Shade Comparative Analysis of Natural tooth Measured by Visual and Two Colorimeters(ShadeEye®,Shadepilot®) (2종 측색기와 시각을 이용한 자연치아의 색조 비교 분석)

  • The objectives were to evaluate the accuracy of shade selection by human visual system(VS) and 2 different colorimeters ($ShadeEye^{(R)}$(SE) and Shadepilot (SP)). Maxillary anterior teeth of 30 volunteers which had no caries or restorations were included in the study. Firstly, the accordance in shade selection by 3 dentists and 2 colorimeters was investigated. Secondly, the color of the teeth were measured by 1 observer's naked eye and 2 colorimeters under different illumination conditions (Sunny versus cloudy day). Additionally testing of inter-observer variability selected colors by 2 novice and 2 experienced dentists were compared. For comparing visual and 2 different colorimeters, SP(60%) showed significantly highest rate of accordance than the visual (23.3%) or SE (16.7%) and lowest mean ${\Delta}E$ ($2.62{\pm}0.74$ versus $3.83{\pm}1.38$;SE or $4.04{\pm}1.61$;VS)(p<0.001). If accuracy of shade selection were measured using VS, the mean ${\Delta}E$ value of cloudy day was higher than that of sunny day ($4.35{\pm}1.70$ versus $3.53{\pm}1.31$; p<0.001). There were no significant difference of the mean ${\Delta}E$ value between sunny and cloudy day in both SE and SP. Inter- observer repeatability was higher in 2 experienced group (73.3%) than novice group (36.7%). The mean ${\Delta}E$ of experienced group was lower than that of novice group ($3.60{\pm}1.47$ versus $4.70{\pm}1.67$; p<0.001). Colorimeters (SE or SP) is more accurate and more reproducible compared with human shade assessment. Using visual system may be limited by cloudy and inexperience of tester, then more experience and using colorimeters may be helpful of raising the accurate repeatability of shade selection.

Homocysteine, insulin, vitamin B12 and folic acid levels in obese children (비만아에서 혈청 호모시스테인, 인슐린, 비타민 B12, 엽산 농도에 대한 연구)

  • Purpose : As the prevalence of childhood obesity is increasing, hypertension, hyperlipidemia, insulin resistance and diabetes mellitus have become problems. High homocysteine levels and low vitamin $B_{12}$ supplementation are acknowledged to have a role in coronary artery disease, but there are few studies on homocysteine, insulin and vitamin $B_{12}$ levels in obese children. We aimed to study whether homocysteine, insulin, vitamin $B_{12}$, folic acid levels could have any difference and relation in obese children. Methods : The disease group consisted of 27 children from 8 to 11 years old, whose obesity index was over 130. The control group consisted of 30 healthy children of the same age group. Obesity index and body mass index were calculated by height and body weight of the children, and their systolic and diastolic blood pressures at resting state were checked. Total cholesterol, triglyceride, homocysteine, insulin, vitamin $B_{12}$, folic acid levels were studied after 10 hours of fasting. Intracellular fluid, extracellular fluid, protein, mineral, muscle mass, lean body fat, fat mass and fat percentages were checked by bioelectrical impedance. Results : Homocysteine levels were higher in obese children($8.1{\pm}2.1{\mu}mol/mL$ vs. $4.9{\pm}1.0{\mu}mol/mL$). Insulin levels were also higher in obese children($26.8{\pm}11.2{\mu}IU/mL$ vs. $12.5{\pm}5.24{\mu}IUl/mL$). Vitamin $B_{12}$ was lower in obese children($798.6{\pm}174.3pg/mL$ vs. $967.8{\pm}405.0pg/mL$). But there was not a difference in the folic acid levels between the two groups. In obese children, systolic blood pressure (r=0.535), triglyceride(r=0.517), total cholesterol(r=0.408), folic acid(r=0.408), vitamin $B_{12}$(r=0.338) and abdomoanl fat %(r=0.306) had a positive correlation. Conclusion : We found definite differences of insulin, homocysteine, and vitamin $B_{12}$ plasma levels in obese children, but we need more study to use those parameters as risk factors of metabolic syndrome in pediatric obese patients.

