임피던스 심장기록법

  • 임피던스 혈량 측정법(impedance plethysmography)은 인체의 어느 특정부위에서의 혈량의 변화를 환자에게 전혀 고통을 주지않고(noninvasive) 안전하고 간편하게 측정하는 방법으로서, 특히 심장의 빅동량(stroke volume)를 측정하는 분야를 임피던스 카디오그라피(impedance cardiography)라고 한다. 혈량 측정법은 인체의 어느 부위에서 박동하는 혈량은 그 부위로 유입되는 혈량과 유출하는 혈량의 차이 만큼 유발한다는데 근거를 두고 있다. 한편 심전도는 주로 심장의 전기적인 면을 측정하는 반면 임피던스 카디오그라피는 박동량과 심장 근육의 수축능력(contractility)등 심장의 기계적인 면을 측정할 수 있다.

Bruxism의 치료 -장치를 이용한 치료법을 중심으로-

  • Bruxism이란, 일반적으로 기능이외의 목적으로 상하악 치아를 악물거나 갈음질 하는 것이라고 정의된다. 동일한 상태를 서술하는 용어로 neuralgia traumatica, Karolyi effect, occlusal habit neurosis, parafunction 등이 있으며, 수면중의 이갈음질을 bruxism, 주간의 습관성 이갈음질을 bruxomania로 구분하기로 한다. 또한 centric bruxism과 eccentric bruxism으로 분류하기도 한다. eccentric bruxism은 통상적 의미의 수면중의 bruxism이며, 하악의 측방운동중의 이갈음질이고 뽀드득하는 갈음질 소리를 냄, 저작근육의 긴장도가 증가되어 있으면서 등장성수축을 나타내고, 비중심위의(eccentric) 교합간섭이 주요 유발인자인 반면에 centric bruxism은 중심교합위에서 상하악 치아를 간헐적으로 악무는 것(clenching)은 의미하고 - 지속적으로 악무고 있는 것은 clamping이라고 함.- 주로 주간에 행하며 갈음질소리를 들을 수 없고, 역시 저자근육의 긴장도가 증가되어 있으며 등률성수축을 나타내고, 중심교합위에 근접하여 존재하는 교두간섭이 유발인자이다. bruxism이나 clenching과 부정교합의 관계는 bruxism과 clenching은 부정교합을 야기할 수 있고, 부정교합도 역시 bruxism과 clenching을 유발시키는 작용을 할 수 있는 악순환적 관계이다. 현저한 부정교합을 가지고 있는 bruxism환자의 교정치료는 그 목적이 bruxism의 치료라기보다는 오히려 부정교합의 치료이며 부차적으로 bruxism의 치료를 기대하게 된다. 그러나 bruxism이 장기간 지속되어 치아 및 그 주위조직, 악관절과 신경근계에 심한 손상을 주었을때의 교정치료는 일반적으로 제한을 받을 뿐만아니라 bruxism의 처치에는 거의 무의미한 치료가 될 수 있다. 따라서 거자는 bruxism 환자의 교정치료라는 제목의 원고 청탁을 받았으나 편집위원의 양해아래 장치를 이용한 bruxism의 치료에 관하여 서술하려 한다.

