Purpose: Health-related quality of life (HRQOL) in patients with cardiovascular disease in Korea has rarely been studied, mostly due to the lack of a psychometrically validated disease-specific instrument. The purpose of this study was to develop and validate a cardiovascular specific-HRQOL questionnaire (CD-QOL). Method: The CD-QOL was developed and validated as follows; item generation, pilot study, and psychometric tests. Patients were recruited from three-university hospitals. The patients were asked to complete the preliminary questionnaire comprising the content-validated items, SF-36, and CES-D. The NYHA and KASI classifications were used to classify the functional performance of the patients. The data was analyzed using correlation, factor analysis, multidimensional scaling, multitrait/multi-item matrix, ANOVA, and Cronbach's alpha. Result: Preliminarily, thirty-nine items were generated. Factor analysisextracted a five-factor solution with a total of twenty-two items. One item was deleted based upon the MDS. The remaining items were moderately correlated with the subscales of the SF-36 and associated with depression measured with the CES-D. The mean scores of patients in NYHA and KASI class I were significantly higher than those in NYHA and KASI class II or/and III, which suggested patients with better functional performance were likely to have a better HRQOL. Cronbach's alphas of the total and subscales were all greater than 0.70. Conclusion: The CD-QOL is a easily applicable instrument with excellent psychometric properties of content, criterion, factorial, convergent, and known-groups validity, and internal consistency reliability in Korean patients with cardiovascular disease.
Purpose: This study was conducted to examine the effects of individual counseling for bus-drivers on cerebrovascular and cardiovascular disease prevention. Methods: The one-group pretest-posttest design was used. This study presents a secondary analysis of data collected in 'contents of cerebrovascular and cardiovascular disease prevention program for bus-drivers in one workplace in Seoul, Korea. The data were collected from 56 bus drivers, using questionnaires from September 10th to November 20th, 2014. The analysis was conducted with ${\chi}^2$ test and paired samples t-test using SPSS/Win 21.0. Results: After the intervention, the participants showed a significant decrease in the level of systolic blood pressure (p=.003) and a significant increase in the total cholesterol level (p=.030). The distribution of cardiovascular risk groups changed after the intervention: 5.3% decreased in the high risk group, 16.1% decreased in the medium risk group, and 3.5% decreased in the low risk group, while 25.0% increased in the normal group. Conclusion: The comprehensive individual counseling including simple screening, tailored education and counseling is effective to manage their lifestyle risk factors, resulting in better maintenance of their health as well as preventing cerebrovascular and cardiovascular disease.
Purpose: The purpose of this study was to identify the link between health-related physical fitness level and cardiovascular disease-related risk factors in adult male workers. Methods: We tested cardiovascular disease-related risk factors (waist circumference, SBP, DPB, fasting glucose, TC, HDL-C, TG, LDL-C) and health-related physical fitness ($VO_2max$, grip, Sit-up, Flexibility, Body fat) and divided health-related physical fitness level of the subjects into 3 groups - A (very good, n=56), B (good, n=59), and C (below-average, n=57) according to the criterion of the Health and Fitness counseling guidelines of KOSHA. The statistical techniques such as standard deviation, one-way ANOVA and multiple regression (p<.05) were used. Results: There were significant differences between group C and group B & A (p<.001) in waist circumference, DBP, Fasting glucose, HDL-C, TG, LDL-C. In TC/HDL-C, TG/HDL-C, LDL-C/HDL-C, Group C was higher than group B and A. Conclusion: On the basis of these results, we identified that improvement of health-related physical fitness level positively effects on the decrease of cardiovascular disease-related risk factors.
