The Effects of Fatigue in the Non-Paretic Plantarflexor Muscle on Spatial and Temporal Gait Parameters during Walking in Patients with Chronic Stroke (만성 편마비 환자의 비마비측 발바닥굽힘근 근피로가 시·공간적 보행변수에 미치는 영향)

  • Purpose: The purpose of this study was to obtain detailed and quantified data concerning the effects of plantarflexor muscle fatigue induced in the non-paretic side on the spatial and temporal gait parameters of the bilateral lower extremities during walking in stroke patients. Methods: This study was conducted on 20 patients with chronic stroke. The load contraction fatigue test was applied to induce muscle fatigue in the non-paretic plantarflexor muscle. Step length, stride length, double support, gait velocity and cadence, and functional ambulatory profile (FAP) score in the bilateral lower extremities were measured using a gait analysis system in order to investigate changes in temporal and spatial gait parameters caused by muscle fatigue on the non-paretic side. The statistical significance of the results was evaluated using a paired t-test. Results: A review of the results for gait parameters revealed a significant increase in double support (p<0.05) and a significant decrease in step length, stride length, gait velocity and cadence, and FAP score (p<0.05). Conclusion: These results indicate that the muscle fatigue in the non-paretic side of the stroke patients also affected the paretic side, which led to a decrease in gait functions. This implies a necessity to perform exercise or training programs in a range of clinical aspects not causing muscle fatigue.

The Effect of Visual Deprivation on Trunk and Lower Extremity Muscle Activity on an Unstable Surface (불안정면에서 시각차단이 체간과 하지의 근활성도에 미치는 영향)

  • Purpose: Visual information is one of the most important factors for postural balance. The purpose of this study was to investigate the effect of visual deprivation on the lower extremities and trunk muscle activity on an unstable surface during quiet standing. Methods: Fifteen healthy males from a university population participated in this study. Surface electromyography of the rectus abdominis, the erector spinalis, the vastus femoris oblique, the semitendinosis, the tibialis anterior, and the medial gastrocnemius was measured using EMG equipment (Telemyo 2400, Noraxon, USA). The participants were asked to maintain postural balance on an unstable surface (Balance pad, Airex, USA) for 30 seconds with eyes open and with eyes closed during quiet standing. The Wilcoxon test was used to compare the muscle activity of the lower extremities and the trunk between open and closed eyes on an unstable surface. Results: The muscle activity of the trunk and the lower extremities was increased more with eyes closed than with eyes open on the unstable surface during quiet standing. Conclusion: The findings from this study suggest that visual information could alter lower extremity and trunk muscle activity. Therefore, visual deprivation can be used as a method to improve postural balance.

A Prospective Investigation into the Effects of Workplace Stress and Working Postures on Work-related Neck Pain in Office Workers (사무직 근로자들에게 나타나는 직업 관련 목 통증에 관한 직장 내 스트레스와 작업자세의 전향적 조사)

  • Purpose: The purpose of this study was to identify the causal relationship between workplace stress and working posture and the development of work-related neck pain in office workers. Methods: The study participants included 62 office workers who had not experienced neck pain in the previous 12 months. A battery of measures to evaluate potential workplace risk factors in an office setting were conducted at baseline, and the 12-month incidence of work-related neck pain was reported via monthly questionnaires. Survival analysis was used to evaluate the longitudinal relationship between the workplace risk factors and the development of work-related neck pain. Results: The incidence of work-related neck pain was 1.91 (95% CI: 1.06-3.45) per 100 person months. The incidence of neck pain was predicted to be less likely to happen when workers had a more upright thorax posture during computer work (hazard ratio, 0.94; 95% CI: 0.89-0.99). However, stress may deteriorate the preventative effects of other risk factors on neck pain and showed a positive relationship with episodes of neck pain (hazard ratio, 1.37; 95% CI: 1.03-1.84). Conclusion: Understanding the psychophysiological effects of neck pain may explain the development of neck pain in office workers. Our interest in prevention plans and treatments should therefore involve a multifactorial pathology of neck pain in the workplace.

The Effects of Temporary Visual Block on the Foot Pressure and Foot Area of Chronic Stroke Patients (일시적인 시각차단이 만성 뇌졸중 환자의 지면 족저압 분포와 족부면적에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effect of temporary visual block existence and nonexistence on the change in foot pressure and foot area in stroke patients. Methods: Sixty-one chronic stroke patients volunteered to participate in the study. Foot pressure and foot area were measured using the Biorescue system. The subject maintain a comfortable standing position on the pressure-measuring plate one meter away from the computer. The sequence of the visual variation data while standing on the measuring plate was collected randomly. The data were collected with three repetitions and used the five-second measuring values except the first second and the last second according to each visual condition. All data were analyzed using SPSS version 21.0. The significance level for the statistical inspection was set as 0.05. Results: The comparison between the visual existence and nonexistence status showed statistically significant effects on foot pressure and foot area. The visual nonexistence status showed more improvement in foot pressure symmetry and area than visual existence status. Conclusion: This study shows that the foot pressure and foot area for the chronic stroke patients changed according to the visual states. It is predicted that these data will be used in rehabilitation training programs and to present temporary changes in visual status for stroke patients.

