Pattern of Pain on Temporomandibular Joint-Muscle Area in Tension-type Headache Patients (긴장성 두통 환자에서의 측두하악부 관절-근육통의 양상)

  • Although most cases of tension-type headache(TTH) are myogenic headache, in some cases, arthralgia appears, such as the disorder of the temporamandibular joint. This study is designed to compare the clinical patterns of arthralgia to those of myalgia, when both symptoms are co-existing. Among 167 patients who visit our clinic during a certain period, whose chief complaint was TTH, 18 patients were the arthralgia group, 50 patients were the myalgia group, and 99 patients who have both arthralgia and myalgia were the arthromyalgia group. Three groups were asked to answer the questionnaires about their age, gender, and oral parafunction such as bruxism or clenching. Then we gathered statistics on the data from the accomplished questionnaires. 1. There was statistical significance in age among the three groups(p=0.02). 2. There was no statistical significance in sex and oral parafunction among the three groups. 3. There was statistical significance the myalgia group was older than the arthromyalgia group(p<0.03). The results of the study show that as patients become older, TTH with myalgia happens more frequently than TTH with arthralgia and myalgia does.

Myofascial restriction will be developed into muscle deficiency and cause pains without releasing the muscle tension and the spasm. 4. Myofascial restriction affect and change the physical posture by losing the muscle elasticity and flexibility and by losing muscle supporting ability from gravitation. 5. The partial myofascial restriction affect the muscle and the adjoining joint supporting gravitation and cause the unbalance of the entire body.

The Effect of the Pain on the TMJ and Masticatory Muscles to Tension-type Headache (측두하악관절과 저작근의 통증이 긴장성 두통에 미치는 영향)

  • Generally, Tension-Type Headache(TTH) patients exhibit muscle pain, but can also have TMJ pain, which includes mouth opening limitation or joint sounds. The purpose of our study is to observe the clinical pain characteristics between TTH patients with muscle pain and TMJ pain. One hundred sixty-seven patients were diagnosed with TTH according to the questionnaires based on the International Headache Society's proposal on the diagnostic criteria of TTH. The patients were classified into three group; arthralgia group (18 patients), myalgia group (50 patients) and arthromyalgia group (99 patients). TTH patients with pericranial muscle pain were classified in the myalgia group. TTH patients with temporal region pain were classified in the arthralgia group. TTH patients with both types of pain were classified in the arthromyalgia group. The parameters in the diagnostic criteria such as quality, intensity, laterality of pain, and aggravation due to physical activities were compared among the three groups. 1. There were no significant differences in the quality of pain among the three groups. 2. There were no significant differences in the intensity of pain among the three groups. 3. There were no significant differences in the laterality of pain among the three groups. 4. A higher percentage of patients in the arthromyalgia group experienced headaches that were aggravated due to physical activity (p=0.03) compared to the other groups. The results of this study show that TTH patients with both arthralgia (TMJ pain) and myalgia (pericranial muscle pain) are more aggravated by physical activity than TTH patients with either one.

Clinical characteristics of congenital myotonic dystrophy diagnosed by molecular genetic method (분자 유전학적 방법으로 진단된 선천성 근육긴장성 이영양증 환자의 임상 양상)

  • Purpose : We performed this study to investigate the perinatal and developmental features of the patients with congenital myotonic dystrophy (CDM) confirmed by the molecular genetic method and the clinical characteristics of their mother, and to identify the relation between the number of CTG repeats and the clinical severity.Methods : A retrospective review of the medical records and the results of the dystrophia myotonica protein kinase (DMPK) gene test was done for the patients who were confirmed as CDM through gene analysis from January 2001 to September 2006. Results : All of the eight patients (male 2, female 6) showed moderate to severe degree of perinatal distress and feeding difficulty associated with profound hypotonia. Three patients had the history of polyhydramnios and two patients had equinovarus deformity. The developmental milestones were delayed in all patients, which improved gradually with age. All of their mothers demonstrated myotonic symptoms and typical myopathic face. The number of CTG repeats in DMPK gene analysis ranged 1,000-2,083, and there was no significant correlation between the number of CTG repeats and the time of walking alone. Conclusion : All patients with CDM presented with severe hypotonia in perinatal period, and developmental delay thereafter, which were improved with age. All of their mothers manifested myotonic symptoms with typical myopathic face, and the identification of such features greatly contributed to the diagnosis of the patients. The number of CTG repeats had no significant influence on the motor development.

