Under the circumstance of no personal and controlled variables, the each group of different medical occupants showed notable positive effects. Nurses corresponded to transformational leaderships, while admission and discharge corresponded to servant leadership. At the same time, medical technicians were most effected by both servant and transformational leadership.

  • This study made an analysis of the relationship between the emotional labor and the organizational commitment and the mediating effect of job satisfaction of employees in senior employment agencies. A survey of 253 employees in Daejeon and Chungcheong areas was conducted. The important analysis methods are confirmatory factor analysis and bootstrap verification. The analysis results are employees' emotional labor had a negative effect on organizational commitment and the mediating effect of self-efficacy was proved significantly. For reducing emloyees' emotional labor, it is important to offer guidelines for improvement of emotional labor, regular inspection and education, and the proper measures of the administrators. To advance their self-efficacy, we need to offer education for the improvement of practice-based professionalism, education and administrative support to improve ethical consciousness, and appropriate emotional support service.

  • The purpose of this study is to investigate the information-providing procedure of public servants responding to the request for the business information through the Information Disclosure mechanism in Korea. Using date collected by in-depth interviews we studied their information providing behaviors and factors that affect the procedure. Based on Grounded Theory approach, we first generated 55 concepts, 18 categories and 6 super-categories about the participants' feeling, experiences and perception related to their information providing. Then we developed a factor model among those generated concepts. And, we discussed various information providing modality of 3 difference type of information according to the formal record keeping level. The main contributions of this study are the results provide practical implications for the enhancing the Information Disclosure institution in Korea.
