The Influence of Physical Activity and Depression on Sleep Quality in Community-dwelling Older Adults: A Comparison between Young-old and Old-old (신체활동과 우울이 지역사회 거주 노인의 수면의 질에 미치는 영향: 전기노인과 후기노인의 비교)

  • Purpose: The purpose of this study was to identify the influence of physical activity and depression on sleep quality among the young-old and old-old community-dwelling elderly. Methods: Participants were 216 community-dwelling older adults in Korea aged 65 or above. Data were collected using structured questionnaires with face-to-face interviews that included demographic and health-related characteristics, International Physical Activity Questionnaires (IPAQ), the Short Form Geriatric Depression Scale (SGDS) and the Pittsburgh Sleep Quality Index (PSQI). A hierarchical multiple regression was conducted to examine whether physical activity and depression would predict sleep quality under other controlled factors. Results: There were differences in demographic and health-related characteristics, physical activity, and depression by age groups, but not in sleep quality. In the young-old elderly, physical activity (${\beta}=-0.22$, p=.043) and depression (${\beta}=0.31$, p=.002) were significantly associated with sleep quality (F=4.46, p=.001, Adjusted $R^2=.16$). In the old-old elderly, physical activity (${\beta}=-0.29$, p=.001) and depression (${\beta}=0.41$, p<.001) were significantly associated with sleep quality (F=10.79, p<.001, Adjusted $R^2=.29$). Conclusion: These finding highlight physical activity and depression as important contributors to sleep quality in both young-old and old-old elderly.

The Effects of TENS and cold application on secondary thermal hyperalgesia in rats induced by muscle pain (근통증이 유발된 흰쥐에 있어 TENS와 냉적용이 이차성 열 통각과민에 미치는 영향)

  • The aim of this study was to investigate the effects of TENS and cold application on secondary thermal hyperalgesia in rats induced by muscle pain. Muscle pain was induced in male Sprague-Dowley rats by intra-muscular injection of gastrocnemius with $3\%$ carrageenan. The paw withdrawal latency(PWL) and tail flick test(TFT) to heat were used to detect secodary thermal hyperalgesia induced by the muscle pain. PWL and TFT were quantified before and 4, 10, and 24 h after induction of muscle pain and after application of TENS(100Hz, $100{\mu}s$, sensory intensity) and cold($4^{\circ}C$). TENS and cold significantly reduced the PWL and TFT to heat stimuli when compared with controls receiving no TENS and cold(p<.05). These results suggested that application of TENS and cold attributed to decrease secodary thermal hyperalgesia in rat induced by muscle pain.

Effects of Home Visiting Physical Therapy on Motor Function and ADL in the Persons with Chronic Stroke (가정방문 물리치료가 뇌졸중 환자의 운동기능과 일상생활동작 수행에 미치는 영향)

  • The purpose of this study was to determine the effects of home visiting physical therapy on the motor function and ADL in the person with chronic stroke. In this study, 50 individuals, lining in Ui-wang city, were chosen by Ui-wang city public health center and the home visiting physical therapy was performed once a week for 8 weeks and scored the MAS, the FIM and the MBI before and after. The results of this study were as follows. (1) The items with the highest score among subscales of the MAS was 'Sitting', the FIM was 'Comprehension', and the MBI was 'Ambulation' and the lowest were the MAS was 'Hand activities', the FIM was 'Bathing', and the MBI was 'Bathing self' respectively. (2) After 8 weeks home visiting physical therapy, average total score of MAS, FIM and MBI were 23.32, 89.00, and 61.72 and MAS and MBI a significant increment(p<0.05) but not FIM. Results shown here indicates that the home visiting physical therapy can result in changed the motor function and the ADL in the person with chronic stroke. Also, the results of this study can provide a reference for the successful therapeutic program in the persons with chronic stroke.

