  • 최근 전 세계적으로 빈번한 홍수, 폭우, 가뭄, 지진 등 기상재해로 인한 돌발적인 식량공황사태가 이어지고 있는 바 식량자급의 중요성은 아무리 강조하여도 지나치지 않을 것이다. 우리나라의 식량자급률은 30% 정도로 부끄러운 수준이며, 매년 1,200만 톤 이상의 엄청난 양의 배합사료를 수입하고 있다. 또한 대가축의 조사료 자급률은 55% 수준으로 너무나 낮으며, 이 중에서도 반 이상을 사료가치가 낮은 볏짚이 차지하고 있어 농후사료 위주의 비정상적인 축산임은 두말 할 나위가 없는 바, 국내 자급 조사료의 안정적 생산기반 구축은 너무나도 당연하고 절실한 실정이다.(중략)

  • 중소기업중앙회는 지난 9월 22일에서 26일까지 '중소제조업 생존전략에 관한 CEO 의견조사'를 실시했다. 이번 조사는 20인 이상을 고용하고 있는 전국의 중소제조업 CEO를 대상으로 Web 및 Fax를 통해 실시했으며, 총 433개 업체의 CEO들이 설문에 응했다. 설문 응답업체의 기업 경영 20년 미만(응답자의 82.6%), 소기업(응답자의 60.7%), 가공조립 산업(응답자의 58.4%), 전통제조업(응답자의 85.2%), 내수위주(75.1%)의 제조업 CEO 들이 주로 설문에 응답했다.

A Study for Anatomical Evidence of Anxiety Using Magnetic Resonance Imaging in Patients with Panic Disorder and Complex Partial Seizure (공황장애와 복합부분간질 환자의 뇌자기공명촬영소견을 통한 불안의 해부학적 증거에 대한 연구)

  • This study was to investigate the anatomical evidence of anxiety. MRI was used to study 11 patients with panic disorder and 15 patients with complex partial seizure, and 21 controls. The regions of interest in the MRI were measured with computer-assisted planimetry using the AutoCad and digitizer. The following results were obtained ; 1) The mean age was 49.7(12.4) years in patients with panic disorder and 30.1(7.5) years in patients with complex partial seizure. 2) There were na signi ficant differences between 3 groups in the values of cerebral area, temporal lobe, caudate nucleus, hippocampus, parahippocampus, amygdala, third ventricle and VBR. The right parahippocampal region which attracted most attention in neurobiological studies regarding anxiety, tended to be larger in both study groups compared to the control group, but with no statistical significance. 3) There was lett-right reversal of temporal lobes in both study groups. And these are mainly due to asymmetrical increase in area of the temporal lobe on right side. These results suggest that temporal lobe, especially right temporal, is the anatomical correspondence of anxiety and functional activation of temporo-limbic system may be accompanied by the structural change of temporal lobe.

Association between Panic Disorder and Dopamine D2 Receptor TaqI A Polymorphism (공황장애와 도파민 D2 수용체 TaqI A 다형성의 관련)

  • Puropse : Disturbances of dopaminergic system might be related to the possible mechanism of panic disorder. This study was aimed to examine the association of DRD2 Taq 1 polymorphism and panic disorder. Methods : One hundred and fourteen patients with panic disorder (62 male (54.4%), mean age $40.96{\pm}0.11$ years) and 200 comparison subjects (114 male (57.0%), mean age $35.57{\pm}8.81$ years)were tested for DRD2 TaqI A polymorphism. We excluded panic patients with comorbid alcohol related disorders, bipolar disorders, and any kinds of psychotic disorders because there have been some reports about association of these disease and DRD2 TaqI A polymorphism. Results : There was significant difference in the frequency of the genotype in DRD2 polymorphism between patients and controls (${\chi}^2$=6.09, df=2, p=0.048). The A1+ allele (A1A1 and A1A2) frequency analysis also showed significant association (${\chi}^2$=4.08, df=1, p=0.043). In addition, we observed a more strong and specific association between panic disorder and the A1+ allele of the DRD2 TaqI polymorphism for men (${\chi}^2$=4.71, df=1, p=0.03), but not for women (${\chi}^2$=0.45, df=1, p=0.50). Conclusion : These results in our Korean sample suggest that the DRD2 TaqI A polymorphism may be associated with panic disorder. Furthermore, we found sex-specific association of DRD2 A1 allele with panic disorder.

