Selective Radiotherapy after Distant Metastasis of Nasopharyngeal Carcinoma Treated with Dose-Dense Cisplatin plus Fluorouracil

  • Purpose: To investigate the efficacy and safety of selective radiotherapy after distant metastasis of nasopharyngeal carcinoma (NPC) treated with dose-dense cisplatin plus fluorouracil. Materials and Methods: Eligible patients were randomly assigned to a study group treated with dose-dense cisplatin plus fluorouracil following selective radiotherapy and a control group receiving traditional cisplatin plus fluorouracil following selective radiotherapy according to a 1:1 distribution using a digital random table method. The primary endpoint was overall survival (OS). Secondary endpoints were progression-free survival (PFS), objective response rate, relapse or progression rate in the radiation field and treatment toxicity. Results: Of 52 patients in the study group, 20 cases underwent radiotherapy., while in the control group of 51 patients, 16 underwent radiotherapy. The median PFS, median OS, survival rates in 1, 2 and 3 years in study and control group were 20.9 vs 12.7months, 28.3 vs 18.8months, 85.2%vs 65.9%, 62.2% vs 18.3%, and 36.6%vs 5.2% (p values of 0.00, 0.00, 0.04, 0.00 and 0.00, respectively). Subgroup analysis showed that the median OS and survival rates of 1, 2, 3 years for patients undergoing radiotherapy in the study group better than that in control group( 43.2vs24.1 months, 94.1% vs 86.7%, 82.4% vs 43.3%, 64.7% vs 17.3%, (p=0.00, 0.57, 0.04 and 0.01, respectively). The complete response rate, objective response rate after chemotherapy and three months after radiotherapy, relapse or progression rate in radiation field in study group and in control group were 19.2% vs 3.9%, 86.5% vs 56.9%, 85% vs 50%, 95% vs 81.3% and 41.3% vs 66.7% (p =0.03, 0.00, 0.03,0.30, 0.01 respectively). The grade 3-4 acute adverse reactions in the study group were significantly higher than in the control group (53.8% vs 9.8%, p=0.00). Conclusions: The survival of patients benefits from selective radiotherapy after distant metastasis of NPC treated with dose-dense cisplatin plus fluorouracil.

A Study on Conception of Friendship in Korean Children of School Age (학령기 아동의 우정관계 개념에 관한 연구)

  • The purpose of this study was to investigate the components in friendship as reported by Korean children of School age Specially, four major aspects of friendship - definition of friendship, friendship formation, friendship expectation, and friendship termination were explored. The subjects of this study consisted of 600 elementary school children in Grades 1 through 6, 50 males and 50 females at each grade level. Based on Bigelow (1977) five open - ended questions were administered. First and second graders were interviewed individually while 3rd-6th graders replied to a questionnaire in class. For content analysis, responses of each child were coded into 22 categories and each category was assigned a score from 1 to 4 according to its frequency. The data were analyzed by factor analysis. The results showed that four major aspects of friendship in Korean children of school age were found to consist of four different factors. DEFINITION OF FRIENDSHIP was found to consist of the factors; 1) reciprocity of liking vs. helping 2) global evaluation vs helping 3) sharing and 4) common activities. FRIENDSHIP FORMATION was found to consist of the factors; 1) situational effects vs complex evaluation 2) structural dimension vs common activities 3) helping and 4) staring. FRIENDSHIP EXPECTATION was found to consist of the factors; 1) mutuality 2) common activities vs. loyalty & commitment 3) exclusive - dyadic activities vs. admiration (character) and 4) interactive a1truism. FRIENDSHIP TERMINATION was found to consist of the factors; 1) physical separation vs disappointment (character) 2) conflict 3) physical separation vs. psychological dimension and 4) general altruism.

