  • Maxillary duplication is a kind of proliferative neurocristopathy and considered to arise from bifurcation of neural crest elements soon after migration into mandibular arch. Sometimes this malformation is accompanied with oblique facial cleft. Usually this type of maxillofacial malformation requires multiple surgical intervention and the results are far from ideal. It became more troublesome if it had not been properly corrected on time, because secondary deformities could be developed from growth and development of abnormal tissues. This is a case of a 25-year-old-female patient who showed severe facial asymmetry thought to secondary deformity of maxillary duplication and masticatory disturbance due to multiple supernumerary teeth on posterior part right maxilla. We successully treated these deformities through four times of surgery of bone resection, orthodontic treatment, zygomatic and orbital reconstruction, orthognathic surgery and scar revision... ect. So we reported this rare case with review of literatures.

  • Return of facial nerve function is important in patients with facial nerve paralysis by trauma. Sometimes, delay in diagnosis of facial nerve paralysis make recovery of facial nerve function difficult. Traumatic facial palsy mostly occur after temporal bone fracture in unilateral. Temporal bone fracture after head trauma are divided into the three group; longitudinal fracture, transverse fracture and mixed fracture. The most common symptoms are hearing impairment, bloody otorrhea, loss of consciousness and facial nerve paralysis. The early care of temporal bone fracture involves facial nerve paralysis. And there has been many discussion and study in the treatment of the immediate or delayed facial palsy ; examply, surgical approach, time and methods. We have managed a patient with unilateral facial nerve paralysis after longitudinal temporal bone fracture in mastoid process and conservative facial nerve decompression was performed. We have obtained good result and report this case with review of literatures.

A CLINICOSTATISTICAL STUDY OF FASCIAL SPACE INFECTIONS OF THE ORAL AND MAXILLOFACIAL REGION (구강 악안면 영역의 근막간극 감염증에 관한 임상 통계학적 연구)

  • Oral and maxillofacial infection is the oldest and most common disease in human history. The infection ranges from the low-grade infection that only requires minimal treatment to the high-grade and life-threatening fascial space infection. In this study, the data on oral and maxillofacial infections were analyzed to aid in the diagnosis and treatment, and to predict the prognosis. This report was based on data from 831 patients with oral and maxillofacial infection (394 males and 437 females) who were hospitalized in the Department of Oral and Maxillofacial surgery of Chosun University Dental Hospital from January 1998 to May 2005. The ratio of males to females was 0.9:1. By age, patients between 60 and 70 years old were the greatest in number (17.1%), while only 5.9% of the patients were between 10 and 20 years old. The most common cause of infection was odontogenic origin (84.4%), followed by post-extraction infection (6.2%), unknown (5.9%), and trauma (3.5%). The most common fascial space involved was the buccal space (39.4%), followed by the canine (20.6%), submandibular (15.9%), pterygomandibular (9.5%), submental (7.6%) and sublingual (2.8%) space. The number of the involved fascial space was one (75.2%), two (19.8%), or more than three (5.0%). In terms of the treatment duration, the hospitalization period of 6 to 10 days was the greatest in number (49.9%). All patients had uneventful recovery without major complication. There are statistically significant correlations between age and treatment period, and the involved space and treatment period, but no correlations between the variables of sex and treatment.

  • Tracheostomy may be used to assure airway protection in various clinical situation. It, as a known operation, has a history spanning 2000 years. The first clear account of a successful tracheostomy was recorded in 1546 by Brasavola. Until 1718 the term "bronchotomy" was used to describe the procedure. Heister then introduce the term "tracheotomy and this was later adopted and popularized by Trousseau about 1830. The term "tracheostomy" appeared in medical literature after 1820 and the two terms "tracheostomy" and "tracheotomy" are used interchangeably today. Indications include relief of upper airway obstruction, facilitation of pulmonary toilet, diminution of dead space and need for prolonged mechanical ventilation. The extent of indication of tracheostomy has a tendency to increase, thus oral and maxillofacial surgeons have some opportunities to face a situation that require tracheostomy. So, we reported retrospective study of 31 cases of tracheostomy patient in oral and maxillofacial surgery with reference review to reveal the significance of surgical skill and management capability of emergercy state maxillofacial surgery patients.

