Survey of Unexpected Antibodies Identified at a General Hospital in Jeju (제주지역 일개 종합병원에서 동정된 비예기항체의 실태조사)

  • Unexpected antibody screening and identification tests are highly important in the prevention of hemolytic transfusion reactions. Therefore, it is highly recommended to perform unexpected antibody screening test in all transfusion candidates. Here, the frequency and distribution of unexpected antibodies identified in Jeju for the past 3 years were evaluated. Between Jan 2014 and Dec 2016, unexpected antibody screening test was performed for 10,360 sera of transfusion candidates in Jeju general hospital using a column agglutination method with the Ortho BioVue system (Ortho-clinical Diagnostics, Raritan, NJ, USA). Eighty-seven (0.84%) of 10,360 cases that underwent unexpected antibiotics screening showed positive results. Among them, unexpected antibodies were identified in 41 cases (0.40%). Unidentified antibodies were detected in 8 cases (19.51%) and autoantibodies were detected in 3 cases (7.32%). The anti-E antibody included in warm antibodies were detected most frequently in 8 cases (19.51%); 6 cases (14.63%) of anti-E + anti-c antibody and 3 cases (7.32%) of $anti-Le^a+anti-Le^b$. $Anti-Le^a$ and $anti-Le^b$ antibodies were detected in 2 cases (4.88%), respectively. The anti-D, $anti-Di^a$, $anti-Fy^b$, $anti-Jk^a$, $anti-Jk^b$, anti-M and anti-P1 were detected in 1 case (2.44%). Complex antibodies were detected in 1 case (2.44%) in anti-C+anti-D and anti-E+anti-c+$anti-Jk^b$, respectively. In this study, we analyzed the frequency and distribution of unexpected antibodies in one general hospital for the past 3 years. However, there has been a general increase in multicultural families and foreign workers in Jeju, and it would be a meaningful study to compare the frequency and distribution of unexpected antibodies.

The First Newborn Screening Study of T-Cell Receptor Excision Circle and κ-Deleting Recombination Excision Circle for Severe Combined Immunodeficiency in Korea: A Pilot Study (국내 최초 T-Cell Receptor Excision Circle과 κ-Deleting Recombination Excision Circle 신생아 선별검사에 관한 연구)

  • Purpose: Severe combined immunodeficiency (SCID) is the most serious form of primary immunodeficiency. Infants with SCID are susceptible to life-threatening infections. To establish newborn screening for SCID in Korea, we performed a screening test for T-cell receptor excision circle (TREC) and ${\kappa}$-deleting recombination excision circle (KREC) in neonates and investigated the awareness of SCID among their parents. Methods: Collections of dried blood spots from neonates and parent surveys were performed at the Samsung Medical Center and Cheil General Hospital & Women's Healthcare Center in Korea. The amplification crossing point (Cp) value <37.0 was defined as TREC/KRECpositive based on cutoff values from measuring multiplex real-time polymerase chain reaction. A Cp value >39.0 was defined as negative. Results: For TREC/KREC screening, 141 neonates were enrolled; 63 (44.7%) were male. One hundred forty neonates (99.3%) had positive TREC/KREC results at the time of the initial test; 82.3% and 75.9% were positive and 17.0% and 23.4% were weakly positive for TREC and KREC, respectively. In one neonate (0.7%), the initial TREC/KREC test result was negative. However, repeated tests obtained and confirmed a positive result. For an awareness survey, 168 parents were engaged. Only 2% of parents (3/168) knew that the newborn screening test for SCID had been introduced and performed in other countries. Eighty-four percent of parents (141/168) replied that nationwide newborn SCID screening should be performed in Korean newborns. Conclusions: In this study, newborn SCID screening was performed along with assessment of public awareness of the SCID test in Korea. The study results showed that newborn SCID screening can be readily applied for clinical use at a relatively low cost in Korea.