Breeding and Development of the Tscherskia triton in Jeju Island (제주도 서식 비단털쥐(Tscherskia triton)의 번식과 발달)

  • The greater long-tail hamster, Tscherskia triton, is widely distributed in Northern China, Korea and adjacent areas of Russia. Except for its distribution, biological characteristics related to life history, behavior, and ecological influences for this species are rarely studied in Korea. This study was conducted to obtain biological information on breeding, growth and development that are basic to species-specific studies. The study adopted laboratory management of a breeding programme for T. triton collected in Jeju Island from March, 2015 to December, 2016. According to the study results, the conception rate was 31.67% and the mice in the large cages had a higher rate of conception than those in the small cages (56.7 vs. 6.7%). The gestation period was $22{\pm}1.6days$ (ranges from 21 to27 days), and litter size ranged from 2 to 7, with a mean of $4.26{\pm}1.37$ in the species. The minimum age for weaning was between $19.2{\pm}1.4days$ (range of 18-21 days). There were no significant differences by sex between mean body weight and external body measurements at birth. However, a significant sexual difference was found from the period of weaning (21 days old) in head and body length, as well as tail length (HBL-weaning, $106.50{\pm}6.02$ vs. $113.34{\pm}4.72mm$, p<0.05; HBL-4 months, $163.93{\pm}5.42$ vs. $182.83{\pm}4.32mm$, p<0.05; TL-4 months, $107.23{\pm}3.25$ vs. $93.95{\pm}2.15mm$, p<0.05). Gompertz and Logistic growth curves were fitted to data for body weight and lengths of head and body, tail, ear, and hind foot. In two types of growth curves, males exhibited greater asymptotic values ($164.840{\pm}7.453$ vs. $182.830{\pm}4.319mm$, p<0.0001; $163.936{\pm}5.415$ vs. $182.840{\pm}4.333mm$, p<0.0001), faster maximum growth rates ($1.351{\pm}0.065$ vs. $1.435{\pm}0.085$, p<0.05; $2.870{\pm}0.253$ vs. $3.211{\pm}0.635$, p<0.05), and a later age of maximum growth than females in head and body length ($5.121{\pm}0.318$ vs. $5.520{\pm}0.333$, p<0.05; $6.884{\pm}0.336$ vs. $7.503{\pm}0.453$, p<0.05). However, females exhibited greater asymptotic values ($105.695{\pm}5.938$ vs. $94.150{\pm}2.507mm$, p<0.001; $111.609{\pm}14.881$ vs. $93.960{\pm}2.150mm$, p<0.05) and longer length of inflection ($60.306{\pm}1.992$ vs. $67.859{\pm}1.330mm$, p<0.0001; $55.714{\pm}7.458$ vs. $46.975{\pm}1.074mm$, p<0.05) than males in tail length. These growth rate constants, viz. the morphological characters and weights of the males and females, were similar to each other in two types of growth curves. These results will be used as necessary data to study species specificity of T. triton with biological foundations.

Effects of Deletion of Ca Supplement (limestone) on Growth and Beef Quality in Hanwoo Finishing Steers (한우 비육후기 사료에 칼슘 첨가제(석회석) 제거가 성장 및 육질특성에 미치는 영향)

