제5 중족골 근위부 골절 환자의 자기공명영상 검사를 통한 족관절 외측 불안정성 평가의 기여도 (Contribution of Lateral Ankle Instability Evaluation with MRI to Proximal Fifth Metatarsal Fracture)

  • Purpose: One of the main contributors to proximal fifth metatarsal fracture is ankle inversion and the incidence of recurrence may increase in patients with ankle instability. So, the authors confirmed the patients of proximal fifth metatarsal fracture with ankle instability by checking the history and magnetic resonance imaging (MRI) and assessed the value of MRI as therapeutic prognosis and clinical indicators for prevention of recurrence. Materials and Methods: Patients with proximal fifth metatarsal fractures visited our hospital during recent five years were reviewed. 35 patients with suspected damage by ankle inversion had been identified a history of ankle instability and checked the hindfoot malalignment through hindfoot alignment view and MRI was performed prospectively. The patients was devided to three groups on the location of fracture site and the groups were compared each other. Results: The mean time from injury to checking MRI was 10.7 days. There was no structural abnormality and was no significant difference according to the location of fracture. The patients with history of ankle inversion were 31(88.6%) and the patients with history of chronic or recurrent injury were 22 patients (62.9%). The lesion of MRI related to lateral ankle instability were identified in all patients. Conclusion: This study noted a high incidence of lateral ankle instability that was identified by MRI in the patients of proximal fifth metatarsal fracture. Aggressive treatment for lateral ankle instability should be needed for complications as proximal fifth metatarsal fracture to reduce the recurrence and occurrence.

상완골 근위부 불안정성 골절의 수술적 치료 (Operative Treatment of Unstable Fracture of the Proximal Humerus)

  • Unstable fractures of the proximal humerus continue to be difficult problems for orthopaedic surgeons. The optimum treatment of these fractures has remained a matter of controversy. We analyzed the clinical results of open reduction and plate fixation underwent for patients of unstable fractures of proximal humerus after minimum 12 months follow up. The purpose of this study is to evaluate the efficacy of open reduction and rigid plate fixation. Twenty-two patients were managed with open reduction and plate fixation. Mean follow up duration was 20.6 months(range, 12 to 28 mon.). Because the age of patient as a maker of degree of osteoporosis was considered the key factor in the success of anatomic reconstruction, we divided into two groups according to age. Group A was comprised of 12 cases with younger than 50 yrs of age. Ten cases of older than 50 yrs of age were Group B. According to Neer's classification, five cases(22%) were two part fracture, 12 cases(64%) were three part fracture, and three cases(14%) were four part fracture. We used the Neer rating system for evaluating the results. In Group A, overall scores were 79.1. In Group B, overall scores were 76.8. Overall scores in two part fracture were 85, overall scores in three part fracture 78.4 and overall scores in three part fracture 68.3. We achieved excellent or good results in nine cases(75%) of Group A and seven cases(70%) of Group B. Also, we obtained excellent or good results in all cases of two part fracture, ten cases(71%) of three fracture and one case(33%) of four part fracture. The complications were three metal loosening, one avascular necrosis of humeral head, one severe stiff shoulder, one superficial wound infection and one ectopic ossification. The results were excellent or good in 16 cases(73%) out of 22 cases. In conclusion, rigid fixation and supervised early exercise would be a good option for unstable fracture of the proximal humerus.

대퇴 근위부 골절환자에서 이중에너지 방사선흡수계측법을 이용한 부위별 골밀도 비교 (A Comparison of Bone Mineral Density in Osteoporotic Facture of the Proximal Femur Using Dual Energy X-ray Absorptiometry)

  • There were some controversies about direct cause of hip fracture. We attempted to look at 40 osteoporotic proximal femur fractures in women over 50 years between March in 1999 and Febuary in 2000. The bone density of the fracture group and the healthy 85 control group was measured by Dual Energy X-ray absorptiometry(DEXA). The result was compared using age matched paired T test. The results were as follows ; 1. The femoral neck fractures were 14 cases and the trochanteric fractures were 26 cases. Mean age at a fracture was 67.1 years in neck fracture group and 76.5 years in trochanteric fracture. 2. In the control group, the bone density of both side of the proximal femur was measured and it showed statistically no difference between both sides in same person. 3. The bone density of neck, Ward's triangle, trochanter(P<0.05) and lumbar spine(P<0.001) was significantly reduced in the proximal femoral fracture group comparing with the control group. 4. The bone density of neck, Ward's triangle, trochanter(P<0.05) was significantly reduced in the proximal femoral neck fracture group comparing with the control group, but there was no statistical difference in lumbar spine comparing with the control group. 5. The bone density of neck, Ward's triangle, trochanter and lumbar spine(P<0.001) was significantly reduced in the proximal femoral neck fracture group comparing with the control group. We concluded that the bone mineral densities(BMD) of proximal femur and lumbar spine had decreased in hip fractures but that the bone mineral density and T-score % of the proximal femur were statistically lower than that of the lumbar spine. We suggest that measuring the bone mineral density of the proximal femur may reflect the weakness of the proximal femur more precisely than measuring the bone mineral density of the lumbar spine.