근육 피로도 분석시 사용되는 매개변수들간의 민감도 비교 연구

  • 근전도(EMG:Electromyogram)를 사용하여 국부 근육 피로(Localized Muscle Fatigue)를 정량화으로 분석 하기 위해 널리 이용되고 있는 AR(Autoregressive)모델의 1차 계수, RMS(Root Mean Square), ZCR(Zero Crossing Rate), MPF(Mean Power Frequency), MF(Median Frequency)를 선택하여, 근육이 발휘하는 힘과 시간의 흐름에 따라 근육 피로의 정도를 민감하게 나타내는 매개변수를 규명하였다. 피실험자 10명의 좌우 척추세움근(Erector Spinae Muscle)을 대상으로 등장수축(Sustained Isometric Contraction)조건에서 허리의 신전(Extension)운동을 실시하였다. 이때 발휘해야 하는 힘의 수준은 15%, 30%, 45%, 60%, 75% MVC 로 정하였고 각 수준마다 20초 동안 근전도를 측정하 였다. 데이터 분석은 총20초 구간의 근전도를 0.5초 간격으로 나누어 매개변수들을 각각 구하고 분석을 실시하였다. 시간의 흐름에 대한 피로도 분석 결과, AR 모델의 1차 계수와 MPF가 유의한 차이를 보였으며, 낮은 수준의 %MVC에서는 AR 계수가, 높은 수준에서는 MPF가 민감한 반응 결과를 나타냈다. 그리고 근육이 발휘하는 힘의 정도를 분석하기 위해 주로 사용되고 있는 RMS 보다는 더 AR 계수가 모든 수준에서 뚜렷하게 차이를 보인 것이 확인되었다. 따라서 AR 모델의 1차 계수가 근육의 피로 정도와 힘의 수준을 다른 매개변수에 비해 더욱 민감하게 구별함이 입증되었다. 이러한 결과는 다른 분야에서도 근육 피로를 정량적으로 측정하는데 사용될 수 있을 것으로 생각되며, 개인적 변이도를 고려한 확률 기법을 사용한다면 보다 정확한 근전도 분석이 이루어질 것으로 기대된다.있음을 알 수 있었다. 사료된다.의 결과는 자전거 에르고노미터의 결과가 트레드밀의 결과에 87.60%정도 나타났다.음을 관찰하였다. 특히 vitamin C와 E의 병용투여는 상승적으로 적용하여 간세포손상을 더욱 억제시킴을 알 수 있었다.mance and on TFP(Total Factor Productivity) growth which is a pure measure of firm performance.   • 본 연구는 슬관전 신전 직전에 수행된 선행 슬관절 굴곡이 슬관절 신전근의 최대 근력발생 및 근육 활성도(activation)에 미치는 영향을 알아보기 위하여 실시되었다. 16명의 정상인이 Cybex II 등속운동기구에서 3가지 다른 운동 속도($100^{\circ}/s$, $200^{\circ}/s$, $300^{\circ}/s$)와 2가지 다른 근육 활성 조건(선행부하가 없는 조건과 선행부하 조건)에서 슬관절의 최대 신전을 수행하였다. 연구대상자들에게 선행부하 없는 조건에서는 $90^{\circ}$ 슬관절 굴곡 상태에서 $0^{\circ}$까지 최대 신전을 수행하도록 하였고, 선행부하 조건에서는 구심성 슬와부 근육의 구심성 수축으로 $0^{\circ}$에서 $110^{\circ}$까지 굴곡한 후, 다시 $0^{\circ}$까지 최대 신전을 하도록 요구하였다. 종속변인으로는 내측광근과 외측광근의 최대 근전도와 최대 신전근력을 측정하였다. 실험 결과 최대 근전도와 최대 근력 모두 선행부하 조건에서 높게 나타났으며, 운동 속도가 증가함에 따라 최대 근력은 감소하는 전형적인 최대 근력-운동속도 상관법칙이 본 실험에서도 나타났다. 또한 운동속도와 전환속도 간의 간섭이 없는 것으로 나타나, 슬관절 신전근에 선행부하(preloading)를 가하는 것이 최대 근력-운동속도 상관법칙(force-velocity relation)에 영향을 미치지 못하는 것으로 해석되었다. 선행부하로 인해 증가된 최대 근력 및 근 활성도의 증가는 주로 신경촉진에 의한 것으로 해석된다.

  • This study was function experiment or inspection of diagnosis x-ray unit at the hospital. It's how many changes tube voltage, tube current, DOSE value through the experiment depending on temperature increasing. The study want to know whether which parameter shown out of range or not how about image quality and so on. Increasing tube current and DOSE were not only too many radiation to the patient and radiation workers and make bad images but also the tube should be damaged by heat. This study was recommended proper exposure at intervals of seconds because passed inspection, reduced radiations for patient and the tube used long term. This results in the hospital's finances will be very helpful.