Comparison of Outcomes after Curative Resection of Primary Lung Cancer between 50 Year or Younger and 70 Year or Older Patients (50세 이하와 70세 이상 원발성 폐암 환자에서의 근치적 수술 후 성적 비교)

  • Background: Previous series have suggested that younger patients with primary lung cancer exhibit a more aggressive disease course with a worse prognosis, as compared to older patients, although this issue is still debatable. Material and Method: We reviewed the medical records of 79 patients (32 patients 50 years and younger (Group I) and 47 patients 70 years and older (Group II)) who underwent curative resection for primary lung cancer between July 2000 and June 2008. Result: The median age of the patients was 46.5 years in Group I and this was 73 years in Group II. The older patients were more likely to have major comorbidities (44% versus 77%, respectively; p=0.003). Histological examinations identified that the minor histological types (excluding non-small cell lung cancer (NSCLC)) were predominantly found in the Group I patients (16% versus 2%, respectively; p=0.037). For the TNM staging of the NSCLC, with excluding the minor histologic types, a higher proportion of patients had stage III disease in Group I (33% versus 13%, respectively; p=0.038). There was no significant difference in major morbidity (16% versus 30%, respectively; p=0.148) and operative mortality (0% versus 4.3%; p=0.512) between the groups. The mean follow-up interval was 33 months (range: $1{\sim}98$ months) for patients in both groups. For the patients with NSCLC, the five-year overall survival rate was 52.3% for Group I and 53.7% for Group II (p=0.955). The rate of freedom from recurrence at five years was significantly lower for the Group I patients than for the Group II patients (39.4% versus 70.4%, respectively; p=0.027), and only being a member of Group I impacted recurrence, based on the Cox proportional hazard analysis (p=0.034). Of the patients who had recurrence, four patients in Group I underwent aggressive surgical treatment. All of these patients exhibited long-term survival (range: $46{\sim}87$ months). Conclusion: In our study, the early outcome and long-term survival were similar for the younger and older patients after curative resection of primary lung cancer. However, we think that younger patients require meticulous follow-up as they had a tendency to proceed to surgery with advanced stage disease, a higher recurrence rate than did the older patients and the survival rates were improved, even for the recurred cases, with early aggressive treatment.

Nutrient Utilization, Body Composition and Lactation Performance of First Lactation Bali Cows (Bos sondaicus) on Grass-Legume Based Diets

  • A study on energy and protein utilization, and milk production of Bali cows on grass-legume diets was carried out using 12 first lactation cows (initial BW $263.79{\pm}21.66kg$) during a period of 16 weeks starting immediately post calving. The animals were randomly allotted into 4 dietary treatment groups R1, R2, R3 and R4, receiving from the last 2 months of pregnancy onwards, graded improved rations based on a mixture of locally available grass and legume feed ad libitum. R1 contained on a DM basis 70% elephant grass (PP, Penisetum purpureum) plus 30% Gliricidia sepia leaves (GS), R2 was 30% PP plus 55% GS supplemented with 15% Hibiscus tilliactus leaves (HT, defaunating effect), R3 and R4 were 22.5% PP+41.25% GS+11.25% HT+25% concentrate, where R3 was not and R4 supplemented with zinc di-acetate. TDN, CP and zinc contents of the diets were 58.2%, 12.05% and 18.3 mg/kg respectively for R1, 65.05%, 16.9% and 25.6 mg/kg respectively for R2, 66.03%, 16.71% and 29.02 mg/kg respectively for R3 and 66.03%, 16.71% and 60.47 mg/kg respectively for R4. Milk production and body weight were monitored throughout the experimental period. In vivo body composition by the urea space technique validated by the body density method and supported by carcass data was estimated at the start and termination of the experiment. Nutrient balance and rumen performance characteristics were measured during a balance trial of 7 days during the 3rd and 4th week of the lactation period. Results indicated that quality of ration caused improvement of ruminal total VFA concentration, increments being 52 to 65% for R2, R3 and R4 above R1, with increments of acetate being less (31 to 48%) and propionate being proportionally more in comparison to total VFA increments. Similarly, ammonia concentrations increased to 5.24 to 7.07 mM, equivalent to 7.34 to 9.90 mg $NH_3-N/100ml$ rumen fluid. Results also indicated that feed quality did not affect DE and ME intakes, and heat production (HP), but increased GE, UE, energy in milk and total retained energy (RE total) in body tissues and milk. Intake-, digestible- and catabolized-protein, and retained-protein in body tissues and milk (Rprot) were all elevated increasing the quality of ration. Similar results were obtained for milk yield and components with mean values reaching 2.085 kg/d (R4) versus 0.92 kg/d (R1) for milk yield, and 170.22 g/d (R4) vs 71.69 g/d (R1), 105.74 g/d (R4) vs 45.35 g/d (R1), 101.34 g/d (R4) vs 46.36 g/d (R1) for milk-fat, -protein, and -lactose, respectively. Relatively high yields of milk production was maintained longer for R4 as compared to the other treatment groups. There were no significant effects on body mass and components due to lactation. From the relationship $RE_{total}$ (MJ/d)=12.79-0.373 ME (MJ/d); (r=0.73), it was found that $ME_{m}=0.53MJ/kgW^{0.75}.d$. Requirement of energy to support the production of milk, ranging from 0.5 to 3.0 kg/d, follows the equation: Milk Prod. ($Q_{mp}$, kg/d)=[-2.48+4.31 ME($MJ/kg^{0.75}.d$)]; (r=0.6) or $Q_{mp}$=-3.4+[0.08($ME-RE_{body\;tissue}$)]MJ/d]; (r=0.94). The requirement for protein intake for maintenance ($IP_m$) equals $6.19 g/kg^{0.75}.d$ derived from the relationship RP=-47.4+0.12 IP; (r=0.74, n=9). Equation for protein requirement for lactation is $Q_{nl}$=[($Q_{mp}$)(% protein in milk)($I_{mp}$)]/100, where $Q_{nl}$ is g protein required for lactation, $Q_{mp}$ is daily milk yield, Bali cow's milk-protein content av. 5.04%, and $I_{mp}$ is metabolic increment for milk production ($ME_{lakt}/ME_{m}=1.46$).