심각한 외측 회전을 동반한 전완 기형의 외과적 교정 치료 2 증례 (Surgical Correction of an Antebrachial Deformity with Severe External Rotation in Two Dogs)

  • 간헐적 전지 파행을 보이는 두 마리 개가 내원하였다. 신체 검사에서 약 $90^{\circ}$ 외측 회전을 동반한 전완 기형을 두 마리 모두에서 확인 하였다. 일반 방사선 사진에서 척골 원위부 아탈구, 요골 앞쪽 휨 현상, 짧아진 요골 척골, 및 앞발의 외측 회전을 확인하였다. 외측 회전을 동반한 전완 기형의 교정을 위해 척골 원위부 골절제와 요골 원위부 쐐기형 절골술 및 요골 원위부 내측 회전을 실시 하였다. 관절 부조화를 예방하기 위해 척골 근위부 절골술을 첫 번째 개에서 실시 하였다. 수술 후 정기 검진은 수의사의 신체 검사를 통해 실시 되었으며 두 마리 모두에서 수술과 관련한 합병 증상 및 파행이 없음을 확인하였다.

제 5중족골 근위부 골절과 동반된 족관절, 족부 손상과 유형 (Ankle and Foot Injuries Accompanying 5th Metatarsal Fractures)

  • Purpose: The proximal fifth metatarsal fracture is one of the most common foot fractures. However, few studies have evaluated the associated injuries in patients with a proximal fifth metatarsal fracture. The purpose of this study was to investigate the incidence of foot and ankle joint injuries associated with proximal fifth metatarsal fractures and compare the incidence of these injuries based on the injury mechanisms and location of the fracture. Materials and Methods: This retrospective study included 157 patients with a proximal fifth metatarsal fracture who underwent surgery from January 2014 to August 2021. Their medical records and radiology images were reviewed to classify and analyze the associated injuries. The proximal fifth metatarsal fractures were classified using Lawrence and Botte's classification. Injury mechanisms were divided into direct and indirect injuries. The incidence of injuries was statistically analyzed according to the injury mechanism and classification. Results: Of the 157 patients with proximal fifth metatarsal fractures, 81 (51.6%) were diagnosed with foot and ankle joint injuries. The incidence of foot injuries was 65.4%, that of ankle joint injuries was 19.8%, and of both foot and ankle joint injuries was 14.8%. In patients with direct injuries, the incidence of foot and ankle joint injuries was 82.5% and that of indirect injuries was 41.0%. Statistical differences were observed between the incidence of direct and indirect injuries (p<0.001). The incidence of injuries, according to Lawrence and Botte's classification, was 54.9% (Zone I), 41.2% (Zone II), and 50.0% (Zone III) respectively. However, there were no statistically significant variations in the locations of the proximal fifth metatarsal fractures (p=0.051). Conclusion: In this study, the incidence of foot and ankle joint injuries associated with proximal fifth metatarsal fractures was found to be high. Therefore, a careful physical examination and appropriate radiological evaluation are recommended for patients with such fractures.

대퇴골 골종양의 치료에 있어서의 Angled Blade Plate의 역할 (The Role of Angled Blade Plate in Treatment of Bone Tumor Occurred in Femur)

  • 목적: 대퇴골에서 골 종양은 주로 근위 전자부와 원위 간단부에 발생한다. 그러나 대퇴골은 체중부하와 연관하여 종양의 크기에 따라서는 병적 골절을 유발하게 된다. 그러므로 종양제거후 대퇴골의 해부학적 특성을 고려한 angled blade plate로 보완해 준다면 생역학적 측면에서 그 예후를 훨씬 개선할 수 있을 것 여겨진다. 대상 및 방법: 1991년 10월부터 2005년 4월까지, 대퇴골에 발생한 골 종양을 제거하고 Blade Plate로 보완해준 환자 16명 17예를 대상으로 하였다. 종양제거 후 11예에서 골 이식으로, 6예에서 골 세멘트로 채워 넣었으며, 모든 예에서 angled blade plate를 사용하여 내고정을 시행하였다. 결과: 평균 추시 기간은 16명 환자(17예)에서 55.5개월 이었다. 시술 후 금속에 관련된 실패는 없었다. 종양의 재발로 인하여 재수술이 4예에서 있었으며 재수술전까지 내고정은 유지되었다. 결론: angled blade plate는 종양수술 후 이식 골의 결합이 이루어질 때까지 그 부위를 골절로부터 보호하며, 골 세멘트를 사용한 경우는 골 세멘트와 골조직과의 취약한 이음새를 골절로부터 보호하였다.