Effects of an Ultrasound-assisted Palpation Training Program on Physical therapy Student's Palpation Skills

  • Objective: This study aimed to explore the effects of a novel training program that integrates ultrasound technology to enhance the palpation skills of physical therapy students. The program was designed to support students in developing more precise palpation techniques by using ultrasound as a supplementary tool for anatomical identification. Design: A randomized controlled trial. Methods: A total of twenty students, all enrolled in the physical therapy department at S University, who met the specified selection criteria, were randomly assigned to one of two groups: the experimental group (EG, n=10) or the control group (CG, n=10). The experimental group participated in an ultrasound-assisted palpation training program, while the control group did not receive this intervention. Results:The experimental group demonstrated significant improvements in their ability to accurately palpate anatomical landmarks, specifically the long head of the biceps brachii (LHBT) and the lateral joint line of the knee (LJLK), as well as an increased level of confidence in their palpation skills (p<0.05). A comparative analysis of changes from pre- to post-training revealed statistically significant differences between the two groups (p<0.05). Conclusions: The findings of this study suggest that the ultrasound-assisted training program can provide valuable educational benefits, offering foundational data to enhance the development of palpation skills in physical therapy students and making a meaningful contribution to educational research within the field.

Association of selected health behaviors with perceived health, depressive symptom and fatalism among the aged 50-69 living in Seoul (서울지역의 50대와 60대의 건강행동과 우울, 운명론(Fatalism)의 관련성)

  • Objectives: The purpose of this study was to investigate association of depressive symptom, fatalism with selected health behaviors among people aged 50-69 with no physical limitation in Seoul. Methods: In 2012, data were collected using a household based interview survey in Seoul. One person in each selected household aged between 50 and 69 was selected and responded. Data analysis was based on 1,190 subjects who answered they do not have any physical activity limitation. Results: Multiple logistic regression analysis showed significant association among variables including perceived health, depressive symptom and fatalism scores on some health behaviors. Multiple regression analysis showed that selected health risk behaviors(current smoking, monthly alcohol consumption, no regular health exam in two years) were significantly associated with depressive symptom and fatality scores in addition to demographic variables. The final regression model's adjusted R square was about 0.235. Conclusion: Demographic variables such as gender, age and socioeconomic status were significant variables in health behaviors and these behavioral factors were associated with perceived health, depressive symptom and fatalistic views. As a conclusion, depressive symptom and fatalism should be monitored and intervened in health education practice.

Analysis of the Nursing Interventions Performed by Public Health Nurses in Health Centers Using the NIC (보건소 간호사의 간호중재 분석 - 간호중재분류[NIC]의 적용 -)

  • Purpose: The purpose of this study was to identify nursing interventions performed by public health nurses in health centers. Method: Data was collected by the taxonomy of Nursing Intervention Classification(NIC 3rd: 486 nursing interventions) from 131 public health nurses in health centers and analyzed using descriptive statistics. Result: As its result, more than 50% of public health nurses performed 137 nursing interventions at least monthly. The most frequently used intervention class was 'activity and exercise management', followed by 'physical comfort promotion', 'community health promotion', 'life span care', 'coping assistance', 'Self care facilitation', 'information management', 'nutrition support', 'community risk management' and 'patient education'. One hundred twenty nursing interventions were rarely performed by 90% or more of the nurses. Most of them were the physical complex domain. Conclusion: In conclusion, 137 interventions were performed by public health nurses at least monthly. NIC is helpful to build a standardized language for public health nursing.

The Trends in Research on the Health of North Korean Refugees (북한이탈주민의 건강 관련 연구 동향)

  • Purpose: This study aims to identify the general characteristics, subjects, and methods of research on the health of North Korean refugees through a systematic literature review. Methods: A total of 140 studies on health were reviewed using the analytical framework developed by the researchers. Results: The quantitative research comprised 90.7% of the studies, whereas the qualitative research were 7.9% of them. Approximately 81.4% of those reviewed have publication dates spanning 2006 to 2015. Only 13.6% of the studies were conducted with the approval of an institutional review board. The subjects of studies were psycho-social health (151.8%), behavioral health (28.5%), cognitive health (15.0%), physical health (12.2%), integrated health (7.8%), and spiritual health (2.8%). Within the quantitative studies reviewed, the most commonly used study design was survey research (86.0%). The two sampling methods used most often were convenience sampling (53.5%) and snowball sampling (19.4%), and the most commonly used data-collection method were questionnaires (94.6%). As for the qualitative studies, the most commonly used study design was phenomenology. Conclusion: The results suggested that the reviewed studies focused on physical health more than psycho-social health, and physiological measurement more than questionnaires. Ethical considerations need to be expanded.

