ORAL REHABILITATION OF UNSTABLE OCCLUSION DUE TO SEVERELY DESTRUCTED FIRST PERMANENT MOLARS (제1대구치의 광범위 치관파괴로 인한 불안정한 교합의 재건)

  • A 9-year old boy visited Department of Pediatric dentistry, Chonbuk National University Hospital due to unstable occlusion. He had permanent 1st molars destructed severely by dental caries and lost vertical stop. He could not occlude in the same position on closing of the jaw since he could not find his centric occlusion. The treatment plan was made, which was oral rehabilitation with crown restorations on permanent 1st molars based on clinical and radiographic examinations and centric relation of the jaw. Treatment plan consisted of endodontic treatment, crown lengthening and restoration of the occlusal vertical dimension (OVD) with gold crown by 3mm-increase of OVD. After endodontic and periodontal treatments, temporary crowns made of acrylic resin were set for assessment of tolerance to an increased OVD. After a month for tolerance, final prosthetics were made and set temporarily. In a month, gold crowns were set finally. Immediately after setting, the patient showed anterior open bite, but at the appointment after 4 months, he showed normal overjet, overbite and stable occlusion.

Application of ARCUS digma I, II systems for full mouth reconstruction: a case report (ARCUS digma I, II system을 활용한 전악수복 증례)

  • Transferring condylar and anterior guidance on an articulator is essential to the diagnosis of a patient for full mouth reconstruction. In this clinical report, ARCUS digma I system was used to measure inherent condylar guidance of a patient requiring full mouth reconstruction in preoperate treatment, and the patients was given provisional restoration based on a functional anterior guidance. Then, ARCUS digma II system was used to mount the final casting model on an articulator, and the definitive prosthesis was placed in the patient. An esthetic and functionally proper clinical result regarding inherent condylar path of the patient was observed, and results from comparison of the two systems are given in this case.

  • Ameloblastoma is a benign odontogenic epithelial tumor with high recurrence rate and requires extensive resection of the surrounding tissue and reconstruction of defect site. Because of the anatomical limitation of the reconstruction site, prosthetic treatment with implants is the first recommendation. This is a case of prosthetic restoration of the reconstruction site with implant fixed prosthesis in patient who underwent mandibular resection and iliac bone reconstruction due to ameloblastoma. However 14 months after completion of implant prosthesis, adjacent natural tooth erupted unexpectedly, resulting in 1mm infra-occlusion occurred including posterior implant prosthesis and anterior natural teeth. In adults, implant infra-occlusion may occur due to residual growth after placement of the maxillary anterior implant. But this case, hypo-occlusion of molar implant and open bite of anterior natural teeth is occurred due to extrusion of adjacent tooth, is rare. Thus we report the treatment process including orthodontic treatment with intrusion of the posterior tooth, and investigate the causes of sudden, unexpected tooth extrusion.

  • Invasive or non-invasive reduction of fractures could be conducted as treatments of traumatic maxillofacial bone fractures. But when suboptimal reduction or malunion of maxillofacial bone fracture occurs, malocclusion could occur as a result of the lost relationship of the mandible and midface. This malocclusion is called post-traumatic malocclusion and orthognathic surgery, orthodontic treatment, selective grinding and prosthetic reconstruction are suggested as treatments for post-traumatic malocclusion after securement of stable TMJ. Stable TMJ is essential for occlusal rehabilitation to prevent occlusal change and relapse of malocclusion. Centric relation and adapted centric posture are suggested as start points of occlusal rehabilitation because they are most stable TMJ position. This case report presents a case in which post-traumatic malocclusion occurred after reduction of panfacial fracture. To rehabilitate full mouth occlusion, selective grinding and prosthetic reconstruction of implant supported fixed prostheses were conducted in centric relation and showed satisfying results in functional and occlusal aspects.

Oral rehabilitation of excessive tooth wear patient using zirconia fixed prosthesis with increased vertical dimension (과도한 치아 마모 환자의 수직 교합 고경 증가를 동반한 지르코니아 고정성 보철물 전악 수복 증례)

  • From the point of view of oral rehabilitation, the treatment of extensive tooth wear requires a prosthetic approach. Physiological tooth wear is considered as a normal process and generally does not require treatment, but excessive tooth wear causes problems like inadequate occlusion and esthetics. Changes of occlusal vertical dimension should be made through accurate diagnosis and analysis. Also, the patient's adaptation to the changed occlusal vertical dimension should be assessed over time. This case was a 60-year-old male patient who complained of a decrease in chewing function and esthetics due to severe tooth wear. Full-mouth rehabilitation was performed with a tooth supported fixed prosthesis. An occlusal stabilization splint and provisional restoration were used to evaluate the adaptation to increased occlusal vertical dimension and induce a stable centric relation position. After that, monolithic zirconia prosthesis was delivered. We report this as a satisfactory functional recovery and esthetics.