Energy Intake according to Expected Body Mass Index of Middle School Students in Seoul (서울 일부지역 중학생의 희망 체질량지수에 따른 에너지 섭취 상태)

  • The purpose of this study was to compare energy intake according to expected body mass index (BMI) in adolescents. A total of 280 middle school students were divided into current or/and expected obesity groups (underweight group; UWG, normal weight group; NWG, and overweight group; OWG) by BMI to compare energy intake and perception on body shape and weight control. The average age of total subjects was 15 years, and the distributions of boys and girls were 52.5% and 47.5%, respectively. The distribution of boys and girls was not significantly different among current obesity groups but was among expected obesity groups (15% and 82% in UWG vs. 71.4% and 16.5% NWG vs. 13.6% and 1.5% in OWG; P<0.001), indicating that girls wanted to lose weight more than boys. For body shape, most subjects in current obesity groups responded "fitness" in UWG (38.7%), "fitness" and "slightly chubby" in NWG (45.5% and 39%), and "slightly chubby" in OWG (65.2%), showing a significant difference (P<0.001). For interest in weight control, most subjects responded "average" in current UWcinterested" in current OWG (52.2%), whereas they responded "very interested" in expected UWG (55.7%), "average" in expected NWG (51.2%), and "not interested" in expected OWG (45.5%), showing a significant difference (P<0.001). The percentage of subjects with experience with weight control was significantly high in current OWG (71.7%), NWG (51.2%) and UWG (34.2%) as well as in expected UWG (57.3%), NWG (40.2%) and OWG (36.4%) in descending order (P<0.001). The daily energy intake of total subjects was 2,057.1 kcal without any significant difference among current obesity groups. However, it was significantly lower in expected UWG than expected NWG and OWG (1,943.1 kcal vs. 2,165.0 kcal or/and 2,152.1 kcal; P<0.001). To sum up these results, girls wanted to lose weight more than boys. The more students wanted to lose weight, the greater their experience and interest in weight control and the lower their energy intake.

