  • The treatment of TMJ ankylosis poses a significant challenge because of technical difficulties and high incidence of recurrence. TMJ ankylosis has been treated by excision and total joint reconstruction with alloplastic, allogeneic, autogenous materials as interpositional materials. The temporalis myofascial flap had been considered to be a successful interpositional material, due to its anatomical, topographical, and functional properties. This study evaluated the efficacy of the temporalis myofascial flap for nine TMJs (five patients) through the preauricular approach and coronoidectomy. Radiographic and physiologic long term result was investigated in this study. The result reveals that the temporalis myofascial flap is a good autogenous tissue satisfying the criteria of an ideal interpositional material, which offers a material that fulfills the physiological function of the disc. In spite of favorable functional outcome, mild postoperative openbite tendency remains another challenge.

Effect of Masticatory Muscle Pain Control by Morphine (Morphine에 의한 저작근 통증의 조절 효과)

  • This study was designed to evaluate the pain control effect by morphine injection to masticatory muscle pain patients. Patients with masticatory muscle pain visited the Department of Oral Medicine, Kyung Hee University Dental Hospital were recruited to this study and diagnosed by RDC/TMD. Experimental group were divided into three group; saline injection group(n=10), lidocaine injection group(n=10) and morphine injection group(n=10). Evaluation list was the subjective pain evaluation(visual analogue scale, Mc Gill pain questionnaire, pain drawing) and the objective pain evaluation(pressure pain threshold, pressure pain tolerance) and evaluation time was injection before, after 10min, 30min, 60min and then it was analyzed statistically. The results were as follows : 1. The subjective pain evaluation and the objective pain evaluation were significantly different statistically in within subject effects(p<0.001). 2. The subjective pain drawing evaluation(p<0.001) were significantly different statistically in between subject effects. 3. The objective pressure pain threshold evaluation(p=0.025) were significantly different statistically in between subject effects. 4. The morphine injection group(p=0.001) were more significantly different than the saline injection group statistically in the subject pain drawing evaluation. Therefore, it was considered that the morphine injection was effective to pain control for masticatory muscle pain patients within 60 minute.

  • The use of music as a means of inducing positive emotions and subsequent relaxation has been extensively studied by researchers. A great deal of this research has focussed on the use of music as a means of reducing feelings of anxiety and stress as well as aiding in the relief of numerous pathologies. The purpose of this study was to evaluate this effect of music using laser doppler flowmeter that monitors relative changes in the muscular blood cell perfusion, concentration of moving blood cells, mean velocity of the myofascial pain disorder patients. Total of 30 subjects were selected. They were monitored by LDF(Laser Doppler Flowmeter) during music listening for 3 minutes. LDF probe was placed over the temporalis muscle. The obtained results were as follows: 1. The average blood perfusion for 3 minutes of music listening at surface of temporalis muscles were elevated on both affected and unaffected parts compared to rest time. 2. The average of blood perfusion elevated during music listening at surface of temporalis muscles were higher in affected part than unaffected part. 3. Increase of blood perfusion during music listening is proportioin to that before music listening. In summary, these results may encouraging start to the elucidation of the hemodynamic events occuring during music listening and aimed to serve as a base for further studies.

The Effect of Wind-instrument Centered Music Therapy on Respiration, Oral Motor and Articulation for Patients with Cervical Cord Injury - Case Study - (취주악기를 사용한 음악치료가 경수손상환자의 호흡, 구강운동 및 조음에 미치는 영향 -사례연구-)

  • The purpose of this study is to investigate the influence on respiration, oral motor and articulation using wind-instrument centered music therapy for patients with cervical cord injury who need continuous train of respiratory muscle. Three patients with cervical cord injury who needed continuous training of respiratory muscle were selected and post data was analyzed. The harmonica using both exhalation and inspiration was selected as an wind-instrument. Each session was taken 30 ~ 40 minutes, once a week from total 6 weeks with small group. Material about every session's work was provided for individual to maintain practice after program. Oral motor and articulation test was done to assess reflecting features of maximal expiratory flow and wind-instrument of factor related breath. Maximal expiratory flow has increased by average 25ml more, articulation has increased by 3.16 points more and the movement of oral motor has increased 11.67 points more than pre-test from the analyzation. In the comparison of the details, the increase of oral motor function was confirmed from scores on the jaw and tongue except for the lips. Based on the results of this study, this study suggests that wind-instrument centered music therapy will be a practical and effective intervention for respiratory rehabilitation in patients with cervical cord injury.