Muscle-Induced Accelerations of Body Segments (근육의 힘이 신체 각 부분의 가속도에 미치는 영향)

  • When the functional electrical stimulation is employed to recover mobility to the plegic, it is very important to understand functions of the selected muscles. I have investigated how a muscle acts to accelerate the body segments, since the body segements are connected by joints so that contraction of a muscle not only rotates the segments to which it is attached but also causes other segments to rotate by creation a reaction force at every joint, which is called the inertial coupling. I found that a single-joint muscle always acts to accelerate the spanned joint in the same direction as the joint torque produced by the muscle. However, a double-joint muscle can act to accelerate the spanned joint in the opposite direction to the joint torque produced by the muscle depending on (1) the body position, (2) the body-segmental parameters, and (3) the type of the movement. Investigating the condition number of the inertia matrix of the body-segmental model gave us some insights into how controllable the body-segmental system is for different values of the factors mentioned above. The results suggested that the upright position is the most undesirable position to independently control the three segments(trunk, thigh and shank) and that the controllability is the most sensitive to variation of the shank length and the trunk mass, which implies that accuracy is required particularly when we estimate these two body-segmental parameters before the paralyzed muscles are innervated by using electrical stimulation.

The Comparison of Low Intensity Eccentric Exercise and Dynamic Stretching on Delayed Onset Muscle Soreness (저강도 원심성 운동과 동적 스트레칭이 지연성 근육통에 미치는 효과 비교)

  • We investigated to compare the effects of a low intensity eccentric exercise and dynamic stretching on symptoms of delayed onset muscle soreness (DOMS). The eighteen women who had not participated in a regular exercise programme for the lower extremities in the previous five months were randomly assigned to one of three experimental groups: control group, a low intensity eccentric exercise group and dynamic stretching group. We measured the joint range of motion (ROM), maximal voluntary isometric exercise (MVIC), muscle soreness rating scale and ultrasound image measurement before eccentric exercise inducing DOMS, and 24, 48, and 72 hours after an eccentric exercise inducing DOMS. The exercise programme in a low intensity eccentric exercise group and dynamic stretching group were respectively performed 3 times a week for 4 weeks before eccentric exercise inducing DOMS. There was significantly different between the groups in muscle soreness rating scale and MVIC (p<.05). However, there was not significantly different between groups in ultrasound image measurement and ROM (p<.05). These results suggest that a low intensity eccentric exercise group and dynamic stretching group effectively reduced muscle soreness rating scale out of the symptoms of DOMS. A low intensity eccentric exercise group may be an effective improvement than dynamic stretching group in muscle soreness rating scale.

The Convergence Effect of Histamine and Atropine on Intestinal Contractility (위장관 수축성에 대한 Histamine과 Atropine의 융합성 조절 효과)

  • The aim of the study was to observe the influence and related mechanism of histamine and its analogues used for hypersensitivity tests and used as an indicator of impurities in drugs on the tissue-specific intestinal contraction. Intestinal contraction includes the activation of thick or thin filament regulation. However, there are few reports addressing the question whether this regulation is involved in histamine-induced regulation. We hypothesized that histamine plays a role in tissue-dependent regulation of intestinal contractility. Denuded ileal/colonic longitudinal and circular muscles of male rats were used and isometric contractions were recorded using a data acquisition system. Interestingly, histamine alone didn't increase the contraction of the circular muscle but increased the contraction of the longitudinal muscle. Histamine together with atropine (M3 receptor antagonist) didn't inhibit the contraction of the longitudinal and circular muscle. Therefore, histamine alone and together with atropine increases the ileal longitudinal muscle contraction suggesting that additional mechanisms (decreased receptor density, postreceptor signaling or distribution of agonists) might be involved in the regulation of ileal muscle contractility. In conclusion, histamine and/or atropine has some effect on the regulation of the longitudinal contractility regardless of M3 receptor and the simpler test would be preferred as the drug impurity test compared to more complicated tests.

