Bursoscopic Evaluation for Degree of the Rotator Cuff Tear with Air Infusion Method (공기 주입과 동시에 시행한 견봉하 관절경 술식을 이용한 회전근개 파열의 정도 평가)

  • Purpose : To evaluate the diagnostic efficiency of the subacromial bursoscopy with glenohumeral air infusion technique for detection of the full thickness tear which we used to think as a partial thickness tear, because it was hidden by bursal tissue. Materials and methods : We chose 65 cases and divided them into 2 groups. The group I was 18 cases with partial thickness rotator cuff tears on glenohumeral arthroscopic evaluation, and the group 2 was 37 cases with full thickness rotator cuff tears which were repaired. We inflated the glenohumeral Joint with 50-100ml of air and observed air bubble leakage simultaneously on bursoscopy. Results : In group I, we could detect air leakage in 3 cases and found that it was full thickness tear. In group II, 2 cases revealed too much air leakage proved that it was incomplete repair and was in need of additional suture. Conclusion : Air infusion technique is though to be a valuable method in differentiation between the partial and full thickness tear and for evaluation of the security of the repair.

  • Paratracheal air cysts are rare lesions and detected incidentally during CT scan or autopsy. Histopathologic diagnoses of paratracheal air cysts include trachocele, tracheal diverticulum and lymphoepithelial cyst. The cysts are lined by ciliated columnar epithelium and have communication with trachea. Previous reports suggested an association with obstructive lung disease because of increased expiratory pressures in these patients. Most of these cysts are asymptomatic but rarely cause productive cough, wheezing, recurrent laryngeal nerve paralysis and difficult intubation. We report a case of paratracheal air cyst causing dyspnea with a review of literature.

Development of 50m Class Airship Structures (복합재를 이용한 50m급 비행선 구조개발)

  • KARI developed 50m class unmanned airship. The airship employ the pressure envelope design principle. The envelope must be considered as a main structural element of the airship. The envelope & three ballonets are fabricated by polyfiber composite laminates. Other structural components (gondola, tailwing, nosecone & engine mounts) are manufactured by carbon fiber & glass fiber laminates. In order to develop a big unmanned airship, a large amount of structural design, analysis and tests had to be made. The paper describes the structural configuration of the 50m class uumanned airship which are basic starting point of the structural development of an airship. The paper includes the various designing processes, components development tests and analysis results. Envelope & ballonets development processes which are very different to conventional airplane design are given in details with actual analysis & test results. The paper also describes the structural design and analysis results for other composite made structures. Each components were tested by static design limit loads and structural safety were confirmed. The paper shows the manufactured structural components and assembled airship.

Clinical effects of additional use of erythritol powder air polishing device on non-surgical periodontal treatment in moderate chronic periodontitis (중등도 만성 치주염 환자에서 erythritol 공기분말 연마기구를 부가적으로 이용하는 비외과적 치주치료의 임상적 효과)

  • Purpose: The purpose of this study was to evaluate the clinical effects of erythritol powder air polishing device (EPAP) in addition to scaling and root planing (SRP) in non-surgical periodontal treatment in moderate chronic periodontitis patients. Materials and Methods: Clinical evaluation was performed at 21 sites treated with SRP (control) and 21 sites treated with the addition of SRP+EPAP (test). All examinations were performed before treatment, 1 month after treatment, and 3 months after treatment. Depth of the periodontal pocket, gingival recession, clinical attachment level, plaque index, and bleeding of probing were measured as clinical parameters. Results: In both test and control groups, there was a significant decrease in the depth of the periodontal pocket, plaque index, bleeding of probing, increased gingival recession, and gain of clinical attachment level at 1 month and 3 months after treatment. However, there was no significant clinical difference between the test group and the control group. Clinical result was improved after 1 month compared to the baseline; in contrast, results at 3 months after treatment were worse than at 1 month after treatment. Conclusion: In this study, we cannot suggest that SRP + EPAP is clinically more effective than SRP alone as non-surgical periodontal treatments. Periodic periodontal therapy, at intervals of at least every three months, is important for sustaining effects of this treatment.