Effects of Complex Direct-Fed Microbial Supplementation on Growth Performance and Nutrient Digestibility for Broilers (복합생균제의 급여가 육계의 성장능력과 영양소 소화율에 미치는 영향)

  • This study was conducted to investigate the effects of dietary direct-fed microbials(DFM) on the growth performance and nutrient digestibility in broiler chickens. A total of two hundred eighty eight broiler chickens were randomly allocated into six treaments with four replications and fed for five weeks. Dietary treatments included 1) CON (basal diet), 2) DFM-1(basal diet + 0.2% Enterococcus sp. & Lactobacillus reuteri), 3) DFM-2(basal diet + 0.2% Enterococcus sp. & Lactobacillus plantarum), 4) DFM-3(basal diet + 0.2% Enterococcus sp. & Lactobacillus reuteri & Lactobacillus plantarum), 5) DFM-4(basal diet + 0.2% Enterococcus sp. & Lactobacillus reuteri & Lactobacillus plantarum & Yeast), 6) DFM-5(basal diet + 0.2% Enterococcus sp. & Lactobacillus reuteri & Lactobacillus plantarum & Bacillus subtilis). During the period of 1∼3weeks, average daily gain (ADG) and Feed/Gain were not significant different among treatments. In the later(3∼5weeks) and overall period(l∼ 5weeks), ADG and Feed/Gain tended to be improved in DFM treatments compared to the control. However, there were no significant differences among DFM complexes and between control and DFM treatments. The broilers fed DFM-3 diet was showd poor growth performance compared to the broilers fed DFM-l diet and similar to the broilers fed DFM-2 diet but there was no significant differences. The treatments of DFM-4 diet added Yeast and DFM-5 added Bacillus subtilis were improved in ADG and Feed/gain but no significant differences were found and also there was no specific DFM treatments in experiment period. In digestibility, the broiler chicks fed DFM treatments tended to improve N digestibility compared to control treatment. However, they were not significantly different among the treatments. In conclusion, DFM tended to improve growth performance in later stage of broiler.

Performance of Laying Period of Two-Way Crossbreed Parent Stock Korean Native Chickens for Producing of Korean Native Commercial Chickens (토종 실용계 생산용 2원 교배종 종계의 산란 능력 검정)

  • This study was carried out to investigate the performance of laying period of two-way crossbred of Korean native chickens. A total of four hundred eighty female 2-crossbred chicks that restored strains and were aboriginal at National Institute of Animal Science. Treatments were four crossbreds (4 replications/crossbred, 30 birds/replication) as A) C strain ${\times}$ S strain, B) C strain ${\times}$ H strain, C) R strain ${\times}$ S strain, and D) L strain ${\times}$ H strain, respectively. There were no significant difference on age, egg weight, and body weight at first egg among crossbreds (P>0.05). Body weight of D crossbred was lower compared to other crossbreds at the age of 24, 32, 40 and 60 week (P<0.05). Feed intake of A crossbred was highest among crossbreds (P<0.05). Weekly egg weight of A crossbred was the highest among crossbreds all laying period except 48~52 and 60~64 weeks. Hen-house egg production of A and B crossbreds was higher than that of C and D crossbreds (P<0.05). Hen-day egg production decreased after at the age of 36 weeks. Weekly egg production of A and B crossbreds was higher compared to C and D crossbreds (P<0.05). These results suggested the basic data on the record of laying period of 2-crossbreed Korean Native Chickens.

Performance of Growing Period of Two-way Crossbreed Parent Stock for Producing of Laying-Type Korean Native Commercial Chickens (산란전용 토종 실용계를 생산하기 위한 2원교배 종계의 육성 능력 검정)

