Radioprotective Effect of Mesna on Mouse Testis (Mesna의 쥐 고환에 대한 방사선 보호 효과)

  • Mesna has been used with ifosfamide to prevent urotoxicity in the treatment of testicular cancers. This drug also protected the toxicities of adriamycin without compromising cytostatic activity. With an idea of radioprotective role of sulfhydryl group of radioprotectors and of mesna decreasing the toxic effect of adriamycin which produces free radicals, mesna and radiation were administered to mice to study the protective effect of this drug and to identify the difference in regenerative capacity of the germ cells in the testis between radiation-treated and both mesna-and radiation-treated groups. The shape and numbers of spermatogenic cells in the seminiferous tubules were examined every week after irradiation. In both groups, initial reduction and later recovery in germ cell numbers and shape was observed. The lowest germ cell number was found around three weeks after irradiation. Mean germ cell number of the mesna-treated group was significantly higher than radiation-treated group at all observed periods (p<0.05). More competent regeneration was present in mesna-treated group. These results suggest that mesna protect the testis from radiation injury. Further study will be necessary to identify whether mesna protects other tissues from radiation and it does not hamper tumor control.

Evaluating the Efficiency of the Device in Shielding Scattered Radiation during Treatment of Carcinoma of the Penis (음경암의 방사선치료 시 자체 제작한 Device의 산란선 차폐 효과에 대한 유용성 평가)

  • Purpose: We evaluated the device that was created for maintaining the patient's setup and protecting the testicles from scattered radiation during treatment of carcinoma of the penis. Materials and Methods: The phantom testicles were made of vaseline cotton gauze and the device consisted of 5 mm of acryl box and 4 mm of lead shielding. $3{\times}3\;cm^2$, $4{\times}4\;cm^2$, $5{\times}5\;cm^2$, $6{\times}6\;cm^2$, $7{\times}7\;cm^2$ field sizes were used for this study and measurement was made at 4, 5, 6, 7, 8, 10 cm from the lower edge of the field for 10 times with lead shielding and without the shielding respectively. 200 cGy was delivered using 6 MV photons. Results: The scatted radiation without lead shielding at 4, 5, 6, 7, 8, 10 cm from the lower edge of the field were 14.8-4.7 cGy with $3{\times}3\;cm^2$, 15.7-5.2 cGy with $4{\times}4\;cm^2$, 17.6-5.5 cGy with $5{\times}5\;cm^2$, 19.9-6.6 cGy with $6{\times}6\;cm^2$, 22.2-7.6 cGy with $7{\times}7\;cm^2$ and the measured dose without lead shielding were 7.1-2.6 cGy with $3{\times}3\;cm^2$, 8.9-3.6 cGy with $4{\times}4\;cm^2$, 12.3-4.8 cGy with $5{\times}5\;cm^2$, 14.6-5.0 cGy with $6{\times}6\;cm^2$ and 21.1~6.4 cGy with $7{\times}7\;cm^2$. As shown above, the scatted radiation decreased after using lead shielding. Depending of the range of field sizes, the resulting difference between without shielding values and with shielding values were: 7.8-1.1 cGy at 4 cm, 5.1-1.2 cGy at 5 cm, 3.8-1.1 cGy at 6 cm, 3.4-1.7 cGy at 7 cm, 2.8-1.7 cGy at 8 cm, 2.4-2.5 cGy at 9 cm and 2.1-1.8 cGy at 10 cm. In the situation as described above, the range in values depending on the distance was 7.8-1.1 cGy with $3{\times}3\;cm^2$, 6.9-1.6 cGy with $4{\times}4\;cm^2$, 5.3-0.8 cGy with $5{\times}5\;cm^2$, 5.3-1.5 cGy with $6{\times}6\;cm^2$ and 1.1-1.8 cGy with $7{\times}7\;cm^2$. Conclusion: Using the device we created to shield the testicles from scattered radiation during treatment of carcinoma of the penis, we have found that scattered radiation to the testicles is decreased by the phantom testicles, and by increasing the distance between the testicles and penis.

