A Study of the Health and Medical Manpower Policy - The Case of dental Technicians - (의료인력(醫療人力)의 수급정책(需給定策) 개선방안(改善方案)에 관한 연구(硏究) - 치과기공사(齒科技工士) 분야(分野)를 중심(中心)으로 -)

  • 인력은 한 사회에 있어서 자본을 축적하며 자연자원을 개발하고 사회 경제 정치적 조직을 성장시키는 변화요인으로, 인간을 중심으로 하는 사회경제적 자원을 종합적으로 지칭하는 말이다. 이렇게 국가 사회가 필요로 하는 인력을 정부가 적절히 계획하여, 형성시키며, 배분 및 활용하는 문제를 논리적이고 일관성 있게 다루는 것을 인력정책이라 한다. 이러한 거시적이고 대 사회적인 정부의 인력정책은 국민의 건강과 생명을 보호하기 위한 보건의료 서비스를 제공해야 하는 의료인력을 대상으로 하는 경우 그 중요성이 더욱 특별하다 할 것이다. 국민에 대한 보건의료 서비스는 훈련된 보건 인력에 의해서 제공되며, 국가의 인력정책의 결과로 나타나는 보건의료인력 공급의 적합성은 인력의 불균형이라는 개념들을 통해서 검토될 수 있다. 의료인력의 불균형이라 함은 의료인력의 수, 종류, 기능, 분포, 질 등과 의료서비스에 대한 국민의 전체적 요구에 대응하여 정부가 생산하여 채용, 지원, 유시할 수 있는 정부 능력의 한계를 의미한다. 다시 말해서 국민에 대한 의료서비스의 적정화는 잘 훈련된(well qualified) 의료인력이 충분히 공급되어야(adequately supplied) 하고, 또한 적절히 분포되어야(well distributed) 한다는 양적, 질적, 그리고 분포의 세 가지 측면에서 살펴볼 수 있다. 질적, 양적, 그리고 분포의 불균형이라는 범주를 통하여 살펴본 치과기공사 분야의 인력정책에 대한 연구 결과와 개선방안은 다음과 같다. 첫째, 수적 불균형의 면에서 치과기공사의 인력은 1970년대 중반이래 계속 과잉 공급되어 왔으며, 이에 대해 정부는 그동안 소극적으로 대처하므로 과잉공급을 가속시켜왔다. 따라서 이러한 과잉공급을 최소하기 위해서는 치과이용에 대한 수요의 확장, 무면허자의 취업규제단속 및 대학의 치과기공학과 정원 축소 등을 생각해 볼 수 있다. 이러한 외형상의 과잉공급에도 불구하고 현업에 종사하는 실제인력은 수용에 비해 부족한 과소 공급현상을 빚고 있다는 점이 문제이다. 이러한 역설적인 현상을 타파하기 위하여 무면허자의 적발을 위시한 제도적 장치가 마련되어야 한다. 둘째, 질적 불균형은 수적 과잉공급에 의한 취업률 저하로 인한 실력 있는 전문인력 확보의 어려움과 전문 교육인력 및 교육시설의 열악한 조건이 원인으로 지적될 수 있으며, 이에 대한 해결방안으로 적절한 인력수요의 조절과 교육인력 및 시설 여건의 향상이 요망된다. 예컨대 3년제로 되어있는 학제를 4년제로 상향조정하는 방안을 고려할 수 있다. 세째, 치과기공사 분야의 인력분포 불균형은 그다지 심각하지는 않은 것으로 나타난다. 그러나 변화하는 소득수준과 사회환경은 의료인력과 균등한 지역적 분포에 대해 지속적인 관심을 가질것을 요청한다고 할 것이다. 이를 위하여 현재의 공중보건의 제도처럼 치기공 분야의 인력을 무의촌지역에 배치하여 공익요원으로 봉사케 하는 제도를 생각해 볼 수 있다.

A Study on Public Health Doctors' Participation in District Public Health Program of Health Sub-centers in Korea (보건지소 공중보건의사의 지역보건사업 참여 실태)

  • Objectives: To investigate the state of medical care around health sub-centers, public health doctors' participation and opinion in the process of district public health programs. Methods: The study included 1,036 public health doctors who worked at health sub-center all over the country. The data were collected for Feb, 2002 using self-administered questionnaire by mail. Results: One or two doctors were working at health sub-center and 33.5% of health sub-centers was located in the region of the separation of prescription and dispensing. There were another medical facilities in 45.9% of the administrative district(eup or myon) where health sub-centers were located. The count of medical utilization went down to 14.8${\pm}$14.8 per a day in Nov, 2001 from 18.0${\pm}$15.6 in May, 2000, and the decline was much more in the region of the separation of prescription and dispensing. Among public health programs in health sub-centers, public health doctors participated mostly in preliminary medical examination for vaccination and least in health education. They participated in implementation rather than planning or evaluation of health program. Over a half of public health doctors were found to be positive that health programs implemented in their health sub-centers would promote the level of health in community people and they were willing to participate in district public health program if community people were in need. Conclusions: Recently health sub-centers are required to turn into health promotion facilities rather than medical practice facilities. Health program in health sub-centers will be advanced in both quality and quantity by turning the role of public health doctors who have provided medical services mainly into managing health program. Persistent education about managing health program and the policy to motivate participation in health program should be provided for public health doctors.