The Effect of Irradiation During Resistance Exercise Using a Diagonal Pattern on the Excitability of Nerves (대각선 패턴에서 저항운동 시 발생한 방산의 효과가 신경의 흥분성에 미치는 영향)

  • Purpose: The human body can experience a variety of injuries. As a result, it may be difficult to directly treat the damaged area. In such a case, indirect treatment is required. Indirect treatment is typically PNF treatment. Morphological changes in muscle have been confirmed through several previous studies; however, few studies have analyzed neurological changes. Therefore, the purpose of this study was to determine how irradiation during resistance exercise using a diagonal pattern effects neurological excitability. Methods: Electromyography was performed on 13 healthy adults. A compound muscle action potential (CMAP) was obtained through a median motor nerve conduction velocity test, which was conducted before and after performing the irradiation exercise and general exercise. Results: Compared to baseline measurements, there was no significant difference in the latency of the irradiation exercise and general exercise. The amplitude of the CMAP measured after the irradiation exercise was significantly higher than after general exercise. Conclusion: Neurological excitability was high in irradiation during resistance exercise using a diagonal pattern. When clinically direct treatment is difficult, it is thought that irradiation can be used indirectly as a technique to induce nerve excitability.

The Effect of Breathing Training Program and Swiss-ball Exercise on Respiratory Circulation Function and Tidal Volume (호흡운동 프로그램과 스위스 볼 운동이 호흡순환기능과 일회 환기량에 미치는 영향)

  • Purpose: The purpose of this current study is to compare the effectiveness of respiratory circulation function and tidal volume according to two different types of practice methods, in terms of breathing training program and abdominis muscle strengthening using a swiss ball. Methods: The subjects were consist 18 college students, were randomly and evenly assigned to either breathing training program group (BTG) or swiss-ball exercise group (SEG). Exercise program was applied for 60 minutes, 3 times a week, for 6 weeks. Before, after 3 weeks and after 6 weeks of exercises, the subjects were tested using the bruce protocol. The significance of differences between the BTG and the SEG was evaluated by analysis of two-way repeated measures ANOVA. Results: There was an increase in respiratory circulation function after both of exercise. Especially, there was significant difference between the before and after 6 weeks in the BTG. Also, change of respiratory circulation function in BTG was significantly greater than SEG. Tidal volume was no significant differences between the before and after 6 weeks in both groups. In addition, there was significant difference between BTG and SEG. Conclusion: These results suggest that direct breathing training program were more useful to improve of respiratory circulation function.

The Effects of 4 Weeks Training using Virtual Reality Game on Balance and Gait Ability, Depression of Elderly People (가상현실 게임을 이용한 4주간 훈련이 노인의 균형 및 보행 능력과 우울증에 미치는 효과)

  • Purpose : This study investigated the effect of 4 weeks training of virtual reality game on balance and gait ability, depression of elderly people. Methods : The subjects of this study were 12 elderly people over 65 age. The subjects were treated by training using virtual reality gam (during 60 minutes, 3 times a week for 4 weeks). The results of the experiment were analyzed using paired T-test along with averages and standard deviations as the statistical methods for data analyses. Results : In comparison of depression, gait and balance ability between pre-test and post-test, there were statistically significant differences. Conclusion : Our study finding indicate that training using virtual reality game could improve balance and gait ability, depression in elderly people. According to our study, training using virtual reality game is judged to be used for elderly people with gait and balance problems, depression to prevent fall down.