Purpose: This study was designed to examine the relation of trait anger and anger expression to blood pressure, cholesterol, and depression in middle-aged Korean women. Methods: This descriptive correlational design was conducted using a convenient sample taken from the health center of K University Hospital located in Kyungki province, Korea. The subjects were 252 women aged 40 to 64 years. Spielberger's state trait anger expression inventory - Korean version and Beck's depression inventory were used for measuring trait anger, state anger, anger expression and depression. Data was analyzed by descriptive statistics, t-test, Pearson correlation, two-way ANOVA, and cluster analysis using a pc-SAS program. Results: The anger expression types by cluster analysis were Anger out/in type, Low anger expression type, and Anger control type. The level of cholesterol and depression were significantly higher in women with high anger in and high trait anger. In addition, the level of depression was significantly higher in women with a high anger temperament. Conclusions: Trait anger and anger in might be related to cholesterol and depression in women. However, this study does not reveal the relation between blood pressure and trait anger and anger expression.
Purpose: Natural menopause resulting in the decline in endogenous estrogen concentrations is responsible for an increased risk of coronary heart disease in postmenopausal women. The purpose of the study was to examine the effects of a 6-month Tai Chi exercise program on cardiovascular risk factors and quality of life in post-menopausal women. Methods: A quasi-experimental design with pretest and posttest measures was used. The participants in the study, 29 women in the Tai Chi group and 31 in the control group, were enrolled for 6 months. Results: After 6 months of Tai Chi exercise, total cholesterol (M=213 to 185), LDL-cholesterol (M=135 to 128), and their 10 yr cardiovascular disease risk (M=2.62 to 2.27) had improved significantly for the Tai Chi participants compared to the control group. Total scores for quality of life along with the sub-dimensions of health perception and mental functioning were also significantly higher in the Tai Chi participants. Conclusion: Tai Chi exercise favorably affected cardiovascular health and quality of life in post-menopausal women after 6 months. Additional rigorous studies are needed to examine long term effects on the prevention of cardiovascular disease in this population.
Purpose: The purpose of this study was to analyze effects of cardiovascular disease prevention in small scale enterprise using community-based approach. Method: The target work site included 4,050 small scale enterprises which were under 50 in non-manufacturing workplace and total 20,986 employees were enrolled. Data collection was conducted from March to December, 2008 by 46 occupational health nurses who were registered in Korean Association of Occupational Health Nurses. Results: The rate of smoking was decreased from 39.9% to 34.6%, and normal BMI group was increased from 61.6% to 64.0%. Evaluation of post-intervention blood pressure(BP) outcomes revealed that significant reductions in systolic BP and diastolic BP(3.25mmHg and 1.89mmHg, respectively) were achieved. In addition, the amount of reduction in total cholesterol level was $180.89{\pm}28.22mg/dL$ at pre-ntervention and $177.71{\pm}24.73mg/dL$ at post-intervention. Conclusion: Cardiovascular prevention program by community-based approach was an effective strategy for quitting smoking and improving BMI, BP, and cholesterol control.
Purpose: The purpose of this study was to evaluate the appropriateness of the replacement time intervals of 18 gauge peripheral intravenous catheters (PICs) by investigating the development of phlebitis. Methods: The subjects were 200 hospitalized patients over 18 yrs old aged who have 18 gauge PICs placed for surgery. After the insertion of PICs, the researcher monitored the insertion site daily for 96 hours for any signs of phlebitis. Results: Phlebitis developed in 25.7% of patients. Patients who developed phlebitis were significantly older and were receiving fluids with faster infusion rate. However, patients with and without phlebitis were not different by gender, insertion site, fluid osmolality, or pH of drugs administered. The incidence rate of phlebitis was higher than 10%(12.9%) starting 24~48 hours after the insertion of 18 gauge PICs. Conclusion: It is recommended to replace 18 gauge PICs within 24~48 hours after insertion. Close monitoring of the PICs insertion site for the signs of phlebitis is recommended.
