The Effects of Performance of Public Health Services and Personal Characteristics on Community Image of Public Hospitals (공공보건의료사업 수행과 주민특성이 공공병원 이미지에 미치는 영향)

  • This study purposes to identify the effects of performance of public health services (PHS) and personal characteristics on community image to public hospitals. The subjects of this study were 33 public hospitals and 1,789 community residents. The data of '2011 Public hospital evaluation programme' were utilized in this study. The personal characteristics consisted of nine items which were gender, age, education, occupation, monthly incomes, medical security, use experience, health state, and location type. The PHS performance consisted of five items which were number of doctors, number of nurses, total number of staff, budget per 1,000 community residents, and amount of activities per 1,000 community residents. The cronbach's alpha of community image instrument was 0.916. As the results of logistic regression, the significant variables of community image, were age (OR=0.34, 95% CI=0.19-0.60), education (OR=3.03, 95% CI=1.60-5.76), use experience (OR=0.57, 95% CI=0.40-0.81), health state (OR=0.69 95% CI=0.49-0.96), location type (OR=2.10, 95% CI=1.11-3.99), and amount of activities per 1,000 community residents (OR=0.58, 95% CI=0.35-0.96). Community image is very important to public hospitals. The workforce and budget related PHS were significantly demanded to improve community image. The Central and Local government should support to public hospitals to perform PHS effectively.

Functional Health Status and Medical Service Utilization Pattern of General and Vulnerable Older People in Community (지역사회 일반 및 취약계층 노인의 건강기능상태와 의료서비스 이용 행태)

  • This study was to investigate the differences of functional health status and medical service experience and needs between general and vulnerable older people in community. This study is a cross-sectional descriptive research. The data obtained through direct visit surveys from November to December 2016. The target population of the study was older people over 65 years old, the final study subjects were 444 older people residing in one district of Seoul. The chi-square test was conducted to confirm the difference in their functional health areas and medical service experiences, and the necessity of medical service utilization in accordance to the social class. In the experience of abnormality in functional health, the vulnerable older people had higher experience in cognitive function, nutrition, hydration, pain, and falling than the general older people. The rate of experience of using medical service to solve the cognitive function problem for general older people was 31.9%, higher than that of the vulnerable older people. In contrast, the medical service utilization needs of the vulnerable older people in the pain management category was significantly higher than that of the general older people. In setting policy of public medical service programs for general and vulnerable older people in community may be differentially developed based on this study. In order to improve the medical accessibility of the vulnerable older people, public medical institutions should be actively supported to overcome obstacles to medical use due to economic barriers.

  • 본 논문은 RFID/USN 서비스의 국내외 시장 전망 및 적용 사례를 분석하였으며, 향후 서비스를 확대 할 수 있는 방안을 제시하였다. 국내외 RFID/USN 서비스 시장 규모는 기기 시장보다 더 커질 것으로 전망되며, 국방/안보, 항공/항만, 도로/교통, 유통/물류, 산업/건설, 제조/공정, 식품/농수 축산, 의료/보건, 도서/출판, 교육/문화/엔터테인먼트, 보안/방범, 재난/재해/환경 등의 분야에서 다양하게 적용되고 있는 것으로 나타났다. RFID/USN 서비스를 확대하기 위해서는 공공 분야를 중심으로 한 대규모 선도 수요 창출이 선행되어야 하며, 모바일 RFID 등 신규 서비스의 창출 및 활성화를 위한 정책 대안이 마련되어야 한다. 그리고 RFID/USN 서비스의 시장 규모와 보급시기 등을 고려하여 우선적으로 적용해야 할 서비스의 발굴을 확대하고, RFID/USN 도입 성과를 가시화하여 민간의 수요를 촉발할 필요가 있다.

A qualitative study on the case managers' experiences of implementing Assertive Community Treatment model in Korea (한국에서의 ACT(Assertive Community Treatment) 적용에 대한 사례관리자의 경험에 관한 연구)

  • The attention of case management currently has been highlighted and its importance has been highly emphasized in mental heath field. At this point, We conducted a qualitative study on the experience of case managers from the Suwon Community Mental Health Center where it has implemented the ACT model. Through the result of this study, we verified feasibility of ACT model into Korea. and it has shown that assertive service and team approach has been a critical ingredients of ACT and it extended case manager's ability and brought improvement in relationship between clients and them. the clients has brought a positive changes. However case managers have experienced the dilemma between experts initiatives and clients dependency, and they empathised the importance of the familism and mental health system in Korea. In order to implement ACT extensively, we suggested several implications.

