The purpose of this study was to evaluate the clinical effectiveness of hydrophilic primer, which claim to retain adequate bond strength on moistened enamel resulting from moisture or saliva contamination, by comparing the shear bond strength and adhesive failure patterns of brackets bonded using hydrophilic primer and conventional hydrophobic primer. Brackets were bonded to human premolars embedded in metal cylinders utilizing light cured adhesive, primed with either a hydrophilic primer(Transbond fm primer) or a conventional hydrophobic primer(Transbond XT primer). Each sample was exposed to varying degrees of artificial saliva contamination during the priming process. The shear bond strength was measured using a universal testing machine, and the adhesive failure patterns after debonding were visually examined by strereomicroscope and assessed using the adhesive remnant index(ARI). The results were as follows 1. In dry conditions, no significant differences in shear bond strength between Transbond W and Transbond XT primers were found. 2. Transbond MIP primer exhibited a significantly higher shear bond strength than Transbond XT primer in saliva-contaminated conditions, regardless of the degree of contamination. 3. When contaminated with one coat of saliva, Transbond MIP primer did not exhibit significant differences in shear bond strength compared to the dry condition. When contaminated with two coats of saliva, Transbond MIP primer exhibited a singnificantly lower shear bond strength compared to the dry condition. 4. The adhesive remnant index of the adhesive failure pattern had a tendency to decrease, as the degree of saliva contamination increased. Bracket-adhesive interface failure was observed in more than half of the saliva contaminated samples utilizing Transbond MIP primer, whereas the bond failure sites of the Transbond XT primer samples occurred almost exclusively at the adhesive-enamel interface in saliva-contaminated conditions. The results of this study suggest that in cases where moisture control is difficult, Transbond MIP primer is an effective alternative to conventional hydrophobic primers.
The purpose of this study was to evaluate the effects of a sealant resin on enamel demineralization In orthodontic bracket bonding. The forty eight extracted sound bovine teeth were subdivided into four groups and treated with Phase II (Reliance, itasca. III) on the surface Group 1 was not treated. Group 2 was acid etched with 37% phosphoric acid for 30 seconds. Group 3 was applied with sealant after acid etching. Group 4 was applied with resin paste after acid etching and sealant application. Each group was demineralized in artificial caries solution. Demineralized enamel depth was measured by confocal laser scanning microscopy. The results were as follows: the mean demineralized enamel depth was $47.4{\mu}m$. (Group 1), $61.8{\mu}m$ (Group 2), $13.9{\mu}m$ (Group 3). $8.2{\mu}m$ (Group 4) the demineralized enamel depth was increased in Group 2 than in Group 1 (p<0.05); the demineralized enamel depth was reduced in Group 3 than in Group 1 and Group 2 with statistically significant differences (p<0.05): and demineralization in Group 4 was very little. The results of the present study indicate that sealant application is useful for reducing enamel demineralization in orthodontic bracket bonding.
The purposes of this study were to evaluate clinical applicability of resin modified glass ionomer cements and to determine the effect of salivary contamination on the tensile bond strength. Fourty extracted human permanent premolars were prepared lot bonding and standard edgewise brackets were bonded with Ortho-One, Fuji Ortho LC, Vitremer and Advance. Fourty extracted human permanent premolars were contaminated with saliva, dried and bonded with same materials above. The tensile bond strength was tested by Instron testing device aster storage in normal saline at ,$37^{\circ}C$ for 24 hours from bonding. The results were as follows : 1. The tensile bond strength of Ortho-One group was $7.68\pm1.76$, Advance group was $7.87\pm2.80$, Fuji Ortho LC group was $4.99\pm2.53$, Vitremer group was $2.80\pm0.88$ MPa. The tensile bond strength in contaminated condition of Ortho-One group was $4.12\pm1.67$, Advance group was $5.37\pm0.68$, Fuji Ortho LC group was $4.41\pm1.61$, Vitremer group was $2.60\pm1.10$ Mpa. 2. Salivary contamination did not affect the tensile bond strength when compared with the uncontaminated enamel group in Fuji Ortho LC and Vitremer (p>0.05) and there was great significant difference in the tensile bond strength of Ortho-One and Advance. 3. Advance, Ortho-One and Fuji Ortho LC seemed to have clinically a proper bond strength.