Revision Rod를 이용한 흉요추 유합 재수술 예비 결과 (Preliminary Result of Revision Fusion Surgery for Thoracolumbar Spine Using Revision Rod)

  • 목적: 척추 유합술을 받은 환자 중에서 척추 유합 재수술을 받는 경우가 증가하고 있다. 척추 유합 재수술을 위해 기존 장치와 연결기를 통해 연결하여 고정술을 연장하는 revision rod를 새롭게 개발하였다. 본 연구에서는 revision rod를 이용하여 척추 유합 재수술을 시행한 후 임상적·방사선적 결과를 분석하였다. 대상 및 방법: 2개의 대학병원에서 척추 유합술 후 유합 연장술을 시행하고 최소 1년 추적관찰이 가능했던 21명의 환자가 포함되었다. 진단명은 인접 분절 병증 16명, 흉요추 골절 4명, 황색인대골화 1명이었다. 임상 결과는 Oswestry Disability Index (ODI)와 수치평가척도(numerical rating scale, NRS)로 평가하였고, 방사선 결과는 요추 전만각, 흉추 후만각, 재수술 부위의 시상면 각도, 그리고 근위부 인접부의 후만각 및 골유합률을 평가하였다. 결과: 수술 전 평균 ODI는 54.6±12.5에서 최종 추시 시 29.8±16.5로 향상되었다. 허리 통증과 다리 통증 NRS는 5.0±1.7, 6.4±2.0에서 2.9±1.6, 2.9±2.2로 향상되었다. 요추 전만각은 수술 전 18.1±11.9도가 최종 추시 시 21.1±10.3도였고, 근위부 인접부 후만각은 수술 전 10.8±10.1도가 최종 추시 시 9.2±10.5도로 유의미한 차이를 보이지는 않았다. 골유합률은 후외측 골유합을 시행한 1명을 제외하고는 모든 예에서 성공적인 유합이 이루어졌다. 결론: Revision rod를 사용한 흉요추의 재유합술에서 우수한 임상 결과를 보였다. 새롭게 개발된 revision rod와 관련된 문제는 발생하지 않았고 방사선상 골유합은 성공적이었다.

불안정성 대퇴골 전자간 골절에 3C형 무시멘트 대퇴 스템을 이용한 고관절 반치환술의 중기 결과 (Midterm Results of Bipolar Hemiarthroplasty for Unstable Intertrochanteric Femoral Fractures Using a Type 3C Cementless Stem)

  • 목적: 고령의 환자에게 발생한 대퇴골 전자간 불안정성 골절에 대하여 관혈적 정복 및 내고정 이외의 다른 대안으로 고관절 반치환술이 선택적으로 사용되고 있다. 최근 의료 시스템과 기술의 발달로 대퇴골 전자간 골절 후 생존율이 높아지면서 중장기 이상의 기간을 고려한 대퇴 스템의 선택이 필요하다. 이에 저자들은 이중으로 가늘어 지는 직각 단면의 대퇴 스템(C2 stem)을 이용하여 불안정성 전자간 골절에 고관절 반치환술을 시행하고 5년 이상 추시한 환자에 대하여 그 임상적 방사선적 결과를 알아보고자 하였다. 대상 및 방법: 2004년 1월부터 2013년 12월까지 대퇴골 전자간 불안정성 골절에 대하여 C2 스템으로 고관절 반치환술을 시행한 43명의 환자(43 고관절)를 대상으로 하였다. 평균 연령은 78.6세(70-84세), 평균 추시 기간은 85.4개월(60-96개월)이었다. 추시 기간 동안 임상적으로는 통증, 보행 능력, 고관절 기능의 변화를 알아보았고, 방사선적으로는 피질골의 골다공증과 골 흡수 등 근위 대퇴골의 변화를 알아보았으며, 탈구와 인공 관절 주변 골절 등 추시 기간 중 발생한 합병증에 대하여 알아보았다. 결과: 통증은 수술 후 유의하게 감소하였는데, 수술 후 4년부터 증가하였고 수술 후 60개월에 보행 능력 평가에서 9예의 환자가 두 단계 보행 능력이 감소하였으며 Harris 고관절 점수는 수술 후 2년부터 3년 사이에 유의하게 감소하였다. 방사선적으로 피질골 골다공증은 14예 발생하였는데, 이 중 5예의 환자가 피질골 흡수 현상으로 진행하였다. 피질골 흡수 현상을 보인 5예 중 4예는 전자부 골절편이 불유합 된 경우였고, 3예는 역사상 골절인 경우였다. 결론: 골다공증을 동반한 고령의 환자에게 발생한 불안정성 전자간 골절에 3C 형태의 무시멘트 스템을 이용하여 고관절 반치환술을 일차로 시행하는 경우 환자의 건강 상태와 잔여 수명을 고려하여 주의 깊은 선택이 필요할 것으로 생각된다.