The Current Status of Workers at Major Industrial Health Institutions in South Korea (우리나라 산업보건 주요기관의 인력 현황)

  • Objectives: The purpose of this study was to identify information on the workers at major industrial health institutions. Methods: The subjects were employees at work environment measurement institutions, specialized health management institutions, special health examination institutions, and health checkup institutions. Data on these employees was identified by age, career, region, and qualification. Results: The sample totaled 6,449 workers at major industrial health institutions. The number of doctors, nurses, and industrial hygienists was identified at 4,609. For their age groups, 34.1% were in their 30s, and 56.3% had less than five years of work experience. The distribution by region was 24.3% in Gyeonggi-do Province, 12.7% in Seoul, and 8.7% in Gyeongsangnam-do Province. Conclusions: Information on occupational health human resources has been published irregularly and is limited by specialty. Therefore, it is necessary to systematize information on human resources and disclose it regularly so that supply and demand conditions can be predicted when establishing industrial accident prevention policies.

The Experience and Competence of Physicians Who Provide Emergency Health Care at Public Health Sub-Centers on Remote Islands in Korea (도서지역 보건지소 공중보건의사의 응급의료 경험 및 대처능력 고찰)

  • Objectives: To investigate the experience and competence of physicians providing emergency medical services at public health sub-centers on remote Korean islands. Methods: This study enrolled 79 doctors who work at public health sub-centers on remote Korean islands. Data were collected in December 2009 via self-administered e-mail questionnaires. The response rate was 44.3%. Results: Emergent situations occurred at most (58.68%) of the public health sub-centers that were surveyed in December 2009. An average of 1.92 cases required treatment by public health physicians. Only 20.25% of the physicians were specialists in emergency medicine, while the remainder were general practitioners (GPs) without clinical experience as emergency doctors. We also found that the physicians we surveyed had insufficient knowledge of emergency medical care. At some health centers only one doctor was available, and there was no medical team in holiday, although most of the physicians indicated that the ideal number of doctors per center was two or three. In cases of emergency, patients were often sent to the mainland by ship without receiving first-aid treatment. The public health sub-centers lacked the necessary medical equipment to save lives in emergencies and lacked escort systems for emergency patients. Conclusions: The Korean government should address the importance of providing emergency care in remote areas. Health administrators should provide suitable manpower, medical equipment, guidelines for emergency medicine, and education for public health physicians on remote islands.

Task Status of Dental Hygienists of Health Centers and Subcenters (일개 도지역 보건기관 근무 치과위생사의 직무실태와 개선방안)

  • This study was performed to investigate the task status and performance improvement plan of dental hygienists of Health Centers and Subcenters. The Data collected by self-administered Questionnaire survey of 203 dental hygienists of Health Centers and Subcenters located in Gyeongsangbuk-do were analyzed. The major results are as follows: For the degree of satisfaction in tasks, 73.9% of dental hygienists of Health Centers and Subcenters felt proud, 52.2% felt overwork, 32.0% hoped transferring to other worksite and the major reason of transferring was lack of promotion opportunity in officials of dental hygiene department. Dental hygienists received job education in addition continuous job education in last 3 years were 47.3%, 19.2% of dental hygienists performed the special dental health program for residents in last 3years. Almost half(53.7%) of dental hygienists replied that dental hygienists who were not engaged in dental health tasks should be engaged in dental health tasks. The major dental hygienic tasks performed by dental hygienic officials were support for dental treatment(41.6%). Dental hygienic officials answered that dental health tasks, school dental health program, vertical dental health program should be conducted as important tasks in order. And they replied that the most serious problem of dental hygienic tasks was not conducting dental health affairs due to 'lack of dental doctor'(40.9%), 'lack of concern for dental health tasks'(26.4%), and 'lack of budget and personnels for dental tasks'(19.0%), and the most important thing to improve dental hygienic tasks was 'posting dental hygienists in ministry of health and welfare(MOHW) and province', 'securing of budget and personnels for dental health tasks'.

