Testing for Self-care Ability Measurement in an Aged Health Center - A Delphi Technique - (일개 노인보건센터에서의 자가관리능력 측정도구 검증 - 델파이기법을 중심으로 -)

  • Purpose: To find out whether the self-care ability measurement used for visiting health care provide an appropriate standard to select service methods using a Delphi survey and to offer basic data that can improve the measurement. Method: A Delphi survey was done with 39 people consisting of theoretical professionals, field managers, and practitioners engaged in visiting health care. Result: It was found that items and weighted points in the currently used self-care ability measurement needed adjustment. Therefore, a modified evaluation standard was presented by domains, items and detailed items. Finally, the self-care ability measurement is composed of 4 domains, 11 items and 19 detailed items. Conclusion: This study is significant in that it presented an evaluation standard applicable directly to business practice by supplementing problems in the existing self-care ability measurement. This study suggests the need to develop various measurements for elderly households based on community.

The Effect of Brain Death Organ Donation Education on Nurses' Knowledge and Attitude (뇌사 장기기증 교육이 간호사의 뇌사 장기기증에 대한 지식과 태도에 미치는 효과)

  • Purpose: This study was conducted to examine the effects of Brain Death Organ Donation Education on nurses' knowledge and attitudes. Methods: It is a quasi-experimental study using the non-equivalent control group. A total of 100 nurses participated in the study; 50 in the experimental group, and the rest in control group. Collected data were analyzed using ${\chi}^2-test$, t-test and ANCOVA by SPSS 12.0 program Results: There was a statistically significant difference in knowledge (F=13.29, p<.001), and attitude (F=4.35, p=.040) after the experimental group received Brain Death Organ Donation Education. Conclusion: The Brain Death Organ Donation Education was revealed as an effective tool in improving nurses' knowledge and attitudes toward organ donation.

The Role of Clinical Research Nurses at Regional Clinical Trials Centers (지역임상시험센터 임상연구간호사의 역할 구명)

  • Purpose: This study aimed to evaluate the performance of Clinical Research Nurses (CRNs) and the importance of their roles at the Regional Clinical Trial Centers (RCTCs). Method: A questionnaire focused on the role of CRNs was crafted by a researcher and the content validity was verified by a panel of experts on clinical research. The subjects of this study were 91 CRNs and Clinical Research Coordinators (CRCs), who were Korean registered nurses working at nine RCTCs. 77 subjects yielded valid data were analyzed using descriptive analysis, the Mann-Whitney U test, Spearman's rank order correlation coefficient, and Kruskal-Wallis test. Results: The performance of CRNs and the recognition in the importance of their roles were statistically significant different in age, education, CRN careers, positions, employment status and the phase of clinical trial. The role of direct caregiver was performed most often by CRNs. The role of coordinator of care and research (pre-study) was considered the most important role but performed the least frequent. Conclusions: The role of CRNs can easily be differentiated from CRCs who are not registered nurses. The domains of CRNs should be clearly identified and established. Moreover, research should be carried out on CRN training programs to cultivate competence in CRNs.

A Model for Community Based Mother Infant Care Center - TMIC(transitional mother infant care center) using a Sanhujoriwon - (산후조리원의 모자건강관리 현황과 제도화방안 - 지역사회중심의 모자건강관리센터(TMIC) 개발을 위한 전략 -)

  • The purpose of this study was 1) analyze the current state of Sanhojoriwon; and 2) to suggest the new model for the community based mother infants health care delivery system: strategies of TMIC are related to Public Health policy, cost-effectiveness, mother infant care provision of medical professionalism, and so on. Method: Forty-seven workers from seventeen Sanhojoriwon participated to analyze several aspects of Sanhojoriwon. Using a questionnaire developed at Korean Sanhojori Research Forum (KSARF), such as the traditional and medical concept of the Sanhojori, postpartum care, Korean traditional postpartum care, job description on women and infant care at Sanhojoriwon, professional management, health care policy and the educational need. Results: Based on the descriptive study results, the TMIC, the community based transitional mother infants care center was suggested as a new model for the cyclic public health care system related on the reproductive health, using an already existing related center, Sanhojoriwon. Also, several strategies were presented on the TMIC.

The Impact of Patient Satisfaction With Nursing Care Services, Switching Costs and Perceived Risk on Intention of Reuse in the Emergency Medical Center (응급의료센터 내원환자의 간호서비스 만족도, 전환비용 및 지각된 위험이 재이용 의도에 미치는 영향)

  • Purpose: The purpose of this study was to identify the factors influencing the intention of the reuse in patients admitted in university hospital emergency medical center. Method: The participants were 253 patients admitted to a niversity hospital emergency medical center. Data were collected with self-administrated questionnaires and analyzed by hierarchical multiple regression. Results: Patient satisfaction with nursing care service and switching cost were positively correlated with reuse by patients while the perceived risk was negatively correlated. As levels of satisfaction with nursing care services and switching cost increase, intention of reuse increases. Satisfaction with nursing care service, switching cost and perceived risk in emergency medical center influence intention to reuse and explain 68.8% of total variation of intention to reuse. Conclusion: Findings provide strong empirical evidence for importance of atient satisfaction with nursing care service, the switching costs and the perceived risk in explaining the intention of reuse an emergency medical center.

