REMOVAL OF A PIERCING BAR IN THE MOUTH FLOOR BY FLUOROSCOPY: REPORT OF A CASE (형광 투시법을 이용한 구강저에서의 피어싱 바의 제거: 증례보고)

  • The piercing is increasing in western culture, and has also become gradually popular among Korean teenagers recently However, the accident hap-pens sometimes such as loss of a bar into the tongue or mouth floor during the procedure. Because of the rare cases, few treatment of choices can be made when a bar is lost inside the mouth floor. In this case, the bar was removed under fluoroscope, without giving significant damage to the adjacent structure. 18 years old female patient visited the emergency room in our hospital. She attempted to pierce her tongue herself and could not find the bar which was lost in the mouth floor. The radiographs revealed that the needle was somewhere inside the mouth floor, but if incision were to be made it would be too deep to find the bar. The bar was removed through fluoroscope easily. Primary closure was done by 5-0 Vicryl and stitch-off was done 1 week later. The patient didn't have any speech problems and complications.

ANTIMICROBIAL SUSCEPTIBILITY TEST ON ORAL FLORA FROM DIFFERENT SAMPLING SITES IN CHILDREN (소아에서 구강내 정상세균총에 대한 채취부위별 항균제 감수성 차이에 관한 연구)

  • The most proper antibiotic must be selected after antimicrobial susceptibility test. If difference in antimicrobial susceptibility was significant between oral sampling sites, it is rationale to use the most susceptible antibiotic agent respond to dental procedure and object of treatment. This study examined sampling site variation from saliva, supragingival plaque and subgingival plaque of 16 children's oral microbes. The cultured bacterial isolates, which were Streptococcus viridans and Neisseria, were examined for 10 antimicribial drugs with the Bauer-Kirby agar disk diffusion method. The used drugs were Penicillin, Ampicillin, Oxacillin, Cephalothin, Imipenem, Gentamicin, Erythromycin, Vancomycin, Ciprofloxacin, Clindamycin. There was no significant difference between three sampling sites for antimicrobial susceptibility test of S. viridans and Neisseria and the sequence of susceptibility was agreed among them. In conclusion, it was suggested that antimicrobial susceptibility test from saliva, supragingival plaque and subgingival plaque of children have no significant sampling site variation.

A STUDY ON MUTATIONS OF P53 TUMOR SUPPRESSOR GENE IN ORAL TUMORS (구강종양에서 p53 종양억제 유전자의 돌연변이에 관한 연구)

  • Nowadays, there are a lot of evidence that mutation of the p53 tumor suppressor gene is one of the most common genetic abnormalities in neoplastic progression. In this study, we analyzed 20 specimens of oral tumors(squamous cell carcinoma 14 cases, ameloblastoma 3 cases, adenoid cystic carcinoma 2 cases, malignant schwannoma 1 case)using polymerase chain reaction and direct sequencing which used an automated DNA sequencer and software for detection of mutations. Polymerase chain reactions were performed with 4 sets of primers encompassing exon 5, 6, 7, 8, and direct sequencing method was employed. The results were as followings. 1. We detected 10 point mutations out of 20 specimens (50%). 2. The genetic alterations included 7 mis-sense mutations resulting in single amino acid subtitutions, 2 silent mutations, 1 non-sense mutations encoding a stop codon. 3. Mutations were mostly in exon 7(7 out of 10 mutations, 70%) and involved codons 225, 234, 235, 236, 238, 247. 4. Therse were 4 cases of $T{\rightarrow}A$ transversion, 2 cases of $C{\rightarrow}A$ transversion, $A{\rightarrow}G$ transition, 1 case of $C{\rightarrow}G$, $T{\rightarrow}G$ transversion respectively. 5. We could find out point mutations more conveniently using PCR - Automated Direct Sequencing method.

ACTIVATION OF NF-$\kappa$B IN THE CISPLATIN-INDUCED APOPTOSIS OF ORAL SQUAMOUS CELL CARCINOMA (구강편평세포암종에서의 Cisplatin 유도 아폽토시스에서의 NF-$\kappa$B의 활성화)

  • Purpose: This study was done to confirm the role of NF-$\kappa$B in cisplatin-induced apoptosis of oral squamous cell carcinoma. Materials and Methods: Five cell lines originated from different oral cancer patients were tested for the apoptosis by the treatment of cisplatin. These cells showed different degree of cisplatin-resistance and the order is OSCC-2>OSCC-3>OSCC-5> OSCC-1>OSCC-4. OSCC-2 and OSCC-4 cells were assayed for the apoptosis by measuring DNA fragmentation and TUNEL staining after cisplatin treatment. While OSCC-4 cells showed apoptosis, OSCC-2 cells showed no or very slight apoptosis by cisplatin treatment. Next, It was determined whether NF-$\kappa$B activation is required in mediating cisplatin-induced apoptosis of OSCC-4. Result: The result was that elevated NF-$\kappa$B activity mediated cisplatin-induced apoptosis. Conclusion: In conclusion, these findings suggest that NF-$\kappa$B activation is essential to cisplatin-induced apoptosis and it may be involved in cisplatin resistance in OSCC cells.

