  • Objective: A case report on the improvement of hypertension patients by Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 59-year-old Korean male with hypertension having bad habits as alcohol, smoking and overweight. Results: Weight loss and blood pressure improved after Ortho-Cellular Nutrition Therapy (OCNT). Conclusion: Ortho-Cellular Nutrition Therapy (OCNT) is effective in relieving the symptoms of patients with chronic symptoms to hypertension.

Factors Affecting Unmet Medical Care in Elderly Hypertensive Patients

  • The purpose of this study is to investigate factors affecting unmet medical care in elderly hypertensive patients. The purpose of this study was to understand the reasons for unmet medical care in elderly hypertensive patients over 65 years of age and to identify the characteristics of each type to improve the medical use system and to contribute to policy making so that unmet medical care does not occur. Using data from the 7th period of the National Health and Nutrition Examination Survey (2016, 2017), 23 sample households were selected using the phylogenetic extraction method among appropriate households and analyzed using the SPSS 18.0 Program. As a result of the study, the unmet medical care of those who had suicidal thoughts compared to those who had not had suicidal thoughts in the past year, when females were higher than males, had worse subjective health status, those who did not subscribe than those who had private insurance, and those who had smoked in the past year. The experience rate was high. This is considered to be able to contribute to improving the medical service system and making policy decisions so that unmet medical care does not occur in the future.

The Effects of Tai Chi Exercise Program on Blood Pressure, Total Cholesterol and Cortisol Level in Patients with Essential Hypertension (태극권운동 프로그램이 본태성 고혈압 환자의 혈압, 총 콜레스테롤 및 코티졸에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of a 6-week Tai Chi exercise program on reducing blood pressure for hypertensive patients. Method: A non-equivalent pretest-posttest experimental design was used. Participants were recruited from the Community Health Center in Busan, Korea. Twenty-eight hypertensive patients participated in this study. Among them, fourteen were in the experimental group and the rest are in the control group. Members in the experimental group participated in a 6- week program of Tai Chi exercise. In order to evaluate the effects of the Tai Chi program, blood pressure, total cholesterol, and cortisol level were measured before and after week 6. Result: After the 6-week Tai Chi program, there were significant differences in systolic pressure (t=-3.13, p=.004) and diastolic blood pressure (t=-4.75, p=.000) in the experimental group when compared to the control group. However there were no significant differences in the total cholesterol (t=1.07, p=.294) and cortisol level (F=1.35, p=.256). Conclusion: These results suggest that a 6-week Tai Chi program can be utilized as an effective nursing program to reduce blood pressure for hypertensive patients.

  • 동맥계에서 맥파의 전달속도는 혈관의 긴장도에 따라 결정된다. 동맥혈관의 긴장도는 혈압에 의존적이며 순간적인 혈압의 증가에 따른 혈관긴장도의 변화는 순환기계의 질병을 야기할 수 있다. 본 연구에서는 말초맥파를 이용한 동맥 긴장도 평가 방법의 유용성을 밝히기 위하여 말초맥파 도달시간을 체위에 따라 측정하고, 체위 변화에 따른 정수압 변화와 말초맥파도달시간의 관계를 관찰하였다. 말초맥파도달시간의 측정은 심전도의 QRS파 피크로부터 말초에서 나타나는 광전용적맥파의 최대기울기점까지 시간을 측정하였다. 생체 신호를 아날로그-디지털 변환하여 PC에서 매번 심장주기마다 맥파도달시간을 분석하였으며 결과는 스플라인 보간법을 사용하여 그래프로 결과를 제시하였다. 소프트웨어는 C++을 사용하여 windows 환경에서 운용하였다. 앙아위에서 말초맥파도달시간은 233.38$\pm$1.84 ms이었으며 서있는 자세에서는 226.77$\pm$2.16 ms로 유의하게 감소하였다. 앙아위의 자세에서 손을 수직으로 들어올림에 따라 말초맥파도달시간은 각도에 비례하여 증가하는 특성을 보였다. 이러한 실험결과는 말초맥파도달시간이 동맥계의 일시적인 압력 변화에 매우 민감하게 반응하는 것을 제시하였으며, 고혈압 및 동맥경화가 있는 환자에서 예후를 판단하는 방법으로 유용하게 사용될 것이라 사료된다.

  • The surgical closure of VSD in patient with severe pulmonary hypertension has been considered a difficult problem for surgeons, because sudden hemodynamic change after closure of the defect could bring on high perioperative mortality. Recently, it was reported that UVP(unidirectional valve patch), which allows some blood to flow from right to left in case of acute right heart failure, is effective in improving the postoperative hemodynamics after closing septal defects. This 42-year old woman had suffered from VSD for 20 years and recently complained of worsening exertional dyspnea for three months, and was diagnosed of a large VSD(2.0 cm in diameter) with severe pulmonary hypertension(116/38 mm Hg), equal to systemic arterial pressure. We could successfully close VSD with severe pulmonary hypertension using one UVP and the other UVP for the creative ASD to be prepared against possible acute right heart failure. She was discharged on the fourteenth postoperative day and has been well for twelve months with spontanenous closure of UVP patch at the ninth postopeative month.