  • A study was conducted to determine the effects of feeding a diet containing no Ca supplement (limestone) during the late finishing period on growth, marbling and serum 1,25-dihydroxy vitamin $D_3$ level in Korean native cattle. Twenty-four steers (20${\sim}$24 mo of age) were divided into two groups of 12 each: one group assigned to a control diet (concentrates containing 2.5% limestone) and the other to a diet containing no calcium supplement. They were allowed to have free access to diets (concentrates and orchard grass hay) and water during the entire feeding period (223 d). Serum $Ca^{2+}$, Ca and P concentrations were not influenced by diets, but serum 1,25-dihydroxy vitamin $D_3$ concentrations determined 2 or 6 mo after the beginning of feeding the experimental diets were higher (P<0.01) in steers fed the diet without Ca supplement than in those fed the control diet (78.3 vs 51.7 and 80.3 vs 51.1 pg/mL, respectively). Steers fed the diet without Ca supplement tended to have a higher intake of concentrates, but a lower intake of hay, compared to those fed the control diet. Average daily gain was higher (P<0.05) in steers fed the diet without Ca supplement than in those fed the control diet. Feeding the diet without Ca supplement remarkably (P<0.01) increased the marbling score (5.1 vs 2.2) and the muscle (M. longissimus dorsi) fat content (10.2 vs 6.7%) with a concomitant decrease in moisture content (67.6 vs 70.4%), compared to feeding the control diet. Ribeye area was increased (77.2 vs 82.8 $cm^2$) with the diet without Ca supplement, compared to the control diet (P<0.05). Meat color, pH and water-holding capacity in longissimus muscle were not different between the two groups. The Warner-Brazler Shear (WBS) force of the longissimus muscle was slightly (P=0.08) lower in steers fed the diet without Ca supplement than in steers fed the control diet (2.9 vs 3.2 kg/1.27-cm diameter core). Sensory evaluation showed that feeding the diet without Ca supplement slightly (P<0.05) improved tenderness (4.9 vs 4.5) and flavor (4.9 vs 4.6), compared to feeding the control diet, but juiciness was not affected by diets. Results showed that deletion of Ca supplement from finishing diets is beneficial, increasing growth and marbling partly through an increased energy intake and induced 1,25-dihydroxy vitamin $D_3$ synthesis that may increase intracellular $Ca^{2+}$ concentration and in turn fat synthesis.

Potential Characteristics of Supercapacitor Based on Ruthenium Oxide-Aqueous Electrolyte (루테늄 산화물-수계 전해액 수퍼캐패시터의 전위 특성)

  • The electrode for a supercapacitor was prepared using an amorphous ruthenium oxide, which was synthesized from ruthenium trichloride hydrate$(RuO_2{\cdot}nH_2O)$. A supercapacitor was assembled with an electrode of ruthenium oxide material on a current collector of tantalum, and an electrolyte of 4.8 M sulfuric acid. The result of the AC impedance analyses on $Ta/H_2SO_4(4.8 M)/Pt$ cell showed that tantalum was stable at the potential range of $0.0\~1.1V(vs. SCE)$. Therefore, Ta film could be used the supercapacitor as a current collector. The irreversible hydrolysis in the supercapacitor occurred over ca. 1.0V(vs.SCE) when the supercapacitor was protonated to 0.5V(vs. SCE). The supercapacitor protonated to 0.5V(vs.SCE) showed good electrochemical properties when it was tested at the potential range of 1.0V in the charge-discharge test. The potential range of the electrodes including the positive and the negative electrode was varied between -0.004 and 0.995V(vs. SCE). The potential ranges of the positive and the negative electrode were $-0.004\~0.515V(vs.\;SCE)\;and\; 0.515\~0.995V(vs.\;SCE)$, respectively.

Does Happiness Always Lead to Reliance on Feelings in Decision Making?

  • Previous research has demonstrated that happy moods are known to promote feeling-based processing, whereas sad moods promote reason-based processing. The current research investigates a boundary condition for the effects of a happy mood on feeling-based decision making. This research proposes that the level of control (low vs. high) one exercises in a happy situation can promote a greater reliance on feelings (vs. reasons) in making judgments and decisions. Specifically, we hypothesize that (1) a happy individual in a situation where control level is low (vs. high) will be more likely to choose a cognitively (vs. affectively) superior option (hypothesis 1), and (2) a happy individual in a situation where control level is low (vs. high) will exert reason- (vs. feeling-) based processing (hypothesis 2). Consistent with the hypothesis 1, the results of two experiments show that happy individuals are more likely to choose cognitively versus affectively superior options in a situation where control level is low (vs. high). Moreover, the mediation analysis confirms that happy individuals are more likely to rely on cognitive, reason-based decision making when their control level is low, which supports the hypothesis 2.