  • "정보시스템의 효율적인 도입 및 운영 등에 관한 법률"이 제정된 이래 다수의 공공기관에서 개별적으로 범정부 ITA 체계를 기반으로 ITA 구축 사업이 활발하게 진행되고 있다. 공공기관, 특히 중앙행정기관의 경우 산하기관이 존재하는 특성으로 인해 ITA 구축 추진시 필연적으로 아키텍처 정보 구축 범위 및 대상 선정에 대한 문제와 산하기관의 아키텍처 정보와의 연계 및 통합이라는 문제에 직면하게 된다. 그러나 현재의 범정부 ITA 체계는 이에 대한 충분한 가이드를 제시하고 있지 못한 실정이다. 또한 산하기관들의 개별적인 사업 추진, ITA 구축의 목표의식 부재, ITA 기반의 IT 거버넌스 청사진 부재, ITA 연계 및 통합을 위한 컨트롤 타워(Control Tower)의 부재 등의 문제는, 각 기관의 정보자원 및 업무의 연계라는 ITA의 목적을 달성을 어렵게 하고 있다. 이에 국가정보화를 관장하는 행정안전부는 향후 3단계에 걸쳐 개별기관이 아닌 중앙행정기관과 산하기관을 아우르는 범정부 차원의 정보화 기본설계도(국가 EA)를 정립하고 개별 기관에 산재된 중복 유사 정보시스템을 연계 통합하기 위한 기준을 마련할 계획이다. 본 연구에서는 '07년 말부터 4개월간 진행된 보건복지가족부 ITA 구축 사례를 기반으로 아키텍처 정보 구축대상 및 범위를 결정하기 위한 분석 방안을 제시하고, ITA 연계 절차 및 모델을 제시하였다. 이는 향후 범정부 차원의 정보화 기본설계도 수립에 많은 시사점을 제공할 것으로 사료된다.

A Study on the Types of Public Hospitals in the Region by Cluster Analysis (군집분석을 통한 지역거점공공병원의 유형화)

  • This study selected indicators that can represent the characteristics of general hospitals, including local medical centers and Red Cross hospitals, which are representative public health institutions, and analyzed clusters. And we present to benchmark in each cluster. According to the analysis, 276 general hospitals were classified into 13 clusters, and local medical centers and Red Cross hospitals were classified into clusters between 1 and 7 of the total 13 clusters because of their small size. Local medical centers and Red Cross hospitals, selected as excellent hospitals in each cluster, showed significant differences in management performance despite similar regional environment and medical performance, and among them, surgical consultation and internal medical care rates, inpatient and outpatient rates. In order for local medical centers and Red Cross hospitals to play their role as secondary acute hospitals in the region, inpatient care services and surgical functions must be activated.

  • On rationale for government intervention is the failure of competition in the market. Health care markets are characterized by such unique aspects as information asymmetry, prevalence of insurance, and cost-increasing competition based on the adoption of costly medical technology. Therefore, government policy to guarantee a sufficient number of providers in markets may not lead to socially beneficisal outcomes such as higher quantity and lower price. This paper examines the unique nature of health services and its implications for competition, the evidence that competition may not reduce health care ex[enditures, and policy tools that government can use to encourage competition which contributes to supporting a sustainable health care system.

  • Health care privatization will have a very significant impact on the quality of medical service and income redistribution and risk diversification. This study was conducted to improve the situation without too much insistence that one did not understand the essence rife about the privatization of health care. In this study, grasp the implications of the current health care debate to be privatized, and dealing with the claims accordingly. Seeks to help establish an objective perspective, looking at the pros and cons at the same time claims for medical privatization policy. The Health and diversification through the capitalist market access as part of the transition process is being overlooked as a new growth engine industries. But health care is constrained to follow the growth of universal access and how to deal with life because it is practical conduct of the person. In addition to services that are not only for the benefit of the profit or government institutions.

  • This paper develops the argument that the 'Healthy Cities Approach' extends beyond the boundaries of officially designated Healthy Cities and suggests that signs of it are evident much more widely in efforts to promote health in the United Kingdom and in national policy. It draws on examples from Leeds, a major city in the north of England. In particular, it suggests that efforts to improve population health need to focus on the wider determinants and that this requires a collaborative response involving a range of different sectors and the participation of the community. Inequality is recognised as a major issue and the need to identify areas of deprivation and direct resources towards these is emphasised. Childhood poverty is referred to and the importance of breaking cycles of deprivation. The role of the school is seen as important in contributing to health generally and the compatibility between Healthy Cities and Health Promoting Schools is noted. Not only can Health Promoting Schools improve the health of young people themselves they can also develop the skills, awareness and motivation to improve the health of the community. Using child pedestrian injury as an example, the paper argues that problems and their cause should not be conceived narrowly. The Healthy Cities movement has taught us that the response, if it is to be effective, should focus on the wider determinants and be adapted to local circumstances. Instead of simply attempting to change behaviour through traditional health education we need to ensure that the environment is healthy in itself and supports healthy behaviour. To achieve this we need to develop awareness, skills and motivation among policy makers, professionals and the community. The 'New Health' education is proposed as a term to distinguish the type of health education which addresses these issues from more traditional forms.

  • This paper searches for programs for creating increased employment in local society in important occupation sectors such as education and training services, insurance and social welfare services, culture and arts, housing and environmental service fields. As part of an improvement plan, while strengthening investment in education training infrastructure for the suppliers of social services, cooperation and coordination by the public and private sectors is necessary. Also necessary are identifying model social enterprises that are based upon joint efforts for success of social enterprises and publicity efforts necessary.
