  • Stimulatory effects of epidermal growth factor (EGF) and transforming growth $factor-{\alpha}$($TGF-{\alpha}$) on the growth of squamous cancer cell lines established from human oral cancer tissue with moderate differentiation were studied in vitro. After culturing in serum-free media for 24 hours, growth factors-EGF only, $TGF-{\alpha}$ only and EGF, $TGF-{\alpha}$ together-were added to the media and numbers of cells were analyzed by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay and compared with the control at 96, 144 hours. Each of EGF and $TGF-{\alpha}$ showed statistically significant stimulatory effects on the growth of cells respectively. Dose-dependent relationship of the stimulatory effects were not clearly demonstrated. The effects of EGF were higher than those of $TGF-{\alpha}$ and combinative administration showed higher effects than those of single uses. In conclusion, EGF may play an important and major role in differentiation and growth of human oral squamous cancer cells. $TGF-{\alpha}$, produced from cells activated by EGF, also can stimulate the cell growth and could be an alternative ligand for EGF receptor.

구강 편평상피세포암 동위종양 모델에서 내피세포의 수용체 타이로신 인산화효소에 대한 표적치료 (TARGETING RECEPTOR TYROSINE KINASE ON ENDOTHELIAL CELLS IN AN ORTHOTOPIC TUMOR MODEL OF ORAL SQUAMOUS CELL CARCINORMA)

  • Purpose: We determined the therapeutic effects of blockade of epidermal growth factor(EGF) and vascular endothelial growth factor(VEGF) receptor tyrosine kinases on the growth of oral squamous cell carcinoma(OSCC) xenografted in athymic nude mice. Experimental Design: We investigated the in vivo antitumor effects of a tyrosine kinase inhibitor for EGFR and VEGFR-2, AEE788 in a mouth floor(orthotopic) tumor model. Nude mice with orthotopic tumors were randomized to receive AEE788, paclitaxel, a combination of AEE788 and paclitaxel, or control. Antitumor mechanisms of AEE788 were determined by immunohistochemical/immunofluorescent and apoptosis assays. Results: Tumors of mice treated with AEE788 demonstrated down-regulation of phosphorylated EGFR, phosphorylated VEGFR and their downstream mediators(pMAPK and pAkt), decreased proliferative index, decreased microvessel density(MVD). As a result, growth of the primary tumor and nodal metastatic potentials were inhibited by AEE788. Conclusion: These data show that EGFR and VEGFR can be molecular targets for the treatment of OSCC.

항암제에 저항성을 가지는 YD-9 human oral squamous carcinoma cell line에서 Pseudomonas aeruginosa exotoxin A의 p53 단백질 누적과 caspase를 활성화 경로를 통해 유도된 세포자멸사 (Pseudomonas aeruginosa Exotoxin A Induces Apoptosis in Chemoresistant YD-9 Human Oral Squamous Carcinoma Cell Line Via Accumulation of p53 and Activation of Caspases)

  • 구강편평상피암종은 말기에서 종종 화학치료요법제들이 유도하는 세포자멸사에 저항성을 보인다. 박테리아의 독에 대한 진전된 이해는 암치료에 대한 새로운 치료전략으로 제기되어지고 있다. 본 연구는 Pseudomonas aeruginosa exotoxin A (PEA)가 세포자멸사 기작을 통해 항암제에 저항성을 보이는 YD-9 구강편평상피암종의 생존율을 현격하게 떨어뜨림을 설명하고 있다. 세포자멸사현상은 핵의 형태학적 변화와 DNA 분절 생성을 통해 입증되었다. PEA는 caspase-3, -6, -9 의 분절과 활성화를 일으켰다. 그리고 이러한 반응들은 caspase 의 기질에 해당하는 poly (ADP-ribose) polymerase (PARP), DFF45, 그리고 lamin A 의 단백질 분해를 야기했다. 사립체 막전위 감소, cytochrome c와 Smac/DlABLO의 사립체로부터 세포질로의 유리, 그리고 AIF의 사립체에서 핵으로 이동 등이 관찰되었다. p53, p21 그리고 $14-3-3{\gamma}$는 증가되는 반면 cyclin B와 cdc2는 감소되었다. 이상의 결과들을 종합해 보면 PEA는 caspase를 활성화시키고, 사립체에 변화를 야기시키고 더 나아가서 세포주기 유전자를 조절함으로써 항암제에 대한 강한 저항성을 보이는 YD-9 세포에서 세포자멸사를 유도한다.

