  • Temporomandibular joint (TMJ) internal derangement, especially disc displacement with reduction (DDwR) is the most common TMJ arthropathy and has been thought to do some effects on masticatory performance. Measuring of maximal bite force has been widely used as objective and quantitative method of evaluating masticatory performance, but previous studies showed various results due to various characteristics of subjects and different measuring devices and techniques. In a few studies about the correlation of bite force and temporomandibular disorders (TMD), some authors reported that bite force and masticatory performance would be reduced in patients with TMD because of pain. But the correlation of changes in structure of articular disc and masticatory performance has not been well investigated yet. In this study, to investigate the influences of non-painful disc change on the masticatory performance, we measured the value of maximal bite force, occlusal contact area and occlusal pressure of 39 patients with non-painful DDwR of the TMJ using pressure sensitive film, and compared it with that of 59 controls. The results are summarized as follows: 1. The maximal bite force (P<0.01) and the occlusal contact area (P < 0.05) of the DDwR patients were greater than the controls. 2. There was no significant difference in occlusal pressure between the DDwR patients and the controls (P > 0.05). 3. The maximal bite force of the male group was greater than that of the female group (P < 0.05). However, the occlusal contact area and the occlusal pressure between the male and the female group didn't show significant difference (P > 0.05). From the results above, we can suggest that DDwR could be a factor of changing bite force, but more controlled, large scaled and EMG related further study is needed.

  • Objective: This study investigated the skeletal and dentoalveolar changes after intrusion of maxillary posterior teeth using miniscrew implants during the treatment and posttreatment period. Method: The subjects consisted of 11 adults (1 male, 10 females) whose open bites were treated by posterior teeth intrusion with miniscrew. The lateral cephalometric x-rays of pre-treatment, post-treatment, and the retention period were evaluated. Results: The average intrusion of maxillary posterior teeth at the end of the treatment period was 2.22 mm (p < 0.001) and the average extrusion of maxillary posterior teeth at the end of the 17.4 month retention period was 0.23 mm (p = 0.359). The relapse rate was calculated at 10.36%. A decrease in overbite after the 17.4 month retention period was 0.99 mm (p < 0.05). The relapse rate was calculated at 18.10%. The skeletal changes and vertical height change of maxillary posterior teeth during the retention period was statistically insignificant, but there was a significant decrease in overbite (mean 0.99 mm, p < 0.05). The amount of intrusion and the overbite improvement from treatment significantly correlates to the degree of relapse. Conclusions: The results indicate that intrusion of the maxillary posterior teeth using miniscrews is an effective, non-surgical treatment modality which can reasonably be used to address adult open bite.

  • Patients who have extruded anterior teeth and deep bite with pathologic tooth migration, it is necessary not only periodontal treatment for reduce inflammation, but also orthodontic treatment for intrusion of anterior teeth. However, it is difficult to place the orthodontic brackets due to the deep bite, and there is a problem that the extrusion of the posterior teeth occurs more easily than the intrusion of the anterior teeth biomechanically. In particular, in patients with long face, relative intrusion of the anterior teeth by extrusion of the posterior teeth causes the clockwise rotation of the mandible and makes the facial profile worse. Therefore the biomechanical consideration and appliance design that can block these problems are required from the treatment plan. This is a patient who had a deep overbite with extruded anterior teeth, treated by periodontal treatment and intrusion of mandibular anterior teeth using cute brackes and miniscrews, and resulted in favorable maintenance during 8-year retention.