The purpose of this study was to evaluate the clinical usefulness of 4 self etching primers by measuring the shear bond strength of orthodontic brackets and examining the failure pattern of bracket-tooth interfaces. Methods: Seventy-five, defect-free, premolars were randomly assigned into five groups: control group (37% phosphoric acid + Transbond XT primer) and self etching primer treated groups (Transbond Plus self etching primer, Unifil bond, Clearfil SE bond, and Adhese). The shear bond strength was measured with a universal testing machine and the amount of residual adhesive remaining on the brackets after debonding was assessed by the adhesive remnant index (ARI). Results: The results showed that the groups conditioned with self etching primer had significantly lower shear bond strength than the control group (p < 0.05), although clinically acceptable. However, there were no significant differences in shear bond strength among the self etching primer groups (p > 0.05). Evaluation of the ARI scores indicated there was less resin remnant on the teeth in the groups conditioned with self etching primers, although not statistically significant. Conclusion: The results of this study suggest that all four of the self etching primers have shown acceptable bond strength for clinical use.
The purposes of this study were to evaluate and compare the frequency of ceramic bracket fracture, frequency of enamel fracture, bond fracture site, adhesive remnant index after mechanical and electrothermal debracketing, to evaluate effectiveness of high and low speed rotary instrument and ultrasonic instrument during residual adhesive remnants removal, and to measure resin film surface(percentage) using by image analyser(Leco 300). Bond fracture site, bracket fracture, and enamel surface damage were examined by scanning electron microscope. The following results were obained : 1. In the mechanical debracketing group, the bond failed predominantly at enamel-adhesive interface with the bulk of adhesive remaining on bracket base. 2. In the eletrothermal debracketing group, the bond failed predominantly at adhesive-bracket interface with the bulk of adhesive remaining on enamel surface. 3. The most effectiveness of residual resin removal was obtained by means of the resin polishing bur and the order of scratch formation was the procedure using tungsten carbide bur, ultrasonic scaler, sof-lex disc, and polishing bur. 4. The order of the resin film surface percentage was ultrasonic scaler, tungsten carbide bur, sof-lex disc, and resin polishing bur.

The use of stainless steel crowns are indicated for restoration of primary or permanent molars with proximal dental caries, extensive dental caries, or previous pulp treatment with increased danger of tooth fracture. Stainless steel crowns were introduced by Humphrey in 1950. For their improved durability, longevity, and success rate, they have been strongly considered for restoring extensive and multi-surfaced dental caries of molars in pediatric dentistry. However, they also have shortcomings, such as possibility of pulpal exposure or damaging proximal surface of adjacent teeth. In addition, when oversized stainless steel crowns are used, eruption of the adjacent permanent teeth may be disturbed by their prominent margin. As a means to compensate the shortcomings of stainless steel crowns, use of orthodontics bands may be considered. It is an alternative restoration method, where an orthodontic band is placed on a tooth first and cavity is restored with filling material, such as composite resin, glass ionomer, or amalgam. The use of an orthodontic band is indicated for molar restoration with cervical dental caries, extensive dental caries, enamel hypoplasia, or previous pulp treatment. Because it requires shorter chair time compared to stainless steel crown, its application is very useful for children with poor behavior. However, restoration using an orthodontic band requires good oral hygiene after its application. This case report illustrates the conservative restoration of primary molars and permanent molars with extensive dental caries using orthodontic bands.
In orthodontic patients, frequently, amalgam restorations are present on the buccal surface of molars. The ability to successfully bond orthodontic brackets and buccal tubes to amalgam restorations would therefore be of clinical value. But the bond strength to total amalgam surface is probably not critical in most instances. Because there is usually a considerable amount of sound enamel surrounding a buccal amalgam filling. The purpose of this study was to evaluate the bond strengths of orthodontic brackets according to surface treatments and size of amalgam restorations. Eighty tooth specimen were assigned to four groups according to amalgam size-1.5mm, 2.0mm, 2.5mm, 3.0mm diameter-and then divided into two groups : one half was sandblasting group the other half was no sandblasting group. After Bracket bonding, shear bond strength for each specimen was determined and bond failure patterns was evaluated. 1. Shear bond strength of amalgam size 1.5mm group was significantly higher than that of the other groups. (p<0.05) 2. There was no significant difference in the bond strength produced by sandblasting. (p<0.05) 3. Shear bond strength of G and H group of which amalgam restoration ratio to the bracket base sizes were $61\%$ were significantly decreased $50-60\% level of that of control group. (p<0.05) 4. There was positive correlation between sandblasting and mARI. (p<0.05) The results of the present study indicate that it may be feasible to bond orthodontic bracket clinically successfully to amalgam restoration with conventional orthodontic resin when its size is less than $50\%$ of that of bracket base.