  • Primary intraosseous carcinoma(PIOC) is defined as a squamous cell carcinoma arising within the jaw, having no initial connection with the oral mucosa. The squamous cell carcinoma within the bone can be presumably developed from residues of the odontogenic epithelium, therefore, it is seen in the jaw only. Metastatic carcinoma from another primary site should be excluded in the diagnosis of Primary Intraosseous Carcinoma. This is a case of 62-year-old man, who initially diagnosed as odontogenic cyst on maxilla, but its pathologic examination was diagnosed as squamous cell carcinoma with odontogenic cyst. We treated this patient with partial maxillectomy, modified radical neck dissection(mRND), and postoperative radiation therapy.

구강 편평상피세포암 마우스 모델에서 상피성장인자 수용체 억제제를 적용한 분자표적치료 (TARGETED MOLECULAR THERAPY IN A MURINE MODEL OF ORAL SQUAMOUS CELL CARCINOMA WITH AN EPIDERMAL GROWTH FACTOR RECEPTOR INHIBITOR)

  • Purpose: We determined the therapeutic effect of an epidermal growth factor receptor (EGFR)-specific monoclonal antibody (mAb), cetuximab (Erbitux) on the growth of oral squamous cell carcinoma (OSCC) xenografted in athymic nude mice. Experimental Design: We induced subcutaneous tumors by inoculating human tumor cell suspension into the right flank of nude mice. Nude mice with subcutaneous tumors were randomized to receive cetuximab alone, paclitaxel alone, cetuximab plus paclitaxel, or a placebo (control). Antitumor mechanisms of cetuximab were determined by immunohistochemical and apoptosis assays. Results: Cetuximab, paclitaxel, and cetuximab/paclitaxel combined therapy resulted in 50%, 52%, 67% in vivo inhibition of tumor proliferation, respectively. Tumors of mice treated with cetuximab plus paclitaxel demonstrated decreased PCNA-positive tumor cells and increased apoptotic tumor cells, which slowed growth of the murine tumors. Conclusion: These data show that EGFR can be a molecular target for the treatment of OSCC. And combination therapy with cetuximab and paclitaxel warrants further clinical study.

  • Several investigators have shown that human papillomavirus(HPV) appear to play an etiologic role in oral and paranasal sinus carcinoma. It was known that 15-25% of head and neck squamous cell carcinoma(SCC) showed HPV-positive infection. Among them, HPV 16 was the most common type but HPV 18 was observed only 2-4% of HPV-positive head and neck cancers. In recent, we treated uncommon 2 oral SCC cases that associated with HPV infection. One is a case of tongue SCC after bone marrow transplantation(BMT), and the other is a case of SCC occurring with aspergillosis in the maxillary sinus. After surgery, HPV 16 and 18 were detected in the surgical specimens by the histological and polymerase chain reaction(PCR) examination. In this report, we present these cases with a review of literature.

백서 혀에서의 4-nitroquinoline 1-oxide 유도 발암과정에서 Bcl-2 계 유전자의 발현 (Expression of Bcl-2 Family in 4-Nitroquinoline 1-Oxide-Induced Tongue Carcinogenesis of the Rat)

  • 전 세계적으로 구강암의 빈도는 점점 증가 추세이며, 특히 한국인의 있어 혀(tongue)는 구강암이 가장 호발하는 장소이다. 구강암은 발암 단계에서부터 과증식 병소(hyperplastic lesion), 이형성(dysplasia) 및 상피내암(carcinoma in situ) 을 거쳐 악성 암종으로 발전하는 다단계 발암과정을 보이며, 분자 생물학적 변이가 구강암을 진행시킴이 널리 알려져 있다. 또한, 구강암은 일반적으로 암세포의 증식 및 고사(apoptosis)의 억제가 중요한 역할을 하고 있다 알려져 있다. 그리고, Bcl-2 family 는 세포 고사에 주요한 역할을 하고 있음이 알려져 있다. 그러나, 이들과 관련한 구강암 발생과정의 변화에 대해서는 널리 연구된 바가 없다. 본 연구는 백서에서 발암 물질인 4-NQO로 구강암을 유도시키고, 구강암 발생 다단계별로 Bcl-2 family의 mRNA 변화를 RT-PCR을 이용해 살펴보았다. Bcl-2 family는 크게 3군, 즉 1) anti-apoptotic, 2) pro-apoptotic, 그리고 3) BH3 only protein으로 분류할 수 있으며, 본 연구에서 anti-apoptotic molecules인 Bcl-w는 모든 군에서 발현이 감소되었으며, Bcl-2는 발현이 증가 되었다. pro-apoptotic molecules에서는 Bad가 제 3군 (편평세포암종)에서 발현이 증가 되었고, 나머지는 감소하였다. BH-3 only protein에서는 Bmf가 제 2군에서, BBC3가 제 3군에서 발현이 증가하였고, 나머지는 모든 군에서 감소하였다. 결론적으로, 4-NQO로 유도된 백서의 발암단계에서, Bcl-2 family의 mRNA 양상은 다양하게 관찰되었으나, Bad 및 BBC3 mRNA가 제 3군에서, Bmf mRNA가 제 2군에서의 발현이 특별함을 알 수 있어, 다단계 발암과정에서의 구강암을 진단하는데 유용하리라 사료된다.