Differences in youngest-old, middle-old, and oldest-old patients who visit the emergency department

  • Objective As aging progresses, clinical characteristics of elderly patients in the emergency department (ED) vary by age. We aimed to study differences among elderly patients in the ED by age group. Methods For 2 years, patients aged 65 and older were enrolled in the study and classified into three groups: youngest-old, ages 65 to 74 years; middle-old, 75 to 84 years; and oldest-old, ${\geq}85years$. Participants' sex, reason for ED visit, transfer from another hospital, results of treatment, type of admission, admission department and length of stay were recorded. Results During the study period, a total 64,287 patients visited the ED; 11,236 (17.5%) were aged 65 and older, of whom 14.4% were 85 and older. With increased age, the female ratio (51.5% vs. 54.9% vs. 69.1%, P<0.001), medical causes (79.5% vs. 81.3% vs. 81.7%, P=0.045), and admission rate (35.3% vs. 42.8% vs. 48.5%, P<0.001) increased. Admissions to internal medicine (57.5% vs. 59.3% vs. 64.7%, P<0.001) and orthopedic surgery (8.5% vs. 11.6% vs. 13.8%, P<0.001) also increased. The ratio of admission to intensive care unit showed no statistical significance (P=0.545). Patients over age 85 years had longer stays in the ED (330.9 vs. 378.9 vs. 407.2 minutes, P<0.001), were discharged home less (84.4% vs. 78.9% vs. 71.5%, P<0.001), and died more frequently (6.3% vs. 10.4% vs. 13.0%, P<0.001). Conclusion With increased age, the proportion of female patients and medical causes increased. Rates of admission and death increased with age and older patients had longer ED and hospital stays.

Effect of OVOIL oil on B6D2F1 Mice Oogenesis

  • This study was performed to investigate the effect of types of oil (OVOIL vs. OIL) on B6D2F1 mice oogenesis. In this study, B6D2F1 F1 mice were used in order to maximize oogenesis. The expansion rate of cumulus cells ($82.0%{\pm}0.2$ vs. $78.0%{\pm}0.1$), in vitro fertilization rate ($92.0%{\pm}0.1$ vs. $88.0%{\pm}0.1$), developmental rate ($91.0%{\pm}0.1$ vs. $87.0%{\pm}0.2$), blastocysts formation rate ($56.0%{\pm}0.1$ vs. $57.0%{\pm}0.1$), and zona hatched rate($41.4%{\pm}0.2$ vs. $24.0%{\pm}0.1$) were not different between groups (NS; P>0.05). However, there was a significant difference in maturation rate; the OVOIL group showed higher maturation rate compared to that of the OIL group ($96.0%{\pm}0.1$ vs. $87.0%{\pm}0.1$; P<0.05). In the blastocysts cell numbers, the total cell numbers ($83.9{\pm}26.1$ vs. $56.9{\pm}23.9$), ICM cell numbers ($15.7{\pm}8.8$ vs. $6.3{\pm}3.5$), TE cell numbers ($68.3{\pm}25.7$ vs. $50.7{\pm}24.1$), % ICM ($21.6%{\pm}0.1$ vs. $12.7%{\pm}0.1$), and the ratio of ICM:TE ($1:6.2{\pm}6.5$ vs. $1:10.3{\pm}7.0$) were significantly higher in the OVOIL group than the OIL group (P<0.05). These results suggested that it is expected to achieve the more developmental ability of B6D2F1 mice depending on the type of oil (OVOIL vs. OIL). In addition, the results can provide essential information for culture condition on B6D2F1 mice. Henceforth, thus, it is expected that these results herein might be used for in vitro culture of human embryos.