Is Radiotherapy Necessary for Stage 1 Testicular Seminoma? (제1병기 성인 고환 정상피종에 대한 임상적 고찰 및 치료결과 분석)

  • Purpose: To report on the clinical outcome of patients with stage I testicular seminoma by postoperative radiotherapy (PORT) or surveillance after radical inguinal orchiectomy. Materials and Methods: This study is a retrospective review of 32 stage I pure seminoma patients treated between 1996 and 2005 at the Samsung Medical Center. Twenty two of the patients were treated by PORT, which was directed at the paraaortic lymphatics with a median dose of 25.2 Gy in 14 fractions for 3 weeks. The 10 remaining patients were managed by surveillance. The median follow-up period was 96 months with a range of 24 to 155 months. Results: Clinically, most patients presented with a testicular mass or discomfort. Two of the patients had a history of undescended testes. Pathologically, 23 of the patients had intratubular germ cell neoplasia with seminoma. Both recurrence-free survival (RFS) and overall survival (OS) rates of patients treated by PORT were 100%. In the control group, 1 of the 10 patients suffered a para-aortic lymph node relapse. The RFS and OS rates of the surveillance group were 88.9% and 100%, respectively. Conclusion: No difference in survival was observed between the two groups. Moreover, symptom recurrence was only observed in 1 patient in the control group. The use of PORT may reduce the risk of relapse. With the availability of effective diagnostic and salvage modalities, surveillance monitoring may be considered for patients in good compliance.

Identification of Tumor Antigens in Lung Cancer Patient by SEREX (폐암 환자에서 면역항원유전자의 혈청학적 동정)

  • Serological anlysis of recombinant cDNA expression libraries (SEREX) has led to identification of several categories of new antigens recognized by the immune system of cancer patients, which are referred to as the cancer immunome. We analyzed normal testis cDNA expression libraries with serumobtained from non-small lung cancer patient and isolated 40 distinct antigen designated KP-LuT-1 through KP-LuT-40. Among these antigens 20 antigens were previously identified by SEREX analysis of other tumor types, and 20 out of 40 antigens (50%) did not match entries in Cancer Immunome Database and were considered newly identified antigens. Sequencing analysis showed that the anti-gens comprised 26 functional known proteins and 14 noble/uncharacterized gene products. Of these, the hypothetical protein KP-LuT-6 was shown tissue-restricted. RT-PCR showed it to be expressed strongly only in normal testis. In addition to normal tissues-restricted expression, KP-LuT-6 mRNA was detected in lung tumor samples(3/l0), stomach tumor samples(3/l0), and breast tumor samples(l/5), whereas not detected in colon tumor samples(O/I2). These data suggest that KP-LuT-6 is a cancer/testis (CT)-like antigen as a potential target for cancer immunotherapies.

A comprehensive Analysis of a Large Panel of Cancer/Testis (CT) Antigens in Korea Breast Cancer (한국 유방암 환자에서 cancer/testis antigen의 발현분석)

  • Cancer/testis (CT) antigens are immunogenic in cancer patients, exhibit highly tissue-restricted expression, and are considered promising target molecules for cancer vaccines. We investigated the expression of 13 CT genes in 29 Korean patients with primary breast carcinoma. The most frequently expressed CT genes were MAGE-3 (66%) and MAGE-1 (57%), followed by LAGE-1 (55%), NY-SAR-35 (49%), MAGE-4 (41%), NY-ESO-1 (38%), CT-7 (24%) and SSX-4 (24%), whereas SSX-1, SSX-2, MAGE-10 and NY-TLU-57 were found to be expressed significantly less often (3-7%) and SCP-l not all. Expression of at least one antigen was observed in 28 breast cancer samples. Immunohistochemistry was performed for NY-ESO-l and MAGE-3 protein expression in breast tumor samples. NY-ESO-l and MAGE-3 proteins were expressed in 11 of 29 (38%) and 12 of 29 (41 %) breast tumors. Our results suggest that CT antigens may be potential candidates for polyvalent immunotherapy in Korean breast cancer patients.

부산점토의 특성 : 조간대 퇴적층의 대자율

  • 낙동강 삼각주의 분포하는 제4기 홀로세 퇴적층 중 북부와 중부, 그리고 남부지역에 해당하는 가덕도 (신항만 조성지역), 범방리 (경마장 조성지역), 양산 (신도시 조성지역)의 조간대 퇴적층에 대한 대자율 연구를 수행하였다. 퇴적층의 층위별 대자율은 입도와 관입저항에 비례하여 대자율도 증가한다. 가덕도의 70여m의 퇴적층 중 20m, 40m의 점토층에서 대자율 변화가 나타나는데 인접한 녹산지역의 연구견과와 비교하면 고환경이 변화되는 지점과 일치한다. 삼각주의 중부에 위치하는 범방리 점토시료는 기반암까지 층위별 대자율이 일정하며, 북부의 양산지역 점토시료는 하부로 갈수록 대자율이 증가하는데 이러한 결과는 시료의 입도 분포와 같은 양상이다. 가덕도 시료와는 달리 모든 점토층이 유사한 대자율을 보여주고 있어 퇴적 동안 환경의 변화는 인지되지 않는다. 이 지역의 시료가 가덕도에 비해 높은 고도에 위치하여 가덕도 시료의 상부 점토에 해당되는 것도 환경변화가 나타나지 않는 이유가 될 수 있다. 따라서 보다 광범위한 지역에 대한 연구가 요구 된다.