  • This work was carried out to investigate the performance of growing period of two-way crossed of Korean native chickens parental stocks. A total of four hundred eighty female 2-crossbred chicks were used in this study and they were from National Institute of Animal Science. Groups were four crossbreds (4 replications/crossbred, 30 birds/replication) as A) C strain ${\times}$ Y strain, B) C strain ${\times}$ L strain, C) C strain ${\times}$ G strain and D) C strain ${\times}$ W strain. Body weight of A crossbred was the highest at the age of 8 week (P<0.05) and that of D strain was the lowest for growing period (P<0.05). Body weights of A and B crossbreds were higher than those of C and D crossbreds at the 12 and 16 weeks (P<0.05). Weekly body weights of A and B crossbreds were higher than C and D crossbreds (P<0.05), and weekly body weight of B crossbred was higher compared to other crossbreds at 0~20 weeks old. Weekly feed intake of D crossbred was the lowest among all crossbreds at 0~12, 0~16 and 0~20 weeks old (P<0.05). Weekly feed conversion ratio of C crossbred was the highest among crossbreds (P<0.05). These results can give the basic information for growth related data in 2-way crossbreed Korean Native Chickens, which can be used for the parental stocks for the laying-type of Korean native commercial chickens.

Radiotherapy for Oral Cavity Cancer (구강암의 방사선치료)

  • Eighty five patients of oral cavity cancer, treated with radiation at the Department of Therapeutic Radiology, Korea Cancer Center Hospital, during the period from March 1985 to September 1990 were analyzed retrospectively. Among 85 patients, 37 patients were treated with radiation only and 48 patients were treated with radiation following surgery. And 70 patients received external irradiation only by $^{60}Co$ with or without electron, the others were 7 patients for external irradiation plus interstitial implantation and 8 patients for external irradiation plus oral cone electron therapy. Primary sites were mobile tongue for 40 patients, mouth floor for 17 patients, palate for 12 patients, gingiva including retromolar trigone for 10 patients, buccal mucosa for 5 patients, and lip for 1 patient. According to pathologic classification, squamous cell carcinoma was the most common (77 patients). According to AJC TNM stage, stage I + II were 28 patients and stage III+IV were 57 patients. Acturial overall survival rate at 3 years was $43.9\%,$ 3 year survival rates were $60.9\%$ for stage I + II, and $23.1\%$ for stage III+IV, respectively. As a prognostic factor, primary T stage was a significant factor (p<0.01). The others, age, location, lymph node metastasis, surgery, radiation dose, and cell differentiation were not statistically significant. Among those factors, radiation plus surgery was more effective than radiation only in T3+T4 or in any N stage although it was not statistically sufficient (p<0.1). From those results, it was conclusive that definitive radiotherapy was more effective than surgery especially In the view of pertainig of anatomical integrity and function in early stage, and radiation plus surgery was considered to be better therapeutic tool in advanced stage.

The Role of Radiation Therapy in the Treatment of Intracranial Glioma : Retrospective Analysis of 96 Cases (뇌 교종 96예에 대한 방사선치료 성적의 후향적 분석)

  • Between March 1983 and December 1989, ninety-six patients with intracranial glioma were treated in the Department of Therapeutic Radiology, Kangnam St. Mary's Hospital, Catholic University Medical College. We retrospectively reviewed each case to evaluate variable factors influencing the treatment results and to develop an optimal therapy Policy. Median follow-up is 57 months (range: 31~133 months). Of the 96 patients, 60 $(63\%)$ were males and 36 $(37\%)$ were females. Ages ranged from 3 to 69 years (median 42 years). The most common presenting symtoms were headeche $(67\%)$ followed by cerebral motor and sensory discrepancy $(54\%),$ nausea and vomiting $(34\%),$ seizure $(19\%),$ mental change $(10\%)$ and memory and calculation impairment $(8\%).$ Eighty five $(88.5\%)$ patients all, except 11 $(11.5\%)$ brain stem lesions, were biopsy proven intracranial glioma. The distribution by histologic type was 64 astrocytomas $(75\%),$ 4 mixed oligoastrocytomas $(5\%),$ and 17 oligodendrogliomas $(20\%).$ Fourty nine patients $(58\%$ were grade I, II histology and 36 $(42\%)$ patients were grade III, IV histology. Of the 96 patients, 64 $(67\%)$ recieved postoperative RT and 32 $(33\%)$ were treated with primary radiotherapy. Gross total resection was peformed in 14 $(16\%)$ patients, subtotal resection En 29 $(34\%),$ partial resection in 21 $(25\%),$ and biopsy only in 21 $(25\%).$ Median survival time was 53 months (range 2~ 133 months), and 2- and, 5-year survival rate were $69\%,49\%$ respectively. 5-year survival rate by histologic grade was grade I, $70\%,$ grade II, $58\%,$ grade III, $28\%,$ and grade IV, $15\%.$ Multivariated analysis demonstrate that age at diagnosis (p=0.0121), Karnofsky performance Status (KPS) (p=0.0002), histologic grade (p=0.0001), postoperative radiation therapy (p=0.0278), surgical extent (p =0.024), cerebellar location of tumor (p=0.0095) were significant prognostic factors influencing on survival.