  • 양성 종양(benign tumor)을 언급하기에 앞서 과오종(hamartoma) 및 과다 형성(hyperplasia)과의 차이를 구분할 필요가 있다. 양성 종양은 기원조직과 유사한 조직이 이상 증식하는 것으로 서서히 성장하지만 일반적으로 치료하기 전까지 이상 증식을 지속하는 진성 신생물을 일컫는다. 이에 비해 과오종은 정상 조직이 무질서하게 과증식하는 것으로 일정기간 후에는 성장을 멈추기 때문에 진성 신생물로 간주하지 않는다. 그러나 일부 과오종이 양성 종양에 포함되기도 하는데, 예를 들어 치아종은 정상적인 치성 조직의 성장이 완료되는 시기와 거의 동일한 시기에 성장을 멈추지만 양성 종양으로 분류된다. 과다 형성은 조직의 세포가 정상적인 배열 양상을 보이면서 세포의 수가 증가하는 것으로 지속적인 성장 양상을 보이지만 그 성장이 제한적이므로 양성 종양과는 구별된다. 양성 종양은 일반적으로 무통성으로 서서히 성장하기 때문에 종양의 크기가 증가하여 안면 종창이나 동통 등을 유발하는 경우에 발견될 수 있으며, 방사선검사에서 우연히 발견되기도 한다. 방사선검사는 병소의 위치, 범위, 특징 및 병소와 인접 해부학적 구조와의 관계 등 많은 방사선학적 진단 정보를 제공한다. 일부 종양은 방사선사진에서 매우 특징적인 소견을 나타내기 때문에 방사선학적 소견으로 예비 진단을 할 수 있을 정도의 진단정보를 제공하기도 하는 반면 어떤 종양들은 방사선사진에서 관찰되는 소견이 매우 유사하여 진단에 어려움을 주기도 한다. 따라서 종양의 확진을 위해서는 생검이 필수적이며, 방사선검사는 반드시 생검에 앞서 진행되어야만 정확한 방사선학적 진단을 할 수 있다. 양성 종양은 각각의 특징적인 방사선학적인 소견을 나타내지만 일반적으로 관찰되는 양성 종양의 특징이 존재하므로 이러한 일반적인 특징을 관찰하여 병소가 양성인지 악성인지를 감별할 수 있다. 첫째, 양성 종양은 대개 호발하는 부위가 있으므로 종양의 발생부위는 감별 진단을 하는 데 매우 중요하다. 일반적으로 치성 병소는 치아가 형성되는 하악관 상방의 치조돌기에서, 혈관성 및 신경성 병소는 하악관 내에서, 연골성 종양은 하악과두와 같이 연골세포가 잔존되어 있는 부위에서 발생하는 경우가 많다. 둘째, 양성 종양은 대체로 명확한 경계와 피질골성 변연을 보이며, 종종 병소가 결체조직으로 둘러싸여 있어 병소 주위에 방사선투과성 띠가 관찰되기도 한다. 셋째, 양성 종양은 일반적으로 인접 주위 조직에 압력을 가하면서 서서히 성장하기 때문에 인접 치아의 변위 또는 흡수, 피질골의 비박, 팽융 등의 소견을 보이며 피질골의 천공은 드물다. 방사선학적으로 양성 종양의 병소 내부는 방사선투과상으로 관찰되거나, 방사선불투과상으로 관찰되거나, 방사선투과상과 방사선불투과상이 혼재된 상으로 관찰된다. 저자는 이 지면에서 이러한 방사선학적 특징을 기초로 하여 구강악안면영역에 발생하는 양성 종양을 분류하고 각각의 특징에 대해 살펴보고자 한다.

  • True midfacial deficiency is defined as a hypoplasia of various components of midface such as maxilla, orbit, zygoma and nasal bone. For treatment of these anomalies Le Fort III osteotomy and its modifications have been used traditionally. Le Fort III osteotomy is the method which advances maxilla with nasal bone and zygomatic bone at a time. At first midfacial osteotomy was introduced by Gillies to treatment of dentofacial deformity in 1950. In 1967 Tessier designed Le Fort III osteotomy according to Le Fort III midfacial fracture line and popularized to treat midfacial deficiency using coronal incision to appoach osteotomy sites. This is a case of patient who had mandibular prognathism with midfacial deficiency with severe discrepancy in maxillomandibular interrelation. First we performed Le Fort III osteomomy for zygomaticomaxillary advancement, and then carried out simultaneous two jaw surgery with Le Fort I osteotomy and BSSRO three months after first surgery.