Comparison of Milk Yield and Milk Composition Between Before and After Auto Milking System (AMS) Use in Dairy Cow (국내 자동착유시스템 이용농가의 설치 전·후 산유량 및 유성분 비교)

  • The present study was conducted to investigate effects of auto milking system (AMS) on milk yield and milk composition in dairy cow using dairy herd improvement (DHI) record. DHI records for 147 cows were compared between a year before and after AMS use. AMS significantly increased daily milk yield compared with the conventional milking system (30.4kg vs 34.3 kg for CMS and AMS, respectively). There were no significant differences in the contents of milk fat (3.7% vs 3.8% for CMS and AMS) and milk protein (3.2% vs 3.2% for CMS and AMS) between CMS and AMS. But the yields of milk fat and milk protein were significantly higher for AMS than for CMS. Average somatic cell counts were 169,400/ml (1st grade) before AMS use but increased by 314,400/ml (2nd grade) after AMS use. Overall, AMS increased milk yield and the yields of milk fat and protein without affecting milk composition but also increased somatic cell count.

Comparative Analysis between Preoperative Radiotherapy and Postoperative Radiotherapy in Clinical Stage I and II Endometrial Carcinoma (자궁내막암 환자에서 수술 전 방사선치료와 수술 후 방사선치료의 성적 비교 분석)

  • Purpose : To obtain the optimal treatement method in patients with endometrial carcinoma(clinical stage FIGO I, II) by comparative analysis between preoperative radiotherapy (pre-op RT) and postoperative radiotherapy (post-op RT). Material and Methods : A retrospective review of 62 endometrial carcinoma patients referred to the Yonsei Cancer Center for radiotherapy between 1985 and 1991 was undertaken. Of 62 patients, 19 patients(Stagel : 12 patients. Stagell;7 patients) received pre-op RT before TAH(Total Abdominal Hysterectomy) and BSO (Bilateral Salphingoophorectomy) (Group 1) and 43 patients(Stage 1;32 patients, Stage 2; 11 patients) received post-op RT after TAH and BSO (Group 2). Pre-op irradiation was given 4-6 weeks prior to surgery and post-op RT administered on 4-5 weeks following surgery. All patients except 1 patient(Group 2: ICR alone) received external irradiation. Seventy percent(13/19) of pre-op RT group and 54 percent(23/42) of post-op RT group received external pelvic irradiation and intracavitary radiation therapy(ICR). External radiation dose was 39.6-55 Gy(median 45 Gy) in 5-6. 5weeks through opposed AP/PA fields or 4-field box technique treating daily, five days per week, 180 cGy per fraction. ICR doses were prescribed to point A(20-39.6 Gy, median 39 Gy) in Group 1 and 0.5cm depth from vaginal surface (18-30 Gy,median 21 Gy) in Group 2. Results : The overall 5 year survival rate was $95{\%}$. No survival difference between pre-op and post-op RT group.($89.3{\%}$ vs $97.7{\%}$, p>0.1) There was no survival difference by stage, grade and histology between two groups. The survival rate was not affected by presence of residual tumor of surgical specimen after pre-op RT in Group 1 (p>0.1), but affected by presence of lymph node metastasis in post-op RT group(P<0.5). The complication rate of pre-op RT group was higher than post-op RT. ($16{\%}$ vs $5{\%}$) Conclusion : Post-op radiotherapy offers the advantages of accurate surgical-pathological staging and low complication rate.

Oocyte-sperm Binding Assay (OSBA) Technique for Rapid Q/C of IVF Culture Condition (체외수정용 배양조건의 신속한 Q/C를 위한 정자-난자 결합분석법(OSBA) 개발)

  • OSBA(oocytes-sperm binding assay) is a tool developed for rapid test of optimal condition of IVF medium and protein source by binding ability of mouse sperm and egg. Mouse oocyte-cumulus complexes were prepared by removing of the cumulus cells with 0.1% hyaluronidase. 10$\pm$2 oocytes per 30 ${mu}ell$ medium drop were inseminated with 3 ${mu}ell$ sperm suspension and were cultured f3r 3 hours and 24 hours, respectively. And the oocytes were recovered gently and the No. of sperm bound on oocytes were counted. In the Exp. 1, the ratio of oocytes bound with one sperm at least were 60.2%(50/83), 2%(2/77) and 100%(79/79) in the medium with no protein, FBS(15%, v/v) and BSA(0.4%. w/v), respectively, Fetal bovine serum(FBS) seriously inhibited sperm binding on oocyte, although bovine serum albumin(BSA) promoted the binding ability. The inhibiting effect of FBS was dependent on the concentration of FBS. The sperm binding ability according to oocyte maturity was tested in the Exp. 2. There was no significant difference between Met. II (mature) and Met. I (intermediate mature) oocytes in the number of oocytes bound with sperm and the number of sperm bound on oocytes. Finally, in Exp. 3, two batches of Ham's F10 medium with good and poor quality by OSBA were tested (The ratios of embryos developed from PN 1-cell stage to hatched blastocyst; 25% vs. 70%). In the medium with good quality, sperm binding ability was significantly increased (P < 0.05). The ratio of oocytes bound with one sperm at least was 66% and 90% in the medium with poor and good quality, respectively. Conclusively, It was possible to test IVF medium condition rapidly and easily by OSBA.