  • 목소리를 많이 사용하는 사람들에서 발생되는 음성장애는 대부분 잘못된 발성방법으로 인한 후두의 과긴장으로 인해 야기되는 것으로, 흡기 및 호기를 자발적으로 조절할 수 있는 호흡방법과 성대 및 주변 근육의 긴장을 줄이도록 하는 훈련하는 음성치료가 주된 치료 방법으로 대두되고 있다. 지금까지 음성치료의 효과는 주로 청음인지적(perceptual) 측면에서 많이 논의되어 왔으며 객관적인 자료는 아직까지 별로 없는 상태이다. 더욱이 호흡 및 발성 방법에 따른 음성 분석은 미진한 상황이다. (중략)

The Effect of Soru Massage Therapy on Pain of the Head and Neck of Elderly (소루마사지요법이 노인의 두경부 통증에 미치는 영향)

  • This study was aimed to analyse the effect of Soru massage therapy on pain of the head and neck of 46 seniors aged over 65. The therapy was given twice a week for eight weeks and a total of 16 sessions for 30 minutes. The results revealed that the pain average on the head and neck of the male and female subjects was statistically significant. For the test group who had a random test, the average of pain on the head and neck of the subjects was statistically significant. Based on the results, Soru massage therapy showed positive results for pain on the head and neck of elderly persons, which indicates that it can improve their health and quality of living. Therefore, this study is an arbitrary alternative therapy which can be applied for the elderly who have pain on the head and neck caused by muscular tension.

Biomechanical Properties of the Cervical Muscles Depending on Using of a Smartphone (스마트 폰 사용에 따른 경추부 근육의 생체역학성)

  • The study was aimed at investigating the relationship between biomechanical properties of cervical muscles and smartphone addiction score in fifty healthy males using smartphone more than 60 minutes each day. The usage of smartphone was evaluated by smartphone addiction survey developed from Korean International Society Agency. Biomechanical properties of three major cervical muscles; splenius capitis, sternocleidomastoid and upper trapezius, were measured by Myoton, and conducted five parameters; frequency, decrement, stiffness, creep, and relaxation time. As results, all parameters had less than 2% of coefficient of variation(CV) between measurement intervals. Also, intra-class correlation coefficient(ICC) indicated a high reliability(ICC>.9, p<.01). Moreover, the smartphone addiction score was significantly different in frequency(r=.353, p<.05) and stiffness(r=.346, p<.05) on upper trapezius; in addition, in decrement(r=-.284, p<.05) and creep(r=.288, p<.05) on sternocleidomastoid. Especially, splenius capitis was closely related with the overuse of smartphone(frequency, r=-.368, p<.01; decrement, r=-.405, p<.01; stiffness, r=-.424, p<.01). In conclusion, this study implied that the overuse of smartphone is significantly related with the damage of cervical muscles, cervical pain, and headache; furthermore, Myoton can be used as an effective device to assess mechanical properties of cervical muscles.

The effect of prone pelvic blocking on hamstring muscle relaxation and sacral movement (엎드린 자세에서의 다리이음뼈 블록중재가 넙다리뒤근육 이완과 호흡시 엉치뼈 움직임에 미치는 영향)

  • The purpose of this study is to investigate the change of hamstring muscle relaxation and craniosacral reciprocal movement by using S. O. T. (sacrooccipital technique) categoryⅠ block-intervention process. 26 subjects with heel tension were randomly divided into two groups (EXP=13 and CON=13). This study carried out the category I block-intervention process on the experimental group and the false block-intervention process on the control group. Then, this study measured the sit-and-reach and sacral reciprocal movement of subjects before and after the intervention. The result values before and after the intervention were analyzed through ANCOVA by using SPSS PC Program (Ver. 20.0). In both investigations, the degree of hamstring muscle relaxation and the change of sacral reciprocal movement, there were statistically significant differences between the experimental group and the control group as [F(1, 23)=7.73, p<0.5] and [F(1, 23)=7.07, p<.05] respectively. According to the results of this study, it is expected that S. O. T category I bllock-intervention technique will have positive effect on both hamstring muscle relaxation and craniosacral reciprocal movement.
