A Case of Subdural Hematoma after Epidural Blood Patch in a Spontaneous Intracranial Hypotensive Patient - A case report - (자발성 두개강내 저혈압성 두통 환자에서 치료 도중 발생한 경막하혈종 - 증례보고 -)

  • Spontaneous intracranial hypotension (SIH) is believed to be a benign disease. However, numerous studies have reported serious complications related to SIH, including subdural hematoma. In this case report, a 54-year-old male patient visited the emergency room with orthostatic headache. A brain magnetic resonance imaging (MRI) study showed diffuse mild thickening and enhancement of pachymeninges, with a suspicious minimal amount of subdural fluid collected in the left posterior parietal area. His orthostatic headache showed no improvement with conservative treatment; but his pain was almost completely relieved after two trials of cervical epidural blood patch. On the 74th day after the onset of his pain, the patient showed a drowsy mental status and slurred speech when he visited the pain clinic. Brain computerized tomography indicated a left subdural hemorrhage, and he underwent emergency operation to drain the SDH. In conclusion, pain clinicians should pay attention to abrupt changes in mental status as well as continuous headache, for the early diagnosis of SDH in SIH patients.

  • This study aimed to investigate the effect of Hwanglyeonhaedok-tang on the clearance of vancomycin-resistant Enterococcus (VRE) colonization in a patient with traumatic subarachnoid hemorrhage and subdural hematoma. A 73-year-old man who was diagnosed with traumatic subarachnoid hemorrhage and subdural hematoma with isolation of VRE colonization was administered Hwanglyeonhaedok-tang and followed up with a rectal swab VRE culture every week. After treatment with Hwanglyeonhaedok-tang, VRE colonization was no longer detected. This case report proved that Hwanglyeonhaedok-tang could be an effective treatment for clearance of VRE colonization in a patient with traumatic subarachnoid hemorrhage and subdural hematoma.

  • Dural arteriovenous malformations(AVM) are not uncommon. Reports of intracranial dural AVM have been increasing but most of them deal with dural AVM in the region of the cavernous sinus, posterior fossa and tentorium, but those of the anterior cranial fossa are very rare. Recently, we experienced two cases of right frontal dural arteriovenous malformation fed mainly by both ethmoidal arteries. The angiographic appearance in these two cases is quite uniform. The nidus was located in the frontal dura, although their main feeders were dural arteries. They were drained through an intracerebral cortical vein associated with aneurysmal dilatation of proximal portion into superior sagittal sinus. Spontaneous intracerebral hematoma was the cause of the clinical symptoms. We report two cases of intracerebral hematoma, caused by dural AVM, which was successfully managed by surgical treatment.

Complex Korean Medical Treatment after Embolization for Myelopathy Due to Spinal Dural Arteriovenous Fistula: A Case Report (척수경막동정맥루로 인한 척수병증 환자의 색전술 이후 한의복합치료: 증례 보고)

  • A 38-year-old Korean man without any other history was diagnosed with myelopathy due to a spinal dural arteriovenous fistula. Following embolization and high-dose steroid therapy, the patient was treated with complex Korean medical therapies including acupuncture, pharmacopuncture, electro-acupuncture, Chuna manual therapy, herbal medicine, and cupping therapy in addition to conventional treatment. To assess the patient's improvement, international standard for neurological classification of spinal cord injury, Korean version of modified Barthel index, functional independence measure, spinal cord independence measure III, walking index for spinal cord injury II, modified Ashworth scale were used. After treatment, the muscle strength and sensory function of the lower extremities were improved, and the spasticity was reduced, resulting in a rapid improvement in performance of daily activities. These results suggest that complex Korean medical therapies may be effective for myelopathy, and further clinical studies are needed to clarify their effects.

  • The purpose of this report is to investigate the clinical application of Korean Traditional treatment to abdominal pain patient after subdural hemorrhage occurred. The main herb medicine used in this study is Ondam-tang (wenda-tang) which is frequently used in psychiatric disorders. Also, acupuncture, moxibustion and physical rehabilitation treatment were practiced. After taking an Ondam-tang (wendan-tang), the clinical symptoms of the patient were improved.