Study on the Weight of Visceral Organs and Live Weight of Korean Native Cattle (한빈우(韓牝牛)의 생체중(生體重)과 장기중량(臟器重量)에 관한 연구(硏究))

  • This study was conducted to determine the interrelationship between the weight of various visceral organs and the live weight of Korean native cattle with particular regard to female ones. One hundred and eighty-five of 2 to 6 years and one hundred and fifty-six Korean native cow of over 7 years of age were examined at the Busan slaughter house during the lest quarter of 1979. The results obtained are summarized as follows: 1. The average live weight of 72 years old cows examined was 297.09kg, while these weight of visceral organs such as liver, lung, heart, left kidney, right kidney and spleen being 4301.09, 2165.22, 1355.43, 346.74, 350 and 991.30g, respectively. The length, width and thickness of the spleen were 49.0, 12.92 and 2.95cm, respectively. 2. The average live weight of 3 years old cows examined was 306.56kg, while these weight of visceral organs such as liver, lung, heart, left kidney, right kidney and spleen being 4099.07, 2144.44, 1383.33, 326.85, 325.93 and 964.82g, respectively. The length, width and thickness of the spleen were 43.3, 12.37 and 2.80cm, respectively. 3. The average live weight of 4 years old cows examined was 314.67kg, while these weight of visceral organs such as liver, lung, heart, left kidney, right kidney and spleen being 4186.67, 2292.50, 1392.50, 346.67, 343.33 and 932.50g, respectively. The length, width and thickness of the spleen were 47.01, 12.55 and 3.08cm, respectively. 4. The average live weight of 5 years old cows examined was 328.90kg, while these weight of visceral organs such as liver, lung, hearts left kidney, right kidney and spleen being 4465.32, 2394.35, 1493.55, 370.16, 335.97 and 997.58g, respectively, The length, width and thickness of the spleen were 49.17, 12.76 and 3.16cm, respectively. 5. The average live weight of 6 years old cows examined was 334.97kg, while these weight of visceral organs such as liver, lung, heart, left kidney, right kidney and spleen being 4585.29, 2552.79, 1555.89, 364.70, 373.24 and 985.29g, respectively. The length, width and thickness of the spleen were 48.27, 12.66 and 2.78cm, respectively. 6. The average live weight of 156 cows (over 7 years of age) examined was 344.91kg, while these weight of visceral organs such as liver, lung, heart, left kidney, right kidney and spleen being 4730.77, 2651.44, 1554.39, 407.37, 409.42 and 1101.06g, respectively. The length, width and thickness of the spleen were 51.04, 13.21 and 3.08cm, respectively. 7. Total average live weight of 341 cows (from 2 to 6 and over 7 years of age) examined was 332.44kg, while these weight of visceral organs being 4550.95, 2497.13, 1500.76, 380.18, 382.27 and 1047.98g, respectively. The length, width and thickness of the spleen were 52.01, 12.96 and 3.02cm, respectively. 8. The correlation coefficients between the live weight of 341 cows and their visceral organs were found to be highly significant (P<0.01), except fort for the length of spleen

Analysis of Relativity Between Invasiveness on Chest of Tomographic Finding and Histopathologic Invasiveness (종격동 종양의 전산화 단층촬영(CT)소견, 수술소견 및 병리조직학적 침윤도 사이의 상관성 분석)