Dosimetric comparison of axilla and groin radiotherapy techniques for high-risk and locally advanced skin cancer

  • Purpose: Radiation therapy targeting axilla and groin lymph nodes improves regional disease control in locally advanced and high-risk skin cancers. However, trials generally used conventional two-dimensional radiotherapy (2D-RT), contributing towards relatively high rates of side effects from treatment. The goal of this study is to determine if three-dimensional conformal radiation therapy (3D-CRT), intensity-modulated radiation therapy (IMRT), or volumetric-modulated arc therapy (VMAT) may improve radiation delivery to the target while avoiding organs at risk in the clinical context of skin cancer regional nodal irradiation. Materials and Methods: Twenty patients with locally advanced/high-risk skin cancers underwent computed tomography simulation. The relevant axilla or groin planning target volumes and organs at risk were delineated using standard definitions. Paired t-tests were used to compare the mean values of several dose-volumetric parameters for each of the 4 techniques. Results: In the axilla, the largest improvement for 3D-CRT compared to 2D-RT was for homogeneity index (13.9 vs. 54.3), at the expense of higher lung $V_{20}$ (28.0% vs. 12.6%). In the groin, the largest improvements for 3D-CRT compared to 2D-RT were for anorectum $D_{max}$ (13.6 vs. 38.9 Gy), bowel $D_{200cc}$ (7.3 vs. 23.1 Gy), femur $D_{50}$ (34.6 vs. 57.2 Gy), and genitalia $D_{max}$ (37.6 vs. 51.1 Gy). IMRT had further improvements compared to 3D-CRT for humerus $D_{mean}$ (16.9 vs. 22.4 Gy), brachial plexus $D_5$ (57.4 vs. 61.3 Gy), bladder $D_5$ (26.8 vs. 36.5 Gy), and femur $D_{50}$ (18.7 vs. 34.6 Gy). Fewer differences were observed between IMRT and VMAT. Conclusion: Compared to 2D-RT and 3D-CRT, IMRT and VMAT had dosimetric advantages in the treatment of nodal regions of skin cancer patients.

Effect of Replacement of Groundnut Cake with Urea-treated Neem (Azadirachta indica A. juss) Seed Kernel Cake on Nutrient Utilisation in Lambs

  • The effect of urea treatment on chemical composition of neem seed kernel cake (NSKC) was investigated by soaking the cake in 2.1% urea solution (1.2 l $kg^{-1}$ NSKC) for five days. The effect on utilisation of nutrients by replacing groundnut cake (GNC) (30%) with urea-treated neem seed kernel cake (UTNSKC) (33%) in a concentrate mixture fed to meet 70% of the protein requirements of lambs (8 males and 8 females), was monitored in a digestibility study. Following urea treatment of NSKC only 9.5% of urea was hydrolysed and the crude protein content of the cake was increased by 6.65%. The tannin content in depulped neem seeds was 37% catechin equivalent. Whereas feeding UTNSKC had no effect on intake of dry matter (72.5 vs 66.3 g/kg $BW^{0.75}day^{-1}$) and digestibility of crude fibre (41.3 vs 43.4%), the cake depressed (p<0.01) the percent digestibility of dry matter (63.7 vs 70.2), crude protein (63.2 vs 70.2), nitrogen free extract (73.8 vs 80.5) and gross energy (64.3 vs 69.1). Digestibility of ether extract (75.8 vs 70.9%) was higher (p<0.05) in animals offered UTNSKC. The nutritive value of the composite ration consumed by lambs offered UTNSKC was lower (p<0.01) in terms of total digestible nutrients (64.7 vs 70.2%) and digestible energy (2.8 vs 3.0 Kcal/g DM). Intake of digestible energy (199.8 vs 194.1 Kcal/kg $BW^{0.75}day^{-1}$) and retention of nitrogen (7.53 vs 8.23 g $day^{-1}$) and calcium (2.12 vs 1.84 g $day^{-1}$) were comparable on the 2 rations. Animals fed UTNSKC retained less (p<0.01) phosphorus (0.37 vs 1.05 g $day^{-1}$). The results indicate that urea treatment increased the protein level of NSKC whereas feeding the treated cake as a replacement of GNC, lowered the digestibility of nutrients and retention of phosphorus.
