Isolated Spinous Process Fracture of the $7^{th}$ Cervical Vertebrae in a Novice Golfer: a case report (골프 초보자에서 발생한 제 7경추 극돌기 골절 -1예 보고-)

  • Fractures of isolated spinous processes of cervical and thoracic vertebrae are called as Clay shoveler's fracture. Such fractures related to sports activities are quite rare. In this report, a case of 27-year-old male with isolated spinous process fracture of C7 during golf swing is reported. The patient was prescribed on muscle relaxant and analgesic therapy. Cervical immobilization was maintained for four weeks with a cervical collar. The patient gradually recovered and was asymptomatic. He started playing golf twice a week after 5 months from the onset of pain.

  • Previous reports have introduced the technique of spinous process osteotomy to decompress spinal stenosis, a procedure which aims to afford excellent visualization while minimizing destruction of tissures not directly involved in the pathologic process. However, bio-mechanically it has not been investigated whether the sacrifice of posterior spinous process might have potential risk of spinal instability or not, even though supra-spinous and inter-spinous ligament are preserved. Therefore the aim of this study is to evaluate the bio-mechanical properties after spinous process osteotomiy, using finite element analysis. In the model of spinous process osteotomy the increase of stress in the disc and segmental rangesof motions were not changed significantly. It is due to the fact that the instability of lumbar spine has been maintained by the two-types of ligaments compared with the prior surgical technique. Therefore, according to the finite element result on this study, these osotetomy was considered to be a clinicallysafe surgical procedure and could not cause the instability of patient.

  • Objectives : Yogaktong is the genernal term of lowback and sciatic pain. Simpley this is not for the syndrom or illiness but for the symptoms. In this study, the effect of Acupuncture and Moxibustion Treatment by Analysing Su-point(背兪穴) compared with X-ray measuring for three patients with Yogaktong(腰脚痛) were evaluated. Methods : After treatment with the Acupuncture and Moxibustion Treatment by Analysing Su-point(背兪穴) compared with X-ray measuring for one time, we evaluated the effect by the figure of x-ray measuring and complaints of patients. Results : In three cases, the change in the figure declined dramatically and symptoms of patients got better after one treatment. Conclusions : Acupuncture and Moxibustion Treatment by Analysing Su-point(背兪穴) compared with X-ray measuring was effective. It will be attempted to the patients with it.

It is due to the fact that the instability of lumbar spine has been maintained by the two-types of ligaments compared with the prior surgical technique. Therefore, according to the finite element result on this study, this osotetomy was considered to be a clinically safe surgical procedure and could not cause the instability of the lumbar spine.

  • This study was proven for the T-spine breathing technique in lateral projection, using computer radiography (CR), charge coupled device (CCD), indirect digital radiography (IDR) and direct digital radiography (DDR). All images were evaluated and compared with CNR and SNR measured with the mean pixels and the standard deviation as setting ROI of spinous process, pedicle, vertebral body, intervertebral foramen and intervertebral disk using Image J. In experiment results of 4 type detectors, T-spine breathing technique was indicated as excellent in ROI of spinous process, pedicle, vertebral body, intervertebral foramen and intervertebral disk. As T-spine breathing technique indicated excellent images compared to the existing T-spine lateral radiography, this method would be useful for elderly patients who have difficulty in deep exhalation. This study was indicated the application possibility of T-spine breathing technique by presenting contrast to noise ratio (CNR) and signal to noise ratio (SNR) with quantitative value in 4 type detectors.

Expansive Laminoplasty for Cerical Compression Myelopathy (경수 압박으로 인한 척수증 환자에서의 경추강 확장술)

  • In an attempt to eliminate some negative aspects of conventional extensive laminectomy, 4 cases of multiple level of cervical compression myelopathy, 1 OPLL(ossification of posterior longitudinal ligament) and 3 cervical stenosis, were treated with a technique of expansive laminoplasty. Operative results in all patients were satisfactory without surgical complications and all patients had a neurological improvement. We suggest that our technique is more effective one for cervical canal enlargement with preservation of stability for treating multiple level of cervical canal stenosis, OPLL, and spondylosis than conventional extensive laminectomy.

