개그맨 김형곤씨의 갑작스런 죽음에 충격을 받은 많은 중년 남성들이 '심근경색'에 이목을 집중하고 있다. 같은 날 슬로보단 밀로셰비치 전 유고연방 대통령이 감방에서 사망했고, 그 전날에 성낙합 서울 중구청장이 집무실에서 숨졌는데, 이들의 사인 또한 급성 심근경색. 최근엔 우리나라의 40대 돌연사 비율이 경제협력개발기구(OECD) 국가 중 최고라는 통계까지 발표된 바 있다. 돌연사 공포증을 불러일으키고 있는 이 증상의 원인과 대책은 무엇일까.

This is a case report about agoraphobia patient treated with oriental medical psychotherapy. We treated with Kyungja-pyungji(Stabilization of anxiety) treatment, Ii- Jeung-Byun-Qi(movement of emotion and degeneration of Qi) treatment, Giungoroen-Focusing(wise saying and lofty opinion) method for 8 weeks. She could not go out herself without her mother for the first teatment time, but she improved step and she could go by herself to the public place at the last time. We found out that the application of systemic oriental medical psychotherapy is effective for the agoraphobia disorder. Therefore, we presented application of oriental medical psychotherapy with bibliological research.

강박증은 우리나라 전체 인구의 $2{\sim}3%$가 앓을 정도로 드물지 않은 정신과 질환이다. 일상생활에서 평범한 많은 사람들은 마음속에 불안과 걱정을 가지고 있다. 사람마다 불안과 우울의 대상은 다양하겠지만, 에이즈라는 특정대상과 결부되어 있는 두려운 공포, 에이즈 포비아와 강박증에 대해 사례를 통해 살펴보고자 한다.

This research as a literature study is to clarify solution method of hypochondriasis through focused distraction. Hypochondriasis is called by various terms such as somatization symptoms, health anxiety, illness phobia, illness anxiety disorder, hypochondriacal disorder, hypochondriacal neurosis, and hypochondria personality disorder. Hypochondria is basically a factor that causes worry, fear, anxiety, and even phobia, lowering the quality of life at home, work, and society. As the quality of life of individuals is decreasing, the field of counseling psychology and psychological counseling has the task of solving this problem and restoring psychological peace. Accordingly, this study is an attempt to explore the symptoms of hypochondria by using the distraction strategy among cognitive strategies as a way to resolve the condition. Focused distraction strategy can be said to be a kind of cognitive 'avoidance strategy'. Focused distraction strategy is a way to avoid being overly focused on one's health. In addition, it weakens health concerns by 'dispersing' or 'distracting' the 'attention' focused on health in another direction.

Because of globalization, flight trips are generalizing, however in proportion to this, it is increasing that people who afraid of riding an airplane because of flight phobia. ‘Flight Phobia’ is individual problem; however it can be factors to suspend of flight schedules. This research is experimental analyzed the recognition degree of flight attendants and cabin attendants about flight phobia and suggested the direction of management about related problems. This research examines meanwhile overlooked importance of flight phobia and the actual condition and if problems are happened, this research will be used valuable to manage quickly and safely.

가상현실은 인간과 컴퓨터가 서로 커뮤니케이션할 수 있도록 하는 새로운 테크놀로지이다. 이는 사용자로 하여금 컴퓨터로 만들어진 가상의 삼차원 환경 속에서 보고, 듣고, 느끼며 그리고 가상환경과 상호 작용할 수 있도록 한다. 본 논문에서, 우리는 정신치료 분야에 가상현실을 도입하고 이를 이용한 고소공포증 노출치료 시스템을 개발하였다. 이러한 가상현실을 이용한 정신치료는 과거의 전통적인 방법보다 더 안전하고 시간적, 경제적으로 효율적이라는 장점을 지니고 있다. 본 시스템은 개인용 컴퓨터를 기반으로 하여 도심 한 가운데에 있는 번지 점프대를 배경으로 가상장면을 구성하였다. 그리고 철 구조물 옆에 사방이 개방된 난간이 있는 리프트를 만들어 가상환경 속에서 피험자가 높이감을 경험할 수 있게 하였다. 또한 이러한 가상현실 정신치료를 이용하여 높이에 대한 두려움을 지니고 있는 환자에게 임상실험을 실시하여 그 주관적 치료효과를 입증하였다.

Objectives : This study aimed to investigate the correlations between neurological and psychiatric symptoms at two weeks after stroke. Methods : For 412 stroke patients, stroke severity was evaluated by the National Institutes of Health Stroke Scale(NIHSS), disability by the Barthel Index(BI) and modified Rankin Scale(mRS), cognitive function by the Korean Mini-Mental State Examination(K-MMSE), and muscle power by grip strength. Psychiatric symptoms were assessed by Symptom check list-90-Revision(SCL-90-R), consisted of nine symptom domains : Somatization, Obsessive-compulsive, Interpersonal sensitivity, Depression, Anxiety, Hostility, Phobic anxiety, Paranoid ideation, Psychoticism, and Additional items. The correlations between the neurological and psychiatric symptoms were investigated at the time of admission and before discharge(i.e. before and after treatment). Results : At the time of admission, NIHSS score was associated with scores on Phobic anxiety and Additional items ; and scores on BI and mRS were associated with Depression, Phobic anxiety and Additional items. At the time of discharge, NIHSS score was associated with scores on Somatization, Depression, Phobic anxiety, and Additional items ; scores on BI and mRS were associated with scores on Depression, Phobic anxiety and Additional items ; MMSE score was associated with Obsessive-compulsive, Depression, Phobic anxiety, and Additional items ; and grip strength was associated with Somatization, Depression, Anxiety and Additional items. Conclusions : More severe neurological symptoms were associated with higher psychiatric morbidity particularly in depression, phobic anxiety, sleep and appetite disturbance at acute stage of stroke. More intensive psychiatric care and intervention are needed for the high risk group.