Frequency of Sodium Lactate Induced Panic Attacks and Blunted Growth Hormone Responses After Clonidine Infusions in Alochol Dependence Patients (주정의존 환자에서 Sodium Lactate 정주 후에 유발되는 공황 발작의 빈도 및 Clonidine에 의한 성장 호르몬 둔화 반응)

  • This study was performed to explore the frequency of panic attack induced by sodium lactate in alcohol dependence patients and to compare the extent of blunted growth hormone reponses after clonidine infusion with that of normal controls. The authors investigated 10 alcohol dependence patients receiving inpatient care in Hangang Sacred Heart Hospital from March 2, 1993 to August 31, 1993 and 10 normal controls. The disagnosis of alcohol dependence was based on DSM-III-R. Thirty minutes after the sodium lactate infusions clonidins were administrated. Venous bloods were sampled before the sodium lactate infusions, and 30, 45, 60, 90 minutes after the administrations of clonidine. Plasma growth hormone levels were measured by RIA method. The results were as follows : 1) In the questionaires of Hamilton Anxiety Rating Scale, Hamilton Depression Raing Scale, CAGE, Korean MAST, the scores of alcohol dependent patients were higher than those of normal controls. 2) Sixty percent of alcohol dependence patients and twenty percent of normal controls had panic attacks induced by sodium lactate. 3) All panic attacks induced by sodium lactate were relieved after clonidine infusions. 4) There were blunted growth hormone responses after clonidine infusions in alcohol dependence patients who had sodium lactate induced panic attacks like panic disorder patients. These results suggest that alcohol dependence patients may have noradrenergic abnormality same as panic disorder patients and two disorder may have high biological correlations each other.

Comparison of Depressive Symptoms between Social Anxiety Disorder and Panic Disorder (사회불안장애와 공황장애에 동반된 우울증상의 특성 비교)

  • Objectives The principal aim of the present study was to investigate the characteristic depressive symptoms in patients with social anxiety disorder (SAD) and panic disorder in comparison to patients with depressive disorder. Methods This study included 132 patients with SAD, 128 panic disorder and 64 depressive disorder (major depressive disorder, dysthymia etc.) patients without comorbid psychiatric disorders. The Beck Depressive Inventory (BDI) is used to measure depressive symptoms. We divided BDI into three categories originally described by Shafer AB, including negative attitude toward self, performance impairment, and somatic symptoms. We compared the depressive symptoms of SAD, panic disorder and depressive disorder by using ANOVA. Results Negative attitude toward self was noticeable in SAD (SAD $0.54{\pm}0.23$, panic disorder $0.41{\pm}0.17$, depressive disorder $0.46{\pm}0.11$, p < 0.001). Performance impairment and somatic symptoms were remarkable in panic disorder than in SAD and depressive disorder (performance impairment : SAD $0.39{\pm}0.21$, panic disorder $0.44{\pm}0.14$, depressive disorder $0.40{\pm}0.09$, p = 0.009 ; somatic symptoms : SAD $0.07{\pm}0.10$, panic disorder $0.15{\pm}0.12$, depressive disorder $0.14{\pm}0.08$, p < 0.001). Conclusions The results facilitate an approach to optimal treatment for patients with comorbidity of anxiety disorder and depression.

Association between Panic Disorder and Dopamine Transporter Gene(DAT1) Polymorphism (공황장애와 도파민 수송체 유전자 다형성과의 연관성)

  • Objectives : There have been many association studies of panic disorder. However, studies about the dopaminergic function in panic disorder have been few. This study was aimed to examine the possible association of dopamine transporter gene(DAT1) polymorphism and panic disorder in Korean population. Methods : Ninety-eight patients with panic disorder(43 male(46.9%), mean age $42.13{\pm}10.88$ years) and one hundred and thirteen comparison subjects(67 male(40.7%), mean age $33.14{\pm}8.55$ years) were tested for DAT1 polymorphism. Genotypes of DAT1 with variable number of tandem repeats(VNTR) were determined using polymerase chain reaction. The differences of allelic frequency and genotype frequency distribution between patient and the control group were tested with Fisher-Freeman-Halton test. Results : There was association between DAT1 polymorphism and panic disorder(allele : p<0.03, genotype : p<0.05). The frequency of 10/10 homozygotes of DAT1 was significantly higher in control group(${\chi}^2$=4.452, df=1, p=0.035). Conclusion : These results in our Korean samples suggest that DAT1 polymorphism might be associated with the vulnerability of panic disorder. Possible association of dopaminergic genes and panic disorder should be investigated with future studies using larger and different population.

