Effect of vitamin C deficiency on the rate of orthodontic tooth movement and alveolar bone remodeling (비타민 C 결핍이 guinea pig의 실험적 치아이동속도와 치조골 개조에 미치는 영향)

  • This study was undertaken to investigate the effect of vitamin C deficiency on the orthodontic tooth movement and bony remodeling processes. Thirty six male guinea pigs were divided on the basis of the given amount of vitamin C (normal group: 5mg/day, deficient group: 0.2mg/day) and 75gm of force was applied to the maxillary incisors. Experimental animals were sacrificed at day 0. day 1 day 3, day 5. day 7 and day 14 after force application and the amount of tooth movement was measured and tissues were studied histologically. The results showed that the amount of collagen fiber in the periodontal ligament and alveolar bone of the deficient group was less than that of the normal group. In the stretched side. the osteoblastic activity and alveolar bone formation of the normal group increased in a time dependent manner during experimental periods, but the deficient group showed less activity and formation. The amount of tooth movement in the deficiency group was more than in the normal group at day 0. day 1, day 3, day 5, and day 7. According to the above results, a deficiency of vitamin C resulted in a defect of collagen synthesis of the periodontium and inhibition of bone formation and stimulation of bone resorption with rapid tooth movement in early periods of force application.

The influence of magnet on tissue healing after immediate implantation in fresh extraction sites in dogs (성견에서 발치 후 즉시 식립 임플란트에 설치한 자석이 주위 조직에 미치는 영향)

  • Statement of problem: The clinical use of electric and electomagnetic fields for fracture healing applications began in the early 1970s. Since then, several technologies have been developed and shown to promote healing of fractures. Developments of these devices have been aided in recent years by basic research and several well controlled clinical trials not only in the medical field but in dentistry. Purpose: The purpose of this study was to compare alveolar bone reduction following immediate implantation using implants onto which magnets were attached in fresh extracted sockets. Material and methods: Four mongrel dogs were involved. Full buccal and lingual mucoperiosteal flaps were elevated and third and fourth premolars of the mandible were removed. Implants with magnets and implants without magnets were installed in the fresh extracted sockets and after 3 months of healing the animals were sacrificed. The mandibles were dissected and each implant sites were sampled and processed for histological examination. Results: The marginal gaps that were present between the implant and walls of the sockets at the implantation stage disappeared in both groups as a result of bone fill and resorption of the bone crest. The buccal bone crests were located apical of its lingual counterparts. At the 12 week interval the mean of marginal bone resorption in the control group was significantly higher than that of the magnet group. The majority of specimens in magnet group presented early bone formation and less resorption of the buccal marginal bone compared to the control group. Conclusion: Within the limitations of this study, it could be concluded that implants with magnets attached in the early stages of implantation may provide more favorable conditions for early bone formation and reduce resorption and remodeling of marginal bone.

The Effects of PDGF and LPS on the Viability of Human Periodontal Ligament Cells (PDGF와 LPS가 치주 인대 세포의 활성에 미치는 영향에 관한 연구)

  • Platelet-derived growth factor(PDGF) and lipopolysaccharide(LPS) may be the important regualtors of bone metabolism Exogenous PDGF is recognized to have a stimulating effect on bone resorption in organ culture but to stimulate the formation of new bone ultimately. LPS is known to be a stimulating agent on the osteoclastic activity. The purpose of this study was to evaluate the effects and the interaction of PDGF and LPS on periodontal ligament(PDL) cells which have important roles in bone remodeling. Cultured human periodontal ligament cells were tented with various concentration or PDGF and/or LPS. The cellular viability was measured by Microtitration(MTT) assay according to the lapse time of culture. The obtained results were as follows: 1. The viability of PDL cells was not different from the con01 in 0.1ng/ml of PDGF, but was significantly increased to be over the level of control in 1ng/ml of PDGF at the second day of culture, and in 10ng/m1 of PDGF at the second and the third day of culture. 2. The cellular viability was decreased in $0.5{\mu}g/ml$ or $5{\mu}g/ml$ LPS at the third day of culture. 3. Incubation with both 1ng/ml or 10ng/ml of PDGF and $0.5{\mu}g/ml$ of $5{\mu}g/ml$ of LPS resulted in the increased cellular viability at the third day, which was greater than LPS only treated group. It was greater than even the control group in 10ng/m1 of PDGF. From the above findings, we could summarize that the admixture of PDGF and LPS could not less increase the viability of the human periodontal ligament cells than PDGF only.