Effect of Selenium Feeding on Selenium Concentration of Blood and Velvet Antler in Sika deer(Cervus nippon) (셀레늄 급여가 꽃사슴의 혈액과 녹용내 셀레늄 함량에 미치는 영향)

  • This study was conducted to investigate the effects of selenium feeding and supplementation in diet on the concentration of selenium in blood and velvet antler of spotted deer(Sika deer). Three spotted deer were fed high selenium concentration(6mg/kg DM). Absorption and retention rates of selenium were examined by evaluating selenium concentrations in feces and urine. Stress-related hormones and serum biochemical parameters in blood were also evaluated for the purpose of detecting any negative effect by the high level of selenium feeding. Eight spotted deers were randomly assigned to two groups and were fed with one of two diets for 20 days, which were with or without the addition of 6mg selenium /kg diet. Concentration of selenium in velvet antler was evaluated. Selenium concentration in blood of spotted deer fed high level selenium for 30 days was significantly increased(p<0.05), retention rate of selenium reached 59.15%. No differences in level of stress-related hormone and biochemical parameters(NEFA, ALT, AST) in blood were observed by feeding high level selenium. The diet with selenium significantly increased concentrations of selenium in top(0.11 vs 0.45ppm; p<0.001), middle(0.08 vs 0.21ppm; p<0.01) and basepart(0.08 vs 0.15ppm; p<0.05) of velvet antler.

Effect of Visual Sensory Improvement by Amblyopia Treatment on Improvement of Ocular Functions (약시 치료에 의한 시감각 개선이 안기능 향상에 미치는 효과)

  • Purpose: This study is to investigate if the improvement of visual sensory (VS) by amblyopia treatment affects the ocular functions in refractive errors, accommodative errors and phoria at distance and near. Methods: 10 subjects (17 eyes, mean age of $10.7{\pm}2.9$ years) who treated amblyopia completely, were participated for this study. Refractive errors, accommodative errors, and distance and near phoria were compared between before and after treatments of amblyopia. Refractive errors and accommodative errors at 40 cm were measured using openfield auto-refractor (NVision-5001, Shin Nippon, Japan) and using monocular estimated method (MEM) respectively. Phoria was determined at 3 m for distance and at 40 cm for near using Howell phoria card, cover test or Maddox rod. Results: Mean corrected visual acuity (CVA) significantly increased from $0.46{\pm}0.11$ (decimal notation) for before amblyopia treatment to a level of $1.03{\pm}0.13$ for after amblyopia treatment (p < 0.001). For spherical refractive error, hyperopia significantly decreased from $+2.29{\pm}0.86D$ to a level of $+1.1{\pm}2.38D$ (p < 0.05) but astigmatism did not significantly change; $-1.80{\pm}1.41D$ for before treatment and $-1.65{\pm}1.30D$D for after treatment (p > 0.05). Accommodative error significantly decreased from accommodative lag of $+1.1{\pm}0.75D$ to a level of $+0.5{\pm}0.59D$ (accommodative lag) (p < 0.05). Distance phoria significantly changed from eso $2.9{\pm}6.17PD$ (prism diopters) to a level of eso $0.2{\pm}3.49PD$ (p < 0.05), and near phoria also significantly changed from eso $0.4{\pm}2.32PD$ to level of exo $2{\pm}4.9PD$ (p < 0.05). There was a high correlation (r = 0.88, p < 0.001) between improvement of visual acuity and decrease of accommodative lag. Conclusions: Hyperopic refractive error decreased with improvement of CVA or VS by amblyopia treatment. And the improvement of VS by amblyopia treatment also improved accommodative error, and changed phoria coupled with accommodation.