본 연구는 간호대학생과 일반대학생의 생명의료윤리의식에 영향을 미치는 요인을 탐색하기 위한 서술적 비교조사연구이다. 본 연구를 위해 S시의 일개 종합대학의 간호대학생과 일반대학생 총 382명을 대상으로 구조화된 설문지로 자료를 수집하였다. 연구결과 생명의료윤리의식의 정도는 평균평점 4점 만점에 간호대학생 3.04점, 일반대학생 3.12점으로, 두 군 모두 생명의료윤리의식 정도가 보통 이상으로 나타났으며, 일반대학생이 간호대학생보다 생명의료윤리의식이 유의하게 높았다. 생명의료윤리의식에 영향을 미치는 요인으로 간호대학생은 생명존중의식과 지각된 윤리가치관이 높을수록, 일반대학생은 생명존중의식이 높을수록, 성태도가 보수적일수록, 자녀 중심적 의식이 높을수록 생명의료윤리의식이 높은 것으로 나타났다. 또한 간호대학생과 일반대학생 두 군 모두 생명의료윤리의식에 영향을 미치는 가장 강력한 예측 변수는 생명존중의식으로 나타났다. 따라서 미래 의료인이 될 간호대학생의 생명의료윤리의식을 높이기 위해 교과 과정에 윤리교과목의 학점 비중을 높이고 학점과 병행한 다양한 매체 활용 및 특강 등을 통해 윤리교육의 양적, 질적 수준이 제고되어야 할 것이다.
본 연구는 간호대학생들의 신종 코로나바이러스감염증-19의 지식 및 위험지각과 예방적 건강행위의 관련성을 파악하기 위한 서술적 조사연구이다. A시 S대학교에 재학 중인 간호대학생 291명을 대상으로 구조화된 설문지로 2020년 8월 1일에서 8월 20일까지 설문조사하였고, SPSS WIN 18.0을 이용하여 Independent t-test, One-way ANOVA, Pearson correlation coefficients, Stepwise multiple regression으로 분석하였다. 연구결과, 대상자 나이, 주관적 성적수준에 따라 지식 차이가 나타났고, 학년에 따라 위험지각 차이가 나타났으며, 나이, 주관적 건강수준에 따라 예방적 건강행위 차이가 나타났다. 예방적 건강행위는 지식, 위험지각과 순상관관계를 보였다. 회귀분석 결과, 예방적 건강행위에 미치는 요인은 나이(β=.188, p<.01), 주관적 건강수준(β=-.245, p<.001), 지식(β=.254, p<.001), 위험지각(β=.191, p<.001), 총 설명력은 18.9%(F=17.904, p<.001)로 나타났다. 본 연구를 통해 간호대학생의 신종 코로나바이러스감염증-19의 예방적 건강행위를 증진시키기 위한 교육 프로그램에서는 나이, 주관적 건강수준, 지식, 위험지각에 대한 고려가 필요할 것으로 사료된다. 추후 간호대학생의 코로나바이러스감염증-19의 예방적 건강행위에 대한 이론적 모형을 검증하고 영향요인을 확인하는 연구가 필요하다.
Purpose: The purpose of this study was to explore the levels of depression experienced by clients (N=152) with Type 2 Diabetes Mellitus(DM), and to compare the levels of self care activity, metabolic control and cardiovascular risk factors between depressed and non-depressed clients. Method: Participants aged 50 and above were conveniently recruited in B city. The levels of depression, self-care activity, metabolic control of glucose and lipids, and cardiovascular risk factors of the participants were measured by using questionnaires and blood tests from November, 2003 to June, 2004. Data was analyzed with descriptive statistics, Pearson correlation, Spearman rho and t-test using the SPSS WIN 10.0 program. Result: The prevalence of depression(CES-D=16) among the participants was $44.1\%$. The levels of self-care activities(p=.012), glucose(p=.019), total cholesterol(p=.022), LDL(.007) and cardiovascular risk factors(p=.012) were significantly higher in the depressed group than those in the non-depressed group. Conclusion: Based on the findings, we concluded that many DM patients experience depression and the depression of type 2 DM clients is significantly related with self care activities, diabetic control, and cardiovascular complications. However, this study did not address causality among these variables. Therefore, further research, such as a longitudinal cohort study, is needed to identify causality among these variables.