SKELETAL RELAPSE AFTER ORTHOGNATHIC SURGERY OF CLASS III SKELETAL OPEN-BITE (개구교합을 가진 3급 부정교합환자의 악교정수술후 재발에 관한 연구)

  • This study was intended to evaluate a post-operative relapse tendency in mandibular prognathism patients with open-bite. 18 patients with or without open-bite have undergone sagittal split ramus osteotomy and were investigated radiologically with cephalogram. The results were as following 1. The preoperative anterior facial height, mandibular plane angle and mandibular gonial angle were larger in open-bite patients than in nonopen-bite patients.(p>0.05) 2. There was significant correlation between surgical change of SNA, mandibular plane length and long-term relapse.(p<0.01). 3. There was relatively stable postoperative anterior facial height in open bite patients. 4. There was no significant correlation between the surgical change of mandibular plane angle, amount of mandibular setback and long-term relapse.(p>0.01) 5. This study showed that horizontal relapse was more significant than vertical relapse.

  • Three Maltese dogs, 5 to 7 months old, were admitted to the Veterinary Medical Teaching Hospital, Chonnam National University with malocclusion including Class IV mesiodistocclusion. In the first case, the treatment was performed by moving the mandibular canine teeth caudally with orthodontic buttons and Masel chains. The second patient was treated for rostroverted mandibular canines using buttons and chains. When distal movement of the mandibular canine teeth was completed, a maxillary arch wire with finger springs was applied to push the incisor teeth forward. In the third case, the treatment began by moving the mandibular canine teeth caudally with buttons and chains. An arch wire with finger springs was applied at maxilla to move the maxillary incisor teeth labially. Additionally, the mandibular incisor teeth were moved lingually by an elastic band attached to the buttons cemented to mandibular canine teeth. As a result, all patients successfully regained a normal occlusion.

  • Hemimandibulectomy and free fibular flap (FFF) in patients with squamous cell carcinoma could disturb the functional movement of the mandible. The muscular function in the sectioned side was compromised because of the incompetency of the muscle attachment to the mandible, leading to the unstable occlusal contact in the centric and eccentric occlusion. In this report, we present a case of a 63-year-old male who underwent cancer surgery and reconstruction with fibular graft, and he needed a fixed dental prosthesis to restore tooth loss in the anterior region. Occlusal contact change and instability were found according to the bite force in the centric relation and eccentric movement. This case report aimed to present the prosthetic procedure for fabricating the functionally optimized fixed prosthesis wherein the functionally generated path (FGP) technique and digital imaging method were applied to replicate mandibular movement and vertical dimension of the patient.

  • In a case of multiple posterior teeth loss, antagonistic teeth extrude to the edentulous space and compensatory occlusion on the remained anterior teeth leads to occlusal trauma. Extrusion of antagonistic teeth breaks down occlusion plane and loss of posterior support bring about severe wear of remained teeth. In this situation, it is needed to restore remained teeth and edentulous space by increasing vertical dimension to obtain prosthodontic rehabilitation space and to correct occlusion plane. In this case report, the patient had a masticatory problem with loss of posterior teeth support and an esthetic problem of shortened anterior teeth. Before the tooth preparation for the prosthodontic restoration, the patient used removable device for 2 months to increase vertical dimension reversibly. After that, he got provisional fixed restoration with irreversible tooth reduction and used it for 3 months. It had spent 5 month to evaluate the adaptation state on final restoration with incresed vertical dimension. The increasing amount was 3 mm, which was relatively in less degree and masticatory system adapted to the increased vertical dimension without any pathologic changes. Final restoration was made to have equal-intensity contacts on all teeth in a verifiable centric relations and immediate disclusion of all posterior contacts the moment the mandible moves in any direction from centric relation. In addition, metal occlusion surface on posterior teeth was applied to prevent excessive muscle activation, occlusal trauma and the porcelain fracture.

  • 본 논문은 수공으로 이루어지는 치과 치료 및 보철물 제작을 3차원 컴퓨터 모델링을 하여 좀 더 정확하고 빠른 시간에 처리할 수 있는 시스템을 소개한다. 이 시스템은 기존 2차원 기반이 아닌 3차원 기반으로 설계되었으며, 구강의 석고 모형을 3차원으로 컴퓨터에 모델링한 후, 모델링 정보로부터 능선(ridge), 교두(crusp), 피트(pit), 열구(fissure), 변연(margin) 등과 같은 치아 특성 정보추출과 교합면 생성을 통하여 보철물, 예를 들어 Conus 내관과 외관을 제작한다. 해부학적으로 무수히 많은 변수를 갖고 있는 구강 구조에 대해 본 논문에서는 이들 일련의 과정에서 요구되는 컴퓨터 모델링의 주요 기술들을 제시한다.