  • Objective: The purpose of this study was to find changes in the occlusal plane related to different vertical facial patterns and suggest treatment goals and conduct possible treatment mechanisms. Methods: 60 adult patients (28 males, 32 females) who had been diagnosed as Class 1 skeletal malocclusion and treated without extraction were selected. Patients were divided into three groups; short face type (group 1), average face type (group 2) and long face type (group 3), using the data on normal occlusion of Korean adults. Results: The results were achieved by analyzing cephalometric tracings of each group at pre-treatment, end-treatment and post-treatment (about 1 year recall check). The inclination of the occlusion plane tends to gradually increase as the face becomes longer In group 1, COP-X, FOP-X, L6/L1, MP-L6 were significantly decreased, and L1-FOP was significantly increased during the retention period (T3-T2). Group 2 showed no significant change, In group 3, FOP-X was significantly increased during the retention period (T3-T2). During the retention period, FOP-X showed significant change among each group, especially between group 1 and group 3. Conclusion: These results suggest that changes of occlusal plane inclination according to facial vertical pattern need to be considered during the retention period for intrusion, extrusion, and incisor overbite.

  • Clinical remounting of complete denture is performed to refine occlusal harmony in maxillo-mandibular relation. It has been reported that patients who used adjusted dentures with clinical remounting felt less complications such as pain and discomfort in mastication. The purpose of this study was to assess effects of clinical remounting with case series. Seven patients with existing complete prosthesis were included. Clinical remounting procedure was done through interocclusal relation recording. In addition, occlusal force was measured with pressure indicating sensor and occlusal contact areas were evaluated with photo occlusion analysis. Occlusal contact areas of prosthesis were enlarged, while bite pressure was not increased. Hit and slide phenomenon of prosthesis was reduced concurrently. Clinical remounting procedure improved denture stability and increased occlusal contact area. Therefore, clinical remounting should be considered.

  • Infraocclusion is defined as tooth whose relative occlusal movement was blocked during the period of active eruption due to ankylosis and so on. Then infraoccluded tooth remains under the occlusal plane composed by adjacent structures showing normal eruption patterns. Untreated infraocclusion may cause: prolonged retention of infraoccluded teeth; extrusion of apposed teeth; destruction of periodontal tissues by occlusal force and food packing; increased sensitivity for dental caries; and disturbances on eruption pathway of succedaneous teeth. Therefore, periodic check-ups and proper treatments are required. There are many treatment options on infraoccluded deciduous molars such as periodic observation, conservative method, restoration and space regaining with extraction of the teeth. The choice of treatment may depend on the presence of succedaneous teeth, time of diagnosis and degree of infraocclusion. In this case report, three patients showing displacement of the second premolars due to infraocclusion of upper second primary molars, were treated by means of space regaining with removable orthodontic appliances and extraction of ankylosed primary molars. All malpositioned permanent premolars in the 3 cases showed ordinary eruption pathways after treatment.

  • Recently several techniques have been reported for the treatment of anterior open bite in adults characterized by molar intrusion with skeletal anchorage. In this study, five adult patients who had anterior open bite malocclusion were selected to undergo upper molar intrusion with midpalatal miniscrew to close the open bite. The aim of this study is (1) to validate true intrusion of molars in adults (2) to test the usefullness of midpalatal miniscrews as anchorage for intruding upper molars (3) to evaluate the skeletal and dental changes of open bite closure. The results are as follows. (1) All had true intrusion of the maxillary molars. Mean amount of molar intrusion was 3.4mm(range 1.5-5mm). (2) No movement of midpalatal miniscrew occurred during their use. (3) Open bite closure was achieved for all 5 patients. The mandible closed and B-point rotated anteriorly and upward. The mandibular plane angle and the occlusal plane angle decreased.

  • The purpose of this study was to determine how to use soft relining material by observing an amount of denture displacement according to the different base area of residual ridge and thickness of soft relining material under masticatory force. Stone models that simulated residual ridge were made with different amount of denture base area and denture was fabricated by conventional heat curing resin with usual manner on the model and relined by silicone type soft relining material with different thickness. Specimen was examined the amount of denture displacement by Instron within range of normal occlusal force. The results were as following : 1. The increasing rate of denture displacement was higher than that of soft relining material thickness. 2. The amount of denture displacement decreased 1.7 times when base area became double at same thickness of soft relining material 3. The increasing rate of denture displacement was higher than that of occlusal force