  • Botulinum toxin type A (BoNT-A) has been applied successfully to treat chronic migraine, dystonia, spasticity and temporomandubular disorders(TMDs) as well as frontal wrinkle and glabella wrinkle. Recently it has been reported that BoNT-A, reversibly blocks presynaptic acetylcholine release, also inhibits the release of substance P, CGRP(calcitonin gene related peptide) and glutamate related to peripheral sensitization and neurogenic inflammation in sensory nerve, In this study we reviewed animal nerve injury model such as rat and rabbit and identify the analgesic effect and mechanism of nerve injury pain after dental treatment.

A CASE OF ORAL LEIOMYOMA (구강내에 발생한 평활근종)

  • Leiomyoma is a benign smooth muscle tumor that usually arise in the uterus, skin and gastrointestinal system. Only 2 percent are located in the head and neck. Leiomyomas of oral cavity are uncommon as only two cases have been reported in the Korean literature. The common location of oral cavity has been tongue, but other sites include buccal mucosa, lips, palate. mouth floor, and gingiva. Our patient was 30 years old female who complained of swelling and discomfort in the mouth floor. Microscopically this tumor showed bundles of intertwining spindle cells within fibrous connective tissue stroma. The nuclei were generally pale staining and blunt ended. Masson's trichrome stain was positive for muscle, and immunohistochemical study for ${\alpha}$-smooth muscle actin revealed strong positivity. It was treated by surgical excision. We experienced a case of leiomyoma of oral cavity, so we report with literature reviews

ORAL SQUAMOUS CELL CARCINOMA ASSOCIATED WITH HUMAN PAPILLOMAVIRUS INFECTIONS; TWO CASES REPORT AND REVIEW OF THE LITERATURE (인간유두종바이러스 감염과 연관된 구강내 편평상피세포암; 문헌고찰 및 증례보고)

  • Several investigators have shown that human papillomavirus(HPV) appear to play an etiologic role in oral and paranasal sinus carcinoma. It was known that 15-25% of head and neck squamous cell carcinoma(SCC) showed HPV-positive infection. Among them, HPV 16 was the most common type but HPV 18 was observed only 2-4% of HPV-positive head and neck cancers. In recent, we treated uncommon 2 oral SCC cases that associated with HPV infection. One is a case of tongue SCC after bone marrow transplantation(BMT), and the other is a case of SCC occurring with aspergillosis in the maxillary sinus. After surgery, HPV 16 and 18 were detected in the surgical specimens by the histological and polymerase chain reaction(PCR) examination. In this report, we present these cases with a review of literature.

A study on the knowledge, attitudes, and performances of 119 EMTs regarding oral and maxillofacial trauma patients (119 구급대원의 구강악안면 외상환자의 지식, 태도, 수행에 관한 연구)

  • Purpose: This descriptive research study aimed to identify and strengthen the knowledge, attitudes, and performances related to first aid for patients with oral and maxillofacial trauma among 119 EMTs working at 119 safety centers and local stations in Jeollanam-do, South Korea. Methods: An online questionnaire was distributed among 144 119 EMTs working at fire stations in Jeollanam-do from August 1 to August 15, 2022. Results: The factors that influenced the knowledge of oral and maxillofacial trauma among the 119 EMTs included qualification (β=.113, p=.005) and performance (β=.837, p=.005), and those influential to attitudes were qualification (β=0.194, p=.005), ordinary interest (β=0.242, p=.005), and the need for education (β=.285, p=.005). The only factor that influenced the performance of first aid for patients with oral and maxillofacial trauma was knowledge (β=.900, p=.005). Conclusion: Considering the nature of first aid for patients with oral and maxillofacial trauma, it is necessary to organize practical knowledge and educational content tailored to the characteristics of each task. This should be simultaneously conducted with practice-oriented simulation education.

CLINICAL STUDY ON SURGICAL MANAGEMENT OF MANDIBULAR CONDYLAR FRACTURES (하악 과두 골절의 외과적 처치에 관한 임상적 연구)

  • 79 surgically managed mandibular condylar fracture patients included the 25 patients treated with Dr, Nam's method were analysed the postoperative resluts in Oral and Maxillofcial Surgery, School of Dentistry, Wonkwang University since 1993 to 1995. Mean patient's age is 32.5 years (range, 8 to 65 years), and follow-up periods were a minimum of 3 months to 28 months. 19% condylar fractures were associated with mostly symphysis portion. According to the patient's age, severity of condylar fractures, clinical signs and symptoms, radiographic findings, treatmenet plans had been performed. Rigid fixation have performed greatly, and then fragment removal of fractured mesial pole of proximal segment of the condylar and little cases of reshaping and eminoplasty and lag screw have been applied. Two cases of the both condylar resorption and deviated condyle posteriorly in Dr. Nam's method. None of infection or necrosis signs of treated condyle surgically. In my opinion, whenever possible, displaced condylar fracture can be managed surgically with rigid fixation, but not Dr. Nam's method. Usually if perform the surgical management of condylar fractures you should maintain maxillomandibular fixation for 2 weeks, or more and has to follow-up functional mandibular exercise should be kept continuously.

  • There is always a risk of injury in sports. With the recent development of the economy, there have been more opportunities to be exposed to risks, and safety issues to prevent accidents have emerged as a central task. However, intensive research on trauma is lacking in the oral & maxillofacial area associated with sports. It has only been partially introduced, with a comprehensive survey of whole-body trauma or mixed with various trauma in the oral & maxillofacial area. In this paper, the overall content of trauma in the oral & maxillofacial area caused by sports activities will be reviewed for clinicians.