The purpose of this study was to evaluate the effects of surface conditioning with $10\%$ polyacrylic acid, etching with $38\%$ phosphoric acid, and polishing with a slurry of pumice on shear bond strengths of light-cured glass ionomer cement, chemically cured glass ionomer cement, and a composite resin to enamel, and to observe the failure patterns of bracket bondings. Shear bond strengths of glass ionomer cements were compared with that of a composite resin. Metal brackets were bonded on the extracted human bicuspids after enamel surface treatments, and samples were immersed in the $37^{\circ}C$ distilled water bath, and shear bond strengths of glass ionomer cements and a composite resin were measured on the Instron machine after 24hrs passed, and the deboned samples were measured in respect of adhesive remnant index. Scanning electron micrographs were taken of enamel surfaces after various treatments. The data were evaluated and tested by ANOVA and Duncan's multiple range test, and those results were as follows. 1. Shear bond strength of light-cured glass ionomer cement showed statistically higher than that of chemically cured glass ionomer cement. 2. Shear bond strengths of light-cured and chemically cured glass ionomer cements to enamel treated with $10\%$ polyacrylic acid and $38\%$ phosphoric acid showed statistically higher than those with a slurry of pumice. 3. According to scanning electron micrographs, enamel surface conditioned with $10\%$ polyacrylic acid is slightly etched and cleaned, that etched with $38\%$ phosphoric acid is severely etched, and that polished with a slurry of pumice is irregulary scretched and not completely cleaned. 4. After debonding, light-cured glass ionomer cement to enamel treated with $10\%$ polyacrylic acid showed less residual materials on the enamel solace than composite resin to enamel etched with $38\%$ phosphoric acid. 5. There was no significant difference in the shear bond strength of light-cured glass ionomer cement to enamel treated with $10\%$ polyacrylic acid and that of composite resin to enamel etched with $38\%$ Phosphoric acid.
현재에는 과거와 달리 표면상 비춰지는 외모의 중요성이 강조되어 치아 교정을 해서 미적 욕구를 충족시키려는 사람들이 증가하고 있다. 특히나 치아를 교정할 때 사용되는 치아교정용 브라켓은 부착되는 위치가 치아 앞면이기 때문에 미관상의 중요성은 치아교정장치 중 가장 크다. 이러한 이유 때문에 미적인 부분을 고려하여 치아색과 유사한 브라켓이 개발되었는데, 이 브라켓은 레진 브라켓이다. 이는 치아 교정에 도움을 주면서 심미성이 우수한 장점이 있지만 음식을 섭취할 때 음식의 색소에 의해 브라켓의 색 변화가 일어난다는 단점이 있다. 본 연구에서는 사람들이 자주 섭취하는 음식이나 음료에 치아교정용 브라켓을 착색시켜 색이 변하는 정도의 차이를 컬러센서를 이용해 측정하였다. 이렇게 측정된 브라켓의 RGB 값은 객관적이고 정량적인 값을 나타낼 수 있다. 이는 브라켓의 변색 뿐만 아니라 치아 미백 치료에도 적용되어 사용될 수 있을 것이다.
The aim of this study was to compare fluoride release and surface changes according to different orthodontic bracket adhesives the application of fluoride products. We used non-fluoridated composite resin Transbond fluoridated composite resins Blugloo and LightBond, resin-modified glass ionomer Rely $X^{TM}$ Luting 2, and conventional glass ionomer Fuji $I^{(R)}$. Fluoride release of five orthodontic bracket adhesives and fluoride release ability after application of three fluoride products (1.23% acidulated phosphate fluoride gel, Tooth Mousse $Plus^{(R)}$, Fluor Protector, and a toothbrush with sodium fluoride-containing toothpaste) were measured using a fluoride electrode that was connected to an ion analyzer. After 4 weeks of fluoride application, the surface roughness and surface morphology were examined using a surface roughness tester and field emission scanning electron microscopy. The amounts of fluoride release were observed not only on application of Tooth Mousse $Plus^{(R)}$ and Fluor Protector on resin-modified glass ionomer Rely $X^{TM}$ Luting 2 and Fuji $I^{(R)}$, but also during tooth brushing using fluoride-containing toothpaste. After application of Tooth Mousse $Plus^{(R)}$, except Transbond XT, the surface roughness increased, and all orthodontic adhesives showed a partial drop of micro-particle filler. On application of 1.23% acidulated phosphate fluoride gel on all orthodontic bracket adhesives, their surface roughness increased. To bond the orthodontic bracket, resin-modified glass ionomer Rely $X^{TM}$ Luting 2 and Fuji $I^{(R)}$ adhesives are highly recommended if the amount of fluoride release is considered to confer a preventative effect on dental caries, and among the fluoride products, Tooth Mousse $Plus^{(R)}$ and Fluor Protector are better than 1.23% acidulated phosphate fluoride gel, and these are expected to prevent dental caries even during tooth brushing with fluoride-containing toothpaste.