상완골 근위부 분쇄 골절에서의 상완골 두 치환술 (Hemiarthroplasty for the Comminuted Fracture of the Proximal Humerus)

  • Purpose: Most proximal humeral fractures are minimally displaced and can be treated satisfactorily with a conservative method. But in many comminuted fractures, hemiarthroplasty is usually done as a primary treatment. The authors evaluated how much functional improvement was achieved after hemiarthroplasty and which factors influence on the final functional results. Materials and Methods: Eleven hemiarthroplasties were performed for three- and four-part fractures(including fracture-dislocation) between April 1992 and June 1999. There were eight women and three men, and the mean age was 65 years. According to Neer classification, six was three-part fracture and five was four-part fracture. Six patients were injured on their right shoulder and five on the left shoulder. Among the five four-part fractures, three had axillary nerve injury and among the six three-part fractures, only one patient had axillary nerve injury. The average follow-up period was 2.4 years(1 year-7 years) after operation. The functional results were evaluated with the UCLA scoring system(Modification for hemiarthroplasty). In addition to the overall results, we compared the results according to the classification of the fracture, the cause of injury, and whether the axillary nerve was injured. Results: At the last follow-up, average total UCLA score was 18.2. The mean score for pain was 7.0, mean muscle power and motion score was 5.5 and 5.7 respectively. The pain relief was more satisfactory than any other functional results. The average score for three-part fractures was 22, and the average for four-part fractures was 13.6. The average score for the patients fractured by vehicle accidents was 15.3, and 19.3 for the patients fractured by slip-down injury. In patients without axillary nerve injury, the average score was 20, and with axillary nerve injury, the score was 15. Conclusion: Shoulder hemiarthroplasty, for the treatment of proximal humeral fractures, cannot restore the shoulder function to normal, but can achieve the functional result to some degree, especially for the activity of daily living. And as for pain, we think that it is relatively effective measure. And we think that the severer the comminution, the more the chance of axillary nerve injury, and the poorer the functional results. In conclusion, the severity of initial injury seems to be the major prognostic factor.

폴라루스 골수정을 이용한 상완골 근위부 골절의 치료 (Treatment of Proximal Humerus Fracture by Polarus Nailing)

  • Purpose: The purpose of this study was to evaluate the usefullness of polarus nailing in the treatment of proximal humerus fractures including 2 part, 3 part and proximal comminuted fractures. Materials and methods: Fifteen cases of proximal humerus fracture treated with Polarus nailing from March, 2002 to March, 2004 were selected. Man was 3 cases, and woman was 11 cases. There were one case of follow up loss due to decease. Average age was 60 years old (range, 23 to 84), and there were 6 cases of 2 part fracture, 3 cases of proximal segmental fracture, 5 cases of 3 part fracture. We analyzed the outcom results between 2 part fracture and 3 part fracture. The average follow up period after the operation was 1.5(range, 1 to 2) years. Range of motion (ROM), pain and functional outcome were evaluated by visual analogue scale(VAS) and american shoulder and elbow surgery (ASES) activity of daily living (ADL) functional scoring system. Results: All cases showed union on radiologic evaluation, with 2.3 months follow up. In 5 cases of 3 part fracture, average union time was 1.9 months regardless of proximal screw loosening in 4 cases. In 2 part fracture union time was 2.2 months (P>0.05). VAS pain score was 1.3, ROM was $160^{\circ}$ in forward flexion, $40^{\circ}$ in external rotation, L3 level in internal rotation, and ASES, ADL functional score was 21 in 2 part fracture. VAS pain score was 1.25, ROM was $160^{\circ}$, $43^{\circ}$ and L1 level, and ASES, ADL functional score was 21 in 3 part fracture. There were no statistically significant difference between two groups (P>0.05) VAS pain score was 1.6, ROM was $170^{\circ}$, $47^{\circ}$ and L3 level, and ASES, ADL functional score was 23 in proximal comminuted fracture. Conclusion: Polarus nailing could be used as an effective modality in certain cases of proximal humeral fracture including 2 part, proximal segmental and in cases of 3 part fractures with large greater tuberosity fragment.
