치매노인의 기능손상으로 인한 가족간호자의 간호부담과 삶의 질에 관한 조사연구

  • 노인인구의 증가에 따라 노인성 질환이 증가하고, 대표적 노인성 질환직 하나인 치매도 점점 증가하고 있다. 이제까지 치매가 노화의 한 정상적인 과점으로 여겨져왔고 최근에 와서야 하나의 질병으로 인식하기 시작하였기 때문에, 그간 치매노인의 의료복지적 측면의 관리가 소홀하였고 가족의 어려움이 켰었다. 본 연구는 치매노인 시설을 이용하고 있는 치매노인 가족간호자를 대상으로 치매노인의 일상생활 기능손상정도 및 건강상태, 가족간호자의 간호부담정도, 사회적지지, 치매노인과 가족간호자의 삶의 질 정도, 그리고 그들이 어떤 사희복지적지지 서비스를 이용하려 하는 지를 파악하여, 앞으로의 치매노인과 그 가족간호자의 부함을 경감하고 삶의 질을 개선할 수 있는 대안마련의 기초자료를 제공할 목적으로 수행되었다.

Differences in Patients' and Family Caregivers' Ratings of Cancer Pain (암환자와 그 가족간호자가 지각하는 환자의 통증강도 차이)

  • Purpose: Undertreatment of canter pain, especially due to the differences in the perception of pain between the patients and caregivers, is a well recognized problem. The purpose of this study were to determine if there exist differences in communication about pain intensity scores between patients and their family caregivers in Korea. Methods: A total of 127 patient-family caregiver dyads who have experienced canter pain participated in this study at a hospital in Seoul for six months. The data were obtained by fare to face interview with a structured questionnaire based on Brief Pain Inventory-Korean version and other previous researches. The clinical information for all patients was compiled by reviewing their medical records. Results: Patients' 'worst-pain for 24-hour' and 'right-now-pain' scores estimated by family caregivers were significantly higher than those by patient themselves. The degree of agreement between patients and family caregivers in the estimate of patients' 'worst-pain for 24-hour' intensity categories was 78.7% for 'severe pain', 40% for 'no pain', 27.5% for 'mild pain' and 22.9% for 'moderate pain'. In case of 'right-now-pain' intensity categories, the agreement was 50% for 'severe pain', 47.2% for mild pain, 46.3% for 'no pain', and 26.3% for 'moderate pain'. Conclusion: This study demonstrates that the degree of agreement between patients and family caregivers in the estimate of patients 'pain intensity categories was less than 50% except for 'severe pain'. The results indicate that Korean family caregivers tend to overestimate the canter pain intensity of their caring patients, especially, when a lancer patient has 'moderate' or 'mild pain'. Health Providers are advised to educate patient-family caregiver dyads to use a pain measurement scale to promote their agreement in pain Intensity stores. Further analyses and studies are needed to identify the factors and differences that influence their communication about pain intensity scores between patients and their family caregivers.

Relationship of Nurse Practice Environment and Work-Family Conflict to Job Satisfaction in Hospital Nurses in Korea (병원간호사의 간호실무환경, 업무-가족 갈등과 직무만족도와의 관계)

  • 연구목적: 본 연구는 병원간호사에게 보다 편안하고 건강한 업무환경을 제공하는 것의 중요성을 병원관리행정가, 교육가, 간호관리자에게 알려주는데 필요한 기초 자료를 제공하고자 수행하였다. 연구방법: 연구진행에 동의한 5개의 종합병원을 선정하여 구조화된 설문지를 이용하여 2010년 7월에 자료를 수집하였다. 연구결과: 직무만족도에 가장 큰 영향을 미치는 요인으로는 간호실무환경으로 긍정적인 영향을 미치는 것으로 나타났으며, 그 다음은 업무-가족 갈등으로 부정적 영향을 미치는 것으로 나타났다. 설명변수는 직무만족도를 39% 설명하는 것으로 나타났다. 결론: 본 연구의 결과 간호실무환경과 업무-가족 갈등이 간호조직에서의 성과인 직무만족도에 중요한 요인으로 나타났다. 따라서 업무환경을 더 매력적으로 만들고, 업무-가족 갈등을 감소시키는 전략은 경력간호사의 보유와 질적 간호를 향상시키는데 도움이 될 것이다.

Nurses' Confidence in Family Nursing and Their Needs for Family Nursing Education: Focusing on the Family of Dying Patients (간호사의 가족간호 자신감과 가족간호 교육요구도: 임종기 환자 가족을 중심으로)

  • Purpose: This study aimed to identify the confidence and educational needs of clinical nurses in care for the family of dying patients. Methods: The subjects of this study were 218 clinical nurses working at two tertiary general hospitals located in D city, Korea, and the data were collected through online questionnaires. The collected data were analyzed using descriptive statistics, t-test, analysis of variance (ANOVA), Scheffe test and 𝝌2 test using the SPSS WIN 20.0 program. Results: 97.6% of clinical nurses recognized the need for family care for dying patients, but 76.7% had never received any education on family care for dying patients. The average score of dying patients' confidence in family care was 3.09. About 90% of clinical nurses were willing to participate in family nursing education for dying patients. The group with more than 5 years of clinical experience was significantly higher than the group with less than 5 years of clinical experience. Conclusion: This study recognized the necessity of family nursing for dying patients highly. It is necessary to develop and apply an educational program based on the education topic that recognizes the need highly.

Factors related to Family Caregiver Satisfaction with Elderly in Nursing Facilities (시설입소 노인환자 부양가족의 만족도 영향요인)

  • This study aims at providing an upgraded suggestion to improve satisfaction for the family caregiver. This study was conducted to find the relationship between satisfaction and family support and outcome indicator to investigate the factors that have influence on the satisfaction. The study population was 126 families of 5 elderly residing in institutionalized elderly homes in seoul and kyunggi. The data collection was measured satisfaction, family support, and outcome indicator instrument. The satisfaction showed meaning relationship with outcome indicator and family support. Explainary power of independent variables of product index was 23%. Factors influencing satisfaction for the family was economic level, family support and outcome indicator. This study was done to give suggestions to improve family caregiver satisfaction and to serve as a basis for policy strategies by examining the current conditions of the nursing facilities.
