The Effect of Oxygen Therapy on VPB in Patients with Chronic Obstructive Pulmonary Disease (만성 폐쇄성 폐질환 환자에서 심실 조기수축에 대한 산소치료의 효과)

  • Background: In patients with chronic obstructive pulmonary disease(COPD). it is well known that hypoxemia increases the frequency of VPB, which is associated with the poor prognosis such as sudden death. The aim of this study is to evaluate the effect of short and long-term low flow oxygen therapy on the development of VPBs in patients with COPD by correcting the hypoxemia. Method: In 19 patients with COPD, oxygen saturation and VPB's were monitored by pulse oxymeter and 24-hour Holter EKG, with room air and oxygen saturation and VPB's were monitored on the 1st and on the 8th day during oxygen therapy with nasal prong (2L/min). Results : The arterial oxygen saturation was significantly higher on the 1st day of oxygen therapy compared with breathing room air, and was also higher on the 8th day of oxygen therapy than on the 1st day. We found that there was significant correlation between the lowest value of the arterial oxygen saturation and the mean value of the arterial oxygen saturation. The number of VPB's per hour was significantly lower on the 1st day of oxygen therapy compared with breathing room air, and also lower on the 8th day of oxygen therapy than on the 1st day. Our results showed positive correlation between the decrease in the frequency of VPB's and the increase in the lowest arterial oxygen saturation, even though correlation was not significant(p=0.056). Conclusion: With oxygen therapy, the arterial oxygen saturation was increased and the number of VPB's was decreased. Long-term oxygen therapy more than 7days, would be helpful to decrease the number of VPB' s in patients with COPD.

  • Software for GafChromic EBT2 film dosimetry was developed in this study. The software provides film calibration functions based on color channels, which are categorized depending on the colors red, green, blue, and gray. Evaluations of the correction effects for light scattering of a flat-bed scanner and thickness differences of the active layer are available. Dosimetric results from EBT2 films can be compared with those from the treatment planning system ECLIPSE or the two-dimensional ionization chamber array MatriXX. Dose verification using EBT2 films is implemented by carrying out the following procedures: file import, noise filtering, background correction and active layer correction, dose calculation, and evaluation. The relative and absolute background corrections are selectively applied. The calibration results and fitting equation for the sensitometric curve are exported to files. After two different types of dose matrixes are aligned through the interpolation of spatial pixel spacing, interactive translation, and rotation, profiles and isodose curves are compared. In addition, the gamma index and gamma histogram are analyzed according to the determined criteria of distance-to-agreement and dose difference. The performance evaluations were achieved by dose verification in the $60^{\circ}$-enhanced dynamic wedged field and intensity-modulated (IM) beams for prostate cancer. All pass ratios for the two types of tests showed more than 99% in the evaluation, and a gamma histogram with 3 mm and 3% criteria was used. The software was developed for use in routine periodic quality assurance and complex IM beam verification. It can also be used as a dedicated radiochromic film software tool for analyzing dose distribution.

Early Result of Surgical Management of the Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery (관상동맥-폐동맥 이상 기시증에 대한 수술의 조기 결과)

  • Background: Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital anomaly, but is one of the most common causes of myocardial ischemia which would result in high mortality within the first year of life. This is our early result of the surgical management for these patients. Material and Method: From June 1989 to July 2003, 6 patients with ALCAPA and one patient with ARCAPA (Anomalous origin of the Right coronary artery from the pulmonary artery) underwent surgical repair. We have reviewed the all medical records, electrocardiogram, chest X-ray and echocardiography retrospectively. Result: Three of the patients were boys and four were girls. The median age at the operation was 5.4 months (Range: 3$\∼$33 months). The average body weight of at the operation was 6.7 kg (Range: 3.7$\∼$11.3 kg). A mean follow up period was 18 months. Only 3 patients were initially diagnosed as ALCAPA. And 3 patients had moderate mitral regurgitation. Immediate coronary artery reimplantation on diagnosis with the aim of restoring a two-coronary system circulation was done. The average bypass time was 114$\pm$37 minutes, and the average aortic cross clamping time was 55$\pm$22 minutes. The average stay of intensive care unit was 5$\pm$3 days, the mean mechanical ventilator time was 38$\pm$45 hours and the hospital stay after operation was 12$\pm$5 days. There were significant improvements in electrocardiogram and chest X-ray of the all patients except one late death patient. The ventricular function showed almost normal recovery after operation; the EF (Ejection Fraction) increased from 41.2$\pm$ 10.3$\%$ to 60.5$\pm$ 15.8$\%$ within 1 month and to 59.8$\pm$13.9$\%$ within 1 year after operation, the SF (Shortening Fraction) increased from 23.6$\pm$4.7$\%$ to 38.6$\pm$8.4$\%$ within 1 month and to 37.4$\pm$7.9$\%$ within 1 year after operation, LVEDDI (Left Ventricular End-diastolic Dimension Index) decreased from 100.8$\pm$25.6 mm/$m^{2}$ to 90.3$\pm$ 19.2 mm/$m^{2}$ within f month and to 79.3$\pm$ 15.8 mm/$m^{2}$ within 1 year after operation. Concomitant mitral repair was done in two patients with anterior mitral leaflet prolapse. In every patient, mitral valve showed less than mild regurgitation during follow up. One late death occurred in which patient Dor procedure was applied 10 months after initial operation due to the dilated cardiomyopathy Conclusion: In the management of this rare and could be fatal Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA), early suspicion and correct diagnosis is of most important. But, after diagnosis, immediate restoration of 2 coronary systems could result in good outcome.