  • The major manifestations of aplastic anemia are characterized by persistent bone marrow aplasia and peripheral pancytopenia due to defective stem cells in which differentiation is not curtailed or preferential elimination of stem cells with self-renewal capacity, which results in serious complications such as bleeding and infection. Recent advances in the therapeutic modalities, such as bone marrow transplantation and immunosuppressive therapy, and in supportive measures as component transfusion and infection control with better quality of antibiotics resulted in an improved survival and subsequently increased possibility of complications which need surgical intervention. Considering the fact that surgery may not only control complications, but offers the opportunity to give effective therapy for aplastic anemia and therefore improves chances for survival, it is strongly suggested that active surgical intervention should be performed. We report a case of patient with aplastic anemia who had been performed surgical intervention of partial maxillectomy for the squamous cell carcinoma on maxilla without serious complications during perioperative and postoperative period.

상악동에 발생한 기저양 편평상피세포암 (BASALOID SQUAMOUS CELL CARCINOMA IN THE MAXILLARY SINUS)

  • Basaloid squamous cell carcinoma(BSCC) is uncommon and distinct variant of squamous cell carcinoma that arises mostly in the upper aerodigestive tract and aggressive, high grade tumor with an increased tendency to be deeply invasive, multifocal, and metastatic even at the initial presentation. The typical microscopic features of carcinoma with a basaloid pattern in intimate association with a squamous component helps in diagnosis of this tumour. Since Wain's report in 1986, BSCC of oral cavity, the palate, floor of the mouth, nasopharynx, oropharynx and mastoid region have been reported. However, BSCC in the nasal cavity or in the paranasal sinuses is rare and there are few reports in the Korean literature. We had experienced a case of basaloid squamous cell carcinoma that occurred in the left maxillary sinus of 72-year-old woman and reported with review of the clinical and pathologic features from the literature.

구강편평상피세포암에서의 치성요인 (THE DENTAL FACTORS IN ORAL SQUAMOUS CELL CARCINOMA)

  • The oral cavity has frequent contacts with many carcinogenic compounds and its soft tissue is continuously stimulated by numerous dental factors. We have examined the detailed dental factors and its correlation with oral squamous cell carcinoma, and denture-wearing effects to analyze the effect of the dental factors on the genesis of oral squamous cell carcinoma. We have studied clinical contributing factors and the dental factors in the genesis of oral squamous cell carcinoma when the effects of smoking and drinking are controlled. The study cases are 100 patients(75 males and 25 females) who were diagnosed histo-pathologically as squamous cell carcinoma at the Yonsei Medical Center. The control group was 154 patients who have no systemic malignant tumors. The effects of 6 dental factors were analyzed in this study. They were divided into the smoking group, the non smoking group, the drinking group, and the non-smoking group. The effects of dental factors were analyzed in each group. In this study, we have drawn some conclusions on the relationship between the dental factors and oral squamous cell carcinoma using $x^2$-test. 1. The repaired teeth have statistical significance on the genesis of squamous cell carcinoma. This is probably due to the combining effects of past poor oral hygiene and continuous stimulation due to poor prosthesis. 2. There is statistical significance of the lost teeth in the smoking group, and the repaired teeth and the degree of alveolar bone resorption had statistical significance in the non-smoking group. 3. Smoking and drinking by-itself have no statistical significance in the genesis of oral squamous cell carcinoma. However, in combination, they have statistical significance. In this study, dental factors had a synergistic effect with smoking and drinking. Together with avoidance of smoking and drinking, appropriate restoration and oral hygiene control are most important factors in the preventive aspects of the oral squamous cell carcinoma.