Effect of Usual Source of Care on Health Behavior of the Middle Aged and the Elderly (상용치료원 보유가 중노년층의 건강행동에 미치는 영향)

  • Background: This study was conducted to analyze the impact of having a usual source of care on health behaviors of the middle aged and the elderly, in order to investigate the potential effect of enhancing primary health care in a Korean context. Methods: This study constructed a balanced panel of middle-aged and elderly samples using the Korea Health Panel 2016-2018, and fixed-effect models were used to analyze the data. Results: Among three sets of dependent variables (physical activity, smoking, drinking), statistically significant results were found only in physical activity. Subgroup analysis showed that this effect was not observed in the late elderly (aged 75 and older) and those without chronic diseases. Conclusion: Results of the study implied that enhancing primary health care among middle age and the elderly may have an effect on improving health behaviors. Moving forward to person-centered primary health care from disease-focused primary health care should be considered in high-risk groups such as the middle aged and the elderly with chronic diseases.

Evaluation of the qualite of life related to oral health among elderly people in some elderly care facilities by OHIP-14 (OHIP-14를 이용한 일부 요양기관 노인들의 구강건강관련 삶의 질)

  • Objectives : The growing elderly population and social changes have fueled a rapid increase in demand for elderly care facilities, but health care services for the elderly, especially oral health services, have long been overlooked. The purpose of this study was to examine the oral health care of elderly residents in elderly care facilities in an effort to provide some information on the elderly's oral health care and the development of dental care programs geared toward institutionalized elderly people. Methods : The subjects in this study were 230 elderly people who were in elderly care facilities in Jeolla Province. An individual interview was held, and they got a dental checkup. As for data analysis, t-test and ANOVA were utilized, and Scheffe post-hoc test was employed. Results : 1. In relation to the subfactors of the quality of life related to oral health, the elderly people investigated got a mean of 4.58, 4.47, 4.38, 4.09, 3.94, 3.91 and 3.76 respectively in activity disorder, mental disorder, social disorder, mental inconvenience, functional disorder, physical pain and physical disorder. 2. Concerning the overall quality of life related to oral health, there were statistically significant gaps in this aspect according to gender, age, presence or absence of systemic disorder, presence or absence of eating difficulties, subjective health status, subjective oral health state and oral health concern. 3. The presence or absence of root caries had a significant relationship to physical pain and social disorder among the subfactors of the OHIP-14, but that had nothing to do with functional disorder, mental inconvenience, physical disorder, mental disorder and activity disorder. Conclusions : The oral health indexes of the institutionalized elderly people in Jeolla Province were measured, and what factors affected the subfactors of oral health was checked. As a result, there appeared a close relation ship between oral health and the quality of life. Therefore effective oral health plans that cater to the elderly should be carried out to improve elderly people's quality of life related to oral health who stay in long-term elderly care facilities. In the future, prolonged research should be implemented from diverse angles for the sake of institutionalized elderly people.

The Study on the Characteristic of Physical Touch in Caring Situation (돌봄상황에서 신체적 접촉의 특성에 관한 연구)

  • This study was performed to investigate the characteristic of physical touch in caring situation. The subjects of this study were adults in caring situation, therefore they were composed of 7 patients, 6 nurses, 3 doctors, 3 pharmacologists, 3 men in paramedicine area, 3 nursing educators and 13 normal general adults. The datas were gathered through nonstructured questionaire from June, 20th to September, 15th in 1995. The datas were subjects' descriptions about the intention, perception, and form of physical touch in caring situation and analysed by content analysis. The results as follow : The datas were divided into four areas. There were the situation being necessiated the physical touch, meaning of physical touch, form of physical touch and perception about physical touch in caring situation. The situation being necessiated physical touch were the situation that required physical treatment, to deliver the active expression of concern about the patients, to determine the treatment due to the identification of physical condition of patients and to induce the psychological eqilibrium into patient's mind. The meanings of physical touch in caring situation were - Good meanings that intention is to encourage, to be have hope about health and to deliver the affection willing to help patient. - therapeutic methods that were to facillitate the circulation of blood, to reduce the pain perception and to facillitate the circulation of qi. - interpersonal affectionate relation that the intentions were to deliver the understanding of patient's pain, were to delivery the meaning to the patient not be alone. The forms of physical touch in caring situation were none invasive forms just like laying hands on hand, head, shoulder, gentle knocking on the shoulder or back, massage of legs and back and finger pressure on acupuncture points. The perception of physical touch in caring situation divided into two parts. In family, the perceptions of physical touch in caring situation were to promote health status because physical touch induce the psychological peace, and to evoke the importance of relationship among family members. In relation with care giver, perception of physical touch in caring situation were inevitable process in treatment, and to deliver the trust and concern about patients.