A LIGHT AND ELECTRON MICROSCOPIC STUDY OF CHANCES IN INTERMAXILLARY SUTURE DURING THE RAPID MAXILLARY EXPANSION OF YOUNG ADULT DOGS (유성견 급속정중이개시 정중구개봉합부위의 조직학적 변화에 대한 연구)

  • Rapid maxillary expansion is widely used for the correction of anteroposterior discrepancies, constriction of the maxillary arch, etc. This experiment was undertaken to examine the serial changes in the osteogenesis as well as the collagen fiber bundles in the intermaxillary suture during the rapid maxillary expansion treatment. Four young female dogs aged 6 to 8 months old and not showing menarche yet were used for the experiment. The maxillary impression of dogs were taken, expansion device cast and Hyrax screw soldered at the midline in the 1st premolar area. RME device was delivered to the dogs and the activation of 0.25 mm per quarter-turn was done 2 times per day for 10 days until 5 mm separation was made. Separation of the maxilla was confirmed by X-ray. The animals were sacrificed on 0, 15, 30, 60 days from the finish of maxillary separation and preparations for light microscopy and surface electron microscopy were made. The sutures were cut into frontal serial sections for examination of the histological reactions. The following results were obtained and the conclusions made. 1. The edges of the two palatal plates bordering the midpalatal suture which at the beginning of the retention period were mainly composed of compact bone, underwent extensive resorption followed by new bone formation and gradually became spongy bone rich in bone marrow which in the 60 day retention animal became the compact bone with short intermaxillary suture space. During this transformation, newly formed trabecular bone tissues were added to the original margin. 2. Throughout the expansion period, the collagen fibers underwent successive changes such as stretching, loss of polarity, and finally fibrillogenesis. Towards the end of the expansion procedure, sharpey's fiber formation in newly formed bones were observed. 3. Bony spicules were found in the initial stage of retention on occlusal topographic X-rays, which later were confirmed to have ossified. 4. Judging from the histological changes occuring during the experimental expansion, excessive expansion will cause an excessive bleeding, and retard the remodeling of intermaxillary suture. According to the above results, the bone remodeling after rapid maxillary expansion was preceded by the migration of migratory cells into the intermaxillary suture area. The bone remodeling phenomena were on-going during the 2 months retention sample.

Finite element analysis of peri-implant bone stresses induced by root contact of orthodontic microimplant (치근접촉이 마이크로 임플란트 인접골 응력에 미치는 영향에 대한 유한요소해석)

  • Objective: The aim of this study was to evaluate the biomechanical aspects of peri-implant bone upon root contact of orthodontic microimplant. Methods: Axisymmetric finite element modeling scheme was used to analyze the compressive strength of the orthodontic microimplant (Absoanchor SH1312-7, Dentos Inc., Daegu, Korea) placed into inter-radicular bone covered by 1 mm thick cortical bone, with its apical tip contacting adjacent root surface. A stepwise analysis technique was adopted to simulate the response of peri-implant bone. Areas of the bone that were subject to higher stresses than the maximum compressive strength (in case of cancellous bone) or threshold stress of 54.8MPa, which was assumed to impair the physiological remodeling of cortical bone, were removed from the FE mesh in a stepwise manner. For comparison, a control model was analyzed which simulated normal orthodontic force of 5 N at the head of the microimplant. Results: Stresses in cancellous bone were high enough to cause mechanical failure across its entire thickness. Stresses in cortical bone were more likely to cause resorptive bone remodeling than mechanical failure. The overloaded zone, initially located at the lower part of cortical plate, proliferated upward in a positive feedback mode, unaffected by stress redistribution, until the whole thickness was engaged. Conclusions: Stresses induced around a microimplant by root contact may lead to a irreversible loss of microimplant stability.

THE LIMITATION OF ALVEOLAR BONE REMODELING DURING RETRACTION OF THE UPPER ANTERIOR TEETH (상악 전치부 견인 시 치아이동에 따른 전방 치조골개조량의 변화에 관한 연구)

  • In many cases of orthodontic treatment the upper anterior teeth are retracted. Periodontal problems may arise during incisor retraction, if the amount of tooth movement and the amount of remodeling in the anterior cortical bone are not the same. Therefore in this study, to find out the relationship between the amount of tooth movement and the amount of bone remodeling during retraction of the upper anterior teeth, lateral cephalograms of 56 female patients over 18-year-old were taken before and after treatment. Among the 56 patients, two groups were divided according to the type of root movement during retraction. 26 patients mainly moved by tipping and 30 by bodily movement. The cephalograms taken before and after treatment were superimposed upon the true horizontal plane. In the Tip-Group, the horizontal bone remodeling/tooth movement ratio was 1:1.63, and in the Torque-Group it was 1:1.66. Because the amount of tooth movement and the amount of bone remodeling were not the same in both groups, in the Tip-Group the root apex moved away from the palatal cortical plate and closer to the labial cortical plate, whereas in the Torque-Group the root moved away from the labial cortical plate and closet to the palatal cortical plate. Therefore, there are limitations in the amount of incisor retraction in patients with a very thin anterior cortical plate in the maxilla, and in patients with severe skeletal discrepancies orthognathic surgery should be considered and when orthodontic camouflage treatment is the only possible method, the orthodontist must be aware of the limitations of treatment.