A Case of Advanced Non-seminomatous Germ Cell Tumor of the Testis Cancer with Retroperitoneal Lymph Node Metastasis : Neoplastic Pain & Neoplastic Fever Treated by Sasang Constitutional Medical Treatment (고환통과 발열을 호소하는 고환암환자의 치험 1례)

  • 1. Objectives In this case report, We report a case of advanced non-seminomatous germ tell tumor(NSGCT) of the testis to retroperitoneal lymph node. We successfully managed with sasang medical treatment. 2. Methods We treated a man with NSGCT of testis with retroperitoneal lymph node metastasis. We evaluated pain grade by VAS(visual analogue scale). and fever grade by body temperature(by axillary thermometer) 3. Results Pain and fever were improved with the Sasang constitutional medicine. 4. Conclusions Cancer pain and fever was markedly improved by Sasang Constitutional management. Further studies are needed to evaluate the cause of the improvement.

A Case of the Bleomycin-Induced Bronchiolitis Obliterans Organizing Pneumonia (Bleomycin 투여 후 발생한 폐쇄세기관지기질화폐렴)

  • A 34-year-old man was admitted to our hospital due to fever and cough. He received the combination anti-cancer chemotherapy for testicular tumor, including bleomycin. The chest X-ray showed consolidation and ground glass opacity on the right upper lobe and subpleural areas of other lobes. This condition was initially misdiagnosed as a pneuomonia, but consolidation did not disappear after antibiotics treatment. We performed transbronchial lung biopsy and bleomycin induced pulmonary toxicity was confirmed. The bleomycin induced lung injury is the most common chemotherapeutically induced pulmonary disease. Bleomycin induced Bronchiolitis Obliterans Organizing Pneumonia(BOOP) is less common than interstitial pneumonitis and responds well to corticosteroid treatment.

  • Cancer-testis (CT) antigens are immunogenic protein antigens with restricted expression in the testes and a wide range of human tumor types, eliciting both humoral and cellular immune responses in cancer patients. They are considered to be ideal targets for vaccine-based immunotherapy, and more than 100 CT antigens, including MAGE, NY-ESO-1, GAGE, BAGE, LAGE, SSX2 and NY-SAR-35 have been identified to date. The CT antigens were identified through various techniques and can be divided in those that are encoded on the X chromosome, the CT-X genes, and those that are not, the non-X-CT genes. CT genes are aberrantly activated and expressed in a proportion of various types of human cancers. The biological role of CT-X in both germ line tissues and tumors remains poorly understood. Cancer vaccine trials based on several CT antigens are currently ongoing. This paper reviews recent advances in and future trends of CT antigens for cancer immunotherapy.

Mineralogical and Geochemical Properties and Origin of Clay-silt Sediments, Suwon, Korea (경기도 수원시에서 산출되는 적갈색 점토-실트 퇴적물의 광물 및 지화학 특성과 기원)

  • Mineral and geochemical analysis were conducted on two sections (~3.5 m) of red-brown claysilt sediments covering the gneiss and granite weathering zones in Suwon-si for establishing Quaternary paleoenvironmental changes in Korea. The sections were divided into four sedimentary layers (Unit 1-4) by vertical changes in mineral composition and chemical composition. The lowermost unit 1 was a sandy sediment with a high K-feldspar content with a significant contribution of weathered bedrock. Unit 2 was a transition layer showing intermediate characteristics. Unit 3 was a reddish brown clay-silt sediment, with a total clay content of 58% on average, and the main clay minerals were illite-smectite mixed layer minerals and hydroxy-interlayered vermiculite/smectite. Unit 3 contained almost no plagioclase, while the content of kaolin minerals derived by the plagioclase weathering was higher than in the other layers. Unit 4 had similar mineral composition and chemical properties to Unit 3, but had a higher content of plagioclase and chlorite and lower content of kaolin minerals. The chemical compositions of the sections were compared with those in other regions of Korea, suggesting the eolian origin of Units 3 and 4. The paleoenvironmental change in the sedimentary section of this region was interpreted as follows. Weathered products of gneiss and granite, which are bedrocks of this region, were eroded and deposited as sandy sediments in the periphery to form the lower layers (Unit 1, 2), followed by the deposition of the claysilty rich eolian sediments (Unit 3) during the glacial. Unit 3 was chemically weathered during the warm humid climate during the last interglacial, developing a reddish brown color. After that, a eolian sediment layer (Unit 4) was deposited during the last glacial.