The Effects of Health Counseling Programs by Occupational Health Nurses on the Unemployed Visiting the Employment Welfare Plus Center (고용복지플러스센터 이용 구직자를 대상으로 산업간호사가 제공한 건강상담 프로그램의 효과)

  • Purpose: The present study was conducted to examine the effects of a health counseling program by occupational health nurses on the unemployed visiting the Employment Welfare Plus Center. Methods: The one-group pretest-posttest design was used. The study presented a secondary analysis of the data collected in a health counseling program run by occupational health nurses for the unemployed visiting the Employment Welfare Plus Center. Kolmogorov-Smirnov, Shapiro-Wilk test, ${\chi}^2$ test and Wilcoxon signed rank test were used to analyze 158 data collected between October 5, 2015 and January 5, 2016. Results: After the intervention, the participants showed a significant decrease in the level of stress index (uBioMacpa), stress (discomfort), musculoskeletal pain and depression. In addition, the participants showed a significant increase in self-rated general health, exercise and regular eating behavior. Conclusion: The health counseling program offered by occupational health nurses is an effective intervention to improve the physical and mental health of the unemployed individuals visiting the Employment Welfare Plus Center.

Determining Nursing Care Priorities among Women in the High Risk of Coronary Heart Disease (일 건강검진센터에 내원한 여성을 대상으로 심혈관질환 위험요인을 활용한 고위험군 간호의 우선순위 설정)

  • Purpose: This study was designed as a epidemiologic study for determining priorities of nursing care in women with high risk groups of coronary heart disease(CHD) using risk factors. Method: Subjects were 1015 women who received health screenings at a hospital in Kyunggi, Korea, over one year period from January to December 2006. Results: The 37.9% of women had multiple risk factors for developing coronary heart disease. The most significant risk factors on CHD were the age older than 70 years (OR=11.45), the age between 60-69 (OR=4.65), the age between 50-59 (OR=3.20), having $HbA_1C$ over 7% (OR=2.38), high risk groups of L/H ratio (OR=1.98), and the waist to stature ratio (WSR) over 0.5 (OR=1.33). Conclusion: The findings suggest that women older than 50 years should be considered as an overt target population for CHD prevention, even in the absence of other risk factors. Also, $HbA_1C$ and WSR can be efficient indicators for CHD screening.

Structural Equation Model for the Analysis of Alcohol-related Problem of Alcohol Use Disorders (알코올사용장애자의 음주관련 문제 분석을 위한 구조모형)

  • Objectives: This study was designed to construct and test the structural equation model for the alcohol-related problem of alcohol use disorders. Methods: Data were collected by structured self-questionnaires from 229 male subjects who received > 8 (greater than 8) score on Alcohol Use Disorder Identification Test (AUDIT). The Data were analyzed by SPSS 21.0 and AMOS 21.0. Results: The model fit indices for the modified hypothetical model showed Q = 2.50, GFI = 0.90, and CFI = 0.94. As a result, Life position, parent's drinking problem, and alcohol expectancy had significantly direct effect on alcohol-related problem. Alcohol expectancy also had mediator effect between life position and alcohol-related problem. Conclusions: Consequently, the more positive life position, the less alcohol-related problem occurred. It is necessary to change their life position, which is individual factor, to prevent or reduce the alcohol related problem of alcohol use disorders.

The Development and Evaluation of the Elbow Restraint on Patients in Intensive Care Unit (중환자실 환자의 팔꿈치 억제대 개발 및 적용 평가)

  • Purpose: This paper was to develop an elbow restraint which can reduce the side effects of the wrist restraint which was frequently used in an intensive care unit and to evaluate its application. Methods: A nonequivalent control group non-synchronized design was used. Subjects were 38 patients (elbow restraint group: 18, wrist restraint group: 20) and 20 nurses who used both restraints in medical intensive care unit at G National University Hospital. Restraints were applied to subjects for 72 hours. Data were collected from December 1, 2009 through March 31, 2010. The data collected were analyzed using $x^2-test$, Fisher's exact test, t-test, and ANCOVA using SPSS 12.0 program. Results: Elbow restraint significantly decreased range of wrist motion reduction, swelling and significantly increased the convenience of wearing restraint compared to wrist restraint. Conclusion: The new elbow restraint are more efficient than the old wrist restraint, which have been used in an intensive care unit, in terms of convenience of restraint application, range of joint motion and prevention of swelling.

The Effects of a Support Program for Family Caregivers of Elderly with Dementia on Empowerment and Attitudes toward Dementia (치매노인 가족 지지프로그램이 치매가족의 임파워먼트와 치매태도에 미치는 효과)

  • Purpose: This research was conducted to identify the effects of providing support programs to families of elderly with dementia on family empowerment and attitudes toward dementia. Methods: A nonequivalent control group pre- and post-test design was used. Participants were 49 family caregivers, experimental (24) and control (25), recruited from families through a dementia support center in Y district. This program implemented Haearim, a support program for family caregivers of demented elders developed by the National Institute of Dementia in 2016. Outcome measures were empowerment and attitudes toward to dementia. Data were analyzed with a ${\chi}^2-test$, independent t-test, Fisher's exact probability test, paired t-test, and repeated measures ANOVA with SPSS/PC version 20.0. Results: Family empowerment (F=6.84, p=.002) and family caregivers' attitudes (F=16.48, p<.001) toward dementia in the experimental group improved significantly more than that of the control group after intervention. Conclusion: Results indicate that support programs for families of elderly with dementia are effective in positively changing empowerment and attitudes toward dementia among family caregivers.