Experiences of Public Officials for the COVID-19 Response in the Community Health Center (보건소 공무원의 코로나19 대응 경험)

  • Purpose: The purpose of this study was to examine the experiences of public officials working for the Coronavirus Disease-19 (COVID-19) response in community health centers in South Korea. Methods: A qualitative thematic analysis was conducted using data collected from three focus groups and two individual interviews. Results: The participants performed quarantine tasks in a poor working environment with several problems, such as significant workload, lack of manpower, and inappropriate compensation system. Participants experienced obstacles in performing quarantine works, which had the lack of the detailed quarantine guidelines, work support and cooperation system. Participants suffered from civil complaints. Participants endured sacrifices in their personal lives while partaking in COVID-19 response work without holidays, and subsequently experienced health problems. And also participants said that it was necessary to secure expertise and effective communication for infectious disease management. Conclusion: The study results suggest that policies are required that are aimed to improve the working environment and the recruitment of experts in infectious diseases. In addition, the job stress related to the COVID-19 response by public officials in community health centers must be evaluated, and the relation of their job stress to physical and mental problems, as well as psychosocial stress, must be examined.

Geographical Distribution of Physician Manpower under the Influence of Public Health Physician (의사인력의 지역간 분포양상 및 공중보건의사의 영향)

  • The purpose of this research is to assess the geographical distribution of physicians and dentists and the degree of maldistribution of the physician. Data were obtained form the Korean Medical Association's report on physicians registry and census for 1990. To assess the degree of disparity in the rural-urban distribution of physician manpower and to identify changes in the distribution pattern, the Gini index of concentration was used. Major findings are as follows; 1. Urban-rural disparity in the distribution of physician manpower exists in all categories of manpower, i.e. physician, dentist, oriental medical doctor, general practitioner, medical specialist, practitioner, public health physician and public health dentist. Urban area which had 74.4% of nation's population, accounted for over 90% of all physician manpower. 2. In terms of the ration of physician manpower per 10, 000 population, in urban area, they were 8.2 physicians, 2.7 general practitioners, 5.5 specialists, 3.0 practitioners, 1.8 dentists and 1.3 oriental medical doctors. In rural area, the ratios were 1.4 physicians, 0.6 general practitioners, 0.9 specialists, 1.0 practitioners, 0.4 dentists and 0.4 oriental medical doctors. 3. Gini indicies computed to measure inequality of physician manpower distribution were 0. 3675 for physicians, 0.3372 for general practitioners, 0.3338 for specialists, 0.2263 for practitioners, 0.3132 for dentists and 0.3293 for oriental medical doctors. 4. Inspite of increase in the number of physician manpower, urban concentration of physician manpower intensified from 1980 to 1990. However, the Gini index for all physician manpower fell by 18.3~36.7% from 1980 to 1990, indicating more even distribution. 5. In rural area, the public health physicians and dentists had increased the ratios of physicians, general practitioners, practitioners and dentists per 10, 000 population remarkebly, and had decreased the Gini indicies of physicians, general practitioners, practitioners and dentists. Thus, public health physicians and dentists contributed to improve the distribution of physician manpower in rural area. Based on the results of this study, long-term and rational manpower policies should be developed to solve the problem of geographical maldistribution of physician manpower as well as short-term policy for inducing physicians to the rural areas.

A Convergence Study on the Current Managerial Status and Policy of Regional Public Medical Centers : Focusing on Jeollabuk-do Province's cases (지방의료원의 운영 현황과 정책에 관한 융합적 연구 : 전라북도 사례를 중심으로)

  • The study selected two regional public medical centers(G, N) in Jeollabuk-do to recognize problems and provide solutions for regional medical centers. The medical centers were analyzed, focusing on availability of medical personnel, budget and management thereof, medical treatment performance, public project performance, and patient satisfaction. The results showed that the medical centers were understaffed with doctors compared to private hospitals or national university hospitals and suffering financial debt. The number of patient with medical benefit was increasing in the medical center G, whereas it was decreasing in the medical center N. Patient satisfaction survey showed that the medical center G was scoring similar to the national average, whereas the medical center N was scoring slightly less than the average. Policies needed for development and efficient management of Regional public medical centers are clarification of Regional medical centers identity, modernization of medical equipment and facilities, recruitment of competent medical personnels, specialization of hospitals and establishment of stronger referral system. The regional medical centers should secure financial supports for public service from regional and national governments, and establish revenues from funerary service, leasing service, and national projects participation.

