Reliability of self-reported data for prevalence and health life expectancy studies: comparison with sample cohort DB of National Health Insurance Services (자가 응답식 자료에 근거한 유병률 및 건강기대수명 연구의 신뢰도 분석: 건강보험 표본코호트 DB와의 비교)

  • Korea Health Panel (KHP) data and Korea National Health and Nutrition Examination Survey (KNHANES) data are collected by self-assess and self-report for individual's health status and medical use. Previous studies have claimed that the reliability for prevalence rates and health life expectancies obtained from these data should be validated. National Health Insurance Services in Korea recently released a sample cohort DB that contain all data related to the use of medical facilities for all entire Korea citizens. It has been shown that disease-specific prevalence rates calculated from these data are representative and reliable for the entire population. In this paper, we evaluate the reliability of prevalence rates derived from self-reported data such as KHP and KNHANES by comparing to the prevalence rates from the sample cohort DB. We found that both KHP and KNHANES underestimate prevalence rates and in turn overestimate health life expectancies. Moreover, the general trends of health life expectancies might be distorted (except for the sample cohort DB) because of sampling and non-sampling errors.

The relationship between smoking and stroke by general characteristics : using the 6th Korea national health and nutrition examination survey (일반적 특성에 따른 흡연과 뇌졸중의 관련성 : 제6기 국민건강영양조사 이용)

  • The purpose of the study was to determine the general characteristics that affect strokes caused by smoking, to provide basic data and political grounds for preventing strokes. Data were used from the 6th (2015) of the National Health and Nutrition Survey. Among the study participants were 2,146 smokers and 132 strokers. Data were analyzed using SPSS ver. 21.0. The findings of the study were as follows: strokes increased the odds ratio 1.7 times(p<0.001) when smoking was performed, and the risk factors for strokes caused by smoking were statistically significant in age(p=0.008, p=0.10), education level(p=0.003, p=0.008), hypertension(p<0.001), and subjective health conditions(p=0.008). Therefore, it is necessary to develop health education contents using smart devices and a consistent national health policy to reduce the occurrence and risk of stroke.

  • 우리나라는 현재 65세 이상 인구가 전체 인구의 7.1%에 달하고 있으며 앞으로 계속 증가하여 2022년에는 14.3%로 고령화 사회에 접어들 것으로 여겨진다. 1998년 국민 건강영양조사에 의하면, 우리나라 노인은 영양취약집단으로 영양상 문제가 발견되었으며, 청장년기까지는 식생활의 부족시대를 지니고, 중년이후의 풍요로움을 누린 후 노년기를 맞고 있는 전체 인구 집단과 또 다른 문제를 보이고 있어 관리가 요구된다. 노인의 경우 여러 가지 신체기능의 변화와 사회생활의 변화로 영양요구량이 달라지고 있으며, 신체기능의 변화는 식품섭취에 영향을 주고 있어서 적절한 관리를 필요로 한다. 그래서 10월의 이달의 건강길라잡이에서는 첫째, 노년층의 영양 무제가 다른 연령층과 어떻게 다른가 이해하고 둘째, 노년기의 노화 현상이 노인들의 건강문제와 영양문제에 어떤 영향을 미칠 것인가 살펴보고 셋째, 일반적으로 노년기의 영양필요량이 청 · 장년기의 영양 필요량과 무엇이 다른가 이해한 후 마지막으로 노인의 식생활을 어떻게 계획할 것인가 알아보고자 한다.

A Study of Association Dining-out, Nutritional Intakes and Health Risk Factors among Korean Women using the Data of Korean National Health and Nutrition Examination Survey (KNHANES VI) (한국 여성의 외식 빈도에 따른 영양섭취 및 건강 위험 인자)

  • The aim of this study was to investigate the association dining-out frequency, nutritional intake level and health risk factors related to metabolic syndrome among Korean women. The data in this study were obtained from the Korean National Health and Nutrition Examination Survey in 2013 (KNHANES VI). Sample of Korean women (n=3,413), aged over 20 with biochemical, dietary and health screening data, was divided into three groups according to dining-out frequency; "Rarely" (< 1 time / week), "Occasionally" (2~4 times / week) and "Frequently" (${\geq}5times/week$). The results showed that there were significantly different intake levels in energy, water, protein, fat, calcium, sodium and vitamin A according to the dining-out frequency. Each dining-out frequency group was significantly associated to obesity and Low HDL Cholesterolemia. In conclusion, this study found significant association between dining-out frequency, nutritional intake and health risk factors among Korean women. Further studies are necessary to examine subdividing into age and dining-out frequency and health risk factors related to metabolic syndrome among Korean.