  • 얼마전까지만 해도 우리나라는 조용한 아침의 나라, 동방예의지국이라 예찬되어 왔으며, 아름다운 산하는 금수강산이라 불리어져 왔다. 비록 창호지 한장으로 바른 창문이었지만 소음공해같은 것은 결코 느끼지 않고 살았으며, 창호지를 통하여 전달되는 첫새벽의 닭우는 소리는 소음이라기 보다는 장부의 단잠을 깨워주는 자연음으로서의 낭만이 있었다. 그러나, 이젠 사정이 크게 달라졌다. 지난 40년 동안에 우리나라 경제가 고도성장을 이룩함으로써 우선 국민생활은 풍요로와 겼으나 대소 공장과 사업장 및 교통혼잡으로부터 The아져 나온 각종 오염물질이 엄청나게 증가하여 호흡하는 공기와 마시는 물, 곡물이 자양되는 토양은 병들어 가고 있을 뿐만 아니라 옛날에는 생각지도 못했던 온갖 소음ㆍ진통으로 우리는 포장되어 살아가고 있다 해도 과언이 아니다. $\ulcorner$소음진동규제법$\lrcorner$에 소음이란 기계ㆍ기구ㆍ시설 기타 물체의 사용으로 인하여 발생한 강한 소리라 정의하고 있다. 말할 것도 없이 이는 인위적으로 발생되는 시끄러운 소리를 의미하며, 이러한 소음을 다소나마 피하기 위해 창문을 알루미늄샤시 유리창으로 바꾸어 커텐을 치고, 출입문을 두중한 원목 도어로 교체 해도 다양해지고 강해진 소음공해를 막기는 어려운 세상이 되 었다. 이로 인하여 우리는 생활의 안정을 위협 받게 되었으며, 학업과 생업에 크고 작은 장해를 받으며 살아가고 있는 것이 오늘의 현실이다. 정부ㆍ기업ㆍ국민 각자가 쾌적한 환경을 보전하고자 하는 확고한 의지와 배경이 없다면 후손들에게도 대물림 될 것임에 틀림없으며, 특히 이러한 현상은 도회지에서 더욱 심화될 것임은 명약관화하다 하겠다.

  • Until early 1980s we have lived without thinking that radon ruins our health. But, scientists knew truth that radon radioactive danger is bedeviling on indoor that we live for a long time. Specially, interest about effect that get in danger and injury for Radon and human body is inactive in our country. Recently, with awareness for Radon contamination, We inform about importance and danger of Radon in some station of the Seoul subway, indoor air of school facilities and We had interest with measure and manages. Usually, Radon gas emitted in base of building enters into indoor through building floor split windage back among radon or indoor air of radon daughter nucleus contamination is increased. Therefore, indoor radon concentration rises as there are a lot of windages between number pipe of top and bottom and base that enter crack from estrangement of the done building floor, underground to indoor. Thus, Radon enters into indoor through architecture resources water as well as, kitchen natural gas for choice etc., but more than about 85% from earth's crust emit. Danger and injury of health by Radon and Radon daughter nucleus that is indicated for cause of lung cancer incerases content of uranium of soil rises specially from inside pit of High area and a mine, cave, hermetical space with house. Safe sub-officer of radon concentration can not know and danger always exists large or small during. So, Important thing reduces danger of lung cancer by lowering concentration of Radon within house and building. Therefore, is thought that need general house Radon concentration measurement, measured Radon concentration monthly using Sintillator radon monitor. Perforated Duodenal Diverticulum after Distal Subtotal Gastrectomy and Billorth II Gastrojejunostomy (위아전절제술 및 위공장 문합술 후 생긴 십이지장 게실 천공 환자 1예)

  • A 69-year old man presented with severe epigastric pain for 1 day. He had early gastric cancer at the antrum and underwent a distal subtotal gastrectomy and Billorth II gastrojejunostomy one month later without any post-operative complications. Radiologic examination revealed a large amount of retroperitoneal free air formation. Because of unremitting pain and unstable vital sign, exploratory laparotomy was followed. During the operation, a perforated duodenal diverticulum at the posterior wall of the 2nd portion of the duodenum was identified. He underwent diverticulectomy and primary closure. He was discharged on the 18th post operative day and has been followed up without any evidence of comlpication for several months.