Dental Service System and Oral Health Providers in Correctional Institutions (구금시설의 치과진료체계 및 구강보건의료인력 현황 조사)

  • The purpose of this study is to analyze the dental service system in correctional institutions and 10 find the factors for improving inmates' oral health. This study is comprised of document review, telephone and questionnaire survey. The subjects of questionnaire survey are public health dentists and doctors in correctional institutions. They responded to questionnaire and the survey was collected from previous research and selected information about the dental service system. The findings of the study were as follow : Documentary survey 1. According to 2004's study, there are 42 dental offices in 46 all correctional institutions. 2. Criminals who took an health examination occupied 69.0% when committed to a jail in 2002's study. Majorities of them(81.5%) responded that they didn't take any oral examination. Telephone & Questionnaire survey 1. Full-time public health dentists are 26 in 2009. There is no correctional institution having oral health providers in 26 correctional institutions surveyed. 2 About 10 patients use the dental services in a day. Part-time dentists visit 4 times a month as average in 80% of institutions. 40% of institutions responded dental treatments can't be progressed conveniently because of the lack of oral health providers. 3. 80% of respondents answered that it is hard to cure prisoners, and that's because they are forbidden to get out of the institutions. 4. Only 20% of correctional institutions offered the oral hygiene instructions. There is no regular oral hygiene education for all inmates. 5. They need to increase the number of oral health providers.

  • 일본의 Soyo town에서 진행된 건강도시사업은 건강증진 프로그램 모형을 통하여 건강한 도시를 건설하겠다는 슬로건을 가지고 주민들이 참여하여 자신의 건강관리능력을 함양하고 서로의 장점을 공유하는 것을 사명으로 하고 있다. 또한 삶의 질 향상, 주관적 건강상태 향상, 삶의 만족도 향상, 정서적 유대감 향상, 수입증가 그리고 조기사망의 감소를 구체적 목적으로 설정하고 있다. 이러한 목적을 달성하게 위한 매우 실제적이며 수량화된 목표들이 설정되어 있다. 1992년에 사전조사를 실시하고 1996년과 2002년에 사후 조사를 실시하였다. 프로그램 내용과 과정에는 보건의료전문가와 시설에 대한 정비, 평가에 대한 계획, 학교를 포함한 포괄적인 보건의료시스템이 포함된다. 모든 부문은 자신들이 설정한 보건정책 결과에 대하여 모니터링을 진행하며 적절한 정책을 다시 설정하고 수량화된 목표를 설정하며 프로그램을 수행한다. 프로그램 수행의 가장 중요한 원칙은 사업의 기획, 수행, 평가 과정에 주민이 최우선적으로 참여한다는 점이다. Soyo town은 사업을 위하여 학교 및 지역 내 사조직과 협력하고 모든 사업의 기획과정에 다양한 구성원들을 참여시켰는데 이는 건강자원 개발에 있어 매우 중요하게 실천되어야 하는 문제이다. 특히 모든 Soyo town의 주민들이 참여하는 것을 원칙으로 하였다. 주민들은 건강관련 자원의 배치상태와 과학적 근거를 고려하여 효과적인 사업을 선정하는 것을 포함한 모든 사업의 과정에 참여하였다. 바람직한 생활양식을 향상시키기 위하여 식품회사, 자원단체 그리고 사회적 관계망 그리고 지역의 단체급식시설, 식당 등이 파악되었으며, 요양원, 가사도우미, 공중보건간호사, 방문간호사, 사회사업가로 구성된 보건의료 시설과 인력들이 협력하였다. 사업이 주민의 건강을 향상시켰는가와 건강향상에 사업이 효과적이었는지를 판단하기 위해서 평가가 진행되었는데, 서비스 제공자 뿐 아니라 소비자와 주민들도 평가를 수행하였다. Soyo town의 건강도시사업의 평가 결과, 조기사망률이 1988년 22.1%이 1992년 18.2%로, 1998년 15.6%로 감소하였다. 또한 주관적인 건강상태는 1988년 48%에서 1992년 67.1%로, 1998년 71.5%로 증가하였다. 전반적으로 주관적 건강상태, 사회적 관계망, 보건의료서비스에 대한 접근성은 증가하였으며, 조기사망, 의료비용은 감소하였다. 한편 보건의료부문의 질적인 향상을 가져왔으며, Soyo town은 건강한 도시의 명성을 얻게 되었다. 사업의 효과성을 향상시킨 요인은 주민 개개인들에게 사업의 기획과정에 처음부터 참여할 기회가 주어졌다는 점과 자원개발에 있어 기존자원의 재개발에 우선순위를 두었다는 점이었다. 효과적인 사업의 수행을 위하여 행정부, 주민, 건강관련 단체들은 수단과 방법에 관한 정보를 교환하는 것이 중요하다. 사업의 기획단계에 모든 이해당사자들이 참여하여 각자의 역할분담을 명확히 하는 것이 필요하며, 건강증진은 개인의 노력 뿐 아니라 사회적 환경의 개선과 자원의 개발을 통해서 달성되는 것이므로 주민들은 건강증진을 위한 환경개선을 위해 노력도 병행되어야 한다.
