Factors affecting the Success of Smoking Cessation for Six Months in the Smoking Cessation Clinic of a Public Health Center Based on the Trans?theoretical Model (범이론적 모형을 기초로 한 보건소 금연클리닉 등록자의 6개월 금연성공요인)

  • Purpose: The purpose of this study was to identify factors affecting on the successful cessation of smoking for six months in the smoking cessation clinic of a public health center. Methods: The subjects of this study were 568 adult smokers who had registered in the smoking cessation clinic of a public health center in B City. Data were analyzed through $x^2$-test, t-test, and logistic regression using the SPSS/WIN 12.0 program. Results: Identified factors affecting the successful cessation of smoking for six months were service type, number of counseling, alcohol problem, previous quit-smoking trial, dependency on nicotine, CO level in expiration at the first visit time, self-reevaluation, helping relationship, negative/affective situational temptation, and Cons. Conclusion: Trans-theoretical Model variables were confirmed as factors affecting the success of smoking cessation. Based on the results of this study, a smoking cessation program was suggested, which uses Trans-theoretical Model variables affecting smoking cessation for six months in a public health center.

The Patterns and Risk Factors of Smoking Relapse among People Successful in Smoking Cessation at the Smoking Cessation Clinics of Public Health Centers (보건소 금연클리닉 금연성공자의 재흡연 양상 및 위험요인)

  • Purpose: The purpose of this study was to investigate the patterns of smoking relapse and to identify risk factors related to smoking relapse among those successful in smoking cessation at the smoking cessation clinics of public health centers. Methods: Data were collected from 1,705 six-month quitters and analyzed by the Kaplan-Meier analysis and the Cox proportional hazard model. Results: The rate of smoking relapse was 38.2% in 1 year, 44.4% in 2 years, and 47.8% in 5 years. The vast majority of relapse (62.3%) occurred within the first six months after quitting. The risk factors related to smoking relapse were age (HR 1.964: 95% CI 1.545, to 2.497), nicotine dependence (HR 1.293: 95% CI 1.087 to 1.539), problem drinking (HR 1.497: 95% CI 1.116 to 2.008), behavioral therapy type (HR 1.398: 95% CI 1.193 to 1.638), and nicotine replacement therapy type (HR 1.363: 95% CI 1.077 to 1.724). Conclusion: For reducing smoking relapse, it is necessary to develop a smoking relapse prevention program for the first six months after quitting and to strengthen behavioral therapy in the course of smoking cessations clinics.

Factors Affecting Smoking Cessation Success during 4-week Smoking Cessation Program for University Students (4주간 금연성공 영향요인에 관한 연구: 금연 프로그램에 참여한 대학생 흡연자 중심으로)

  • Purpose: Smoking cessation interventions are important because university students are vulnerable to smoking and good health practices during their university lives greatly affects their health status. The purpose of this study is to investigate the factors affecting smoking cessation success among the university students who participated in the 4-week smoking cessation program. Methods: This study was conducted with 101 university student smokers. To identify the factors affecting the success of smoking cessation, a logistic regression analysis was performed. Results: In Model 1, without the smoking cessation aids variable, when the frequency of face-to-face counseling was greater, the success rate of smoking cessation was 1.70 times higher. In Model 2, including the smoking cessation aids variable, when the nicotine dependence score was higher, the success rate of smoking cessation was 0.72 times lower, and when the number of smoking cessation aids used was greater, the success rate of smoking cessation was 1.40 times higher. Conclusion: Smoking cessation aids are essential to increase the success of short-term smoking cessation, and face-to-face counseling and telephone counseling should be provided continuously to maintain long-term smoking cessation.

Factors which Differentiate Ex-smokers from Smokers among Patients with Cardiovascular Disease (심혈관계 질환자의 금연성공 예측 요인)

  • Purpose: The purpose of this study was to identify factors which differentiate ex-smokers from smokers among patients with cardiovascular disease. Ex-smokers were patients who abstained from smoking for at least 6 months. Method: The subjects were 92 male cardiovascular patients who try to quit. The stepwise discriminant analysis was used to identify predictors of smoking cessation. Result: 1. The ex-smokers(n=47) and smokers(n=45) were significantly different regarding the age, duration after diagnosis, nicotine dependence, family support for smoking cessation, and intention-to-quit. 2. The family support for smoking cessation, intention-to-quit, nicotine dependence, and age were the predictors which differentiate ex-smokers from smokers among cardiovascular patients. The percentage of grouped cases correctly classified was 92.4%. Conclusion: The higher the family support for smoking cessation, intention-to-quit, age and the lower the nicotine dependence, the more likely these smokers were to success smoking cessation.