  • 홍수시 하천의 유량측정은 매우 어렵고 위험하며 많은 노력과 비용이 드는 작업이다. 이러한 홍수시 유량측정을 위해 영상을 이용하여 하천의 표면유속을 측정하고 여기서 유량을 산정하는 기술은 하천 유량 측정의 자동화와 안전한 유량 측정을 위한 대안으로 크게 주목받고 있다. 그런데, 유속측정을 위해 20~40초 정도의 영상의 평균유속을 구하고자 하면, 방대한 양의 영상처리에 많은 시간이 소요되어 실시간 측정이나 분석이 어렵게 된다. 본 연구는 영상을 이용하여 홍수시 하천의 유량을 실시간으로 측정하기 위해 투영변환과 시공간영상 분석법을 적용하여 실용적인 표면영상유속계를 개발하기 위한 것이다. 이를 위해, 3차원 투영변환(11변수 변환)을 적용하여 측정선과 유속측정점의 위치를 영상내에 특정하고 이 부분만을 추출하여 시공간 영상(spatio-temporal images)으로 구성하고, 이 시공간 영상을 분석하여 유속과 유량을 산출하는 기법을 개발하였다. 즉, 하천의 주흐름 방향의 유속만을 산정하도록 하여 영상의 분석에 소요되는 계산량과 계산시간을 단축하였다. 또한, 시공간 분석과정도 기존의 CASTI (Correlation Analysis of Spatio-Temporal Images)를 훨씬 간단하고 빠르게 계산할 수 있도록 개량하였다. 그 결과 영상의 유속분석 및 유량산정에 소요되는 시간을 획기적으로 줄일 수 있었으며, 실시간으로 유량 측정이 가능하게 되었다.

Comparison of Muscle Activities between the Diagonal Pattern of Shoulder Exercises Using the %Normalization and %Isolation Method (독립화 비율과 정량화 비율을 사용한 대각선 패턴의 어깨운동 간의 근활성도 비교)

  • Purpose: The present study aims to investigate the effects of a diagonal exercise pattern on selective activation of the upper extremity muscles using both normalization and isolation methods. Methods: In total, 17 asymptomatic subjects participated in this study. During the two diagonal patterns of exercise (diagonals 1 and 2), muscular activities of the upper trapezius (UT), lower trapezius (LT), serratus anterior (SA), anterior deltoid (AD), and infraspinatus (IS) were measured. The collected data were analyzed in two ways, according to the dominance of muscle activities (%Isolation) and according to normalized activities (%MVIC). Results: There were significant differences in LT, SA, AD, and IS between %MVIC and %Isolation (p<0.05), and the diagonal 1 pattern of exercise showed significantly more LT activities compared with the diagonal 2 pattern (p<0.05). Further, except for LT, there were no significant differences in muscle activities between the diagonal 1 and 2 exercises. Conclusion: The present study suggests that a diagonal pattern of exercise is advantageous for strengthening shoulder muscles, but caution is needed when applying to patients requiring selective strengthening. Regarding both the concentric and eccentric phases of exercise, there was no significant difference in muscular activation, except in LT, between the two diagonal patterns of exercises.

A Measurement Technique of Flow Velocities in Small and Medium Sized-Rivers using Drone Images (드론 영상을 이용한 중소하천의 유속 측정)

  • 영상을 이용하여 하천유속과 유량을 측정하는 방법은 신속하고 간결하게 유속을 측정할 수 있는 방법으로 주목을 받고 있다. 또한 새로운 기술인 드론을 활용하여 하천의 유속과 유량을 측정하는 방법도 다양하게 시도되고 있다. 본 연구는 드론을 정지영상과 동영상을 이용하여 별도의 측량이나 복잡한 과정없이도 하천의 유속분포를 추정할 수 있는 방안을 제시하였다. 이 때, 중소하천의 유속 측정에 보다 쉽게 활용할 수 있도록 카메라 보정을 통한 카메라 내부변수 획득과 정지영상에 수록된 드론의 위치와 자세에 대한 EXIF 자료를 이용하는 방안, 약간의 흔들림이 있는 영상에서도 유속을 추정하는 방안을 제안하였다. 먼저, 드론의 카메라 보정을 위한 프로그램을 작성하고, 카메라의 내부 변수를 추정하였다. 드론의 위치와 자세에 대한 정보는 정지영상(JPG 파일)에 수록된 EXIF 정보를 이용하여 드론의 위치(GPS)와 자세(자이로스코프)를 알아내었다. 이를 이용하여 현장의 참조점에 대한 위치 정보를 확인하고, 또 수면촬영을 위한 정지비행시의 카메라의 대략의 위치와 자세 정보를 확인하였다. 이 자료들은 실제 유속측정에 이용하는 동영상에서 나타나는 참조점의 위치 정보를 결정하기 위한 것이다. 그리고 연이어 촬영된 동영상에서 시공간영상분석법으로 측정 단면의 유속분포를 분석하였다. 이 때, 동영상 내에 있는 약간의 흔들림은 FFT 분석으로 적절히 보완할 수 있다. 개발된 방법을 밀양강의 단장천 대리 수위표 지점 인근에서 시험한 결과 기존의 유속계로 측정한 방법과 상당히 근사한 결과를 얻을 수 있었다.