Contemporary Approach to Autotransplantation of Teeth with Complete Roots Using 3D-printing Technology (3D 프린팅 기술을 이용한 치근단 완성 치아의 자가이식에 대한 최신 접근)

  • Autotransplantation of teeth with complete roots is being increasingly performed for orthodontic treatment or for replacement of unsalvageable teeth, but this procedure has lower survival and success rates than those obtained following transplantation of teeth with incomplete root formation. While previous autotransplantation procedures used only models of the donor tooth and recipient site, surgical guide templates created using 3D-printing technology are now available. They allow shaping of the recipient site in proper direction and to the correct depth, thereby reducing the treatment time and improving the success and survival rates. Herein, we report a case involving autotransplantation of a tooth with complete roots at the site of a congenitally missing permanent tooth using surgical guide templates generated with 3D-printing technology. The procedure resulted in favorable healing of the transplanted tooth.

MITOGENIC EFFECTS OF NICOTINE TO HUMAN PERIODONTAL LIGAMENT(PDL) CELLS IN VITRO (In Vitro에서 니코틴이 치주인대세포에 미치는 세포분열효과에 대한 연구)

  • Nicotine is one of the major components of cigarette smoking which causes various systemic and local diseases to human body. Mitogenic effects of nicotine to systemic disease are interesting factors in the results of cellular Proliferation especially to vascular and pulmonary tissue or cells. The study of local effects concerns with destruction of tissue and delayed healing rate after various surgical treatment. Platelet-Derived Growth factor(PDGF) and Insulin-like growth factor(IGF) are blown as major mitogens to human PDL cells. The purpose of this study was to investgate the mitogenic effects of nicotine to human PDL cells. We studied the expression of PDGF-$\alpha$ receptor, PDGF-$\beta$receptor, and IGF-l receptor mRNA from the nicotine treated human PDL cells by northern analysis. The experimental groups were divided into different serum($1\%,\;10\%$) and nicotine (100ng/m1,1000ng/m1) concentrations and each group was studied by time course. The results of this study showed upregulation of PDGF-${\alpha},\;{\beta}$ receptor and IGF-l receptor mRNA at 100ng/ml nicotine concentration and $10\%$ serum group to the time course. These results suggest that physiologically attainable nicotine concentrations may stimulate the mitogenic gene synthesis to human PDL cells in vitro.

THE STUDY OF COMPARISON BEFORE AND AFTER ORTHODONTIC TREATMENT IN CLASS I BIALVEOLAR PROTRUSION CASES WITH FOUR BICUSPID EXTRACTION (제 1 소구치 발치를 동반한 제 I 급 치조성 양악 전돌 환자의 치료 전${\cdot}$후 비교에 관한 연구)

  • The purposes of present study were to evaluate changes in models and lateral cephalometric head films during orthodontic treatment and to compare the amount of incisal retraction and anterior movement of molars with the two approaches of the retraction method of canine(sectional canine retractions vs sliding canine retractions) and the anchorage management(head gears vs no head gears, transpalatal arches vs no transpalatal arches and lingual arches vs no lingual arches) and to evaluate changes during orthodontic treatment in models with relation to lateral cephalometric head films. 67 Korean women with Angle's Class I bialveolar protrusion were selected, whose initial chronologic age was above 16 yrs. Models and lateral cephalometric head films were taken before and after orthodontic treatment with four bicuspid extraction. the results were obtained as follows. 1. Significant decreases were observed in intermolar width, arch length and arch perimeter of maxilla and mandible but significant difference was not observed in intercanine width of maxilla and mandible during treatment period. 2. The linear change of the upper incisor to upper lip was 2.84:1 and the linear change of the lower incisor to lower lip was 1.45:1 3. There were no significant differences between the two groups(sectional canine retractions vs sliding canine retractions), the !we groups(transpalatal arches vs no transpalatal arches) and the two groups(lingual arches vs no lingual arches) in the amount of incisal retraction and anterior movement of molars. There were a greater amount of maxillary incisal retraction and a lesser amount of anterior movement of maxillary molars with the use of head gears than no use of head gears. 4. Changes during orthodontic treatment in models with relation to lateral cephalometric head films were obtained as follows : 1) Maxilla Central incisors were moved 3.79mm backward, canines were moved 0.22mm laterally and 3.70mm backward, and molars were moved 0.535mm medially and 2.29mm forward. 2) Mandible Central incisors were moved 3.04mm backward, canines were moved 0.145mm laterally and 3.92mm backward, and molars were moved 0.755mm medially and 1.77mm forward.