  • 건강관리서비스는 금연, 절주, 식이 운동 등 생활습관을 개선하여 스스로 건강을 증진하도록 평가 교육 상담 등을 제공하는 서비스이다. 본 연구의 목적은 건강관리 서비스 분야에서 성공적인 건강상담사례들 중 유사한 사례를 추출하여 이를 건강관리 상담에 활용하기 위한 CBR(Case based Reasoning) 기반의 웰니스멘토지원시스템을 개발하는 것이다. 자가 건강관리 서비스를 위해 수집한 개인 건강관리 관련 정보로부터 운동처방 및 식이영양 처방을 위한 판단기준이 되는 변인들을 추출하였고, 변인들 간 유사도 비교를 위한 함수를 선정하고 가중치를 부여하였다. 또한 기존의 성공사례들에 대한 DB를 구축하고 유사도 비교(Retrieve)를 통해 가장 적절한 사례를 추천하기 위한 웰니스멘토지원시스템의 프로토타입을 개발하였다. 웰니스멘토지원시스템을 통해 과거의 성공적인 상담사례를 참고하여 상담해줌으로써 상담에 필요한 시간을 단축시키고 서비스의 질 향상을 도모할 수 있을 것이다.

Predictors of Institution Level of Smoking Cessation Rate and Job Satisfaction of Counselors in Smoking Cessation Clinics of Community Health Centers (보건소 금연클리닉의 금연 성공률과 금연상담사의 직무만족도 영향요인)

  • The purposes of this study were to identify factors of institution-level 6-month quit rates and job satisfaction of counselors in smoking cessation clinics of community health centers. The study was a mixed-method study including descriptive study and secondary data analysis and the data collection period was April 3, 2020, to October 31, 2020. Data were collected using a self-report survey with counselor representatives in 267 smoking cessation clinics in the nation. Part of the data regarding institutional and community characteristics was obtained from the national database. Most survey respondents were middle-aged 42.4±9.2 years old women (96%). The institutional level of 6-month quit rates was 35.3±0.1%, and the job satisfaction of survey respondents was 7.4±1.7/10 on average. Work competency was the only statistically significant contributor of both institution-level 6 month quit rate (aOR=3.57, 95% CI=1.65, 7.74, p=.001) and job satisfaction of counselors (aOR=4.64, 95% CI=2.01 10.74, p<.001). More studies are needed on the work competency of smoking cessation counselors and the development of interventions to improve their competency.

A Study on Taxi Drivers' Smoking Behavior, Self-efficacy and Expectation Level of Success in Smoking Cessation (택시기사들의 흡연실태 및 금연에 대한 자기효능감과 성공기대)

  • Purpose: The purpose of this study is to find taxi drivers' smoking behavior, self-efficacy and expectation level of success in smoking cessation. It will provide useful information for developing nursing interventions in health promotion programs. Method: The subjects of this study were 271 taxi drivers in Seoul and its metropolitan area. Data were collected using a self-reported questionnaire asking smoking history, smoking behaviors, health problem, stages of change for smoking cessation, self-efficacy and the expectation level of success in smoking cessation. Data were analyzed through descriptive statistics. Pearson's correlation, Mann-Whitney U test, Kruskal-Wallis test and Cronbach's a. Results: Of the taxi drivers, 83.8% were current smokers. Stages of change were as follows: 45.8% were in the precontemplation stage, 26.2% in the contemplation stage, 13.7% in the maintenance stage, 12.2% in the preparation stage and 1.5% in the action stage. In addition. 20.3% of the subjects reported health problems such as hypertension, heart disease, diabetes mellitus and respiratory disease. The expectation level of success in smoking cessation was positively related with self-efficacy. There were statistically significant differences in the expectation level of success in smoking cessation according to the number of cigarettes per day and nicotine dependency. In addition. there were statistically significant differences in self-efficacy according to the number of cigarettes per day, duration of smoking (years) and nicotine dependency. Conclusion: The results of this study suggest that taxi drivers' health problems and smoking rate were serious. Therefore more systematic health promotion programs for smoking cessation should be developed and executed by health care specialists at individual taxi companies.

Factors Associated with Success of Smoking Cessation during 6 Months (6개월 금연 성공의 영향 요인 연구 - 인천광역시 보건소 금연클리닉을 방문한 흡연자를 중심으로 -)

  • Purpose: This study was to identify which factors are likely to influence the effectiveness of smoking cessation on adults who smoke in Metropolitan Incheon. Method: Data from 9,083 smokers, who visited a smoking cessation clinic of a public health center from Jan. to Oct. 2005, were provided by the Korean Health Research Society, Among 9,083 smokers, 1,495 people were selected for follow up care at 6 months in order to analyze the differences between two groups one is a successful group and the other is a failure group. Results: The successful group included 639 people and the failure group 856 people. In the demographic profiles such as sex, age and motive registration, there was a significant difference between the two groups. In the view of smoking pattern and factors such as the expiratory CO level, the age of starting to smoke, the duration of smoking, alcohol, and dependence on alcohol use and nicotine, there were significant differences between the two groups. The smoking cessation method, results of uni variate analysis, the total number of visits to the smoking cessation clinics, and the use of nicotine gum or a patch(stage 1, stage 2) were significantly different in the two groups. Conclusion: The results of multi variate analysis have shown that the factors associated with the success for smoking cessation is the total number of visits to the smoking cessation clinic, and the dependence on alcohol.