  • This case describes a case of desmoplastic fibroma of the mandible. A 9-year-old boy was seen with a history of progressive swelling and expansion of the left mandible for one month period. Desmoplastic fibroma was diagnosed on histopathologic examination This report reviews the diagnostic criteria, differential diagnosis and surgical treatment of choice in brief of this uncommon primary bone tumor of the oral and maxillofacial region.

  • Dentofacial trauma can result in tooth fracture, avulsion, facial bone fracture. The Unites states and Japan mandated the use of mouthguards for contact sports. But, Korean didn't. Mouthguards divided into ready-made type and custom-made type. Mouthguards protect the lips, intraoral soft tissues, teeth and provide the mandible with resilient support to prevent jaw fracture and dislocations. Sports-related accidents have been reported to be one of the most common causes of dentofacial trauma. Sports trauma of involving teeth with incomplete root formation cause long chair time, multiple visit, economic considerations, additional dental services. So, mouthguards can offer considerable protection against sports-related trauma.

Consideration for the Importance of Diseases Associated with Oral Medicine by Analyzing the Dental Cyber Consultation (사이버 치과 진료 상담을 통해 살펴본 구강내과 질환의 중요성에 대한 고찰)

  • The moderners of industrial society suffer from various stresses, which bring about increase in prevalence of temporomandibular disorders, oral mucosal disease and chronic neuropathic pain, therefore, the number of patients seeking help of those symptoms tend to increase. The purpose of this study was to discuss the importance of oral medicine related disease by investigating questions that appeared in cyber consultation of Kangnung National University Dental Hospital web site. Among the nearly 2000 questions, the rate of questions related to oral medicine was 20.92%, and the rate of questions related to other departments were oral and maxillofacial surgery 16.87%, conservative dentistry 16.67%, orthodontics 14.02%, prosthodontics 12.25%, periodontics 8.36%, pediatric dentistry 4.93%, preventive dentistry 2.08% and otherwise questions 3.90%. Among the 403 questions related to oral medicine, the frequent questions were oral mucosal diseases 44.17%, temporomandibular disorders 41.19%, halitosis 4.47%, xerostomia 3.23%, other orofacial pain 2.23%, forensic dentistry 1.49% and otherwise questions 2.98%. From the higher rate of questions related to oral medicine compared with other fields of dentistry, we would consider that the people are considerably concerned about the oral medicine related disease, such as oral mucosal diseases and temporomandibular disorders.

A Study of Sound Analysis of Temporomandibular Joint (악관절 잡음에 관한 연구)

  • 저자는 서울대학병원 치과진료부 구강진단과를내원한 두 개하악장애 환자 중, 악관절음이 있음을 호소하는 환자 106명을 대상으로 SonoPAK을 이용하여 악관절음을 분석하고 Transcranial TMJ Series와 파노라마상 판독결과를 종합하여 다음과 같은 결론을 얻었다. 1. SonoPAK에 기록된 210회의 발생 악관절음 중 왕복성 관절음은 44건, 88회 발생하였고, 개구시 관절음은 74회, 그리고 폐구시 관절음은 48회 발생하였으며, 개구시에 관절음이 더 빈번히 나타났다. 2. 왕복성 악관절음이 있다고 인정되는 혼자군에서, 개구시 평균 최대 단위는 18.6(%)$\pm$15.6(%), 폐구시 평균 최대 진폭은 16.6(%)$\pm$17.4(%) 이었으며, 개폐구간 진폭의 차이에는 유의성이 없었다. (p>0.05). 3. 악관절음을 미약한 소리, 중간소리, 큰소리로 분류할 수 있었으며 각각 110, 79,21회의 발생을 기록하였다. 4. (>300Hz)/(<300Hz)의 비율이 0인 경우는 34회 였고, 300Hz 이상의 주파수가 관찰된 경우는 176회였다. 5. Transcranial TMJ projection series 와 파노라마상의 판독 결과 하악과두의 골 변화가 인정된 환자군에서 SonoPAK 기록의 평균 주파수 비율((>300Hz)/(<300Hz))은 0.387$\pm$0.284이었고 골 변화가 인지되지는 않았으나 주파수 비율이 0.01 이상인 환자군의 평균은 0.286$\pm$0.227이었다.