Compliance with Health Behavior among Patients who are Hypertensive (고혈압 환자의 건강행위 이행)

  • Purpose: This study was done to investigate compliance with health behaviors among patients who are hypertensive. Method: The participants in this study were 121 outpatients with hypertension. Compliance with health behaviors was measured using a questionnaire developed by the researchers. Data were analyzed using t-test, ANOVA, Duncan's multiple range test and chi square test. Results: According to tile general characteristics of the participants, compliance with medication and diet was higher among women than men, among high school graduates than middle school graduates or those with lower academic qualifications, and among those who were unemployed compared to those who were employed. Compliance with alcohol abstinence or not smoking was higher among women than men, and among those who were unemployed compared to those who were employed. According to the risk factors of hypertension, compliance with health behaviors was significantly higher in those who had associated diseases. The participants in the group with a hypertension morbidity period longer than 10 years did exercise more than 3 times a week and regularly weighed themselves once or more a week. The item showing highest compliance concerning medication and diet was 'Take prescribed drugs regularly', while the one showing lowest was 'Take drugs with knowledge about their side effects'.

The impact of health literacy on self-care behaviors among hypertensive elderly (지역사회 노인 고혈압 환자의 건강정보이해능력이 고혈압 자가관리행위에 미치는 영향)

  • Objectives: This study thus set out to examine the health literacy and self-care behaviors of community-dwelling elderly patients with hypertension and investigate their relations and influential factors, thus providing basic data to develop a nursing intervention program to promote self-care behaviors among hypertension patients. Methods: The subjects include 180 hypertension patients using community health centers, public senior centers, and welfare centers in Jeju. Results: The subjects scored a mean of 49.8 (${\pm}12.4$) points on health literacy and 55.8 (${\pm}6.7$) points on self-care behaviors. In the results of hierarchical regression analysis, health literacy independently predicted self-care behaviors (${\beta}=.12$, p=.050) after controlling for general characteristics, hypertension-related knowledge and self-efficacy. Conclusions: These results suggest that interventions for improving health literacy are important to enhance elder's ability of self-care behaviors. It is necessary to understand the health literacy of elderly patients with hypertension before providing them with educational intervention and information and to develop appropriate educational materials and intervention programs.

The Development and Effects of a Medication Adherence Intervention Program for Hypertensive Patients (범이론 모형을 적용한 고혈압 환자 약물순응 중재 프로그램 개발 및 적용 효과)

  • Purpose: This study was to estimate the effectiveness of an intervention program using the trans-theoretical model on the stages of change, the process of change, decisional balance, self-efficacy, the levels of medication adherence, and blood pressure difference. Methods: A similar experiment was conducted using a nonequivalent control group pre- and post-test design. The study subjects were 108 hypertensive patients. The program was administered to the experimental group while educational materials were given to the control group. Descriptive analysis, 2-test, t-test, and repeated measures ANOVA were used with SPSS 17.0 to analyze collected data. Results: After treatment with this program, the experimental group showed stage of change ($x^2=52.917$, p<.001), cognitive (F=13,528, p<.001), behavioral process of change (F=10.808, p<.001), benefits (F=5.569, p=.012), loss of medication adherence (F=15.661, p<.001), self-efficacy (F=5.407, p=.011), levels of medication adherence (F=51.442, p<.001). Conclusion: There was a significant improvement in the experimental group who participated in the TTM medication adherence intervention program. Given that this intervention program motivated and stimulated hypertensive patients to adhere to the prescribed medications, the TTM medication adherence intervention program is expected to be an effective and practical intervention method for health improvement.

Factors Influencing the Stages of Change in Medication Adherence in Patients with Hypertension (고혈압 환자의 복약순응행위 변화단계에 영향을 미치는 요인)

  • Purpose: The purpose of this study is to identify factors associated with the stages of change in medication adherence in patients with hypertension. Methods: Participants were 323 patients with hypertension. Sociodemographic/medication-related characteristics, stages of change, processes of change, self efficacy and decisional balance were self-administered. Results: Stages of change were significantly different according to gender, age, job and living arrangement. A multinominal logistic regression analysis has revealed that gender, age, living arrangement, self-liberation, and self-efficacy were significantly associated with the precontemplation stage. Age and self-liberation were significantly associated with the contemplation stage. Gender, age, living arrangement, and self-liberation were significantly associated with the preparation stage. Gender and helping relationship were significantly associated with the action stage. This model explained 52.0% of the stages of change in medication adherence. Conclusion: The tailored intervention strategies based on the stages of change may be needed for improving medication adherence in patients with hypertension.

Influencing Factors on Health Related to Quality of Life in Hypertension Patients (고혈압 환자의 건강관련 삶의 질에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify the factors influencing health related to quality of life with hypertension patients. Methods: The subjects of this study were 409 hypertensive patients. Data were collected from March 1st to April 30th, 2008. A questionnaire consisting of exercise barrier, Psychosocial Wellbeing Index (PWI), EuroQol 5D (EQ-5D) was given. The collected data were analyzed with the SPSS program which was used for descriptive statistics, univariate and multivariate analysis. Results: There was a significant relationship between exercise barrier (t=3.57, p=.000), psychosocial wellbeing (F=29.96, p=.000) and health related to quality of life. According to the result of multiple regression, the identified significant factors were an exercise barrier (F=7.09, p=.000) and psychosocial wellbeing (F=21.5, p=.000) with health related to quality of life. Conclusion: Hypertension patients experienced exercise barrier and psychosocial distress which led to a negative effect on health related to quality of life. Therefore it is needed to encourage motivation of lasting of exercise compliance and relieving of psychosocial distress for better health promotion and high quality of life.