No Association between Genetic Polymorphism of Tryptophan Hydroxylase A218C and Serotonin Transporter Linked Polymorphic Region and Panic Disorder (한국인 공황장애 환자의 트립토판 가수분해 효소와 세로토닌 전달체 유전자 다형성)

  • Objectives:Genetic variations of the tryptophan hydroxylase(TPH) gene and the serotonin transporter linked polymorphic region(5-HTTLPR) polymorphism have been associated with its functional capacity. The authors investigated whether the allelic constitution of the TPH gene and 5-HTTLPR are associated in Korean panic patients. Methods:244 Korean patients with panic disorder and 227 normal healthy controls were tested for a genetic polymorphism of TPH A218C and 5-HTTLPR polymorphism. To assess the severity of panic disorder during the last one month, anticipatory anxiety, panic difficulty, panic distress, agoraphobic difficulty and agoraphobic distress were measured with visual analogue scale(VAS) score, STAI-S & T, BDI, SCL-90-R, ASI-R, CGI, PDSS, and HAMD. Results:There was no significant difference in genotype and allele frequencies of TPH A218C and 5-HTTLPR polymorphism between panic patients and controls. Although we observed some differences in genotype and allele frequencies of TPH A218C polymorphism among male subjects, these differences disappeared after Bonferroni correction. And there were no significant differences in clinical variables. Conclusion:Our results suggested that there are no association between the genetic polymorphism of TPH gene and 5-HTTLPR with panic disorder.

A Pilot Study of the Effectiveness of a Session of Group Cognitive Behavioral Therapy for Patients with Panic Disorder (공황장애에서 1회기 집단인지행동치료의 효능 ; 예비 연구)

  • Objectives : The purpose of this pilot study was to investigate the potential clinical benefits a single group session of cognitive behavioral therapy in the treatment of patients with panic disorder. Methods : The study participants were 18 patients (14 males, 4 females; mean age=38.9 years), all of whom were assessed as meeting the DSM-IV-TR criteria for panic disorder. All participants attended one two-hour session of structured group cognitive behavioral therapy (CBT). Clinical symptoms was assessed before and eight weeks after the single therapy session using the Panic Disorder Severity Scale (PDSS). Results : Eight weeks after a single session of group CBT significant improvements were found in panic attack frequency, distress during panic attacks, severity of anticipatory anxiety, agoraphobic fear/avoidance, panic-related sensation fear/avoidance, impairment in work functioning, impairment in social functioning. Conclusions : One session of group CBT appears to be an effective treatment of panic disorder by reducing the severity of all symptoms assessed on the PDSS. An attempt should be made to replicate the findings of this pilot study in a larger and controlled, comparative clinical trial.

  • In this special article, we presented the organization of the work group, basic principles of the algorithm, future plan and methods for developing a treatment algorithm for panic disorder in Korea. The psychiatrist work group from the Korean Association of Anxiety Disorders began to develop a treatment algorithm designed to improve the management of Korean patients with panic disorder by incorporating better evaluation techniques and treatment procedures. We have reviewed the treatment guidelines and algorithms for panic disorder published thus far, including the Practice Guideline for the Treatment of Patients with Panic Disorder established by the American Psychiatric Association, the Management of Anxiety (Panic Disorder, with or without Agoraphobia, and Generalized Anxiety Disorder) in Adults in Primary, Secondary and Community Care established by the National Institute for Clinical Excellence, and the Clinical Practice Guidelines established by the Canadian Psychiatric Association. We developed the basic materials to be used in the treatment algorithm for the management of panic disorder in Korea. Therefore, in this special article, we intro-duce the goal of the algorithm and the details of the algorithm development.