  • Immunoglobulin G4 (IgG4) related disease (IgG4-RD) is currently considered an immune-mediated condition and is recognized as a disorder group with common pathological, hematological, and clinical characteristics. This disease may involve diverse organs of the head and neck, and include mainly the lacrimal gland, orbit, thyroid gland, pituitary gland, and the meninges. Here, we report a case of IgG4-RD in a 65-year-old female showing head- and neck-limited but synchronously and mainly manifesting as otalgia and facial neuritis.

Effect of Post Operative Daily Activity on Post-dural Puncture Headache after Spinal Anesthesia (척추마취수술 환자의 수술 후 일상활동이 경막천자 후 두통 발생에 미치는 효과)

  • Purpose: The purpose of this study was to compare and analyze differences in effects of postoperative daily activity on headaches in patients who underwent surgery under spinal anesthesia. Methods: The study was conducted with 219 adults, 20 years or older. Official approval (Approval number: KMC IRB 1434-01) was received from K university hospital clinical trials review board. The research design was a nonequivalent control group non-synchronized design with a daily activities group and the bed rest group. Data was collected after receiving written consent from the participants. Results: There were no participants in either group who experienced headaches. Changes in a physiological index were also not significantly different between the daily activities group and the bed rest group. Conclusion: The results indicate that allowing daily activities in the ward, rather than maintaining bed rest for 6 hours, the existing method of nursing care for the prevention of postoperative headaches, in spinal anesthesia patients, is not detrimental to these patients post operatively.

  • The characteristics of mass transfer in an inorganic ion exchange system where Cs and/or Sr are separated by means of zeolites was investigated. Experimental work to separate Cs or Sr was carried out for Cs-AW300 and CS-AW500 systems in case of Cs whereas for Sr-4A and Sr-13X systems in case of Sr. The experimental conditions were chosen as follows in the batch type separation : temperature $25^{\circ}C$, agitation speed 300rpm, amount of zeolite 4g, volume of solution $0.5{\ell}$, and concentrations of solution 1000ppm, 2000ppm, respectively. As a result, it was found that the mass transfer rate is controlled mainly by the liquid film diffusion. The mass transfer coefficients in the film were found to be in the range of $10^{-4}{\sim}10^{-3}cm/sec$, while the apparent diffusivity inside the particles was found to be in the order of $10^{-8}cm^2/sec$.

  • In the patient who has intradural mass associated with spinal stenosis, if the operation for spinal stenosis is performed alone, the symptom may remain. We report with literature review that we achieved the successful outcome after simultaneous decompression of spinal stenosis and space occupying mass removal in the case of intradural and extradural compression. A 71-year-old female patient suffering from low back pain and radiating pain of both lower extremities admitted. In magnetic resonance imaging, spinal stenosis on L4-5 and spondylolisthesis on L5-S1 compressed dural sac and intradural space occupying mass on L4 level compressed. By posterior approach, decompression and interbody fusion were carried out. Then mass was removed with median durotomy. Pathologic diagnosis was schwannoma and the symptom was improved remarkably.

Epidural Space Identification Device Using Air-filled Catheter (공기도관을 사용하는 경막외강 자동탐지기구)

  • An assist device was developed to identify the epidural space by continously monitoring the air-filled catheter pressure. The pressure signal appropriately amplified and filtered enabled to alarm the needle introduction into the epidural space by thresholding detection. Ten LEDs provided a visual change of catheter pressure before alarming for user convenience. Clinical trials were performed in 30 patients with 83% success rate at the first trial. When failed, the second trial was enough for successful anesthesia. The air volume introduced during each anesthesia was less than 1ml, causing side effects. Air filling of the catheter could also minimize infection possibility. Therefore, the present device guarantees safe anesthesia with user convenience.