Early histological change in hard tissue from orthodontic force placed on microscrews in ovariectomized rats (난소 적출 백서에 식립된 마이크로스크류에 교정력 부여 시 나타난 초기 경조직 변화에 관한 연구)

  • Most elderly women experience a decrease in their bone density due to a deficiency of calcium intake, ovariectomy, or menopause. This study evaluated the usability of the microscrew as a skeletal anchorage system in these orthodontic treatment cases, using rats as a research group. The 4 month old sprague-dawley species rats were divided into two groups, the OS (Ovariectomy Screw), and the SS (Sham operation Screw) group. In both the OS and SS groups, microscrews were implanted into the palatal bone between the upper molar teeth and two upper incisors were retracted using NETE coil spring with 75 g of force. After 3days, the again after 7 days, 7 rats in each group were sacrificed. Three days before they were sacrificed, Alizarin red S was intraperitoneally injected, and their maxillary bone, tibia and blood from their hearts were taken. The components of the extracted blood were biochemically analyzed and non-decalcified grinding resin sections for maxillary bone and tibia were made. The sections were examined with a polarization microscope, and fluorescent microscope. Smaller concentrations of Ca and P, the inorganic substances closely related to bone density, were found in the extracted blood of the OS group. Both OS and SS groups showed a possibility of bone remodeling with a high concentration of ALP after 7 days. An increase in bone density on the tension and compression sides of the microscrew and the tension side of the tooth for both OS and SS groups was confirmed with a polarization microscope. However, the bone density of the pressure side of the tooth and apical side was decreased. More deposits of Alizarin red S in the bone after 7 days rather than 3 days seen with a fluorescent microscope suggested the existence of new bone formation.

OSSEOUS REPAIR OF THE FRACTURED MANDIBULAR CONDYLE IN CHILDREN (소아에서 발생된 골절된 하악과두의 골회복)

  • 본 연구는 1987년 1월부터 1991년 12월까지 연세대학교치과병원 구강악안면외과에 과두 골절이 의심되어 내원한 환자중 임상 및 방사선학적 소견에서 하악과두 골절로 진단되어 비외과적 치료를 받은 12세 미만의 13명의 소아환자를 연구 대상으로 하여 임상 및 방사선학적 소견을 분석하여 다음과 같은 결론을 얻었다. 비외과적인 보존적 또는 기능적 치료를 받은 후의 결과는 양호하였으며, 골의 재생과 개조는 신속하여 골 구조는 기능 중에 가해진 힘에 반응한다는 Wolff의 법칙과 일치하는 듯이 보였다. 두 예의 골절탈구에서 과두의 단축 및 과두가 골절된 악관절에서 clicking음을 보였으나 저작하거나 개구시에 큰 어려움은 없었다. 술후 관찰기간이 불충분하였으나 본 예의 대부분에서 비외과적인 술식에도 만족스러운 치료결과를 보였다.

Effects of bisphosphonate and indomethacin on alveolar bone remodeling in rats (Bisphosphonate와 Indomethacin이 백서 치조골의 골개조에 미치는 영향)

  • The purpose of this study was to examine the effects of bisphosphonate and indomethacin, blockers of bone resorption with different mechanisms, on alveolar bone remodeling. Male rats were divided into control, bisphosphonate and indomethacin groups, and then each group was divided info an experimental side and a control side according to the force application. Bisphosphonate(6.3mg/kg,$2.52x10^{-2}mol/L$) and indomethacin (9mg/kg, $2.52x10^{-2}mol/L$) were injected 6 hours and 1 hour before or 24 hours after the force application. The rats were killed 72 hours after the force application and histologic examination was perfomed. The values of serum acid phosphatase and lactate dehydrogenase were also measured in the control md experimental groups treated with bisphosphonate or indomethacin 1 hour before the force application. In the experimental side, the least number of osteoclasts was noted in the groups treated 1 hour before the force application with indomethacin or bisphosphonate, while there were no differences between the control and the groups treated with drugs 6 hours before or 24 hours after the force application. In the control side, the number of osteoclasts was not inecreased with no differences among the groups. Histologic examination revealed a severe alveolar bone resorption in the control group and the groups treated with indomethacin 6 hours before or 24 hours after the force application. Indomethacin treatment 1 hour before the force application and bisphosphonate treatment at any time significantly attenuated the bone resorption. Electron microscopically, ruffled border and clear zone of osteoclasts were observed in the control and indomethacin groups, while some osteoclasts were detached from the bone surface and exhibited dull cellular projections in the bisphosphonate groups. The bisphosphonate and indomethacin groups showed lower values of acid phosphatase and lactate dehydrogenase than the control group. The acid phosphatase value in the bisphosphonate group was lower than that in the indomethacin group, whereas there was no difference in the lactate dehydrogenase value between the groups. These results suggest that bisphosphonate reduces the activity of osteoclasts as well as the number of osteoclasts and that indomethacin reduces the number of osteoclasts without affecting the activity of osteoclasts. Bisphosphonate has a larger inhibitory effect on bone resorption md thus less limitation in the application time than indomethacin.