Arthroscopic Capsular Release in Refractory Adhesive Capsulitis of the Shoulder (견관절 난치성 유착성 관절 낭염에서 관절경 하의 관절낭 유리술)

  • Purpose: To evaluate the clinical results and prove the effectiveness of arthroscopic capsular release in refractory adhesive capsulitis of shoulder. Materials and Methods: We preformed arthroscopic capsular refractory adhesive capsulitis that not responded by stretching execies for above 1 year. 21 cases were followed above 1 year and average follow up 3 years 1 months (1${\sim}$5 years). We checked VAS of pain, ADL of function. UCLA score which were evaluated at preoperation, postoperation 6 months, 1 years and last follow up period, and compared with each other at last follow up. Results: The VAS score improved average preoperative score 8 to average postoperative score1, the ADL score improved average preoperative score 7 to average postoperative score 26, the UCLA score improved average preoperative score 8 to average postoperative score 34. Forward elevation improved average preoperative 75 degrees to average postoperative 175 degrees, external rotatiion at side improved average preoperative 4 degrees to average postoperative 52 degrees, abduction improved average Preoperative 60 degrees to average postoperative 170 degrees, internal rotation at posterior improved preoperative thigh-lumbar 3 spinous process to postoperative 7th thoracic spinous process~9th thoracic spinous process. Conclusion: Arthroscopic capsular release in refractory adhesive capsulitis that non responsive to stretching exercise for above 1 year were effective treatment method.

A Comparison of Body Shape Changes Between Deep Tissue Massage and Illite-Combined Deep Tissue Massage - Focusing on women in their 30s - (딥티슈마사지와 일라이트병행 딥티슈마사지의 체형변화 비교 -30대 여성을 대상으로-)

  • This study aims to put forth an efficient way of improving body shapes by examining the effects of deep tissue massage and illite-combined deep tissue massage on body shape changes, and identifying body shape changes when applying each method. This study targeted twenty women in their thirties, and ten separate subjects were placed in different groups. Then deep tissue massage and illite-combined deep tissue massage were performed once a week, for a total of eight weeks. Moire Topography was applied before the experiments, four weeks later and eight weeks later to compare changes in spinous process inclination, shoulders and hips. The data collected were analyzed using SPSS v. 21.0, and the study results are as follows. In relation to general characteristics of the subjects, professionals occupied the highest proportion of them, and 90% of them were married. Here, 77.8% of them had experience in giving birth, and 78.6% of them chose natural birth. In addition, 57.1% of the subjects holding a majority had two children. When measuring spinous process inclination, shoulders and hips in the illite-combined deep tissue massage group and in the deep tissue massage group before the experiments, the illite-combined deep tissue massage group showed somewhat higher values in every area than the deep tissue massage group, but no statistically significant differences were not found, which means the homogeneity existed between them. When comparing body shape changes between the two massage methods, there were significant differences(p<.05, p<.01), because the illite-combined deep tissue massage group showed a much higher decline in spinous process inclination, shoulders and hips than the deep tissue massage group. This implies illite-combined deep tissue massage was more effective in improving body shapes than deep tissue massage. Therefore, illite-combined deep tissue massage is considered to be helpful in improving body shapes, and it is anticipated that this massage method can be used in relevant fields, including the skin care industry.

Arthroscopic Treatment of Calcific Tendinitis of the Rotator Cuff (회전근 개 석회화 건염의 관절경적 치료)

  • Purpose : The purpose of this study was to evaluate results of the arthroscopic treatment of calcific tendinitis of the shoulder resistant to conservative treatment. Materials and Methods : From March, 1996 to June, 1998, fourteen patients underwent shoulder arthroscopy to treat resistant calcific tendinitis of the rotator cuff despite conservative treatment for more than 6 months. Calcium deposits were localized to the supraspinatus tendon only in eleven patients, the supraspinatus and infraspinatus tendon in two patients, and to the supraspinatus and subscapularis tendon in one patient. Each shoulder was evaluated with UCLA shoulder rating scale and Constant-Murley score Results : The Constant-Murley pain score improved from average score 3.2 before surgery to average score 8.3 after surgery, and the UCLA functional average score improved from 4.5 preoperatively to 8.3 postoperatively. Preoperative ROM averaged $110^{\circ}$ of flexion, $45^{\circ}$ of external rotation, $90^{\circ}$ of abduction, and internal rotation with the thumb reaching to the spinous process of the third lumbar vertebra, but postoperative range of motion averages improved as follows: $170^{\circ}$ or flexion, $50^{\circ}$ of external rotation, $140^{\circ}$ abduction, and internal rotation with the spinous process of the twelveth thoracic vertebra. Overall 3 patients were rated excellentm 9 were good, 2 were fair. Conclusion : Shoulder arthroscopy is an effective treatment in patients with refractory calcific tendinitis.