Plant Proteins in Relation to Health-related Quality of Life in South Korean Individuals Aged 50 Years or Older: Korea National Health and Nutrition Examination Survey 2016-2018 (50세 이상 한국인의 식물성 단백질 섭취와 건강 관련 삶의 질과의 연관성: 국민건강영양조사 2016-2018)

  • This study aimed to investigate the association between plant protein intake and health-related quality of life (HRQoL) in Korean individuals aged 50 years or older. Using the 2016-2018 Korea National Health and Nutrition Examination Survey data, 7,956 participants (3,434 men and 4,522 women) were included in the study. HRQoL was measured using the Euro-quality of life five-dimension (EQ5D), composed of physical function, self-care, daily activities, pain/discomfort, and anxiety/depression problems. The EQ5D estimates were converted into an EQ5D index score. The association between daily intake of plant proteins and HRQoL was evaluated using regression analysis. The intake of total plant protein, legume nut seed (LNS), and fermented bean (FERMB) proteins were all positively associated with HRQoL in both men and women (p < 0.01). Of EQ5D's five dimensions, physical function and daily activities were most commonly associated with plant protein intake. Compared to LNS protein or FERMB protein intake, total plant protein intake showed a better association with HRQoL. In conclusion, a significant association was observed between plant protein intake and HRQoL. It suggests that adequate intake of plant protein might be helpful for the maintenance or improvement of HRQoL in Korean adults.

Factors Related to Allergic Rhinitis in Korean Adults (한국 성인의 알레르기 비염 관련요인)

  • This study was conducted to investigate the factors associated with allergic rhinitis in Korean adults. The investigation was based on secondary data analysis using the National Health and Nutrition Survey data. The subjects of this study were 5,841 adults aged 19 years or older who participated in the National Health and Nutrition Survey for the first year (2016). The collected data were analyzed by complex sample multiple logistic regression analysis using SPSS Statistics 24. The prevalence of allergic rhinitis was 15.4%. In addition, factors related to allergic rhinitis included gender, age, marital status, stress, depression, asthma and atopic dermatitis. The results of this study can be used as basic data for policy and interventions to prevent allergic rhinitis and manage symptoms.

  • The objective of this study was to develop a model for predicting Korean adult consumers who frequently eat food-away-from-home. A total of 7,032 adults aged 19 years and older from the 2001 National Health and Nutrition Survey in Korea were used as subjects. The data were analyzed using a data mining procedure including logistic regression and decile analysis. The model developed in the study was proven to be valid in predicting the consumers who frequently eat food-away-from home(once a day or more often). This model showed that consumers eating food-away-from-home frequently tend to be younger men, living in a big city, working full time, receiving more stress and eating snacks and fried food more frequently. The model could be used to identify targets for nutrition and related education and consumer segments for the marketing of restaurant businesses.

  • A total of 5,597 Korean subjects aged 20-64 years were analyzed using the KNHANES 2013-2015. Among the subjects, 41.1% of males and 5.5% of females were smokers, and risk of developing metabolic syndrome in smokers was significantly increased in men with age, alcohol, physical activity, obesity, and sleep(aOR 1.785, 95% CI 1.004-3.174), whereas it was not significantly higher in women. As a result of analyzing the difference of average nutrient intake according to smoking and metabolic syndrome, the energy, retinol and vitamin C intake were significant when age and gender were corrected in the smoker and metabolic syndrome group. In this study, we found that smoking behavior and metabolic syndrome were related to nutrient intake which requires a national level of lifestyle intervention for the prevention and management of metabolic syndrome.

  • This study was conducted to analyze the relationship between the levels of lead (Pb) exposure and cardiovascular disease (CVD) in Korean adult women. We used cross-sectional data on blood lead and self-reported diagnoses of ischemic heart disease (IHD), stroke and hypertension in a subsample of 1.821 adults 19 years and older who participated in the 2017 Korea National Health and Nutrition Examination Survey (KNHANES). CVD and blood Pb concentrations were analyzed through logistic regression analysis, and correlations between factors were confirmed using the pearson correlation coefficient. An increase of blood Pb was associated with an increased risk of IHD (OR 5.68, 95% CI 1.01-17.51) and hypertension (OR 3.37, 95% CI 2.24-5.07) only in women. Additionally, there was a correlation between blood Pb and nutrient intake. This suggest that blood Pb levels may be used as a key predictor of CVD development, and that women are more susceptable to IHD and hypertension associated with Pb exposure.

Factors Influencing Quality of Life among Cancer Survivors: Using KNHANES 2010-2014 (암 생존자의 삶의 질에 영향을 미치는 요인: 2010-2014 국민건강영양조사 자료 활용)

  • The purpose of this study was to explore factors influencing quality of life among cancer survivors in order to identify strategies for cancer survivors to go back to normal life and carry on stable and high quality life. This study used Korea National Health and Nutrition Examination Survey(2010-2014), conducted by Korea Centers for Disease Control and Prevention(KCDC). The sample of this study was 610 cancer survivors and their average time since diagnosis was about 11 years. The most powerful influencing factors for quality of life among cancer survivors were activity limitation(${\beta}$=.30, p<.001), followed by age(${\beta}$=-.20, p<.001), subjective health status(${\beta}$ =-.19, p<.001), and economic status(${\beta}$=.12, p=.004). According to the result, limitation on daily activities or social activities due to physical or mental debilitation was a strong factor that lowers quality of life among cancer survivors. Hence, there is a need for manpower and infrastructure that facilitates, supports, and manage daily activities of cancer survivors.