  • Objective: The purpose of this study was to perform gender-wise analysis of the related cephalometric factors affecting Korean patients with obstructive sleep apnea (OSA). Methods: We examined 118 adults who had visited the Sleep Disorder Clinic Center in Keimyung university, Daegu, Korea, and evaluated them by using poly-somnography (PSG) and lateral cephalograms. The patients were divided into 4 groups (male simple snorers, male OSA patients, female simple snorers, and female OSA patients) according to AHI (apnea-hypopnea index) and sex. Results: The position of the hyoid bone in the female OSA group was inferior to that in the female simple snorer group. Multiple regression analysis showed that tongue length and soft palate width were significant determinants for the severity of AHI in male OSA patients. However, inferior position of the hyoid was a significant determinant only in women. Conclusions: From a cephalometric point of view, OSA in male and female adult patients may be characterized by different pathogeneses. In particular, in female OSA patients, they might be managed by individualized treatments such as hormone replacement therapy in addition to conventional treatment.

THE INFLUENCE OF $CARISOLV^{TM}$ ON SHEAR BOND STRENGTH OF COMPOSITE RESIN RESTORATIONS ($Carisolv^{TM}$의 사용이 복합레진 수복물의 전단결합강도에 미치는 영향)

  • This study evaluated the influence of chemomechanical caries removal agent $Carisolv^{TM}$(MediTeam, Sweden) for composite resin adhesion to sound human permanent and primary dentin. The buccal/labial surfaces of 80 permanent molars and 80 primary incisors were used. Four types of adhesives and one composite resin were used; AQ Bond(Sun Medical, Japan), Clearfil SE Bond(Kuraray, Japan), Single Bond(3M, USA), Scotchbond Multi-Purpose(3M, USA) and Z100(3M, USA). One drop of $Carisolv^{TM}$(MediTeam, Sweden) was pretreated on the dentin for 0 second(control) and 60 seconds. The specimens were thermocycled for 1,000 times in baths kept 5 degrees C and 55 degrees C with a 30 seconds dwell time. Shear bond strengths were tested and the data was statistically analyzed using one-way ANOVA with subsequent post hoc Scheffe test at p<0.05. $Carisolv^{TM}$ treatment significantly decreased the shear bond strength. Shear bond strength of permanent dentin was significantly higher than that of primary dentin. Clearfil SE Bond treatment groups showed the highest shear bond strength and AQ Bond treatment groups showed the lowest shear bond strength.

Comparison of arch form between Koreans and Egyptians (한국인과 이집트인의 치열궁 형태 비교)

  • Objective: The purpose of this study was to evaluate morphologic differences in the mandibular arch between Egyptian and Korean subjects. Methods: The Egyptian sample consisted of 94 mandibular casts (35 Class I, 32 Class II and 27 Class III). The Korean sample consisted of 462 mandibular casts (114 Class I, 119 Class II, and 135 Class III). The most facial portion of 13 proximal contact areas was digitized from photocopied images of the mandibular dental arches. Clinical bracket points were calculated for each tooth. The subjects were grouped according to arch form to compare the frequency distribution of the 3 arch forms between the ethnic groups in each Angle classification. Results: Egyptians had significantly narrower intermolar and intercanine widths ($p$ < 0.001), and shallower intermolar and intercanine depths ($p$ < 0.001) than Koreans. There was an even frequency distribution of the 3 arch forms within the Egyptian group ($p$ = 0.46). However, in the Korean group, the most frequent arch form was the square arch form (46.7%), while the frequency of the tapered arch form was significantly lower (18.8%). Conclusions: These results might provide helpful information in evaluating morphologic differences between ethnic groups in selection of preformed superelastic archwires.

A Cephalometric Study of Lateral Morphologic Features in Adult Cleft Lip and Palate Patients (구순 구개열 환자의 성장후 측모형태에 관한 두부계측방사선학적 연구)

  • Background: Cleft lip and palate deformity have unknown patterns of maxillofacial growth and development. The maxillofacial growth can be affected either by congenital or environmental factors such as infection and trauma. Surgical repair of cleft lip and palate may interfere the subsequent growth and development of maxillofacial region. The purpose of this study is to evaluate the characteristics of maxillofacial growth patterns in adult cleft lip and palate patients. Materials and Methods: The material for this study consisted of 17 adult male patients with cleft lip and palate. Cephalometric tracing and measurements were done by one investigator. The relationship between 17 cleft lip and palate patients and Korean norms were evaluated statistically. Results: There were statistically differences in Na. perpendicular to point A, SNA angle, effective maxillary length, maxillofacial differencial. Wit's appraisal and upper incisor to point A(p < 0.01). Pogonion to Na. perpendicular also statistically differed(p < 0.05). Other measurements didn't statistically differ. Conclusion: It was evident that in adult cleft lip and palate patients, maxilla was retruded and short. Careful cleft lip and palate repair and treatment are recommended for facilitating normal growth of maxilla.