Biological Activity of Extracts of the Starfish Asterias amurensis (아무르불가사리 추출물의 생리활성)

  • Nowadays, starfish is one of problems about interruption of marine ecosystem, so many researchers are focusing on application of the starfish (Asterias amurensis). In this study, we investigated the contractile and relaxant activity of the smooth muscles and the antimicrobial and antioxidant activity of six different tissues (muscle, gut, liver, tube feet, gonads, and body) of A. amurensis. Frozen samples were extracted with distilled water containing 1% acetic acid. Extracts from all the tissues, except the body tissue, showed potent antimicrobial activity against Escherichia coli D31. The dorsal retractor muscles (DRM) of muscle and gut extracts showed strong contraction responses. On the other hand, contractile activity on esophagus of squid Todarodes pacificus could be detected in all tissues tested. The contractile activity of the liver extract was higher than that of the other tissues. The body, tube feet, and liver extracts showed the contractile activity on the intestine of the panther puffer fish (Takifugu pardalis). Relaxation response on the DRM of starfish (A. pectinifera) was observed in all tissues tested. Increased antioxidant activity was observed in the gut, liver, and body extracts. The results suggest that the starfish (A. amurensis) is a potential source of novel bioactive compounds.

Study on the Short-Term Hemodynamic Effects of Experimental Cardiomyoplasty in Heart Failure Model (심부전 모델에서 실험적 심근성형술의 단기 혈역학적 효과에 관한 연구)

  • Background: To evaluate the short-term effect of dynamic cardiomyoplasty on circulatory function and detect the related factors that can affect it, experimental cardiomyoplasties were performed under the state of normal cardiac function and heart failure. Material and Method: A total of 10 mongrel dogs weighing 20 to 30kg were divided arbitrarily into two groups. Five dogs of group A underwent cardiomyoplasty with latissimus dorsi(LD) muscle mobilization followed by a 2-week vascular delay and 6-week muscle training. Then, hemodynamic studies were conducted. In group B, doxorubicin was given to 5 dogs in an IV dose of 1 mg/kg once a week for 8 weeks to induce chronic heart failure, and simultaneous muscle training was given for preconditioning during this period. Then, cardiomyoplasties were performed and hemodynamic studies were conducted immediately after these cardiomyoplasties in group B. Result: In group A, under the state of normal cardiac function, only mean right atrial pressure significantly increased with the pacer-on(p<0.05) and the left ventricular hemodynamic parameters did not change significantly. However, with pacer-on in group B, cardiac output(CO), rate of left ventricular pressure development(dp/dt), stroke volume(SV), and left ventricular stroke work(SW) increased by 16.7${\pm}$7.2%, 9.3${\pm}$3.2%, 16.8${\pm}$8.6%, and 23.1${\pm}$9.7%, respectively, whereas left ventricular end-diastole pressure(LVEDP) and mean pulmonary capillary wedge pressure(mPCWP) decreased by 32.1${\pm}$4.6% and 17.7${\pm}$9.1%, respectively(p<0.05). In group A, imipramine was infused at the rate of 7.5mg/kg/hour for 34${\pm}$2.6 minutes to induce acute heart failure, which resulted in the reduction of cardiac output by 17.5${\pm}$2.7%, systolic left ventricular pressure by 15.8${\pm}$2.5% and the elevation of left ventricular end-diastole pressure by 54.3${\pm}$15.2%(p<0.05). With pacer-on under this state of acute heart failu e, CO, dp/dt, SV, and SW increased by 4.5${\pm}$1.8% and 3.1${\pm}$1.1%, 5.7${\pm}$3.6%, and 6.9${\pm}$4.4%, respectively, whereas LVEDP decreased by 11.7${\pm}$4.7%(p<0.05). Comparing CO, dp/dt, SV, SW and LVEDP that changed significantly with pacer-on, both under the state of acute and chronic heart failure, augmentation widths of these left ventricular hemodynamic parameters were significantly larger under the state of chronic heart failure(group B) than acute heart failure(group A)(p<0.05). On gross inspection, variable degrees of adhesion and inflammation were present in all 5 dogs of group A, including 2 dogs that showed no muscle contraction. No adhesion and inflammation were, however, present in all 5 dogs of group B, which showed vivid muscle contractions. Considering these differences in gross findings along with the following premise that the acute heart failure state was not statistically different from the chronic one in terms of left ventricular parameters(p>0.05), the larger augmentation effect seen in group B is presumed to be mainly attributed to the viability and contractility of the LD muscle. Conclusion: These results indicate that the positive circulatory augmentation effect of cardiomyoplasty is apparent only under the state of heart failure and the preservation of muscle contractility is important to maximize this effect.