  • 두개하악장애는 저작근, 측두하악관절 및 그와 관련된 구조물의 많은 임상적인 문제를 포함하는 포괄적인 용어이다. 두개하악장애의 원인은 다양하고 종종 다인성이며 위험을 증가시키는 여러가지 소인들이 존재한다. 이갈이나 이악물기 같은 이상기능습관은 흔하며 또한 임상적으로 많은 관련이 있는 것으로 사료된다. 그러나 이전의 연구에서는 연령증가에 따른 기능적 마모를 고려한 치아교모에 관한 연구가 시행되지 않고 단순한 교모도의 평가비교만이 시행되었다. 본 연구는 법치의학적 연령감정에서 널리 사용되고 있느 치아교모도를 이용한 연령감정법을 이용하여 두개하악장애환자의 치아교모도에 관한 연구를 시행하였다. 연령은 기능적마모에 따른 정도를 평가하기 위해서 고려되었다. 단국대학교 치과병원 구강내과에서 두개하악장애로 진단된 환자18명과 대조군으로 외래환자 14명을 대상으로 하였다. 연령차는 연령감정법의 추정연령과 환자의 실제나이의 차이를 구해서 비교분석하였다. 이상의 결과로 두개하악장애환자의 중절치, 측절치, 제1소구치, 제2소구치의 치아교모도가 정상군보다 유의성 있게 높았다.

  • Ameloblastoma is the most frequently accuring odontogenic tumor in mouth. The biologic behavior of this neoplasm is locally invasive tumor with a high rate of recurrence. But in case of unicystic ameloblastoma, it was known that the neoplasm can be treated by marsupialization and the recurrence rate is lower. In our clinic, we tried to treat one of ameloblastoma cases by marsupialization and finished the treatment by enucleation via intra-oral approach with sagittal ramus osteotomy. This is a report of that case about 29 years old female patient.

Delphi analyses for the activation of program for the public oral health center (보건소 구강보건센터운영 활성화 프로그램 개발을 위한 델파이 분석)

  • Objectives : The purpose of this study was to investigate the oral health center operation and to activate the program development of public oral health center. Methods : Delphi analyses were utilized by the recruited expert panel from August to September in 2012. Results : The difficulty in operation of the oral health centers is the human resources management. In order to solve the human resources operation, utilization of the local human resources is the best choice of the solution. The next choice was the designation of the oral health expert in Ministry of Health and Welfare. So the expert can activate the public oral health care system for the disabled and the underprivileged classes in the long term. Conclusions : In order to activate the oral health centers, Ministry of Health and Welfare should promote the public health center function connected with the cooperation of the Association of dental hygienists.

  • The purpose of this study was to analyze the prevalence of Oral and maxillofacial injuries of elementary school children in Suwon, Korea. The sample consisted of 850 boys and girls in a elementary school children in the city. This study conducted a survey by sending questionnaires to school-aged boys and girls, to study a condition of traumatic injuries of teeth. 1. The students who had a traumatic injuries in a oral & maxillofacial area are 125 persons(14.7%); boys 93 persons(21%), girls 32 persons (7.9%). 2. Incidence of traumatic injuries was 1 time 102 persons(81.6%), 2 times 14 persons(11.2%), others 9 persons(7.2%). 3. The causes of traumatic injuries were fall down 65.6%, blow 10.4%, sports 9.6%, traffic accidents 7.2%, tripping 3.2%, others 4%. 4. The distributions of sports related traumatic injuries were roller blade 7 persons, bicycle 2 persons, football 1 person, baseball 1 person, basketball 1 person. 5. Types of traumatic injuries were soft tissue injury(40.8%), tooth fracture(36.8%), extrusion(8.0%), jaw fracture(3.2%), others(11.2%).

  • Epithelial-myoepithelial carcinoma(EMC) of the salivary glands is a rare tumor first discribed in 1972. The EMC comprises approximately 1% of all salivary gland tumors. It is characterized by tubular and solid growth pattern with a dual cell population including an inner layer of epithelial cells, which is peripherally bounded by a layer of clear myoepithelial cells. It is demonstrated that tumor with solid slowly growing pattern, generally have a higher frequency of local recurrence.   • Retrograde peri-implantitis (RPI) is defined as a clinically symptomatic periapical lesion. RPI is generally accompanied by symptoms of pain, tenderness, swelling and fistula. Several etiologic factors of RPI were possible, residual bacteria would be the main cause of RPI. Various treatment modalities have been introduced: debridement only or a combination of debridement with the grafting material accompanied by a detoxification of the infected implant surfaces, apicoectomy and so on. Although the definitive management methods remain undefined, many favorable clinical results of a treatment of RPI have been published. This case report introduces the 7-year long-term clinical result of the application the principle: implant surface detoxification using saline and chlorhexidine and guided bone regeneration with bone graft material and barrier membrane. If the implant was not mobile, it would be possible to treat RPI according to surgical approach and good results will be maintained over long term.