  • Mediastinal tumor had been fascinated by its location on heart, great vessels, esophagus, and nervous tissue, its convenience of surgical treatment and superiority of its operative result. Between January 1989 and June 1995, eighty-seven patients with mediastinal tumor which were treated surgically in the Department of Thoracic and Cardiovascular Surgery, Asan Medical Center, School of Medicine, University of Ulsan. To provide the appropriate surgical management of mediastinal tumor, the demographic data, diagnostic evaluation, clinical presentation, location, size, operative finding and histopathologic distribution were reviewed and we analyzed relativity between invasiveness in chest computed tomographic finding or invasiveness on operative finding and histopathologic invasiveness. The anterosuperior mediastinum was the most commonly involved site of a mediastinal tumor(57%), followed by the posterior mediastinum(35%) and middle mediastinum(8%). The most frequently encountered tumors were thymic neoplasia(31%), followed by primary cyst(22%), neurogenic tumor(22%) and teratoma(10%) in decreasing order of frequency. Histopathologically invasive tumors were present in 17 patients(20%) and its site included anterosuperior mediastinum(16%) and posterior mediastinum(4%). All patients in this study underwent chest CT. In chest CT's finding, 15 patients(17%) showed invasiveness. A total excision of the tumor was performed 80 patients(92%), subtotal excision 6 patients(7%) and biopsy only 1 patient(2%). In operative finding, 14 patients(16%) were suspected invasiveness. The mean size of the tumor was 6.0$\pm$ 3.2cm. In anterosuperior mediastinum, the mean size was 6.2$\pm$3.1cm, in middle mediastinum, it was 3.9$\pm$1.1cm, in posterior mediastinum, it was 5.8$\pm$2.6cm. In malignant tumors, the mean size was 7.3$\pm$4.6cm, in benign tumor, it was 5.5$\pm$2.6cm(P<0.05). Relativity between histopathological invasiveness(17 patients) and invasiveness in chest CT's finding(15 patients) included sensitivity 35%, specificity 87% and predictability 35%, relativity between histopathological invasiveness(17 patients) and invasiveness on operative finding included sensitivity 52%, specificity 93% and predictability 64%. In conclusion, since it was proved that the compatibility of preoperative chest CT findings or operative findings and histopathological invasiveness is quite low, it is considered that wide excision of the mediastinal tumor except cystic lesion including adjacent tissues would yield better postoperative results.

Different $^{18}$F-FDG Uptake According to Tumor location and Morphology of Cholangiocarcinoma and Its Clinical Implication (담관암의 위치와 형태에 따른 $^{18}$F-FDG 섭취량의 차이와 임상적 의의)

  • Purpose: $^{18}$F-flurodeoxyglucose (FDG) uptake on positron emission tomography (PET) scan has been found to reflect tumor aggressiveness and prognosis in various types of cancer. However, pattern of FDG uptake in biliary malignancies and its clinical significance have not been studied well. The purpose of this study was to assess the additional value of $^{18}$F-FDG PET in differential diagnosis and prognosis of cholangiocarcinoma (CC) according to the tumor location and tumor morphology. Materials and Methods: From April 2005 to May 2008, eighty two patients (M:F = 55:27, age 66.2$\pm$9.6 yrs) with CC underwent $^{18}$F-FDG PET. For semiquantitative analysis, the maximum standardized uptake value (SUV$_{max}$) was obtained from the primary tumor. The difference of SUV$_{max}$ according to the tumor location and tumor growth pattern, such as scirrhous type, nodular type, polypoid type were compared. Results: Overall sensitivity of PET scan was 81.7% in CC. SUV$_{max}$ on PET scan in intrahepatic CC was significantly higher compared to extrahepatic CC. In extrahepatic CC, polypoid type showed significantly higher SUV$_{max}$ compared to scirrhous type. Conclusion: $^{18}$F-FDG PET may have a significant impact on clinical decision-making and on the management of intrahepatic cholangiocarcinoma. And it is related to the shape of the tumor and the sensitivity of detection is higher in the mass-forming type than in the scirrhous type.
